Práce hodnotí výsledky vyšetření 52 pacientů s neuroboreliózou, u nichž byla vyšetřena intratekální sekrece specifických antiboreliových protilátek. Všichni pacienti měli klinicky manifestní neurologický nález suspektní z neuroinfekce (meningoencefalitida, meningoradikulitida) a prokazatelné antibořeliové protilátky IgG v séru i mozkomíšním moku (konfirmováno Western blotem). Sekrece IgG antiboreliových protilátek (stanovená metodou Reibera a spol.) byla prokázána celkově pouze u 1/2 (54 %) pacientů, nejmenší podíl pozitivity byl ve skupině meningoencefalitid (10 pozitivních z 26), sekrece byla dále prokázána u 10 z 18 meningoradikulitid a u 8 z 11 encefaloradikulitid. Vyšetření může pomoci odlišit specifické protilátky proniklé pasivně ze séra při poruše hematoencefalické bariéry a zvýšit specifitu diagnostického procesu u neuroboreliózy.
The authors evaluate the results of examinations made in 52 patients with neuroborreliosis where the intrathecal secretion of specific antiborreha antibodies was examined. All patients had a clinically manifest neurological finding suspect of neuroinfection (meningoencephalitis, meningoradiculitis) and detectable antiborrelia IgG antibodies in serum and cerebrospinal fluid (confirmed by Western blot). Secretion of IgG antiborrelia antibodies (assessed by the method of Reiber et al.) was proved only in half the patients (54 %), the smallest ratio of positive patients was in the group with meningoencephalitis (10 positive of 26), secretion was furthermore detected in 10 of 18 patients with meningoradiculitis and in 8 of 11 patients with encephaloradiculitis. The examination can help to differentiate specific antibodies which penetrated passively from serum through an impaired haematoencephalic barrier and increase the specificity of the diagnostic process in neuroborreliosis.
