Úvod: Panická porucha je v populaci často zastoupené psychiatrické onemocnění charakterizované náhlými a nečekanými záchvaty panické úzkosti, doprovázené pocitem masivního strachu. Tyto záchvaty bývají asociovány s celou řadou tělesných symptomů. Vzhledem k velkému množství příznaků lze očekávat, že do průběhu panické ataky bude zapojena řada mozkových struktur. EEG (elektroencefalografie) je jednoduchá laboratorní metoda, která může být s výhodou využita k měření mozkové aktivity. V současné chvíli je EEG v psychiatrii užíváno spíše k hodnocení demencí nebo v rámci diferenciální diagnostiky záchvatových onemocnění. Díky pokrokům ve zpracování signálu je však dnes možno využít EEG nejen k hodnocení kortikální aktivity, ale lze získat informace i o aktivitách hlubších mozkových struktur. Cílem článku je nabídnout přehled současných znalostí o EEG nálezech u pacientů s panickou poruchou. Výsledky: Navzdory řadě studií doposud žádná práce neidentifikovala specifický EEG nález pro panickou poruchu. Podařilo se však odhalit asymetrii frontální alfa-aktivity, změny koherence a pomocí LORETA i změny v absolutním výkonu frekvenčních pásem beta 1 a beta 3 v oblasti laterálního prefrontálního kortexu. Navzdory tomu, že stížnosti na kvalitu spánku a noční panické ataky jsou u pacientů s panickou poruchou časté, ani polysomnografické nálezy nejsou jednoznačné a neprokazují žádný specifický nález. Závěr: Ačkoliv existuje větší množství studií věnujících se problematice EEG změn u pacientů s panickou poruchou, zatím nebyl nalezen žádný specifický nález. Nicméně EEG se ukázalo být dobrým nástrojem pro zkoumání neurobiologické podstaty tohoto onemocnění.
Introduction: Panic disorder is frequent psychiatric disorder characterized by sudden and unexpected onset of a panic attack, characterized by terror or impending doom, and associated with many somatic symptoms. Variety of symptoms suggests, that there are many brain structures involved in panic reaction. EEG is simple laborathory method, which can be used for measuring of brain activity. Nowdays is EEG in psychiatry rather used for evaluation of dementia or in differential diagnosis of seizures. Due to development of singal processing can be EEG used both to evaluate cortical acitivity and also gives information about activity of deep brain structures. Aim of this article is to review recent informations about EEG findings in patients with panic disorder. Main findings: Up to date, there is no study identifying specific EEG finding in patients with panic disorder. Some studies detect frontal alpha asymmetry, changes in coherence and using LORETA also changes in absolute power of beta 1 and beta 2 frequency bands in lateral prefrontal cortex. Despite complains about dyssomnia and nocturnal panic attacks are frequent in patients with panic disorder, there was no specific changes found using polysomnography. Conclusions: Although there are many studies monitoring changes in EEG of patients with panic disorder, there are no specific findings for panic. Thus EEG proved to be the usefull method that can be used to study neurobiology of panic disorder.

OBJECTIVE: Previous functional brain imaging studies have described various and contradictory activation findings in patients with panic disorder (PD). Our study focused on patients with a chronic PD, who were investigated and treated in a conventional manner, which represents the real PD patients in clinical practice. METHODS: Continuing their medication, patients were included in a six-week cognitive-behavioral therapy (CBT) program in the psychiatry department. At the onset of the study, participants underwent clinical evaluation using standard scales and were examined using fMRI while listening to verbal threat-related stimuli contrasted to neutral words. According to the therapeutic outcome, they were subsequently divided into two groups, responders, and nonresponders and the two groups were mutually compared. RESULTS: In non-responders compared to responders, we found increased pre-treatment activation in dorsolateral prefrontal cortex bilaterally, left orbitofrontal cortex, left frontal eye field, right parietal lobule and left amygdala. In addition, both groups showed negative fMRI BOLD correlation with BAI improvement and positive correlation with CGI improvement across the ROIs. We suggest that DLPFC over-activation may reveal a lack of cognitive control over emotional processing, which makes subsequent CBT less effective. CONCLUSION: Despite several limitations, we found neuroimaging predictors of poor CBT response, under the conditions of standard clinical practice, in real PD patients.

Východiska a cíle: Záchvaty paniky jsou charakteristické pro pacienta znepokojivými somatickými symptomy, zejména kardiovaskulárními pøíznaky. U pacientů s panickou poruchou byly zaznamenány zvýšená srdeèní morbidita i mortalita. Velmi úspěšný nástroj k detekci vegetativních nestability u rùzných klinických stavů je spektrální analýza RR intervalu na elektrokardiogramu. Bylo zjištěno, že variabilita srdeční frekvence (HRV) je výsledkem rychlých kontrolních kardio'askulárních systémů. Cílem naší studie byío měření velmi nízké frekvenčního pásma (VLF), nízkofrekvenčního pásma (LF) a vysokých frekvencí (HF) složky intervalu RR při ortostatické experimentu u pacientů s panickou poruchu před léčbou a po léčbě a jejich porovnání se zdravými kontrolami. Metoda: Hodnotili jsme variabilitu srdeční frekvence u 19 pacientů s panickou poruchou před a po 6týdenní léčbě antidepresivy v kombinaci S kognitivně-behaviorální terapií (KBT) a srovnávali s 18 zdravými kontrolami. Diagnóza byla provedena v souladu s ICD-10 výzkumnými diagnostickými kritérii potvrzena pomocí dotazníku M.I.N.I (Mezinárodní . [europsychiatrické Interview). Pacienti byli léčeni kombinací KBT a psychofarmak. Pravidelně každý týden byli hodnoceni pomocí CGI (Clinical bal Impression), BAI (Beck Anxiety Inventory) a BDI (Beck Depression Inventory). Variabilita srdeční frekvence byla hodnocena ve 3 polohách (1. poloha - 5 minut vleže na zádech, 2. poloha - 5 minut stojící, 3. poloha - 5 minut vleže) před zahájením a na konci terapie. Pomocí rychlé Fourierovy transformace byly vyhodnoceny, velmi-nízce-frekvenční pásma-VLF (0,0033-0,04 Hz), nízce-frekvenční pásma- LF (0,04-0,15 Hz) L vysoce-frekvenční pásma - HF (0,15-0,40 Hz). Výsledky: Otevřené ôtýdenní studie se účastnilo 19 farmakorezisLtních pacientů s panickou poruchou, kteří byU léčeni kombinací SSRI a KBT. Léčba se ukázala jako účinná léčba u všech 19 pacientů. Pacienti se výrazně zlepšili v průběhu sledovaného období podle všech hodnořících měřítek. Ukázaly se vysoce statisticky významné rozdíly mezi zái chvaty u pacientů a kontrolní skupinou ve všech komponentách výkonové spektrálni analýzy (VLF, LF a HF) v pozici ve stoje a ve dvou komporozdíl mezi dvěma sledovanými skupinami v poměru: LF/HF v pozici ve stoje. Během léčby byla pozorována tendence zvýšení hodnot energie ve ní poloha na zádech byla jedinou, kdy nárůst dosáhl úrovně os statiscké významnosti. Závěr: Nálezy prokázaly nízkou autonomní činnost při změnách 30sturálních pozic u pacientů s panickou poruchou ve srovnání se zdravými kontrolami a tendencí ke zvýšení energie autonomní činnos¬ ti v průběhu léčby.
Objectives and aims: Alarming somatic symptoms and in particular the cardiovascular symptoms, are the characteristic features of panic attacks. Increased cardiac mortality and morbidity have been proposed in these patients. Power spectral analysis of electrocardiogram R-R intervals is known to be a particularly successful tool in the de- tection of autonomic instabilities in various cli nical disorders. Heart rate variability (HRV) has been found to be the outcome of rapidly re- acting cardiovascular control systems. The aim of our study is to mea- sure very low frequency band (VLF), low frequency band (LF) and high frequency band (HF) components of R-R interval during orthostatic ex- periment in patients with panic disorder efore and after treatment and compares it with healthy controls. Methods: We assessed heart rate variability in 19 patients with panic disorder before and after 6-weeks treatment with antidepressants combi- ned with cognitive behavioral therapy (CBT) and in 18 healthy controls. Diagnosis was done according to the ICD-10 research diagnostic criteria confirmed with MINI (MINI international neuropsychiatric interview). Patients were treated with CBT and psychotropics. They were regularly every week assessed using CGI (Clinical Global Impression), BAI (Beck Anxiety Inventory) and BDI (Beck Depression Inventory). Heart rate va- riability was assessed during 3 positions (1st – 5 min supine; 2nd – 5 min standing; 3rd – 5 min supine) before and after the treatment. Power spectra were computed for very low frequency – VLF (0.0033–0.04 Hz), low-frequency – LF (0.04–0.15 Hz) and high frequency – HF (0.15–0.40 Hz) bands using fast Fourier transformation. Results: Nineteen panic disorder patients resistant to pharmacologi- cal treatment entered a 6-week open-label treatment study with combi- nation of SSRI and CBT. The combination of CBT and pharmacotherapy proved to be an effective treatment in these patients. The patients signi- ficantly improved during the study period in all rating scales. There were highly statistical significant differences between panic patients and con- trol group in all components of power spectral analysis in 2nd (VLF, LF and H in standing) and in two component of 3rd (LF and HF in supine) positions. There was also a statistically significant difference between the- se two groups in LF/ HF ratio in standing position (2nd). During thera- py there was a tendency increasing values in all three positions in compo- nents of HRV power spectra, but HF in 1st supine position was the only component where the increase reached the level os statistical significance. Conclusions: These findings demonstrate a Loir autonomic activi- ty in panic disorder patients measured during the changes of postural position in comparison with healthy controls and tendency to increase this autonomic power during the treatment.

Úvod: Cílem naší práce bylo pokusit identifikovat faktory spojené s odpovědí na léčbu pomocí psychofarmak a kognitivně-be"Kaviorální terapie u farmakorezistentních pacientů se sociální fóbií. Metodika: Do práce byla zařazena data získaná od 50 pacientů s dg. sociální fobie hospitalizovaných na psychoterapeutickém oddělení Kliniky psychiatrie Fakultní nemocnice Olomouc. Zkoumali jsme základní delografické údaje (věk, délka trvání nemoci a věk počátku nemoci), index užívaného antidepresiva a skóre hodnotících škál (LSAS, CGI, BAI, BDI). Výsledky: U pacientů došlo v průběhu terapie k statisticky významnému poklesu ve všech posuzovacích škálách. Na konci terapie dosáhlo celkem 22 pacientů (tj. 44 %) hodnocení v CGI 1 nebo 2 (kritérium retise). Dále se podařilo prokázat, že změna v LSAS statisticky významaě negativně koreluje s BAI na počátku a pozitivně koreluje s věkem počátku poruchy. Při rozdělení na respondéry a nonrespondéry se pak podařilo prokázat statisticky významný rozdíl v počáteční hodnotě BAL Závěr: Léčba pomocí kognitivně behaviorální terapie byla v naší sku3ině spojena s poklesem ve všech posuzovacích škálách. Mezi skupinani respondérů a nonresponděrů byl však prokázán statisticky významý rozdíl pouze v hodnotě BAI na počátku terapie.
Introduction: Aim of our study was to identify predictors of therapeutic response in pharmacoresistant patients with social phobia treated by cognitive behavioral therapy therapy. Methods: We measured 50 patients with social phobia treated in psychotherapeutic department of department of Psychiatry University Hospital Olomouc. We examined basic demographic information (age, duration of illness and age of onset of illness), the index used antidepres- sants, and scores of rating scales (LSAS, CGI, BAI, BDI). Results: There was a statistically significant decrease in all ratings scales. At the end of therapy a total of 22 patients (44 %) acheve the CGI score 1 or 2 (criterion of response). We also demonstrated that a change in LSAS significantly negatively correlated with BAI at the beginning and positively correlated with age of onset. We also found statistically significant difference in the BAI score on the beginning of the therapy between responders and non-responders. Conclusion: In our group was treatment with cognitive-behavioral therapy associated with decreases in all assessment scales. Between groups of responders and non-responders was statistically significant difference only in the value of BAI at the beginning of therapy.

BACKGROUND: Standardized low-resolution electromagnetic tomography (sLORETA) is a new quantitative EEG method for determining distribution of neuronal electrical activity in the form of three-dimensional images of current density of the cerebral cortex. Unlike standard quantitative EEG, it allows noninvasive and detailed localization of neuronal generators responsible for surface EEG with zero localization error. The study aimed at finding electrotomographic differences between patients with panic disorder who respond well to cognitive behavioral therapy (CBT) and those with an inadequate response and to determine factors predicting a response to treatment. METHODS: The study comprised 24 patients diagnosed with panic disorder with or without agoraphobia (ICD-10 F41.0). The severity of symptoms was measured with the Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI), Sheehan Anxiety Scale, subjective and objective Clinical Global Impression (CGI) and Dissociative Experiences Scale (DES). Additionally, quality of life was evaluated using the Q-LES-Q questionnaire. Based on final BAI score decreases by 25%, the patients were classified into two groups - responders and non-responders. 21-channel EEGs were recorded at baseline and after completion of therapy. Power spectra and intracortical tomography were computed by sLORETA in seven frequency bands and compared between (responders vs. non-responders) and within (pre- vs. post-treatment) groups. RESULTS: There were no differences between responders and non-responders with respect to age, gender and baseline disorder symptomatology. Statistical analysis of sLORETA values demonstrated no significant inter-group differences in the pretreatment current density distribution. After treatment, only responders showed a significant decrease of alpha-2 sources (p<0.05) in the occipital lobes and cuneus and a statistical trend for increased beta-3 sources (p<0.10) in the posterior cingulate. In non-responders, there were no statistically significant changes in sLORETA findings following therapy. CONCLUSIONS: The study failed to use pretreatment sLORETA in the prediction of therapeutic response in patients with panic disorder. However, we clearly demonstrated that only treatment response was associated with significant changes of electric neuronal activity. An analysis of demographic data suggested that duration of the disease, age, level of dissociation and employment may be considered as factors influencing the response.

Cílem naší studie bylo předpovědět efekt přídatné kognitivně-behaviorální psychoterapie u pacientů s panickou poruchou. Pacienti byli léčeni pomocí existující farmakoterapie a přidané KBT. Před léčbou a po terapii byli vyšetřováni pomocí fMRI v průběhu expozice slovům navozujícím úzkost a neutrálním slovům, dále tvářím vyjadřujícím různé emoce a neutrálním tvářím. Pacienti byli léčeni pomocí KBT a již zavedené farmakoterapie. Na základě poklesu ve stupnici úzkosti BAI o 25 % byli rozděleni na responděry a nonresponděry. Efekt KBT u responděrů se projevil poklesem reaktivity amygdaly, silněji pravostranné. Negativním prediktorem terapeutické odpovědi ve fMRI je zvýšená aktivita v oblasti dorzolaterálního prefrontálního kortexu (DLPFC) - Brocova oblast BA 45 a 46. Dostatečnou terapeutickou odpověď predikuje vyšší reaktivita pravé amygdaly na úzkostné tváře, dále aktivace pravého angulárního gyru, pravé inzuly a pravého temporálního pólu na všechny emoční tváře. Pres omezení naší studie, může být fMRI slibným nástrojem pro predikci terapeutické odpovědi na adjuvantní KBT u pacientů s panickou poruchou léčených zvyklým způsobem.
The aim of our study was to predict CBT treatment effect in patients suffering from panic disorder. Patients were studied in fMRI during the exposition of threat-related words and with neutral words and emotional faces (anxiety, surprise and neutral) compared with fixation points. Inpatients were treated with CBT (5 hours per week group CBT, 1 hour per weak individual CBT ) and existing pharmacotherapy (SSRI antidepressants) for 5 weeks. Patients were studied before and after CBT treatment. Based on the 25 percent decrease of BAI (before and after treatment), patients were divided into responders and non-responders. CBT effect was observed on decrease of amygdala activity, stronger in right amygdala in responders. Negative treatment predictor is hyperactivity in left dorsolateral prefrontal cortex (DLPFC) – Broca Area (BA) 46 and 45. Sufficient therapeutic response predicts higher reacti- vity of right amygdala for anxious faces, activation of right angular gyrus, right insula and right temporal pole for all emotional faces. Despi- te the limitation of our study, may be fMRI promising tool for predicting therapeutic response to adjunctive CBT in patients with panic disor- der treated as usual.
