Uvedená kazuistika poukazuje na ne zcela typický případ diabetu – jak terapii diabetes mellitus (DM) 2. typu dezintenzifikovat – protože většinou v našich praxích terapii tohoto chronického a progresivního metabolického onemocnění postupem měsíců a let stále více zintenzivňujeme. Jsou však pacienti a pacientky, u nichž po prvotním zaléčení hyperglykemie inzulinoterapií je k zamyšlení, zda z této léčby budou profitovat i nadále, nebo nikoliv. Inzu- linoterapie nastavená v době nezbytně nutné při její indikaci totiž neznamená, že by se měla stát léčbou doživotní, a při nevšední motivaci spolupracujícího subjektu je reálné dokonce její vysazení. Tato varianta alternativní terapie DM 2. typu, navazující na třídenní hladovku, se mi již několikrát osvědčila a níže popsaná kazuistika je zářným příkladem faktu, že když se správně nastaví psychika našich pacientů, lze v některých, vhodně indikovaných případech sledovat úspěšné zjednodušení terapie diabetu, jaké bychom ani nečekali.

The presented case report points to a not entirely typical case of diabetes and shows how to de-intensify the therapy of type 2 diabetes mellitus (DM) – because in most cases in our practice we increasingly intensify the therapy of this chronic and progressive metabolic disease over the course of months and years. However, there are patients in whom, after the initial treatment of hyperglycaemia with insulin therapy, it is worth considering whether they will continue to benefit from this treatment or not. Insulin therapy set at the time absolutely ne- cessary for its indication does not mean that it should become a lifelong treatment, and with unusual motivation of the cooperating subject, it is even possible to stop it. This variant of alternative therapy for type 2 diabetes mellitus, following a three-day fast, has already proven itself several times for me, and the case report described below is a shining example of the fact that when the psyche of our patients is set up correctly, in some appropriately indicated cases, successful simplification of diabetes therapy can be observed, such that we would not even expect.
