Neterapeutické užívání léčiv a jeho nárůst představují celosvětový a celospolečenský problém. Tento zasahuje nejen do oblasti zdravotnictví, ale má i značný lidský, sociální a ekonomický dopad. Vývoj lékových forem (LF) omezujících nebo zabraňujících zneužití léčiv je jedním z kroků komplexního řešení tohoto problému. Za poslední desetiletí došlo k vývoji odolnějších LF, které jsou schopné zneužívání léčivých přípravků (LP) omezit. Technologie a strategie zabraňující zneužívání LP se zaměřují většinou na obvyklé nebo očekávané aplikační cesty využívané při zneužití. Technologicky je možné omezit extrahovatelnost účinné látky, biologickou dostupnost po manipulaci s LF, zabránit distribuci léčiva v organismu po užití alternativní aplikační cesty, snížit účinek látky po manipulaci, případně účinek znepříjemnit. Nejčastěji se technologický přístup zaměřuje na formulace fyzikálně a chemicky odolných přípravků, které jsou často také schopny modifikovat farmakokinetiku léčiva po nežádoucí manipulaci. Postupně se do praxe prosazují i další možnosti, jako je použití antagonistů léčiv, proléčiv, averzivních látek a využití inovativních excipientů.
The non-therapeutic use of medicines and its increase is a global and societal problem. It affects not only the health sector, but it also has significant human, social and economic consequences. The development of dosage forms (DFs) to reduce or prevent the abuse of medicines is one step in a comprehensive solution to this problem. The last decade has brought the development of more robust DFs capable of reducing drug abuse. Technologies and strategies to prevent drug abuse typically target common or expected application routes. Technology can limit the extractability of the active ingredient and the bioavailability after DF manipulation, prevent drug distribution in the body after using an alternative administration route, reduce the effect of the substance after manipulation, or make the effect less pleasant. The technological approach often focuses on formulations that are physically and chemically resistant, often also capable of modifying the drug's pharmacokinetics after adverse manipulation. Other options, such as drug antagonists, prodrugs, aversive agents, and innovative excipients, are also gaining attention.
Nanočástice se řadí mezi moderní a rozvíjející se skupinu lékových forem s velkým potenciálem pro další budoucí využití. Do farmakoterapie přináší řadu výhod. Kombinace jejich velikosti, specifických vlastností pomocných látek a správně zvoleného léčiva může mít za výsledek cílenější terapii u některých vážných diagnóz. Se subcelulární velikostí lékové formy jsou ale spojeny i potencionální nežádoucí účinky, a ačkoliv výzkum probíhá s bezpočtem pomocných látek a typů nanočástic, do klinické praxe se ve skutečnosti prosadil jen jejich menší podíl. Tento stručný přehled pojednává právě o těchto v praxi používaných třídách nanočástic, přináší základní charakteristiku, rozdělení a případně klinické využití. Součástí článku je také aktuální přehled zástupců jednotlivých tříd nanočásticových lékových forem, které jsou v současnosti registrovány v ČR.
Nanoparticles belong to a modern and developing group of drug delivery systems with great potential for further future use. To pharmacotherapy, it brings a number of advantages. The combination of their size, specific properties of used excipients and a suitable drug can result in a more targeted therapy for some serious diagnoses. However, potential adverse effects are also associated with the subcellular size of the pharmaceutical form, and although research is being conducted with countless excipients and types of nanoparticles, only a small proportion of them have made it into clinical practice. This brief overview discusses these classes, it presents the basic characteristics, classification, and possible clinical use. The article also includes an up-to-date overview of representatives of individual nanoparticle drug delivery classes currently registered in the Czech Republic.

U akné se nejčastěji vyskytuje mastný typ pleti. Doporučený přístup k péči o mastný typ pleti a o akné se proto prolíná. Společným rysem je cílení na klíčové faktory (zvýšenou produkci mazu, folikulární hyperkeratózu, zvýšenou kolonizaci Cutibacterium acnes a zánět), které se v různé míře na patogenezi akné podílejí. Úlohou odborného personálu v lékárně je doporučit, jak správně o mastnou/aknózní pokožku pečovat a poradit při výběru vhodného (kosmetického) přípravku, který musí být bez obsahu dráždivých a komedogenních látek.

A common feature is targeting key factors (sebum overproduction, follicular hyperkeratinization, increased colonization of Cutibacterium acnes, and inflammation) that contribute to varying degrees in the acne ́s pathogenesis. The role of professional staff at a pharmacy is to recommend how to properly care for oily/acne-prone skin and to advise in a selection of an appropriate (cosmetic) product, which must be irritant-free and without comedogenic substances.

Inhibitory Janusových kináz (JAK) patří mezi cílené malé molekuly uplatňující se při přenosu signálu z extracelulárního prostoru do jádra buňky. Díky své velikosti působí intracelulárně, jsou selektivní k cílům, ke kterým vykazují afinitu, a jejich účinnost mimo cíl může být zvýšena v závislosti na dávce. K aktivaci Janusových kináz dochází po navázání adenosintrifosfátu (ATP). Obsazením katalytického místa pro ATP inhibitorem JAK s vyšší afinitou, než jakou má ATP, je tak zabráněno vazbě ATP a kinázové aktivitě, přičemž u inhibitorů JAK je tato inhibice reverzibilní a částečná. Jelikož existují čtyři izoformy JAK (JAK1, JAK2, JAK3 a TYK2), jsou vyvíjeny inhibitory JAK s různými profily selektivity. Signální dráhy významné v případě atopické dermatitidy (AD) jsou charakteristické zejména přítomností JAK1. Upadacitinib je reverzibilní inhibitor JAK1, který vykazuje nejvyšší selektivitu pro JAK1 oproti jiným izoformám JAK v enzymatických i buněčných testech in vitro. Účinnost upadacitinibu v léčbě pacientů se středně závažnou až těžkou AD byla prokázána v rámci klinických studií III. fáze Measure Up 1 a Measure Up 2 se srovnatelným výskytem nežádoucích účinků oproti placebu. Z hlediska dosažení příznivého bezpečnostního profilu hraje důležitou úlohu právě selektivita inhibitorů JAK.
Janus kinase (JAK) inhibitors are targeted small molecules involved in signal transduction from the extracellular space to the cell nucleus. They act intracellularly, are selective to targets to which they show affinity, and their off-target efficacy may be increased in a dose- dependent manner. Activation of JAK happens when adenosine triphosphate (ATP) binds the ATP binding site in JAK enzymes. If this site is occupied by JAK inhibitor with a higher affinity than ATP, ATP binding and, therefore, kinase activity is prevented. Inhibition by JAK inhibitors is reversible and partial. As there are four isoforms of JAK (JAK1, JAK2, JAK3 and TYK2), JAK inhibitors with different selective profiles are being developed. Signaling pathways critical in atopic dermatitis (AD) are characterized by the presence of JAK1. Upadacitinib is a reversible JAK1 inhibitor showing the highest selectivity for JAK1 over other JAK isoforms in in vitro enzymatic and cellular assays. The efficacy of upadacitinib in the treatment of patients with moderate to severe AD has been demonstrated in phase III clinical trials Measure Up 1 and Measure Up 2 with a comparable incidence of adverse events compared to placebo. The selectivity of the JAK inhibitors plays an important role in achieving a good safety profile.

Nowadays, analogues of gonadotropin-releasing hormone are among the standard drugs in several indications, often using modern dosage forms. Globally, their market is estimated at billions of $ annually. This review article summarizes basic information on the relationship between structure and effect, pharmacology, and application of gonadotropin-releasing hormone and its synthetic analogues. Due to their properties, namely a higher efficiency and a higher binding affinity for the gonadotropin-releasing hormone receptor compared to native gonadotropin releasing hormone, the analogues have found applications in the treatment of many diseases, where it is necessary to suppress the secretion of gonadotropins and sex hormones. They are usually indicated for the treatment of cancers of various origins, premature puberty or artificial insemination. Due to their peptide nature, they are not administered orally, but require injection or intranasal administration. Compared to agonists, antagonists show faster onset of action and the absence of the so-called flare-up phenomenon, i.e., temporarily increased serum levels of lutropin, follitropin and, consequently, of testosterone and estradiol.
The use of solid oral dosage forms depends on the degree of bioavailability of the active pharmaceutical ingredient. The rate and extent of the drug released from the dosage form and subsequently dissolved in the gastrointestinal fluids greatly affects its fate in the human body. In vitro dissolution test may provide an in-depth understanding of a drug formulation's behaviour in vivo, as long as it sufficiently simulates relevant gastrointestinal conditions. Therefore, the development of in vitro gastrointestinal systems, which reflects advanced technology and knowledge about the human body, is receiving considerable attention. This article is focused on the biorelevant dynamic apparatuses and their sophisticated design, overcoming many limitations of conventional dissolution devices and allowing a better correlation with in vivo behaviour of solid oral dosage forms.
Recently, the stimuli-responsive polymers get increased scientific interest due to the ability to reversibly alter their physicochemical properties. They are often referred to as smart, environmental-sensitive or intelligent polymers. This review provides fundamental information on various types of smart polymers responsive to biological, physical and chemical stimuli with examples of their use in the preparation of smart hydrogels and in situ gels with controlled or targeted drug release.
Oblast vývoje, výroby a bezpečnosti vakcín se v poslední době dostává do zorného pole veřejnosti. Cílem tohoto přehledového článku není poskytnout vyčerpávající informace o vývoji a výrobě vakcín, což by ani na omezeném prostoru nebylo možné. Jeho smyslem je v krátkosti nastínit stručný přehled principů, vývoje a výroby základních druhů vakcín a poukázat na benefity a rizika z pohledu farmaceuta. Ten se může podílet nejen na základním výzkumu, ale jeho role je především ve formulaci lékové formy, registraci vakcíny, její distribuci a osvětové a edukační činnosti směrem k laické i odborné veřejnosti.
The field of development, production and safety of vaccines has recently come under the public eye. The aim of this review article is not to provide comprehensive information on the development and production of vaccines, which would not be possible even in a limited space. Its purpose is to outline a brief overview of the principles, development, and production of basic types of vaccines and point out the benefits and risks from the perspective of a pharmacist. He can participate not only in basic research, but his role is mainly in the formulation of a dosage form, registration of a vaccine, its distribution, and educational activities towards the lay and professional public.

Cílem článku je představit základní design používaných léčivých přípravků s řízeným uvolňováním léčiva pro terapii nespecifických střevních zánětlivých onemocnění a objasnit jejich chování v gastrointestinálním traktu z pohledu farmaceutické technologie. Konkrétně se zaměřuje na přípravky s obsahem 5-aminosalicylové kyseliny (Asacol®, Pentasa®, Salofalk®,) a budesonidu (Budenofalk®, Cortiment®, Entocort®). Součástí příspěvku jsou také základní doporučení a praktické informace, která lze využít v klinické praxi.
The aim of this article is to introduce the basic design of used medicinal products with controlled drug release for the treatment of inflammatory bowel diseases and to clarify their behaviour in gastrointestinal tract from the perspective of pharmaceutical technology. Specifically, it focuses on pharmaceutical drugs containing 5-aminosalicylic acid (Asacol®, Pentasa®, Salofalk®) and budesonide (Budenofalk®, Cortiment®, Entocort®). As a part of this paper, basic recommendations and practical information that can be used in clinical practice are also given.
