Cíl studie: Cílem studie bylo zjistit incidenci ventilovaných dětí na zúčastněných pracovištích, základní demografická data ventilovaných dětí, používané ventilační režimy a jejich nastavení a letalitu ventilovaných dětí na JIP a její příčiny. Typ studie: Prospektivní observační multicentrická studie v období od 1. 2. 2002 do 30. 4. 2002. Místo studie: 7 jednotek intenzivní a resuscitační pediatrické péče nemocnic ČR. Materiál a metoda: Všechny děti ve věku 0–18 let přijaté na zúčastněná pracoviště, které vyžadovaly intubaci a umělou plicní ventilaci.U všech pacientů byly zaznamenány demografické údaje (věk, hmotnost, pohlaví), původ základního onemocnění, závažnost stavu (Pediatric Risk of Mortality Score – PRISM, Multiorgan System Failure – MOSF, Lung Injury Score – LIS), původ respiračního selhání, chronicita onemocnění, imunosuprese, délka ventilace, délka hospitalizace, použitý ventilační režim konvenční ventilace, nastavení ventilátoru, použití nekonvenční ventilace, acidobazická rovnováha a krevní plyny, alveolo-arteriální diference kyslíku – AaDO2, oxygenační index – OI, hypoxemické skóre – PaO2/FiO2, ventilační index – VI, poměr dechového objemu k mrtvému prostoru – Vd/Vt a dynamická poddajnost respiračního systému – Cdyn. Byla sledována letalita a příčiny úmrtí. Výsledky: Celkem144 dětí (42%dívek) bylo zařazeno do studie (23%všech přijatých pacientů).Průměrný věk byl 70 měsíců a průměrná hmotnost 23 kg. PRISM skóre a délka hospitalizace ventilovaných dětí byly dvojnásobné oproti průměrným hodnotám (11,7 vs 5,7, resp. 10,4 vs 4,8 dne).Průměrná délka ventilace byla 117 hodin,66%pacientů mělo mimoplicní původ respiračního selhání, 19 % pacientů bylo chronicky nemocných a 0,7 % pacientů imunosuprimovaných. Nejčastěji používanými ventilačními režimy jsou tlakově regulovaná objemově řízená ventilace a BIPAP. Permisivní hyperkapnie a pronační poloha byly použity shodně u 8 % pacientů, nekonvenční ventilační postupy celkem u 13 % pacientů. Letalita souboru byla 3,5%. Závěr: Děti vyžadující umělou plicní ventilaci tvoří 23%pacientů přijatých na pediatrické jednotky intenzivní péče.Závažnost stavu i délka hospitalizace jsou dvojnásobné oproti průměru, 66 % pacientů mělo mimoplicní původ respiračního selhání. Tlakově regulovaná objemově kontrolovaná ventilace a BIPAP jsou nejčastěji používané ventilační režimy, nekonvenční ventilace byla použita u 13 % pacientů. Letalita souboru byla 3,5 %, hypoxie nebyla příčinou smrti u žádného pacienta.
Objective: The aims of this study were to evaluate the incidence of mechanically ventilated children in participating units, to find out the demographic data of the patients, to evaluate ventilator settings and to assess the mortality of ventilated children. Design: Prospective observational multicenter study between 1. 2. 2002 and 30. 4. 2002. Setting: Seven paediatric intensive care units in tertiary hospitals in the Czech Republic. Material and Method: All children between 0 and 18 years admitted to the participating paediatric intensive care units who required intubation and mechanical ventilation were enrolled. Following parameters were recorded in all patients: demographic data (age, weight, gender), the origin of the admitting diagnosis, severity of illness (Paediatric Risk of Mortality Score – PRISM, Multiorgan System Failure – MOSF, Lung Injury Score – LIS), the origin of respiratory failure, presence of chronic disease and immunosuppression, length of ventilation, length of stay, ventilator setting, the use of unconventional ventilation, outcome (mortality), blood gas analyses and indices (alveoloarterial oxygen difference – AaDO2, oxygenation index – OI, hypoxemia score – PaO2/FiO2 and ventilation index – VI) and dynamic respiratory system compliance (Cdyn). Results: One hundred and forty four children (42% girls) were enrolled in total, which represented 23% of all admitted children. The mean age of the patients was 70 months and mean weight was 23 kg. PRISM score and the length of stay were twofold compared to mean values (11.7 vs. 5.7 and 10.4 vs. 4.8 days respectively). The mean length of ventilation was 117 hours, 66 % of the patients had an extrapulmonary origin of respiratory failure, 19% of the patients were chronically ill, and 0.7 % had the evidence of immunosuppression. The pressure regulated volume controlled and biphasic positive airway pressure were the most frequently used ventilator settings. Both permissive hypercapnia and prone position were used in 8% of the patients and unconventional ventilation in all was used in 13% of the patients. The mortality was 3.5%. Conclusion: Children on mechanical ventilation represent 23% of all patients admitted to paediatric intensive care units. The severity of illness and length of stay were twofold compared to mean values and 66% of the patients had an extrapulmonaryorigin of respiratory failure.The pressure regulated volumecontrolled and biphasic positive airway pressure were the most frequently used ventilator settings and unconventional ventilation was used in 13% of the patients. The mortality rate was 3.5% and hypoxia was not a cause of death in any patient.

Autoři popisují snahu propracovávat koncept autologní krevní transfuze v perioperativní péči malých dětí, u nichž je plánovaný odběr k autotransfuzi z různých důvodů technicky nemožný. Jsou stanoveny indikace, kontraindikace, nutná předoperační vyšetření, monitorování, způsob anestezie, technika odběru, nutná laboratorní kontrola a pooperační péče. Záměrná normovolemická hemodiluce byla provedena u 3 dětí předškolního věku, průměrné tělesné hmotnosti 22 kg. Průměrné množství odebrané krve bylo 193 ml. Sledovaná skupina pacientů je sice malá, ale dokazuje použitelnost této metody i u dětí předškolního věku.
The authors try to further improve the concept of autologous blood transfusion in perioperative care for small children who are unsuitable for planned pre-operative blood donation for technical reasons. There are outlined indications, contraindications, necessary preoperative evaluation, monitoring, type of anesthesia, technique of donation, necessary laboratory assessment and postoperative care. The intentional normovolemic hemodilution was performed in 3 children of pre-school age, mean weight 22 kilograms. The mean blood volume aspirated was 193 mililitres. The followed group is small; however, it proves the applicability of the method even in pre-school children.
