V systémové léčbě inoperabilního cholangiocelulárního karcinomu byly zaznamenány v posledních letech zásadní změny. Do první linie paliativní léčby se jednoznačně prosadila kombinace chemoterapie s imunoterapií, a to ať již s pembrolizumabem, nebo s durvalumabem. Již v úvodu diagnosticko-terapeutického procesu je vhodné u pacientů v celkově dobrém výkonnostním stavu indikovat testování přítomnosti specifických mutací, které umožňují využití specifické cílené léčby. Vlastní konvenční chemoterapie by měla být v tuto chvíli rezervována pouze pro pacienty, kteří z nějakého důvodu nejsou vhodní k vlastní cílené léčbě.

There was a significant progress in the systemic treatment of the inoperable cholangiocellular carcinoma. In the first palliative line the combination of the chemotherapy with immunotherapy (durvalumab or pembrolizumab) represents the new treatment standard. The testing for specific mutation should be performed as early as possible in the diagnostic process in all patients with good performance status, where targeted therapy could be the treatment of choice. Chemotherapy should be reserved only for the patients which are not candidates for possible targeted therapy.
