The aim of the study was to present the effect and advantages of surgical decompression and dynamic transpedicular stabilisation in patients with degenerative spondylolisthesis of the lumbosacral spine. MATERIAL AND METHODS: This prospective study involved patients undergoing dynamic transpedicular stabilisation using Isolock or Isobar TTL (Scient X, France) systems. Between June 2003 and June 2009, 65 patients were treated and followed-up. They were aged 35 to 75 years (average, 57.17 years), and there were 32 men and 33 women. Follow-up ranged from 1 to 6 years. Based on indications for surgery they fell into two groups. Group 1 included 52 patients with grade I or II degenerative spondylolisthesis or retrolisthesis. Group 2 (control) consisted of 13 patients with degenerative disc disease or failed back surgery syndrome. The disorder had always been manifested by combined axial and radicular symptoms. Treatment included posterior decompression of nerve structures by laminectomy in conjunction with semi-rigid stabilisation, without fusion. Followup clinical (VAS, ODI), neurological and radiographic examinations were carried out at 6 weeks, 6 months and 1 to 6 years after surgery. The VAS and ODI results of both groups were statistically analysed and compared. RESULTS: During follow-up the ODI values decreased by 54 % (from 58.4 % to 26.8 %) and VAS values by 62 % (from 7.9 to 3.0) as compared with the pre-operative values, and this was statistically significant. When both groups were compared, the VAS values decreased significantly (by 5.61) in Group 1, as compared with Group 2 (decrease by 3.54). DISCUSSION: In the treatment of pseudospondylolisthesis, the semi-rigid stabilisation with spinal decompression, as presented here, is a convenient alternative to simple decompression without fixation or to various forms of instrumented or non-instrumented arthrodesis. A disadvantage associated with arthrodesis is a higher risk of ASD development; dynamic systems do not allow for reduction of spondylolisthesis and involve a change in sagittal spinal balance, and simple decompression carries the risk of slip progression and recurrent problems. CONCLUSIONS: The authors demonstrated that decompression combined with semi-rigid stabilisation had a very good effect on the clinical state of patients with degenerative spondylolisthesis (retrolisthesis) at medium-term follow-up. The procedure was less effective in other indications. Semi-rigid stabilisation with Isobar TTL or Isolock systems prevented the progression of anterolisthesis or retrolisthesis; none of the patients experienced instrumentation failure. Neither symptomatic restenosis nor disc herniation was found in the instrumented segment. Semi-rigid stabilisation can, if necessary, be converted to fusion or disc replacement.

Cíl studie: Zjištění četnosti výskytu a rizikových faktorů pro rozvoj pooperační bolesti v krku po celkové anestezii. Typ studie: Dotazníková, observační, zaslepená. Typ pracoviště: Oddělení a JIP chirurgie, traumatologie, plastické a estetické chirurgie, urologie a gynekologie fakultní nemocnice. Materiál a metoda: 1942 pacientů po elektivním chirurgickém výkonu bylo dotázáno na bolest v krku v den operace, 1. a případně 3. pooperační den. Při bolestech přetrvávajících 3. den jsme provedli otorinolaryngologické vyšetření. Z dokumentace dospávacího pokoje příslušného oddělení a anesteziologického záznamu jsme zapsali demografické údaje, použité anesteziologické techniky a podaná léčiva, která mohla ovlivnit výskyt pooperačních bolestí v krku. K hodnocení jsme použili statistický software SPSS s příslušnými testy na 5% hladině významnosti. Výsledky: Bolest v krku v den operace udávalo 16,2 % pacientů, četnost závisela na způsobu zajištění dýchacích cest při celkové anestezii. Nejvyšší byla po orotracheální intubaci (18,3 %) a zavedení laryngeální masky (12,8 %), nejnižší po ventilaci obličejovou maskou (4,0 %). Výskyt bolestí ovlivnil i způsob lubrikace konce tracheální rourky před intubací. Mesocain gel a voda zvyšovaly výskyt bolesti dvojnásobně ve srovnání s použitím K-Y gelu nebo ponecháním bez ošetření. Pohlaví (p = 0,712), BMI (p = 0,982), zkušenosti anesteziologa (p = 0,311), peroperačně zavedená nazogastrická sonda (p = 0,522) ani kuřáctví (p = 0,691) bolest v krku výrazně neovlivnily. Závěr: Způsob zajištění dýchacích cest během celkové anestezie a použitá lubrikancia významně ovlivňují četnost pooperačních bolestí v krku.
Objective: To assess the incidence and risk factors for postoperative sore throat. Design: A prospective, observational, double-blind study. Setting: Departments and ICUs of Surgery, Trauma, Plastic and Aesthetic Surgery, Urology and Gynaecology, University Hospital Olomouc. Materials and methods. The study comprised of 1,942 patients after elective surgery who were inquired about sore throat and other peri-operative complaints on the 1st postoperative day. If sore throat or hoarseness were present on the 1st postoperative day, the patients were re-examined on the 3rd postoperative day. Patients with sore throat persisting till the 3rd postoperative day were referred for an ENT examination. Patient records from the recovery room and wards as well as the anaesthetic records were used to find out about the demographic data and anaesthetic management. The data were processed using appropriate statistical tests. Results: A total of 16.2% of patients had postoperative sore throat on the day of surgery, occurring at various rates depending on the type of airway management during general anaesthesia. Sore throat was the most frequent after endotracheal intubation (18.3 %) and laryngeal mask insertion (12.8 %). The lowest rates of sore throat were reported after face mask ventilation (4 %). The way the tracheal tube tip was prepared prior to tracheal intubation had a prominent influence as well. Both local anaesthetic (Mesocain) jelly and water increased the incidence of sore throat two-fold as compared with K-Y jelly or no pre-treatment. Gender (p = 0.712), BMI (p = 0.982), anaesthesiologist’s experience (p = 0.311), nasogastric tube insertion (p = 0.522) or smoking (p = 0.691) did not influence the incidence of sore throat. Conclusion: The technique of peri-operative airway management and lubricant use during general anaesthesia significantly affect the incidence of postoperative sore throat.

Cíl studie: Srovnání farmakodynamiky cisatrakuria, rokuronia a vekuronia u mladých a starších mužů během totální intravenózní anestezie. Typ studie: Klinická, prospektivní, kontrolovaná, randomizovaná. Typ pracoviště: Klinika anesteziologie a resuscitace fakultní nemocnice. Materiál a metoda: Po schválení etickou komisí a informovaném souhlasu nemocných jsme po úvodu do anestezie podali 240 mužům dvou věkových skupin (20–40, 60–75 let) ekvipotentní dávku (2 × ED95) cisatrakuria, rokuronia nebo vekuronia. Monitorováním relaxace (TOF, stimulace n. ulnaris, elektromyografická odpověď m. adductor pollicis) jsme u každého nemocného stanovili rychlost nástupu účinku použitého relaxancia (do maximální deprese T1), délku klinického trvání (do 25% zotavení T1) a délku účinku do plného zotavení (dosažení TOF-ratio ? 0,9). Rozdíly mezi skupinami mladších a starších nemocných jsme pro každé relaxans porovnali příslušnými statistickými testy (Mann-Whitney, Kruskal-Wallis). Výsledky: Medián rychlosti nástupu účinku (v sekundách) cisatrakuria byl u mladších mužů 205 vs 255* u seniorů, rokuronia 95 vs 133* a vekuronia 210 vs 250* (p < 0,001). Medián délky klinického účinku (v minutách) cisatrakuria byl u mladších 44 vs 43,5* u seniorů (p = 0,568), rokuronia 36 vs 58* a vekuronia 32 vs 60. Medián délky účinku do plného zotavení (v minutách) cisatrakuria byl u mladších mužů 71 vs 77* u seniorů (p = 0,095), rokuronia 60 vs 103 a vekuronia 56 vs 92*. Závěr: Nástup účinku studovaných relaxancií je ve stáří pomalejší. Trvání klinického účinku i doba do plného zotavení jsou pro rokuronium a vekuronium delší u seniorů, délka účinku cisatrakuria na věku nezávisí.
Objective: To compare the pharmacodynamics of cisatracurium, rocuronium and vecuronium following a single dose in young and elderly males during TIVA. Design: Clinical, prospective, controlled, randomized study. Setting: Department of Anaesthesiology, University Hospital. Materials and methods. Following ethics committee approval and patients' informed consent, 240 males of two age groups (20–40, 60–75 years) received cisatracurium, rocuronium or vecuronium (2×ED95). For each patient, neuromuscular monitoring (TOF, electromyography) determined the onset time (maximal T1 depression), clinical duration (25% recovery of T1) and full recovery (TOF-ratio ? 0.9). Mann-Whitney or Kruskal-Wallis test were used to compare the differences between young and elderly patients. Results: The median onset times (seconds) were 205 vs. 255* for cisatracurium, 95 vs. 133* for rocuronium and 210 vs. 250* for vecuronium in the young and elderly patients, respectively (*p < 0.001). The median clinical duration (minutes) was 44 vs. 43.5 (p = 0.568) for cisatracurium, 36 vs. 58* for rocuronium and 32 vs. 60* for vecuronium in the young and elderly patients, respectively. Finally, the median full recovery times (minutes) were 71 vs. 77 (p = 0.095) for cisatracurium, 60 vs. 103* for rocuronium and 56 vs. 92* for vecuronium, in the young and elderly patients, respectively. Conclusion: The onset was slower in the elderly. Both clinical duration and full recovery for rocuronium and vecuronium were longer in the elderly whereas these parameters for cisatracurium were age-independent.
