Cíl: Cílem této práce bylo analyzovat vztah mezi novými případy klinické formy klíšťové encefalitidy a různými meteorologickými a sezonními prediktory. Materiál a metodika: Modelování vychází z národních dat České republiky za období 2001–2016 v denním rozlišení, a to z hodnot průměrné teploty, průměrné relativní vlhkosti vzduchu a počtu případů klíšťové encefalitidy klasifikovaných podle data prvních příznaků. Používají se čtyři varianty negativně binomického modelu z třídy zobecněných aditivních modelů. Základní model dává výskyt klíšťové encefalitidy do souvislosti se zpožděnou průměrnou denní teplotou okolí a denní průměrnou relativní vlhkostí vzduchu a jejich interakcí se zpožděním odrážejícím inkubační dobu a další faktory. Hodnota zpoždění byla odhadnuta optimalizačním postupem založeným na Akaikeho informačním kritériu. Model také zahrnuje vliv sezony a vliv dne v týdnu. Pro zvýšení biologické věrohodnosti byl základní model rozšířen a byl použit distributed lag model, který zohledňuje možné časově proměnlivé účinky meteorologických proměnných a zahrnuje více zpoždění. Výsledky: Statisticky nejvýznamnějším efektem je sezonnost v rámci roku a pak interakce teploty a relativní vlhkosti vzduchu. Vztah obou meteorologických faktorů a jejich interakce se mění v průběhu sezony aktivity hladových klíšťat Ixodes ricinus. To také mění podmínky výskytu nových klinických případů klíšťové encefalitidy. Časově proměnlivý vliv meteorologických faktorů na výskyt klíšťové encefalitidy vykazuje netriviální změny v průběhu roku. V období před polovinou kalendářního roku (kolem 22. týdne) je efekt nižší, poté následuje zvýšení až do 35. týdne. Závěr: Byly vyvinuty flexibilní modely s kvantitativně charakterizovanými vlivy teploty, vlhkosti vzduchu a jejich interakce, se zpožděním efektu odhadnutým optimalizačním procesem. Výkonnost finálního modelu byla zkontrolována pomocí nezávislých dat, aby se ověřila možnost využití výsledků ke zlepšení predikce rizika nárůstu klinických případů klíšťové encefalitidy.
Objectives: The aim of this work was to analyze the relationship between new cases of clinical tick-borne encephalitis (TBE) and various meteorological and seasonal predictors. Material and Methods: The modelling is based on national data from the Czech Republic for the period 2001–2016 in daily resolution, namely on average temperatures, average relative air humidity and the number of TBE cases classified according to the date of the first symptoms. Four variants of a negative binomial model from the generalized additive model class are used. The basic model relates the occurrence of TBE to the lagged ambient daily average temperature and daily average relative air humidity and their interaction with the lag reflecting the incubation period and other factors. The lag value was estimated via the optimization procedure based on Akaike information criterion. The model also includes the effect of the season and the effect of the day of the week. To increase the biological plausibility, the basic model has been expanded to account for possible time-varying effects of meteorological variables and to incorporate multiple lags. Results: The most statistically significant effect is the within-year seasonality and then the interaction of the temperature and relative air humidity. The relationship of both meteorological factors and their interactions vary throughout the activities season of the hostquesting Ixodes ricinus. This also changes the conditions of occurrence of the new clinical cases of TBE. The time-varying effect of meteorological factors on the incidence of TBE shows non-trivial changes within a year. In the period before the middle of the calendar year (around the week 22) the effect decreases, then it is followed by an increase until the week 35. Conclusion: Flexible models were developed with quantitatively characterized effects of temperature, air humidity and their interaction, with the delay of the effect estimated through the optimization process. Performance of the model with multiple lags was checked using independent data to verify the possibility of using the results to improve the prediction of the risk of clinical cases of TBE uprise.

Tick-borne encephalitis (TBE) is a serious acute neuroinfection of humans caused by a tick-borne flavivirus. The disease is typically seasonal, linked to the host-seeking activity of Ixodes ricinus (predominantly nymphs), the principal European tick vector species. To address the need for accurate risk predictions of contracting TBE, data on 4,044 TBE cases reported in the Czech Republic during 2001-2006 were compared with questing activity of I. ricinus nymphs monitored weekly at a defined location for the same 6-year period. A time shift of 21 days between infected tick bite and recorded disease onset provided the optimal model for comparing the number of cases of TBE with numbers of questing nymphs. Mean annual distribution of TBE cases and tick counts showed a similar bimodal distribution. Significantly, the ratio of TBE cases to questing nymphs was highest in the summer-autumn period even though the number of questing nymphs peaked in the spring-summer period. However, this pattern changed during a period of extreme meteorological events of flooding and abnormally high temperatures, indicating that changes in climate affect the incidence of TBE. Previous studies failed to link human behavior with changes in incidence of TBE but showed extrinsic temperature impacts arbovirus replication. Hence, we hypothesize the apparent discrepancy between peak nymphal tick activity and greatest risk of contracting TBE is due to the effect of temperature on virus replication in the tick vector. Relative proportions of questing nymphs and the numbers of weeks in which they were found were greater in summer-autumn compared with spring-summer at near-ground temperatures >5°C and at standard day and weekly average temperatures of >15°C. Thus, during the summer-autumn period, the virus dose in infected tick bites is likely greater owing to increased virus replication at higher microclimatic temperatures, consequently increasing the relative risk of contracting TBE per summer-autumn tick bite. The data support the use of weather-based forecasts of tick attack risk (based on daytime ambient temperature) supplemented with weekly average temperature (as a proxy for virus replication) to provide much-needed real-time forecasts of TBE risk.

Cíl práce: Ve čtyřech topograficky odlehlých oblastech České republiky byl proveden tříletý výzkum (2011–2013) populační denzity klíštěte Ixodes ricinus a jeho infikovanosti jím přenášených hlavních patogenů. Bylo vybráno 13 lokalit se zvýšenou incidencí onemocnění klíšťovou encefalitidou v předchozí dekádě (2001–2010), indikující tím pravidelný kontakt obyvatelstva s I. ricinus, a tak i významná místa pro studium výskytu dalších patogenů. Do pracovního programu bylo zařazeno zjištění druhového spektra spirochét komplexu Borrelia burgdorferi sensu lato a podmínek jejich výskytu. Materiál a metodika: Ve vybraných lokalitách krajů Ústeckého, Olomouckého, Jihočeského a kraje Vysočina byly stanoveny fixní plochy (každá 600 m2) v optimálních místních habitatech I. ricinus, na nichž byl prováděn každoroční kontrolní sběr v jaroletní a podzimní sezoně aktivity klíšťat. Sebraná dospělá klíšťata I. ricinus (1 396 samců a 1 404 samic) byla individuálně vyšetřena na přítomnost genospecií komplexu B. burgdorferi s. l. Výsledky: Spirochéty B. burgdorferi s.l. byly detekovány v klíšťatech ve všech 13 zkoumaných lokalitách a v celém rozsahu nadmořské výšky 280–1030 m. Celková pozitivita dosáhla 11,4 % (samci 10,4 %, samice 12,4 %) v rozmezí hraničních hodnot 1,3 % (Ústí nad Labem v roce 2011) a 19,7 % (Jihočeský kraj, 2012). V různém poměru a kombinacích byly detekovány: Borrelia afzelii, B. garinii, B. burgdorferi s. s., B. bavariensis, B. bissettii, B. valaisiana, B. spielmanii a B. lusitaniae. Tříletá pozorování ukazují, že regionální rozdíly v infikovanosti I. ricinus jsou dány charakterem lokální biocenózy jednotlivých oblastí. Dynamika sezonních změn, podmíněná klimatickými faktory, určuje jejich meziroční rozdíly. Závěry: Tři zdravotně nejvýznamnější genospecie tvoří hlavní část detekovaných borélií. Převládá B. afzelii (115 detekcí), následována B. garinii (100) a B. burgdorferi s. s. (19). Ostatní genospecie byly detekovány ojediněle. Je třeba zdůraznit význam zjištěného výskytu B. bissettii, vzhledem k závažnosti patogenního působení prokázaného v současnosti a dosud jen sporadickým znalostem o rozšíření této borélie na území České republiky. O významu v humánním lékařství se diskutuje také u ostatních sporadicky se vyskytujících genospecií.
Study objective: Three years long research study (2011–2013) on population density of Ixodes ricinus and the infection rate of the pathogens that they transmit was conducted in four topographically distant areas in the Czech Republic. In the previous decade (2001–2010) thirteen loci with increased incidence of tick borne encephalitis cases were defined, suggesting the permanent interaction of human population with ticks and indicating the landmarks for study of the presence of other tick borne pathogens. The work program included the identification of existing spectrum of spirochetes from Borrelia burgdorferi sensu lato complex and the conditions of their occurrence and distribution. Material and methods: In the areas of the Ústí nad Labem Region, Olomouc Region, South Bohemian Region, and Highlands Region, 600 m2 plots were selected in the local optimal I. ricinus habitats where tick flagging was performed every year in the spring-summer and autumn seasons of the tick questing activity. Collected adult ticks (1369 males and 1404 females) were individually screened for B. burgdorferi s. l. spirochets. Results: Spirochetes from B. burgdorferi s.l. complex were detected in all 13 studies sites in all altitudes from 280 to 1030 meters a. s. l. The total rate of infection was determined as 11.4% (males 10.4%, females 12.4%) with range limits from 1.4% (Ústí nad Labem in 2011) to 19.7% (South Bohemian Region, 2012). Genospecies were detected in various proportions and in different combinations: Borrelia afzelii, B. garinii, B. burgdorferi s. s., B. bavariensis, B. bissettii, B. valaisiana, B. spielmanii and B. lusitaniae. The three-year observation justifies the assumption that the regional differences in infectivity of I. ricinus are based on the character of the local biocenosis of the respective region. The dynamics of its seasonal changes, conditioned by climatic factors, determines the annual differences. Conclusion: Three of the medically most important Borrelia species formed a core group among all detected genospecies. B. afzelii was a dominated one (115 detections), followed by B. garinii (100) and by B. burgdorferi s.s. (19). Other genospecies were detected sporadically. However, the detection of B. bissettii should be emphasized due to the recently proven pathogenic effects of this genospecies and yet little-known sporadic expansion in the Czech Republic. The medical importance and distribution of other sporadically occurred genospecies is also discussed.

Cíl práce: Cílem tříletého sledování (2011–2013) výskytu klíštěte obecného Ixodes ricinus bylo ověřit v oblastech se zvýšenou incidencí klíšťové encefalitidy (zjištěnou v předchozí dekádě 2001–2010) jednak stavy místních populací tohoto přenašeče, jednak jeho infikovanost virem klíšťové encefalitidy. Takto komplexně zaměřený a dlouhodobý výzkum založený na předchozích epidemiologických zjištěních nebyl dosud v Evropě realizován. Materiál a metodika:Ve vybraných lokalitách krajů Ústeckého, Olomouckého, Jihočeského a kraje Vysočina byly stanoveny fixní plochy (každá 600 m2) v optimálních místních habitatech I. ricinus, na nichž byl prováděn každoroční kontrolní sběr v jaroletní a podzimní sezoně aktivity klíšťat. Celkem bylo shromážděno a laboratorně vyšetřeno 18.721 klíšťat I. ricinus (z toho 1448 samic, 1425 samců a 15.848 nymf). Výsledky a závěry: Výsledky ukazují, že pozorované regionální rozdíly v infikovanosti I. ricinus jsou založeny na stavu místní populace těchto klíšťat ovlivněné charakterem celé lokální biocenózy. Dynamika jejích sezonních změn, podmíněná především klimatickými faktory, určuje meziroční rozdíly infikovanosti I. ricinus, a tak i změny epidemiologické situace nákaz klíšťaty přenášených. Číselný poměr aktivních dospělců a nymf klíšťat vykazuje v různých oblastech značnou variabilitu a mohl by být v budoucnu použit jako jeden z indikátorů charakterizujících místní populace I. ricinus, úroveň zoonotické cirkulace viru klíšťové encefalitidy, a tak i zdravotní riziko dané oblasti. Dále bylo zjištěno, že populace klíšťat dotčená lokálně jednorázově působícími extrémními meteorologickými jevy, se rychle meziročně obnovuje. Ačkoliv je stále pozorováno pronikání I. ricinus a jimi přenášených patogenů do vyšších nadmořských poloh, vysoké riziko lidských nákaz virem klíšťové encefalitidy je ve sledovaných regionech dosud do úrovně 600 m n. m.
Study objective: The aim of the three-year study (2011–2013) was to monitor population density of Ixodes ricinus ticks and its infection rate with the tick-borne encephalitis virus in areas with a high incidence of tick-borne encephalitis as reported in the previous decade 2001–2010. Such a comprehensive and long-term study based on existing epidemiological findings has not previously been conducted in Europe. Material and methods: In the areas of the Ústí nad Labem Region, Olomouc Region, South Bohemian Region, and Highlands Region, 600 m2 plots were selected in the local optimal I. ricinus habitats where tick flagging was performed every year in the spring-summer and autumn seasons of the questing activity. In total, 18,721 I. ricinus ticks (1448 females, 1425 males, and 15,848 nymphs) were collected and investigated. Results and conclusion: The results have shown that the differences in the infection rate of I. ricinus observed between regions are driven by variation in the density of the local I. ricinus populations which is influenced by the characteristics of the whole local biocenosis. The overall prevalence estimate of TBE virus in Ixodes ricinus ticks at the altitudes below 600 m a.s.l. was 0.096 % (95% CI 0.055–0.156) for nymphs, and 0.477 % (95% CI 0.272–0.773) for adults. The dynamics of the seasonal variation in I. ricinus populations, depending primarily on the climatic factors, are behind the interyear differences in the infection rate of ticks and, consequently, in the epidemiological situation of tick-borne encephalitis. The nymph to adult ratio was 5.5 on average but showed great interregional variability (from 10.3 in the Ústí nad Labem Region to 1.8 in the Highlands Region). It might be used in the future as one of the indicators of the composition of the local I. ricinus population and of the level of the circulation of tick-borne pathogens in zoonotic sphere and also for use in the health risk assessment in a given area. Despite the permanent expansion of ticks and tick-borne pathogens in higher altitudes the high risk limit for human infection with tick-borne encephalitis is 600 m a.s.l. in the Czech Republic.

BACKGROUND: Abiotic conditions provide cues that drive tick questing activity. Defining these cues is critical in predicting biting risk, and in forecasting climate change impacts on tick populations. This is particularly important for Ixodes ricinus nymphs, the vector of numerous pathogens affecting humans. METHODS: A 6-year study of the questing activity of I. ricinus was conducted in Central Bohemia, Czech Republic, from 2001 to 2006. Tick numbers were determined by weekly flagging the vegetation in a defined 600 m(2) field site. After capture, ticks were released back to where they were found. Concurrent temperature data and relative humidity were collected in the microhabitat and at a nearby meteorological station. Data were analysed by regression methods. RESULTS: During 208 monitoring visits, a total of 21,623 ticks were recorded. Larvae, nymphs, and adults showed typical bimodal questing activity curves with major spring peaks and minor late summer or autumn peaks (mid-summer for males). Questing activity of nymphs and adults began with ~12 h of daylight and ceased at ~9 h daylight, at limiting temperatures close to freezing (in early spring and late autumn); questing occurred during ~70 % calendar year without cessation in summer. The co-occurrence of larvae and nymphs varied annually, ranging from 31 to 80 % of monitoring visits, and depended on the questing activity of larvae. Near-ground temperature, day length, and relative air humidity were all significant predictors of nymphal activity. For 70 % of records, near-ground temperatures measured in the microhabitat were 4-5 °C lower than those recorded by the nearby meteorological observatory, although they were strongly dependent. Inter-annual differences in seasonal numbers of nymphs reflected extreme weather events. CONCLUSIONS: Weather predictions (particularly for temperature) combined with daylight length, are good predictors of the initiation and cessation of I. ricinus nymph questing activity, and hence of the risk period to humans, in Central Europe. Co-occurrence data for larvae and nymphs support the notion of intrastadial rather than interstadial co-feeding pathogen transmission. Annual questing tick numbers recover quickly from the impact of extreme weather events.

Batai virus (BATV) is a poorly studied arthropod-borne virus belonging to the genus Orthobunyavirus (Bunyamwera serogroup) within the family Bunyaviridae. It has been associated with human influenza-like febrile illness in several Asian, African, and European countries. Čalovo virus (CVOV), isolated in 1960 in Slovakia, has been classified as BATV based on high antigenic similarity, and since then both CVOV and BATV were used as synonyms. In order to fully clarify the phylogenetic relationships between CVOV, BATV, and other members of the Bunyamwera serogroup, we performed whole genome sequencing of four CVOV strains isolated in Europe and phylogenetic analyses of all related viruses. The nucleocapsid protein, encoded by the S genomic segment, contains 233 amino acids, 60 of which, putatively critical for protein function, are conserved. Within the CVOV polyprotein encoded by the M genomic segment, putative cleavage sites, N-glycosylation sites, and seven transmembrane regions were identified. The RNA-dependent RNA polymerase, encoded by the L genome segment, exhibits conservation of the three regions known to be conserved among bunyavirus and arenavirus L proteins. Phylogenetic analyses of all three genomic segments of selected orthobunyaviruses clearly revealed that European and Asian/African strains of BATV are phylogenetically different and form two distinct lineages, indicating the existence of two different genotypes of BATV, tentatively named European genotype (with CVOV as a type strain) and Afro-Asian genotype (with BATV as a type strain) of BATV.

Virus klíšťové encefalitidy (KE) byl ve střední Evropě izolován poprvé v roce 1948 na území České republiky z pacienta i z klíšťat Ixodes ricinus sebraných v místech, kde jimi byl pacient napaden (Berounsko – střední Čechy) a současně nezávisle také z pacienta na Moravě (Vyškovsko). Dalším prioritním objevem byl alimentární přenos viru KE mlékem pasených koz napadených infikovanými klíšťaty, učiněný při epidemii KE ve městě Rožňava (JV Slovensko). Tato epidemie (660 nemocných) je dosud největší svého druhu. Oba objevy podnítily rozsáhlý výzkum, který soustředil odborníky všech oborů nutných pro komplexní analýzu přírodní ohniskovosti KE. Výsledky pětileté práce byly publikovány českými a slovenskými autory v první evropské monografii KE (1954), a vytvořily tak základ znalostí pro další výzkum v tomto prostoru. Z hlediska epidemiologie, byl mimo jiné již tehdy jasně definován vliv meteorologických faktorů (ovlivňujících incidenci KE prostřednictvím aktivity I. ricinus) a rekreační charakter této nákazy. Současně se KE stala povinně hlášenou nákazou (od roku 1971 pouze laboratorně potvrzené případy). V dalších desetiletích byl intenzivně studován fenomén přírodní ohniskovosti KE (včetně působení antropických vlivů) a byly analyzovány faktory vhodné pro vymezení rizikových oblastí v terénu. Výsledky byly využity při tvorbě predikčních map zvýšeného rizika výskytu I. ricinus a onemocnění virem KE v ČR realizované poprvé v Evropě na základě družicových dat LANDSAT 5 a s využitím technologií GIS. Začátkem 90. let minulého století (zejména od roku 1993) došlo v ČR (shodně s dalšími zeměmi rozšíření KE) k prudkému vzestupu incidence KE, jejíž vysoká úroveň s určitými výkyvy se udržuje dodnes. Spolupráce s klimatology při analýzách historických dat, aktuálních epidemiologických pozorováních i při výzkumu I. ricinus v terénu, ukázala rozhodující vliv probíhajících modifikací klimatu. Rozbor socio-ekonomických podmínek v rizikových oblastech neprokázal vliv na nemocnost KE. Výrazný je vliv rekreačního faktoru, který je ovlivňován průběhem meteorologických změn. Sezonnost incidence KE podléhá velkým výkyvům (demonstrováno na příkladu 2006, 2009 a 2010) rovněž pod vlivem povětrnostních podmínek jednotlivých sezon. Z toho plyne nutný požadavek používat pro závěry obecného rázu dostatečně dlouhé časové řady vstupních dat, minimálně desetiletého období, jak tomu je v předložené práci (2001–2010). Data prezentovaná podle krajů dokládají podstatné regionální rozdíly. Ze 14 krajů (na něž je ČR administrativně rozčleněna) mají 3 regiony lineární trend incidence KE s minimální odchylkou od výchozích hodnot. Čtyři kraje vykazují v různé míře sestupný lineární trend, avšak 7 regionů demonstruje v různé míře vzestupný trend. Nejzřetelnější je tato situace v kraji Vysočina, kde souvisí s orografickými podmínkami převažujícími v této oblasti a vzestupem výskytu I. ricinus ve vyšších polohách. Místo, kde byl pacient napaden I. ricinus, registrované v databázi Epi-Dat jako „místo pravděpodobné nákazy“ pomáhá při stanovení rizikových oblastí. Porovnání s místem bydliště pacienta naznačuje pohyb obyvatelstva do těchto oblastí, a tak i jejich význam v celostátním měřítku. Nejzřetelnější je to u obyvatel regionu Praha (NUTS3 CZ010), kde v rámci území vlastního regionu bylo infikováno jen 37,7 % pacientů, zatímco při návštěvě Středočeského kraje 33,4 % a Jihočeského kraje 13,9 % (v dalších krajích jen jednotlivá procenta nebo jejich zlomky). Na rozdíl od toho rezidenti Jihočeského kraje (CZ031, který je na prvním místě v početnosti případů KE v ČR) byli infikováni v 99,5 % na vlastním teritoriu. Je tu zřejmá souvislost s rekreačním potenciálem jednotlivých krajů. Místo pravděpodobné nákazy je registrováno s určením přesnosti katastru obce. Přesnost této informace se ztrácí podle typu použitého kartogramu a základní plošné jednotce, k níž je incidence vztažena. Vzhledem k přírodní ohniskovosti KE je ke stanovení rizikových oblastí a úrovně místního rizika nutná analýza lokálních environmentálních faktorů umožňujících koloběh viru KE ve volné přírodě. Výskyt lidských případů existenci přírodního ohniska KE indikuje, ale sám o sobě není dostačující. Stejně tak delší období bez výskytu lidských případů nemusí vymezovat bezrizikové území.
Tick-borne encephalitis (TBE) virus was isolated for the first time in Central Europe in 1948 from both a patient and Ixodes ricinus ticks collected in the area where the patient had been tick bitten (the Beroun area – Central Bohemia) and concomitantly from a TBE patient in Moravia (the Vyškov area). Another priority discovery was alimentary transmission of TBE virus via the milk from tick infected grazing goats that was made during a TBE outbreak in Rožňava (SE Slovakia). This outbreak of 660 cases has been the largest of its kind. Both of these discoveries were a challenge to multidisciplinary research into the natural focality of TBE. The results obtained were published by Czech and Slovak authors in the first European TBE monograph (1954) and were the stimulus for further research in this area. From the epidemiological point of view, among others, the impact of meteorological factors (on TBE incidence associated with I. ricinus host-seeking activity) and recreational nature of TBE were clearly defined then. At the same time, TBE became a notifiable disease (since 1971 laboratory confirmed TBE cases only). In the following decades, the phenomenon of natural focality of TBE (including anthropic impacts) was extensively studied and the determinants of high-risk areas in the field were analyzed. The results were used in the creation of I. ricinus and TBE risk prediction maps for the Czech Republic generated for the first time in Europe using LANDSAT 5 satellite data and GIS technology (1990). In the early 1990s (in particular since 1993), similarly to other countries, the Czech Republic reported a sharp rise in TBE cases that continues, with some fluctuations, until now. The cooperation with climatologists in the analysis of historical data, current epidemiological observations, and study of I. ricinus in the field have shown a decisive impact of the ongoing climate change. The analysis of the socio-economic conditions in high-risk areas for TBE has not revealed any impact of these conditions on TBE morbidity. The recreational factor that isinfluenced by the weather changes has a considerable impact. The seasonal trend of TBE cases shows large fluctuations as were seen in 2006, 2009, and 2010, also as a result of weather changes with seasons. This clearly implies the need for using long time series of data, covering at least a decade, to be able to draw general conclusions as is the case in the present study (2001–2010). The data broken down by Administrative Region display substantial interregional differences. Of 14 Administrative Regions of the Czech Republic, three exhibits a linear trend in TBE incidence, with a minimum deviation from the baseline, four Regions show different downward linear trends, but seven Regions display different upward trends. The upward trend is most obvious in the Highlands (Vysočina) Region where it is associated with the prevalent orographic conditions and increase in the incidence of I. ricinus ticks at higher altitudes. The knowledge of the area where the patient was tick bitten that is entered in the Epidat database as the „probable area of TBE infection acquisition“ is helpful in identifying high-risk areas for TBE. By matching the area of TBE acquisition with that of the patient’s domicile we revealed that TBE patients had to travel to areas other than their area of domicile to acquire TBE and thus also significance of the areas of TBE acquisition at the country level. The population of the Prague Region (NUTS3 CZ010) can be used as an illustrative example, with 37.7% of TBE cases only reported to be acquired in the Prague Region while 33.4% of TBE cases were associated with travel to the Central Bohemian Region and 13.9% of TBE cases were imported from the South Bohemian Region (the rates of TBE cases imported from other Regions were less than 5%). And conversely, the residents of the South Bohemian Region (CZ031), with the highest number of TBE cases ever in the Czech Republic, acquired TBE in the region of domicile at a rate of 99.5 %. These rates are clearly associated with the recreational potential of various Regions. The probable area of TBE acquisition is identified by cadastral community. In the light of the natural focality of TBE, the analysis of the local environmental factors involved in the circulation of TBE virus in the wild environment is required to determine the high-risk areas and local risk level. Although outbreaks of TBE cases in humans are indicative of TBE natural foci, more data is needed to delineate such areas. And similarly, the absence of TBE cases in humans over a period of time may not be indicative of a no risk area.

The altitudinal shift in the limit of Ixodes ricinus occurrence above the previously established altitude of 750 m above sea level has been monitored over the long-term (2002-2008) in the Krkonose Mts. (Giant Mts.), the highest in the Czech Republic, along two vertical transects in their eastern and central parts (600-1020 and 600-1270 m). Ticks were collected by flagging three times annually, and examined individually by PCR or RT-PCR for the presence of Borrelia burgdorferi sensu lato or tick-borne encephalitis virus (TBEV). A total of 5999 I. ricinus ticks were tested. TBEV RNA was detected in 26 ticks at up to 1140 m. Demonstration of TBEV in two larvae of I. ricinus indicates transovarial transmission. Similar infection rates in larvae and nymphs show vertical transmission in TBEV circulation to be very important under these mountain conditions. B. burgdorferi sensu stricto was found at up to 1040-1065 m, Borrelia garinii and Borrelia afzelii up to 1080-1140 m, and Borrelia valaisiana up to 1270 m. The total infection rates of nymphs and larvae were 7.3% and 2%, respectively. B. garinii was the most prevalent (37%), followed by B. afzelii (29%), B. burgdorferi s.s. (11%), and B. valaisiana (9%). Double to quadruple coinfections were detected in 32% of the infected ticks, most frequently B. garinii/B. afzelii. Predominance of B. garinii and B. valaisiana over B. afzelii suggests that small passerine birds moving on the ground are responsible for permanent local populations of I. ricinus in mountain localities with low numbers of small terrestrial mammals. The detection of B. burgdorferi sensu lato and TBEV in host-seeking larvae indicates an autochthonic infection. Upon analysis of the local climate we consider climate warming to be responsible for the spreading of ticks and tick-transmitted pathogens to higher altitudes.

Výskyt onemocnění klíšťovou encefalitidou v první polovině roku byl výrazně nižší než v roce předešlém. Vše nasvědčuje tomu že v podzimních měsících došlo k druhé vlně případů onemocnění, tak jak tomu bylo například v roce 2006. Geografická distribuce případů podle okresu pravděpodobné infekce se v posledních letech zásadním způsobem neliší.
The incidence rate of tick-borne encephalitis (TBE) was clearly lower in the first half of 2010 than in the previous year. The available data suggests a second increase in TBE cases in autumn 2010, e.g. similarly to 2006. The geographical distribution of cases by district of probable infection has not varied substantially over the last years.
