Inhibitory kontrolních bodů imunitního systému (immune checkpoint inhibitors, ICI) mají své pevné místo v léčbě lokálně pokročilého a metastazujícího nemalobuněčného karcinomu plic (non-small cell lung cancer, NSCLC); nyní vstoupily do paradigmatu časných stadií NSCLC. Nedávné údaje ukazují, že přidání ICI do neoadjuvantní nebo adjuvantní léčby zlepšuje míru přežití u časných stadií resekabilního NSCLC a v současné době představuje standardní možnost léčby v tomto stadiu. Nedávné studie fáze III zkoumaly přidání ICI v neoadjuvantní i adjuvantní fázi léčby, tzv. perioperační strategii, aby se zjistilo, zda by tato kombinace mohla pomoci maximalizovat přínos pro přežití. Hlavním cílem tohoto přehledu je představit výsledky nejnovějších studií, které podporují perioperační přístup u pacientů s časným stadiem NSCLC.

Immune checkpoint inhibitors (ICI) have an established role in the treatment of locally advanced and metastatic non-small cell lung cancer (NSCLC). ICI have now entered the paradigm of early-stage NSCLC. Recent data shows that the addition of ICI to neoadjuvant or adjuvant settings improves the survival rate in early-stage resectable NSCLC and is now a standard of care option in this setting. Recent phase III trials have explored the addition of ICI in both neoadjuvant and adjuvant treatment phases, a so-called perioperative strategy to establish if the combination could help to maximize the survival benefit. The main objective of this review is to narratively present the results of the latest studies that support the perioperative approach in patients with eariy-stage NSCLC.
