Parkinson's disease and parkinsonism are relatively common neurodegenerative disorders. This study aimed to assess potential genetic risk factors of haplotypes in genes associated with parkinsonism in a population in which endemic parkinsonism and atypical parkinsonism have recently been found. The genes ADH1C, EIF4G1, FBXO7, GBA, GIGYF2, HTRA2, LRRK2, MAPT, PARK2, PARK7, PINK1 PLA2G6, SNCA, UCHL1, and VPS35 were analyzed in 62 patients (P) and 69 age-matched controls from the researched area (C1). Variants were acquired by high-throughput sequencing using Ion Torrent workflow. As another set of controls, the whole genome sequencing data from 100 healthy non-related individuals from the Czech population were used (C2); the results were also compared with the Genome Project data (C3). We observed shared findings of four intron (rs11564187, rs36220738, rs200829235, and rs3789329) and one exon variant (rs33995883) in the LRRK2 gene in six patients. A comparison of the C1-C3 groups revealed significant differences in haplotype frequencies between ratio of 2.09 for C1, 1.65 for C2, and 6.3 for C3, and odds ratios of 13.15 for C1, 2.58 for C2, and 7.6 for C3 were estimated. The co-occurrence of five variants in the LRRK2 gene (very probably in haplotype) could be an important potential risk factor for the development of parkinsonism, even outside the recently described pedigrees in the researched area where endemic parkinsonism is present.

Farmakoterapie extrapyramidových onemocnění zažívala svůj boom v průběhu devadesátých let minulého století, spíše v jeho druhé polovině, kdy většina klinických hodnocení v neurologických indikacích zkoumala efekt různých molekul na Parkinsonovu nemoc (PN) a další extrapyramidová onemocnění. Další dekáda byla v této oblasti ve znamení pozvolného útlumu velkých klinických hodnocení a na významu nabývalo zavádění nákladných forem léčby pokročilého stadia Parkinsonovy nemoci, spolu s rozšiřováním indikací léčby botulotoxinem (BoNT). Následná globální ekonomická krize zde v podstatě ukončila aktivity farmaceutického průmyslu a korporátní výzkum se soustředil na jiná odvětví medicíny či na jiné oblasti neurologie. Prakticky až posledních 5-7 let lze pozorovat jisté oživení farmakologického výzkumu, akademického i korporátního, na poli extrapyramidových onemocnění. Jsou zkoumány nové lékové formy L-DOPA, jsou zkoušeny nové molekuly ovlivňující u Parkinsonovy nemoci jiné systémy než dopaminergní; pozornost je věnována i non-motorické symptomatice nemoci, probíhá i intenzivní translační výzkum. Jsou zkoušeny nadějné molekuly v terapii neurodegenerativního atypického parkinsonismu. Léčba extrapyramidových dyskinezí (či "hyperkinezí") se spíše pozvolna zbavuje starších, "folklórních" stereotypů a standardizuje postupy na bázi "evidence-based medicine" (EBM). Velký rozvoj zaznamenávají nové postupy v léčbě dyskinezí botulotoxinem, ať už se jedná o distribuci lokální terapie, inovaci dávkovacích schémat nebo cílenou implikaci centrálního efektu léčby. Jakkoliv nelze oblast extrapyramidových onemocnění nazvat jedním z nejdynamičtěji se rozvíjejících odvětví neurologické terapie, je situace mnohem méně stacionární, než byla před deseti lety.
The pharmacotherapy for extrapyramidal diseases experienced a boom during the 1990s, particularly in the second half of the decade, when most clinical trials in neurological indications investigated the effect of various molecules on Parkinson's disease and other extrapyramidal diseases. The following decade witnessed a slow decline in large clinical trials, and an issue of growing importance was the introduction of costly forms of treatment of advanced-stage Parkinson's disease as well as expansion of indications for the treatment with botulinum toxin. The subsequent global economic crisis essentially ended the activities of the pharmaceutical industry in this area, and corporate research focused on other branches of medicine or on other fields of neurology. Only in the last five to seven years, there has been some revival of pharmacological research, both academic and corporate, into the field of extrapyramidal diseases. Novel L-DOPA dosage forms are being investigated and new molecules targeting other than dopaminergic systems in Parkinson's disease are being tested; attention is being paid to non-motor symptomatology of the disease and intensive translational research is underway. Promising molecules are being tested in the treatment of neurodegenerative atypical parkinsonism. The treatment of extrapyramidal dyskinesias (or hyperkinesias) rather slowly abandons old, traditional stereotypes and standardizes evidence-based medicine strategies. There is a major development in novel strategies in treating dyskinesia with botulinum toxin, whether it be distribution of local therapy, innovation in dosing schedules, or targeted implication of central treatment effect. Even though the field of extrapyramidal diseases is not one of the most dynamically developing areas of neurological treatment, the situation is much less stationary than it was ten years ago.

Idiopatická či sporadická Parkinsonova nemoc (PN) je považována za jednu z nejčastějších neurodegenerativních chorob. Toto onemocnění má poměrně heterogenní fenotypický charakter i původ: je pravděpodobné, že se na manifestaci PN podílí jak endogenní (genetické), tak i exogenní (environmentální) příčiny, je ovlivněna i mnohými dalšími faktory, skutečná patofyziologická příčina PN však dosud uspokojivě objasněna není. Je proto nepochybně mimořádně důležité primárně zmapovat všechna data, která mohou k pochopení vzniku PN napomoci. K těm patří nepochybně i data epidemiologická, mohou totiž poukázat i na faktory, které by dále mohly být experimentálně ověřovány. Je nutno zde konstatovat, že spolehlivá data týkající se prevalence a incidence PN jsou spíše výjimkou. Zatím bylo provedeno jen málo epidemiologických studií za pomoci validizovaných metod; pokud provedeny byly, jednalo se jen o relativně malé populace v geograficky omezených regionech a jiné studie byly prováděny pouze v oblastech s endemických výskytem parkinsonismu. Nicméně z dostupných studií lze říci, že prevalence PN a neurodegenerativního parkinsonismu vzrůstá s věkem, dále že odhadovaný rozptyl incidence je v rozpětí 10-50 případů na 100 000 ročně a pravděpodobná míra prevalence onemocnění je 100-300 nemocných na 100 000 obyvatel.
Idiopathic (sporadic) Parkinson's disease (PD) is one of the most frequent neurodegenerative disorders. The disease has heterogenous pfenotype and origin: both endogenous (genetic) and exogenous (enviromental) factors probably participate on manifestations of the PD and many other factors are involved. However, the exact pathophyziological cause has not been indentified yet. Therefore it is exceptionally important to collect all data which could help understanding the development of the Parkinson's disease. The epidemiological data can expose factors which can be further experimentally verified. It is essential to point out that relevant data about prevalence and incidence of PD are relatively rare. Just few epidemiological studies has been conducted yet using validated methods and if so, just small groups in geographically restricted areas were examined and other studies were contucted in regions with endemic incidence od parkinsonism. However, available studies suggest that the prevalence of PN and neurodegenerative parkinsonism increases with age, estimated incidence is in the range of 10-50 cases per 100 000 per year, and the probable prevalence rate is 100-300 patients per 100 000 inhabitants.

An increased prevalence of familial neurodegenerative parkinsonism or cognitive deterioration was recently found in a small region of southeastern Moravia.The aim of the study was to assess the genetic background of this familial disease.Variants in the ADH1C, EIF4G1, FBXO7, GBA + GBAP1, GIGYF2, HTRA2, LRRK2, MAPT, PRKN, DJ-1, PINK1, PLA2G6, SNCA, UCHL1, VPS35 genes were examined in 12 clinically positive probands of the pedigree in which familial atypical neurodegenerative parkinsonism was identified in previous epidemiological studies. Libraries were sequenced by massive parallel sequencing (MPS) on the Personal Genome Machine (PGM; Ion Torrent). Data were analyzed using Torrent Suite and IonReporter software. All variants were then verified and confirmed by Sanger sequencing.We identified 31 rare heterozygous variants: 11 missense variants, 3 synonymous variants, 8 variants in the UTR region, and 9 intronic variants. Six variants (rs1801334, rs33995883, rs35507033, rs781737269, rs779760087, and rs63750072) were evaluated as pathogenic by at least one in-silico predictor.No single "founder" pathogenic variant associated with parkinsonism has been found in any of the probands from researched pedigree. It may rather be assumed that the familial occurrence of this disease is caused by the combined influence of several "small-effect" genetic variants that accumulate in the population with long-lasting inbreeding behavior.

Atypické parkinsonské syndromy tvoří pestrou škálu fenotypů charakterizovaných přítomností parkinsonského syndromu s měnlivou akcentací jednotlivých symptomů, motorických, kognitivních i behaviorálních, a s velmi variabilním průběhem. Klasifikace těchto onemocnění prochází rychlým vývojem, a to hlavně díky molekulární genetice a klinicko-patologické korelaci.
Atypical parkinsonian syndromes include a diverse range of phenotypes characterized by the presence of the parkinsonian syndrome with variable accentuation of the different symptoms, motor, cognitive and behavioural, and with very variable course. Classification of these diseases is rapidly evolving, mainly owing to molecular genetics and clinical-pathological correlation.

BACKGROUND: A higher prevalence of parkinsonism was recently identified in southeastern Moravia (Czech Republic). Further research confirmed 3 large pedigrees with familial autosomal-dominant parkinsonism spanning 5 generations. METHODS: This case report concerns a patient belonging to one of these 3 pedigrees, in whom motor and oculomotor symptoms were accompanied by frontal-type dementia, who finally developed a clinical phenotype of progressive supranuclear palsy. Molecular genetic examinations were performed due to the positive family history. RESULTS: No previously described causal mutation was found. After filtering against common variants (minor allele frequency (MAF) < 0.01), 2 noncoding and 1 synonymous rare mutation potentially associable with parkinsonism were identified: GIGYF2-GRB10 Interacting GYF Protein 2, PARK11 (c.*2030G > A, rs115669549); VPS35 gene-vacuolar protein sorting 35, PARK17 (c.102 + 33G > A, rs192115886); and FBXO7-F-box only protein 7 gene, PARK15 (c.540A > G, rs41311141). CONCLUSION: As to the changes in the FBXO7 and VPS35 genes (despite phylogenetic conservation in primates), probably neither the FBXO7 nor the VPS35 variants will be direct causal mutations. Both described variants, and possibly the influence of their combination, could increase the risk of the disease.
