Dialýza je metodou náhrady funkce ledvin (angl. RRT - renal replacement therapy), díky níž dochází k očišťování krve od nahromaděných katabolitů organismu a přebytečné vody. Za rok je v České republice dialyzováno kolem 8 000 pacientů. Základem včasné diagnostiky a léčby renálního selhání je spolupráce lékaře prvního kontaktu s nefrologem a v případě pacientů s terminálním selháním ledvin s dialyzačním střediskem. Dialýza je v ideálním případě pouze "přemosťující" terapií k transplantaci ledviny, která je jedinou kauzální léčbou renálního selhání. Podle statistik ji v roce 2021 v ČR podstoupilo celkem 449 dospělých pacientů. Zařazení pacienta do dialyzačního programu vede často k významné změně kvality života pacienta i jeho rodiny. Dialyzovaní pacienti jsou často těmi nejvíce nemocnými a jejich obtíže se prolínají napříč celým medicínským spektrem. V péči o dialyzovaného je tak nutný multidisciplinární přístup včetně psychologicko‐spirituální roviny a sociální péče.
Dialysis is a method of renal function replacement (RRT - renal replacement therapy), where the blood is purified from the accumulated catabolites and excess water. Approximately 8,000 patients undergo dialysis in the Czech Republic per year. The basis of early diagnosis and treatment of renal failure is the cooperation of the doctor of first contact with the nephrologist and, in the case of terminal kidney failure, with the dialysis center. Dialysis is ideally only a "bridging" therapy to kidney transplantation, which is a causal treatment for renal failure. According to statistics, a total of 449 adult patients underwent it in the Czech Republic in 2021. Placing a patient on a dialysis program often leads to a significant change in the quality of life of the patient and their family. Dialysis patients often exhibit a high degree of polymorbidity, with their diseases covering the entire medical spectrum. Therefore, a multidisciplinary approach, including psychological-spiritual level and social care, is necessary in the care of a dialysis patient.

Goodpastureova nemoc a glomerulonefritida s pozitivitou anti‐GBM protilátek (protilátky proti glomerulární bazální membráně) patří mezi vzácné autoimunitní vaskulitidy malých cév postihující převážně mladé muže. Zaujímá důležité místo v rámci diferenciální diagnostiky pulmorenálního syndromu. V diagnostice hraje zásadní roli průkaz cirkulujících autoprotilátek, typický histologický obraz biopsie ledviny s nálezem srpkovité glomerulonefritidy a klinický obraz nefritického syndromu. Naše kazuistika popisuje případ mladého pacienta s anti‐GBM nefritidou charakteru rychle progredující glomerulonefritidy (rapidly progressive glomerulonephritis, RPGN) s progresí do terminálního renálního selhání dependentního na dialýze. Netypicky došlo ke vzniku nefrotického syndromu s volum‐dependentní hypertenzí. Případ byl komplikován rozvojem heparinem indukované trombocytopenie. Při kombinované imunosupresivní terapii s plazmaferézou se stav stabilizoval. K obnově renálních funkcí ale již nedošlo. Toto dříve fatální onemocnění má v dnešní době díky imunosupresivní terapii, plazmaferéze a hemodialyzačním metodám velmi dobrou prognózu přežití.
Goodpasture's disease and anti-glomerular basement membrane nephritis (anti-GBM nephritis) are rare autoimmune small vessel vasculitis predominantly affecting young men. Goodpasture's disease plays an important part in differential diagnosis of pulmonary - renal syndrome. The evidence of circulating autoantibodies, a typical histological appearance of the kidney biopsy with finding of the crescent glomerulonephritis and clinical presentation of nephritic syndrome play an important role in diagnosis. Our case report describes a case of a young male with anti-GBM nephritis that presents as rapidly progressive glomerulonephritis (RPGN) with progression to dialysis-dependent renal failure. The atypical sign of the case was development of nephrotic syndrome with volume-dependent hypertension. The case was complicated by heparin-induced thrombocytopenia. During combined immunosuppressive therapy with plasmapheresis, the condition of the patient has stabilized. However, renal function did not recover. This previously fatal disease has nowadays a very good survival rate because of immunosuppresion therapy, plasmapheresis and hemodialysis.

The complexity and application range of interventional and diagnostic procedures using contrast media (CM) have recently increased. This allows more patients to undergo procedures that involve CM administration. However, the intrinsic CM toxicity leads to the risk of contrast-induced acute kidney injury (CI-AKI). At present, effective therapy of CI-AKI is rather limited. Effective prevention of CI-AKI therefore becomes crucially important. This review presents an in-depth discussion of CI-AKI incidence, pathogenesis, risk prediction, current preventive strategies, and novel treatment possibilities. The review also discusses the difference between CI-AKI incidence following intraarterial and intravenous CM administration. Factors contributing to the development of CI-AKI are considered in conjunction with the mechanism of acute kidney damage. The need for ultimate risk estimation and the prediction of CI-AKI is stressed. Possibilities of CI-AKI prevention is evaluated within the spectrum of existing preventive measures aimed at reducing kidney injury. In particular, the review discusses intravenous hydration regimes and pre-treatment with statins and N-acetylcysteine. The review further focuses on emerging alternative imaging technologies, alternative intravascular diagnostic and interventional procedures, and new methods for intravenous hydration guidance; it discusses the applicability of those techniques in complex procedures and their feasibility in current practise. We put emphasis on contemporary interventional cardiology imaging methods, with a brief discussion of CI-AKI in non-vascular and non-cardiologic imaging and interventional studies.

AIM: Pentose phosphate pathway (PPP) with key enzyme transketolase (TKT), represents a potentially 'protective' mechanism in hyperglycaemia. Diabetic kidney disease (DKD), a common complication of both type 1 and type 2 diabetes associated with significant morbidity and mortality, represents the most common cause of chronic kidney disease (CKD). We hypothesized that protective PPP action in diabetes and eventually even more severely in concomitant DKD might be compromised by limited intracellular availability of an active TKT cofactor thiamine diphosphate (TDP). METHODS: Effect of hyperglycaemia on gene expression and protein levels of key PPP loci was studied in vitro using human cell lines relevant to diabetes (HUVEC and HRGEC) and (together with measurement of TKT activity, plasma thiamine and erythrocyte TDP concentration) in vivo in diabetic vs. non-diabetic subjects with comparable renal function (n=83 in total). RESULTS: Hyperglycaemia significantly decreased protein levels of RFC-1, THTR1, THTR2 and TKT (P<0.05) in vitro. Analysis of blood samples from CKD patients with and without diabetes and from controls did not reveal any difference in gene expression and protein levels of thiamine transporters while TKT activity and TDP in erythrocytes gradually increased with decreasing kidney function being highest in patients with CKD3-4 of both diabetic and non-diabetic aetiology. Hyperglycaemia and uremic serum mimicking CKD in diabetes did not affect TKT activity in vitro (P<0.05). CONCLUSION: Both in vitro and human experiments showed decrease or unchanged expression, respectively, of thiamine transporters induced by hyperglycaemia while TKT activity in parallel with intracellular TDP was increased in CKD patients with or without diabetes. Therefore, lack of adaptive increase of thiamine transmembrane transport allowing further increase of TKT activity might contribute to compromised PPP function in diabetes and CKD and to the development of glycotoxic injury.
