Ranné komplikace jsou nejčastější pooperační komplikací, i když většinou neohrožující pacienta na životě, zhoršují jeho kvalitu. Současně také zvyšují náklady na léčbu. Hojení ran ovlivňuje řada faktorů – charakter samotné rány, infekce, komorbidity, nutriční stav pacienta. Ale i stres a deprese mohou snížit zánětlivou odpověď organismu, usnadnit vznik ranných infekcí a prodloužit dobu hojení. Ovlivnit duševní rozpoložení pacienta zcela nedokážeme, ale je dobré se držet doporučení společnosti ERAS (Enhanced Recovery After Surgery) a snažit se odpovídajícím vysvětlením snížit stres z operace, dbát na dostatečné tišení bolesti a příjemné prostředí.
Surgical wound complications remain a major cause of morbidity; although usually not life threatening, they reduce the quality of life. They are also associated with excessive health care costs. Wound healing is affected by many factors – wound characteristics, infection, comorbidities and nutritional status of the patient. In addition, though, psychological stress and depression may decrease the inflammatory response required for bacterial clearance and so delay wound healing, as well. Although the patient´s state of mind can be influenced only to a certain extent, we should nevertheless stick to ERAS (Enhanced Recovery After Surgery) guidelines and try to diminish fear and anxiety by providing enough information preoperatively, pay due attention to postoperative analgesia and seek to provide an agreeable environment.

V následujícím souhrnném sdělení se věnujeme vlivu psychologických faktorů na vznik a progresi maligních chorob. Osoby trpící stresem, úzkostí či depresí mají vyšší riziko vzniku a rychlejšího růstu nádoru. Na prognózu má vliv i rodinný stav. Osoby žijící v manželském svazku mají lepší celkové i tumor specifické přežití.
The following review is dedicated to the impact of psychological factors on the genesis and development of malignant diseases. People suffering from stress, anxiety and depression have a higher risk of tumour occurrence and its faster growth. The prognosis is also influenced by the marital status; married patients have a better overall survival and cancer-specific survival compared to unmarried patients.

Plerixafor je novým typem léku, jehož podání vede nezávisle na chemoterapii a na G-CSF k vyplavení progenitorů krvetvorby (HSC) do periferní krve. Plerixafor je derivát bicyklamu o malé molekule, který reverzibilně blokuje vazbu SDF-1/CXCR4. Tato vazba patří mezi nejdůležitější mechanismy, které udržují HSC v prostředí kostní dřeně a její inhibice vede k rychlé mobilizaci HSC do krve. Plerixafor je velmi cenným lékem zejména pro skupinu pacientů, u kterých standardní mobilizace (G-CSF ± chemoterapie) nevede k dostatečnému vyplavení HSC do krve a u kterých tak nelze získat bezpečný štěp periferních kmenových buněk (PBSC, peripheral blood stem cells) pro autologní transplantaci. Cílem této retrospektivní studie je vyhodnotit výsledky dosažené při podávání plerixaforu v transplantačních centrech České republiky. Soubor pacientů a metody: Plerixafor byl podáván od 2/2009 do 8/2011celkem 93 pacientům ve věku od 4 měsíců do 71 let (63 % muži, 37 % ženy), u kterých byla předchozí nebo stávající mobilizace neúspěšná (82 %) nebo u kterých byla vysoká pravděpodobnost neúspěšné standardní mobilizace (13,5 %). Výsledky: Primární cíl odebrat bezpečný transplantát PBSC ( ≥ 2x106 CD34+ buněk/kg) byl splněn u 71,6 % mobilizací. Nežádoucí účinky plerixaforu byly mírné a dobře tolerované. Bylo provedeno 69 transplantací s použitím odebraných PBSC u 66 pacientů (71,0 %), s uspokojivě rychlou a trvalou obnovou krvetvorby. Závěr: Autoři uzavírají, že plerixafor je velmi účinný mobilizační prostředek umožňující bez závažných nežádoucích účinků získat bezpečný a kvalitní autologní transplantát krvetvorby u většiny pacientů, u kterých standardní mobilizace selhala nebo selhává. Použití plerixaforu tak těmto pacientům umožňuje absolvovat vysokodávkovanou chemoterapii, která může výrazně zlepšit jejich prognózu a v některých případech vést ke kurativnímu léčebnému výsledku.
Plerixafor is a novel kind of drug which administration leads to a hematopoietic stem and progenitor cells (HSC) mobilization independently of chemotherapy or G-CSF. Plerixafor is a small molecule bicyclam derivate and inhibits reversibly the SDF-1/CXCR4 interaction, which belongs to the most important mechanisms binding HSC to the bone marrow microenvironment. Inhibition of SDF-1/CXCR4 interaction results in a fast mobilization of HSC into a peripheral blood. Plerixafor is a very valuable substance especially for patients who are not able to mobilize sufficient numbers of HSC after G-CSF ± chemotherapy and, therefore, who are not the candidates for autologous peripheral blood stem cells (PBSC) transplantation because they are not able to collect adequate transplants. The aim of this study is to evaluate results reached with plerixafor in transplant centres in Czech Republic. Patients and methods: Plerixafor was given between 2/2009 and 8/2011 to 93 patients of age from 4 months to 71 years (63% males, 37% females), who were assessed to be proven (failed previous or current mobilization attempt; 82%) or predicted (high risk of failed mobilization; 13.5%) poor mobilizers. Results: The primary objective to collect a safe transplant (≥ 2x106 CD34+ cells/kg) was reached in 71.6% of mobilizations. Plerixafor adverse events were mild and well tolerated. Overall, 66 patients (71.0%) were treated by 69 transplantations using collected PBSC, engraftment was fast and durable. Conclusion: We conclude that plerixafor is a very efficient mobilization agent which allows collecting safe PBSC transplants of a good quality without significant adverse events in a majority of patients who failed standard mobilization. Plerixafor administration enables these patients to proceed to high dose therapy with autologous transplantation, which could significantly improve their prognosis or have a curative effect in certain patients.

Jako jedna z možností léčby pacientů s nádorovými onemocněními se v posledních letech rozvíjí imunoterapie s využitím dendritických buněk. Tyto antigen prezentující buňky hrají důležitou roli v indukci primární imunitní reakce na specifické antigeny. In vitro studie účinnosti vakcín dendritických buněk naložených individuálním antigenem ve formě nádorové RNA poskytují slibné údaje a základ pro další vývoj vakcín založených na dendritických buňkách. Vliv vakcinace RNA-transfekovanými buňkami na stimulaci T-buněčné imunity a terapeutických protinádorových odpovědí byl nejdříve studován u myších modelů a získané výsledky odůvodnily další klinické studie u lidí. Následující přehled shrnuje dosavadní studie zaměřené na použití RNA vakcín u pacientů s nádorovými onemocněními.
Immunotherapy using dendritic cells develops is considered as one of the possible treatments of patients with tumour diseases in the last years. These antigen presenting cells play important role in the induction of primary immune reaction on specific antigens. Data from in vitro studies of the efficiency of vaccines of dendritic cells loaded with individual tumour RNA provides promising information and the base for further dendritic cell vaccines development. Impact of RNA-transfected cell vaccine on T-cell immunity and preparation of therapeutic anticancer responses was first studied on murine models and acquired results subsequent clinical studies on humans. Following survey summarizes studies focused on the use of RNA vaccines in tumour patients.
