V léčbě silné bolesti u křehkých polymorbidních pacientů je třeba brát v potaz klinické charakteristiky pacienta, rizika nežádoucích účinků, ale i lékových interakcí. Významně zde pomáhají transdermální lékové systémy, v České republice jsou v této lékové formě dostupné fentanyl a buprenorfin. Lepší pochopení mechanismu analgetického účinku a znalost nežádoucích účinků umožňují individuální volbu, která zohledňuje širší klinický kontext i individuální charakteristiky pacienta.
The treatment of severe pain in frail polymorbid patients should take into account the clinical characteristics of the patient, the risk of adverse reactions, as well as drug interactions. Transdermal drug systems are of considerable help here, fentanyl and buprenorphine being available in this drug form in the Czech Republic. A better understanding of the mechanism of analgesic action and knowledge of adverse reactions allows for an individual choice that takes into account the broader clinical context as well as the individual characteristics of the patient.
