Health care facilities and hospitals generate significant amounts of wastewater which are released into the sewage system, either after a preliminary treatment or without any further treatment. Hospital wastewater may contain large amounts of hazardous chemicals and pharmaceuticals, some of which cannot be eliminated entirely by wastewater treatment plants. Moreover, hospital effluents may be loaded with a plethora of pathogenic microorganisms or other microbiota and microbiome residues. The need to monitor hospital effluents for their genotoxic hazard is of high importance, as detailed information is scarce. DNA-based information can be acquired directly from samples through the application of various molecular methods, while cell-based biomonitoring assays can provide important information about impaired cellular pathways or mechanisms of toxicity without prior knowledge of the identity of each toxicant. In our study, we evaluated samples of chlorinated hospital wastewater discharged into the sewage system after this disinfection process. The assessment of cytotoxicity, genotoxicity and mutagenicity of the hospital effluents was performed in vitro by using a broad battery of biomonitoring assays that are relevant for human health effects. All the tested hospital wastewater samples could be classified as potentially genotoxic, and it is concluded that the microbiota present in hospital wastewater might contribute to this genotoxic potential.

Health care facilities use for therapeutic purposes, diagnostics, research, and disinfection a high number of chemical compounds, such as pharmaceuticals (e.g. antibiotics, cytostatics, antidepressants), disinfectants, surfactants, metals, radioactive elements, bleach preparations, etc. Hospitals consume significant amounts of water (in the range of 400 to 1200 liters/day/bed) corresponding to the amount of wastewater discharge. Some of these chemicals are not eliminated in wastewater treatment plants and are the source of pollution for surface and groundwater supplies. Hospital wastewater represents chemical and biological risks for public and environmental health as many of these compounds might be genotoxic and are suspected to contribute to the increased incidence of cancer observed during the last decades. The changes of the genetic information can have a lethal effect, but more often cause tumor processes or mutations in embryonic development causing serious defects. A review of the available literature on the mutagenicity/genotoxicity of medical facilities wastewater is presented in this article.

Úrazy a jejich následky jsou závažným problémem z hlediska zdravotního, sociálního i ekonomického. Podle údajů zdravotnické databáze ÚZIS ČR celková úrazovost české dětské populace v období let 2000–2005 meziročně vzrostla o 0,7 %, počet zlomenin o 1,3 %. Oproti předchozímu decenniu tak došlo ke zvýšení úrazovosti dětí o 8 % a počet zlomenin na 100 tisíc dětí vzrostl téměř o 10 %. V období let 2006–2009 trend v počtu úrazů byl již klesající (v přepočtu na obyvatele bylo ošetřeno ročně v průměru 394 tisíc úrazů dětí ve věku 0–14 let, tj. 266 úrazů na 1 tisíc dětí) [1]. Podíl zlomenin na počtu úrazů se u dětí do 15 let pohybuje dlouhodobě kolem 25 %. V období pubertálního spurtu podíl zlomenin u školních dětí v celkovém počtu úrazů převažuje. Diskutujeme užitečnost a přínos nových poznatků semilongitudinální studie somatického vývoje a úrazovosti u školních dětí a referujeme o těsné asociaci růstového profilu a výskytu (četnosti) dětského úrazu v časném období dospívání. Zjištěné poznatky vnášejí nový pohled na podmíněnost vzniku úrazového rizika v období pubertálního spurtu. Využití a aplikace poznatků semilongitudinálního šetření v pedagogické a pediatrické praxi by mohly významně přispět k řešení aktuálních úkolů dětské úrazové problematiky v ČR a k efektivitě úrazové strategie a prevence na ZŠ, a tím k významnému snížení počtu úrazů u současné populace školních dětí.
Injuries and their consequences pose serious health, social and economic concerns. Based on the data of the Institute of Health Information and Statistics of the Czech Republic (IHIS), the overall child injury incidence rate increased year over year by 0.7% and the number of fractures rose year over year by 1.3% in the Czech Republic in 2000–2005. In comparison with the previous decade, the child injury incidence rate increased by 8% and the fracture incidence rate per 100,000 children was nearly 10% as high. In 2006–2009, a downward trend in the injury incidence was observed (394 thousand injuries on average were reported annually in children 0–14 years of age, i.e. 266 injuries per 1000 children per year) [1]. In children under 15 years of age, fractures account for about 25% of the overall injury incidence on a long-term basis. During the pubertal spurt, fracture is the leading cause of injury. The usefulness and benefit of the new data from the semi-longitudinal study of somatic development and injury incidence in school children are discussed and a close association is reported between the growth profile and injury incidence in early adolescence. The new data contribute to a new insight into the higher injury risk during the pubertal spurt. The data from the semi-longitudinal survey could be considerably helpful in pediatric and pedagogical practice for solving the current tasks in child injury prevention, for identifying the effective child injury prevention strategies to be implemented in primary schools and thus for significantly reducing the injury incidence in the present population of school children.

The proposal of grant study deals with problems of evaluating tendencies in the growth dynamics of parameters of linear somatic proportionality and injury incidence in school children. The velocity curves of child growth as well as of the increase (or decrease) in the number of injuries will be constructed by means of applying a model of linear regression with mixed effects . This model made it possible to establish average characteristics of both items for periods of mid-growth spurt and pubertal spurt. Timings and durations of both phases will be compared with regard to injury incidence. Findings and conclusion based on the grant study are supposed to contribute to the reduction of injuries in schoollers by their implementation or utilization in forthcoming injury prevention projects in the Czech Republic.
Vývojová perioda - mladší školní věk- představuje z pohledu somatického vývoje prepubertální období. Na ni navazuje druhá největší akcelerace růstu v dětství- pubertální růstový spurt. Na pozadí aktuálních informací o nepříznivém současném trendu, zaznamenávajícím nárůst školních úrazů v posledních letech, předpokládáme význam získaných poznatků grantové studie pro změnu tohoto trendu, zejména pak s ohledem na snížení rizik vzniku dětských úrazů typu zlomeniny a vykloubeniny v období akcelerace a decelerace růstu dítěte Pro komparaci úrazových trendů a jednotlivých somatických etap ve vývoji dítěte bude využito matematických postupů pro modelaci růstové a úrazové charakteristiky žáka ZŠ Na základě takto získaných informací budou stanoveny úrazové determinanty , zejména s ohledem na zlomeniny a vykloubeniny u školních dětí v období MGS a PS.
