Autoimunitní pankreatitida (AIP) je idiopatické zánětlivé onemocnění slinivky břišní, se kterým se vzácně setkáváme i u pediatrických pacientů. Ve srovnání s dospělými pacienty se však může klinická manifestace u dětí lišit. Rozdílná jsou také kritéria pro potvrzení diagnózy AIP. Prezentujeme kazuistiku 13letého chlapce přijatého na naše pracoviště pro měsíc trvající bolesti břicha, hmotnostní úbytek, zvracení a náhlý rozvoj ikteru. Vstupní laboratorní parametry vykazovaly známky cholestázy, zánětlivé parametry a pankreatické enzymy byly v normálním rozmezí. ERCP vyšetření prokázalo stenózu pankreatické části ductus choledochus, sonografické vyšetření a MRCP vyšetření zobrazilo lézi v oblasti hlavy pankreatu a známky typické pro autoimunitní pankreatitidu. Na základě těchto nálezů byla zahájena léčba orálními kortikosteroidy v iniciální dávce 30 mg/den s postupným snižováním až na aktuální udržovací dávku (5 mg prednisonu na den). Pacient je aktuálně v klinické remisi. Přetrvává u něj ale exokrinní dysfunkce pankreatu, kterou řešíme substitucí pankreatických enzymů. Cílem sdělení je prezentovat základní znaky pediatrické formy AIP a poukázat na rozdíly mezi tímto onemocněním u dětí a dospělých.
Autoimmune pancreatitis is an idiopathic inflammatory disease of the pancreas that is rarely encountered even in paediatric patients. However, compared to adult patients, the clinical manifestation and course of the disease in children may differ. We present a case report of a 13-year-old boy admitted to our department with a history of abdominal pain, weight loss, vomiting, and sudden development of jaundice. Initial laboratory parameters showed signs of cholestasis, inflammatory parameters and pancreatic enzymes were in the normal range. ERCP examination demonstrated stenosis of the pancreatic part of ductus choledochus, sonography and MRCP examination showed lesion in the head of the pancreas and signs typical for autoimmune pancreatitis. Based on these findings, treatment with oral corticosteroids was started at an initial dose of 30 mg/day with a gradual reduction to the current maintenance dose, which is 5 mg of prednisone per day. The patient is currently in clinical remission; however, the exocrine dysfunction of the pancreas persists which we are solving by replacement of pancreatic enzymes. The aim of this paper is to present the basic features of the paediatric form of AIP and to point out the differences between AIP in children and adults.

Aktuální doporučené postupy pro resuscitaci vycházejí z konsenzuálních stanovisek a léčebných doporučení z roku 2020. Byly uveřejněny jako doporučení Evropské resuscitační rady v roce 2021. Resuscitace dětí představuje samostatnou kapitolu a týká se resuscitace pro věkovou kategorii 0–18 let s výjimkou novorozenců bezprostředně po porodu. Resuscitaci novorozenců po porodu je věnována samostatná kapitola. V článku jsou uvedeny postupy při základní i rozšířené pediatrické resuscitaci s důrazem na zajištění dýchání a krevního oběhu. U resuscitace novorozenců jsou probrány terapeutické postupy na porodním sále a následné zajištění péče o novorozence s poruchou poporodní adaptace. Jsou uvedena kritéria nezahájení resuscitace a jejího ukončení.
Current resuscitation guidelines are based on consensual opinions and medical guidelines from 2020. They were published as the European resuscitation council's guidelines in 2021. A separate chapter is dedicated to paediatric resuscitation. It examines the resuscitation of the age category between 0 and 18 years, with the exception of newborn shortly after birth. The resuscitation of newborn shortly after birth is studied in another separate chapter. Practices of basic and advanced paediatric resuscitation with particular focus on the respiratory and blood flow ensuring are also mentioned in the article. As for the neonatal resuscitation, the therapeutic procedures in delivery rooms and subsequent provision of care for newborns with problems of postnatal adaptation are examined. Finally, the criteria of non-initiation of resuscitation as well as its discontinuation are presented.
It is generally known that prevalence of allergic diseases in developed countries is increasing. Same trend we registered in the most serious allergic reaction, which is called anaphylaxis. So far there has not been not enough proof of anaphylaxis in children in the Czech Republic. The goal of our study is to bring the data about occurrence of anaphylaxis in Kolín and Pardubice district, triggers and treatment of anaphylaxis. In our study we enrolled 52 paediatric patients, who suffered from anaphylaxis between June 2014 and June 2019 and they were hospitalized or examined in the paediatric department in hospitals in Pardubice and Kolín. The most common trigger of anaphylaxis was food (29 cases), predominantly peanuts that caused 10 anaphylactic reactions in our cohort. With increasing patient’sage we observed more reactions caused by insect stings. That was the second most common trigger in our study (18 cases). Majority of our patients was treated by antihistamines (83% of patients) and corticosteroids (76% of patients). Epinephrine was used in 24% of patients, predominantly route of application was intrasmuscular. No fatal reaction was observed

Infantilní hemangiomy jsou nejčastější benigní nádory, které postihují 10–12 % kojenců, vyšší incidence je u nedonošených a nezralých dětí. Rozlišují se tři základní typy – superficiální, hluboký a smíšený. Většina hemangiomů nevyžaduje žádnou léčbu, po fázi intenzivního růstu v prvních 4 až 5 měsících života nastává stagnace a po ní involuce. Asi u poloviny hemangiomů přetrvává více či méně významné reziduum. Kolem 10 % hemangiomů může způsobovat závažné komplikace – ohrožení vitálních funkcí (postižení víček, nosu apod.), exulcerace, krvácení a závažné kosmetické postižení. Lékem první volby je neselektivní betablokátor propranolol podávaný ve formě perorálního roztoku, v dávce 2–3 mg/kg/den po dobu 6 měsíců. Propranolol způsobuje vazokonstrikci kapilár hemagiomů, blokuje produkci endogenních faktorů (VEGF – vaskuloendothelial growth factor) podporujících novotvorbu cév a podporuje apoptózu (přirozenou smrt cévních buněk). Propranolol navodí involuci hemangiomu a rezidua po léčbě jsou významně nižší a méně závažná. Nejčastějšími nežádoucími účinky jsou poruchy spánku (až u 20 % dětí). Léčbu je nutné provádět v centrech, kde je k dispozici dětský dermatolog, onkolog, kardiolog, radiolog a chirurg příslušné specializace k řešení reziduálních lézí.
Infantile hemangiomas are the most common benign tumors that affect between 10–12% of infants, a higher incidence in premature and immature children. There are three basic types – superficial, deep and mixed. Most hemangiomas do not require any treatment, stagnation and involution occur after the phase of intense growth in the first 4 to 5 months of life. About half of the hemangiomas persist in more or less significant residues. Around 10% of hemangiomas can cause serious complications – endangering vital functions (eyelid, nose, etc.), exudation, bleeding and severe cosmetic impairment. The first-line drug is the non-selective betablocker propranolol administered at a dose of 2–3 mg/kg/day for 6 months. Propranolol acts as a vasoconstrictor of capillary hemangiomas, blocking vascular endothelial growth factor (VEGF) promoting vascular development and promoting apoptosis (natural death of vascular cells). Propranolol induces involution of hemangioma and residues after treatment are significantly lower and less severe. The most common complications are sleep disorder (up to 20% of children). Treatment should be performed in centers where a pediatric dermatologist, oncologist, cardiologist, radiologist, and surgeon are provided with the appropriate specializations to resolve residual lesions.
