Učebnice Psychiatrie a pedopsychiatrie přehledným způsobem zachycuje obor psychiatrie v celé jeho šíři. Na rozdíl od jiných učebnic podrobně zpracovává pedopsychiatrii, což je velmi důležité v kontextu osamostatnění této specializace v České republice.Publikace systematicky probírá jednotlivé duševní poruchy z hlediska jejich klinického obrazu, epidemiologie, etiopatogeneze, vyšetřovacích metod, průběhu a léčby. K zajímavým tématům, která nebývají do učebních textů zařazována, a přitom jsou pro klinickou praxi i přehled studentů nezbytná, patří například: • psychiatrická problematika somatických onemocnění a konziliární psychiatrie, • resocializace a komunitní péče, • organizace oboru s návrhem potřebné reformy, • urgentní stavy v psychiatrii. Významným prvkem učebního textu je kapitola o organizaci psychiatrické péče v České republice v současnosti a o připravovaných změnách. Léčba duševně nemocných by se měla z velkých psychiatrických nemocnic, často značně vzdálených bydlišti pacientů, stále více přesouvat do jejich přirozeného životního a společenského prostředí. Stejně tak autoři podrobně rozpracovali kapitolu o jednotlivých způsobech psychoterapie, která se někdy v klinické praxi obtížně prosazuje proti rychlé a levné farmakoterapii.
Investigation into the earliest signs of autism in infants has become a significant sub-field of autism research. This work invokes specific ethical concerns such as use of 'at-risk' language, communicating study findings to parents and the future perspective of enrolled infants when they reach adulthood. This study aimed to ground this research field in an understanding of the perspectives of members of the autism community. Following focus groups to identify topics, an online survey was distributed to autistic adults, parents of children with autism and practitioners in health and education settings across 11 European countries. Survey respondents (n = 2317) were positively disposed towards early autism research, and there was significant overlap in their priorities for the field and preferred language to describe infant research participants. However, there were also differences including overall less favourable endorsement of early autism research by autistic adults relative to other groups and a dislike of the phrase 'at-risk' to describe infant participants, in all groups except healthcare practitioners. The findings overall indicate that the autism community in Europe is supportive of early autism research. Researchers should endeavour to maintain this by continuing to take community perspectives into account.

BACKGROUND: The idea of latent social skills in autism emerged as a possible interpretation of the rapid (but temporary) improvement of autistic subjects in oxytocin studies. We tested a hypothesis that a normal response to Item No 59 "Secure Base" from the third version of the Autism Diagnostic Interview - Revised (ADIR-59) could indicate the presence of latent social skills in autism. METHODS: We examined 110 autistic children (88 boys and 22 girls) with a mean age of 6.0±2.5 years (range: 2.2-14.8 years) using the Autism Diagnostic Observation Schedule (ADOS) - Generic. A diagnosis of mental retardation was established in 68 autistic children (62%). RESULTS: The difference in the ADOS social domain between children aged ≤5 years on one side and children older than 5 years on the other side was significant in subjects with normal responses to ADIR-59 (9.60 vs 6.47; P=0.031) but not in those with abnormal responses to ADIR-59 (10.62 vs 9.63; P=0.537). In a predictive model, lower ADOS social domain scores were predicted by older age (P=0.001), lower scores on the ADIR-59 (P=0.01), and the absence of mental retardation (P=0.049). CONCLUSION: The results support the hypothesis that the normal response to item ADIR-59 "Secure Base" indicates the presence of latent social skills in autism that might foretell further social growth in older autistic subjects.

OBJECTIVE: The objective of this study was to explore the potential of the Infant/Toddler Sensory Profile (ITSP) as a screening tool for autism spectrum disorders (ASD) in prematurely born children. METHODS: Parents of 157 children with birth weights <1,500 g (aged 2 years, corrected for prematurity; 88 boys, 69 girls) completed a screening battery that included the ITSP, Modified Checklist for Autism in Toddlers (M-CHAT), and the Communication and Symbolic Behavior Scales Developmental Profile Infant-Toddler Checklist (CSBS-DP-ITC). Children with known disabilities were excluded. All the children who were screened positive on any of the screening tools subsequently underwent clinical examination including the Autism Diagnostic Observation Schedule. RESULTS: We used classification trees to answer the question whether ITSP (or some of its subscales) could be combined with the M-CHAT and/or the CSBS-DP-ITC or its subscales into an effective ASD screening tool. Using the CSBS-DP-ITC, overall score, and the Sensation Seeking subscale of the ITSP, we obtained a screening tool that was able to identify all of the ASD children in our sample (confirmed by cross-validation). The proposed screening tool is scored as follows: 1) if the overall CSBS-DP-ITC value is <45.5, then the screening is positive; 2) if the overall CSBS-DP-ITC value is ≥45.5 and the z-score of the Sensation Seeking subscale of ITSP is ≥1.54, then the screening is positive; 3) otherwise, the screening is negative. CONCLUSION: The use of CSBS-DP-ITC in combination with the Sensation Seeking subscale of the ITSP improved the accuracy of autism screening in preterm children.

Little is known about use of early interventions for autism spectrum disorder in Europe. Parents of children with autism spectrum disorder aged 7 years or younger (N = 1680) were recruited through parent organisations in 18 European countries and completed an online survey about the interventions their child received. There was considerable variation in use of interventions, and in some countries more than 20% of children received no intervention at all. The most frequently reported interventions were speech and language therapy (64%) and behavioural, developmental and relationship-based interventions (55%). In some parts of Europe, use of behavioural, developmental and relationship-based interventions was associated with higher parental educational level and time passed since diagnosis, rather than with child characteristics. These findings highlight the need to monitor use of intervention for children with autism spectrum disorder in Europe in order to contrast inequalities.

Cíl: Strmý nárůst prevalence poruch autistického spektra (PAS), zejména v posledním desetiletí, povzbudil celosvětově zájem o širší okolnosti péče o autistické děti. Cílem naší studie bylo zjistit, jak v České republice ovlivňuje rodinný socioekonomický status (SES) věk dítěte při stanovení diagnózy PAS, dále pak blíže zmapovat informační zdroje, které české rodiny využívají k získávání informací o autismu. Soubor a metodika: Soubor zahrnul 50 klinicky vyšetřených dětí (41 chlapců, 9 dívek) o průměrném věku 7,0 ? 2,8 roku (rozmezí 3,1-14,8 roku), jejichž rodiče vyplnili dotazník týkající se rodinné situace a informovanosti o problematice autismu. U 43 dětí (86 % souboru) byla skutečně potvrzena diagnóza PAS. Výsledky: Věk pacienta při stanovení diagnózy významně negativně koreloval se vzděláním matky (p = 0,011) i se vzděláním otce (p = 0,041), tedy vyšší vzdělání rodičů bylo asociováno s časnější diagnostikou dítěte. SES rodiny významně s věkem pacienta nekoreloval. Většina rodičů (72 %) přišla k diagnostickému pobytu s názorem, že dítě je autistické. Naprostá většina respondentů (94 %) uvedla, že před hospitalizací již získala nějaké informace o autismu. V získávání informací byly aktivnější buď matky (64 %), nebo byli aktivní oba rodiče stejnou měrou. V průzkumu se nevyskytly rodiny, kde aktivnější by byl otec. Průměrný počet zdrojů, z nichž respondenti čerpali své informace o autismu, byl 3,2 (? 1,8), rozmezí bylo 0-9. Počet zdrojů nekoreloval významně s věkem dítěte při určení diagnózy. Nejčastěji uváděným zdrojem informací byl internet (72 %), následován dětským psychiatrem (56 %). Výsledky výzkumu byly diskutovány v kontextu zahraničních zkušeností. Závěr: Jako významný faktor časnější diagnostiky bylo nalezeno vyšší vzdělání obou rodičů. Socioekonomický status rodiny ani počet informačních zdrojů o autismu neovlivnily věk dítěte v době stanovení diagnózy.
Objective: A marked increase in prevalence of autism spectrum disorders (ASD), particularly in the past decade, has stimulated worldwide interest in exploring broader circumstances of care of autistic children. The aims of our study were to found how family socioeconomic status (SES) influences the age at diagnosis of ASD, and to find out more about information sources that Czech families use for gaining information on autism. Methods: Our sample comprised 50 children (41 boys, 9 girls; mean age 7.0?2.8 years, range 3.1-14.8) who were clinically examined and whose parents completed a questionnaire focused on the family situation and family information about autism. Diagnosis of ASD was confirmed in 43 children (86% of the sample). Results: The age at the diagnosis correlated significantly negatively with the education of the mother (p=0.011) as well as of the father (p=0.041); thus, higher education of the parents was associated with an earlier diagnosis of the child. Family SES did not correlate with the age at the diagnosis. The majority of the parents (72%) came to the diagnostic hospitalization with a belief that their child was autistic. The vast majority of responders (94%) confirmed that they had found some information on autism already before the diagnostic stay at the hospital. Mothers were the more active family members in gaining information than fathers (64%). In remaining cases, both parents were equally active. We did not observe any case when father would have been the more active parent. The mean number of information sources on autism was 3.2 (? 1.8) with a range 0-9. The number of sources did not correlate with the age at the diagnosis. The most used information sources were the Internet (72%) and child psychiatrists (56%). The results were discussed in the context of international experience. Conclusion: Higher education of both parents was found to be a significant factor for the earlier diagnosis of the child. Socioeconomic status or number of information sources on autism did not influence the age at the diagnosis.

BACKGROUND: Studies of children with very low birth weight (VLBW, 1,000-1,500 g) and extremely low birth weight (ELBW, less than 1,000 g) indicate that this population seems to be at increased risk of autism spectrum disorder (ASD). METHODS: Parents of 101 VLBW and ELBW children (age 2 years, corrected for prematurity) agreed to participate in the study and signed informed consents; however, parents of only 75 children (44 boys, 31 girls) completed the screening questionnaires. The screening battery included the Modified Checklist for Autism in Toddlers (M-CHAT), Communication and Symbolic Behavior Scales Developmental Profile Infant-Toddler Checklist (CSBS-DP-ITC), and the Infant/Toddler Sensory Profile (ITSP). Children with disabilities were excluded. All children who screened positive on any of the screening tools were subsequently invited for a detailed assessment. RESULTS: Thirty-two children (42.7%) screened positive on at least one of the screening questionnaires. The screening tool with the most positive results was the CSBS-DP-ITC (26 positive screens), followed by the M-CHAT (19 positive screens) and the ITSP (11 positive screens). Of the 32 children who tested positive, 19 participated in the detailed follow-up assessment. A diagnosis of ASD was confirmed in eight of the 19 children. ASD prevalence, calculated from those 19 children and those with negative screening results (43 children), yielded a prevalence of 12.9% in the sample. The difference in frequency of positive screens between the tests was significant (P=0.011). In pair comparisons, ITSP was found to be significantly less positive than CSBS-DP-ITC (P=0.032). No significant differences were found between the M-CHAT and CSBS-DP-ITC or between the M-CHAT and ITSP. CONCLUSION: The results strongly support the hypothesis of an increased prevalence of autism in children with a birth weight less than 1,500 g.
