Nevelký nebo dočasný hmotnostní úbytek, kterého se dosahovalo s většinou antiobezitik, která byla v minulosti k dispozici, nesplnil očekávání jak pacientů, tak lékařů. Kombinované léky a analoga gastrointestinálních hormonů představují nový, účinnější a bezpečnější přístup k léčbě obezity a s ní spojených zdravotních rizik. Analoga intestinálních hormonů, jako je např. analog glucagon like peptidu-1 (GLP-1) liraglutid, byla zprvu úspěšně použita v léčbě diabetu 2. typu a až později v léčbě obezity. Článek informuje o klinických studiích 3. fáze s novými analogy intestinálních hormonů, o agonistovi GLP-1 semaglutidu a o duálním agonistovi intestinálních hormonů tirzepatidu; jeho jedna molekula cílí na receptory dvou střevních hormonů GLP-1 a GIP (glucose-dependent insulinotropic polypeptide). Jak semaglutid, tak tirzepatid se aplikují podkožně jednou týdně. Hmotnostní redukce docílená s těmito léky, zvláště s tirzepatidem, je srovnatelná s tou, kterou pozorujeme po méně invazivních bariatrických výkonech a je provázena významným zlepšením kardiometabolických rizik a příznivým bezpečnostním profilem. Gastrointestinální nežádoucí účinky po těchto lécích jsou obvykle přechodného charakteru a jsou mírné až střední závažnosti. Semaglutid byl již schválen k dlouhodobé léčbě obezity Evropskou lékovou agenturou (EMA), zatímco schválení tirzepatidu se očekává. Klinické studie s orálním semaglutidem, trojitým inkretinovým agonistou retatrutidem, nepeptidickým orálním agonistou GLP-1 orforglipronem a kombinací cagrilintidu se semaglutidem přinesla rovněž nadějné výsledky v redukčním režimu. Na druhé straně setmelanotid je lék, který působí na melanokortinový receptor 4 a je vyhrazen pro léčbu velmi vzácných monogenních onemocnění, která se projevují těžkou obezitou manifestující se v raném dětství, např. při deficitu proopiomelanokortinu nebo leptinového receptoru. Individualizovaná léčba antiobezitiky by měla být integrální součástí komplexní léčby obezity zahrnující dietu, pohybovou aktivitu a kognitivně behaviorální intervenci. Řádně vyškolení lékaři by měli být garantem úspěšnosti léčby antiobezitiky. Navíc, ministerstvo zdravotnictví a zdravotní pojišťovny by měly vzít v úvahu, že cena nových antiobezitik zabraňuje této účinné léčbě u většiny indikovaných pacientů, a proto by měla být přijata opatření k zajištění patřičné dostupnosti těchto léků.
Only modest or transient weight loss achieved with most anti-obesity medications available in the past failed to meet expectations of both patients and physicians. Drug combinations and gut hormone analogues represent a new, more efficient and safe approach to the treatment of obesity and related health risks. Initially, gut hormone analogues, e.g. glucagon like peptide-1 (GLP-1) agonist liraglutide, have successfully been applied for the treatment of type 2 diabetes and later on for the treatment of obesity. The paper reports on the phase 3 clinical trials in obesity management with novel gut hormone analogues, GLP-1 agonist semaglutide and dual gut hormone agonist tirzepatide, a single molecule which targets receptors of two gut hormones, GLP1-1 and glucose-dependent insulinotropic polypeptide (GIP). Both semaglutide and tirzepatide are applied subcutaneously once-weekly. Weight loss achieved with these new drugs, particularly with tirzepatide, is comparable to that observed with less invasive bariatric procedures and is accompanied by significant improvements in cardiometabolic risks and favorable safety profile. Observed gastrointestinal adverse events with these medications were usually transient and mild to moderate in severity. Semaglutide has been approved as an anti-obesity drug for long-term weight management by the European Medicines Agency (EMA) while tirzepatide is waiting for its approval. Clinical studies with oral semaglutide, triple incretin agonist retatrutide, non-peptide GLP-1 oral agonist orforglipron and cagrilintide/semaglutide combination also provided promising results in the weight management. On the other hand, setmelanotide is a drug that acts on the melanocortin-4 receptor (MC4R) and is designated for the treatment of very rare monogenic disorders exhibiting severe obesity in early childhood, e.g. deficiency of proopiomelanocortin or leptin receptor. Individually tailored treatment with antiobesity drugs should be an integral part of the comprehensive obesity management which includes diet, exercise and cognitive behavioral intervention. Well educated physicians should ensure the success of treatment by antiobesity drugs. In addition, health authorities and health insurance companies should consider that the cost of new antiobesity drugs prevents such efficient treatment in most of indicated patients and therefore the measures to arrange appropriate availability of these drugs should be adopted.

Zvyšující se prevalence obezity a komplikací s ní spojených vede k nutnosti zintenzivnění její prevence a léčby. V léčbě obezity se v současnosti uplatňuje především úprava životního stylu, která však mnohdy dlouhodobě selhává. Pro příští desetiletí se jeví jako nezbytná součást léčby obezity – spolu s chirurgickým přístupem – dlouhodobé podávání antiobezitik, tedy léků, které příznivě ovlivňují nejen pokles nadměrné hmotnosti, ale i zdravotní rizika s obezitou spojená. Tento text podává přehled o současných možnostech farmakoterapie obezity včetně jejich indikací, vhodném výběru pacienta a nežádoucích účincích léčby. Přináší také přehled studií, které prokazují dlouhodobou účinnost a bezpečnost této léčby. Přestože účinná a bezpečná antiobezitika jsou v současné době k dispozici, není ani částečně hrazena ze všeobecného zdravotního pojištění. Náklady na léčbu jsou však pro velkou část obézních dlouhodobě neúnosné. Faktická nedostupnost účinných antiobezitik pro indikované pacienty má závažné zdravotně-ekonomické dopady. Nevyužívání účinných možností terapie, potvrzených postupy medicíny založené na důkazech, má za následek vysokou prevalenci s obezitou spojených nemocí, jejichž léčení je ekonomicky ještě nákladnější a v případě diabetu 2. typu dokonce méně efektivní. Alespoň částečnou úhrada antiobezitik z prostředků všeobecného zdravotního pojištění pro spolupracující pacienty za jasně vymezených podmínek považujeme za nezbytný krok ke zlepšení situace, ve svých důsledcích jednoznačně ekonomicky efektivní.
The increasing prevalence of obesity and its associated complications leads to the need to intensify its prevention and treatment. The treatment of obesity is currently based on lifestyle modification, which often fails in the long term. For the next decade, the long-term administration of anti-obesity drugs, i.e. drugs that have a positive effect not only on the reduction of excess weight but also on the health risks associated with obesity, seems to be a necessary part of obesity treatment, along with surgical approaches. This text provides an overview of the current options for the pharmacotherapy of obesity, including their indications, appropriate patient selection and adverse effects of treatment. It also provides an overview of studies that demonstrate the long-term efficacy and safety of these treatments. Although effective and safe anti-obesity drugs are currently available, it is not even partially covered by general health insurance. However, the cost of treatment is unaffordable in the long term for a large proportion of the obese. The virtual unavailability of effective antiobesity drugs for indicated patients has serious health-economic consequences. Failure to take advantage of effective therapeutic options, confirmed by evidence-based medicine, results in a high prevalence of obesity-related diseases, which are even more costly to treat economically and, in the case of type 2 diabetes, even less effective. We consider at least partial reimbursement of antiobesity drugs from general health insurance for cooperating patients under clearly defined conditions to be a necessary step towards improving the situation, and clearly cost-effective in its consequences.

BACKGROUND: Dysfunctional adipose tissue (AT) is known to contribute to the pathophysiology of metabolic disease, including type 2 diabetes mellitus (T2DM). This dysfunction may occur, in part, as a consequence of gut-derived endotoxaemia inducing changes in adipocyte mitochondrial function and reducing the proportion of BRITE (brown-in-white) adipocytes. Therefore, the present study investigated whether endotoxin (lipopolysaccharide; LPS) directly contributes to impaired human adipocyte mitochondrial function and browning in human adipocytes, and the relevant impact of obesity status pre and post bariatric surgery. METHODS: Human differentiated abdominal subcutaneous (AbdSc) adipocytes from participants with obesity and normal-weight participants were treated with endotoxin to assess in vitro changes in mitochondrial function and BRITE phenotype. Ex vivo human AbdSc AT from different groups of participants (normal-weight, obesity, pre- and 6 months post-bariatric surgery) were assessed for similar analyses including circulating endotoxin levels. RESULTS: Ex vivo AT analysis (lean & obese, weight loss post-bariatric surgery) identified that systemic endotoxin negatively correlated with BAT gene expression (p < 0.05). In vitro endotoxin treatment of AbdSc adipocytes (lean & obese) reduced mitochondrial dynamics (74.6% reduction; p < 0.0001), biogenesis (81.2% reduction; p < 0.0001) and the BRITE phenotype (93.8% reduction; p < 0.0001). Lean AbdSc adipocytes were more responsive to adrenergic signalling than obese AbdSc adipocytes; although endotoxin mitigated this response (92.6% reduction; p < 0.0001). CONCLUSIONS: Taken together, these data suggest that systemic gut-derived endotoxaemia contributes to both individual adipocyte dysfunction and reduced browning capacity of the adipocyte cell population, exacerbating metabolic consequences. As bariatric surgery reduces endotoxin levels and is associated with improving adipocyte functionality, this may provide further evidence regarding the metabolic benefits of such surgical interventions.
The fatty acid composition is associated with obesity. Omega 3 polyunsaturated fatty acid (PUFA) could have a beneficial role in the prevention and treatment of many disorders, including cardiometabolic diseases. A cohort of 84 men and 131 women were examined in adolescence and after 8 years. Body weight (BW) and fat mass (FM) were measured. The composition of fatty acids (FAs) of serum phospholipids was assessed using gas chromatography. Statistics: PLS method. Aim: to determine the relationships between FAs in adolescence and FM (explanatory variable 1, EV1) and BW (explanatory variable 2, EV2) in adulthood. In the predictive models, a cluster of FAs in boys explained 47.2 % of EV1 and a cluster of 6 FAs in girls explained 32.3 % of EV1 measured in adulthood. FAs measured in adolescents explained 23.7 % of EV2 in early adults regardless of gender. A significant negative association was found between 18:1n-9c and EV1 in males and EV2 in both genders. We found a significant negative association between 18:2n-6 and 20:0 and both EV1 and EV2. In all analyses, we found a significant negative association of 20:1n-9 and 18:3n-3 with EV1-2 in both genders. A significant positive association was found in 20:3n-6 with EV1 and EV2 in males. 20:4n-6 was positively associated with EV1 in females and EV2 in both genders. A positive association between FM and very long chain n- 6 PUFAs was also observed. It is concluded that serum MUFAs and essential PUFAs in adolescence are associated with lower BW and FM in adulthood.

Hlavní úlohou antiobezitik v komplexní léčbě obezity je podpořit příznivé účinky behaviorální intervence životního stylu na redukci hmotnosti a na zdravotní rizika spojená s obezitou. Nicméně pouze nevelký hmotnostní pokles, kterého se docílilo při léčbě většinou antiobezitik dostupných v minulosti, nesplnil očekávání jak pacientů, tak lékařů. Nová generace účinnějších antiobezitik zahrnuje kombinace léků a analoga hormonů zažívacího traktu. Originální přístup k léčbě obezity, diabetu 2. typu a s nimi souvisejících onemocnění představuje tirzepatid, jehož jedna molekula cílí na receptory dvou hormonů zažívacího traktu, a to na receptory glukagonu podobného peptidu 1 (GLP-1) a inzulinotropního peptidu závislého na glukóze (GIP). Článek shrnuje údaje o tirzepatidu, které se vztahují k mechanismu jeho působení a k nedávno publikovaným klinickým studiím, které se prováděly u pacientů s diabetem 2. typu a u pacientů s obezitou, kteří neměli diabetes. Podávání tirzepatidu bylo provázeno významnou redukcí koncentrací glykovaného hemoglobinu a zlepšením inzulinové senzitivity a lipidového profilu. Pozorovaný pokles hmotnosti o více než o 20 % nebyl docílen po žádném léku, který byl doposud používán v léčbě otylosti a je tak srovnatelný s výsledky méně invazivních bariatrických výkonů. Podávání tirzepatidu jedenkrát týdně spolu s příznivým profilem ohledně bezpečnosti a výskytu nežádoucích účinků může přispívat ke compliance pacienta. Zevrubně jsou diskutovány budoucí perspektivy léčby obezity tirzepatidem.
The main role of anti-obesity drugs in comprehensive obesity management is to support favorable effects of behavioral lifestyle intervention on weight loss and obesity-related health risks. Nevertheless, only modest weight loss achieved with most anti-obesity medications available in the past has not met expectations of both patients and physicians. A new generation of more efficient anti-obesity drugs includes drug combinations and gut hormone analogues. A novel approach to the treatment of obesity, type 2 diabetes and related diseases is represented by tirzepatide, a single molecule which targets receptors of two gut hormones, glucagon-like peptide-1 (GLP1-1) and glucose-dependent insulinotropic polypeptide (GIP). The paper summarizes the data on tirzepatide related to the mechanism of action, and recent clinical trials conducted in patients with type 2 diabetes and in patients with obesity without diabetes. Tirzepatide administration was associated with significant reductions of glycated hemoglobin concentrations and with improvement in insulin sensitivity and lipid profile. Observed weight loss by more than 20 % has never been achieved with any drug until now used for the treatment of obesity and is comparable to results of less invasive bariatric procedures. Once-a-week administration of tirzepatide together with favorable safety and side-effects profile may contribute to patient compliance. Future perspectives of obesity treatment with tirzepatide are discussed in detail.

Prezentujeme případ 76letého muže v anamnéze s tromboembolickou chorobou a ischemickou chorobou srdeční, který byl přijat do nemocnice pro akutní hypoxemickou respirační insuficienci při oboustranné pneumonii v důsledku onemocnění COVID-19. Vzhledem k tomu, že druhý den hospitalizace poklesla spontánní saturace O2 na 60 %, byl pacient přeložen na jednotku intenzivní péče, kde byla zahájena intenzivní oxygenoterapie (vysokoprůtoková nazální oxygenoterapie a neinvazivní ventilace prostřednictvím CPAP: Continuous Positive Airway Pressure). Saturace O2 postupně stoupala a docházelo ke zlepšení klinické symptomatologie. Po následné dechové rehabilitaci byl pacient po 17 dnech hospitalizace propuštěn do domácí péče. Ve sdělení jsou zvažovány hlavní faktory, které ovlivňují výsledek léčby ARDS (Acute Respiratory Distress Syndrome). Vedle intenzivní oxygenoterapie přispěly velmi pravděpodobně k úspěšnému zvládnutí závažného stavu u tohoto pacienta včasné podání nízkomolekulárního heparinu, remdesiviru a dexamethasonu. Klinické zlepšení doprovázelo zlepšení laboratorních nálezů (CRP, prokalcitonin, D-dimery, kreatinin, hs-troponin, počet lymfocytů). Závěrem je diskutována možná kauzální souvislost koronavirové infekce s herpetiformním exantémem, který se objevil před propuštěním pacienta z nemocnice.
We present a 76-year-old male with a past medical history of thromboembolism and coronary heart disease. He was admitted to the hospital with acute hypoxemic respiratory insufficiency due to bilateral COVID-19 pneumonia. As spontaneous O2 saturation dropped to 60% on the second day of the hospital stay, the patient was transferred to the intensive care unit where an intensive oxygen therapy was initiated (HFNO: High-Flow Nasal Oxygenotherapy and non invasive ventilation via CPAP: Continuous Positive Airway Pressure). His oxygen saturation gradually increased and follow-up showed improvement in symptoms. After 17 days of in-patient treatment, he was discharged to homecare. Main factors affecting the outcome of ARDS (Acute Respiratory Distress Syndrome) are considered. Besides the intensive oxygen therapy, an early administration of low molecular heparin, remdesivir and dexamethasone most probably contributed to reversing the initial serious finding in this patient. Clinical improvement was accompanied by improved laboratory tests (CRP, procalcitonin, d-dimer, creatinine, hs-troponin, lymphocyte count). Finally, we discuss a potential association of coronavirus infection with herpetiform exanthema, which appeared before the patient was discharged from the hospital.

Childhood obesity is a serious global health problem. Waist circumference (WC) and waist-to-height ratio (WHtR) reflect body fat distribution in children. The objectives of this study were to assess WC and WHtR in 7-year-old children and to determine body mass index (BMI), WC, and WHtR differences in children from 10 selected countries across Europe (Bulgaria, Czechia, Greece, Ireland, Latvia, Lithuania, North Macedonia, Norway, Spain, and Sweden) participating in the World Health Organization (WHO) Europe Childhood Obesity Surveillance Initiative (COSI). The 50th and 90th percentile of WC (according to COSI and "Identification and prevention of Dietary- and lifestyle-induced health EFfects In Children and infantS" (IDEFICS) cutoff values) and WHtR above 0.5 were used as measures of abdominal obesity in a unique sample of 38,975 children aged 7.00-7.99 years. Southern European countries, including Greece and Spain, showed significantly higher BMI, WC, and WHtRin both genders (p < 0.0001) than Eastern and Northern Europe. The highest values for WC were observed in Greece (60.8 ± 7.36 cm boys; 60.3 ± 7.48 cm girls), North Macedonia (60.4 ± 7.91 cm boys; 59.0 ± 8.01 cm girls), and Spain (59.7 ± 6.96 cm boys; 58.9 ± 6.77 cm girls). WC and WHtRin may add an information about the occurrence of central obesity in children.

Polyunsaturated fatty acids (PUFA) influence many physiological functions. Associations have been found between single nucleotide polymorphisms (SNP) in the FADS1 (Fatty acid desaturase 1) gene and the relative abundance of PUFA in serum lipids. This study examines the relationship between two SNPs in the FADS1 gene (rs174546, rs174537) and the fatty acid (FA) composition of serum lipids in adolescents (13-18 years). We used DNA samples (670 children; 336 girls and 334 boys) from the Childhood Obesity Prevalence and Treatment (COPAT) project. Genomic DNA was extracted from peripheral blood leukocytes in whole blood samples. For genotype analysis, TaqMan SNP Genotyping assays (Applied Biosystems) were used. Fatty acid composition of serum lipids was assessed using gas chromatography. The T-statistic and regression were used for statistical evaluations. Minor allele T carriers in both SNPs had significant lower level of palmitic acid (16:0, phospholipids) and arachidonic acid (20:4[n-6], phospholipids) in both sexes. In girls, we found a significant positive association between minor allele T carriers and eicosadienoic acid (20:2[n-6], cholesteryl esters) in both SNPs. Being a minor allele T carrier was significantly positively associated with dihomo-γ-linolenic acid (20:3[n-6], phospholipids) in boys in both SNPs. SNPs (including rs174546, rs174537) in the FADS gene cluster should have impacted desaturase activity, which may contribute to different efficiency of PUFA synthesis.
