Osimertinib je tyrosinkinázovým inhibitorem třetí generace, vysoce účinný i v oblasti centrálního nervového systému, kde často nádory plic generalizují. Léčba je vyhrazena pro nemocné s nemalobuněčným plicním karcinomem s mutací receptoru epidermálního růstového faktoru (epidermal growth factor receptor, EGFR). V současné době je hrazena u pacientů s pokročilým či generalizovaným onemocněním, ale i v adjuvanci po dobu tří let, kdy významně snižuje riziko vzniku relapsu nemoci.
Osimertinib is a third-generation tyrosine kinase inhibitor, highly effective in the central nervous system, where lung tumors often metastasize. Treatment is specifically reserved for patients with an epidermal growth factor receptor (EGFR) mutation in non-small cell lung cancer. It is currently reimbursed for patients with advanced or metastatic disease, and also as adjuvant therapy for up to three years, significantly reducing the risk of disease recurrence.
Imunoterapie je dnes standardní součástí terapeutického algoritmu u nemocných s nemalobuněčným plicním karcinomem. Jde o velký léčebný posun vpřed a naději na dlouhodobé přežití nemocných. Daní za tuto léčbu bývá neobvyklé spektrum toxicit. Jejich výskyt není tak častý, jako bývá u cytostatické léčby, ale mnohdy je obtížněji podchytitelný a toxicity mohou být i závažné s nutností hospitalizace a ukončením imunoterapie. Jedním z léků této kategorie (imunoterapie) je nivolumab, který byl ve druhé linii léčby podán naší nemocné. Přestože léčbu doprovázely různé nežádoucí účinky, pacientka měla z léčby profit, a maximálně jsme se proto snažili ji ponechat na této účinné a moderní terapii.
Immunotherapy is now a standard part of the therapeutic algorithm for patients with non-small cell lung cancer. It is a major therapeutic advance and offers hope for long-term survival. This treatment comes with its own unique set of toxicities. Their occurrence is not as frequent as with cytostatic therapy, but they are often more difficult to manage and can be severe, leading to the discontinuation of immunotherapy. One of the drugs in this category is nivolumab, which was administered to our patient as a second-line treatment. Although various side effects accompanied the treatment, the patient benefited, and we made every effort to maintain her on this effective, modern therapy.
Skeletální metastázy se v době diagnózy vyskytují až u 20-30 % pacientů s nemalobuněčným karcinomem plic (non-smah cell lung cancer, NSCLC); v průběhu onemocnění se vyvinou až u 60 % nemocných s NSCLC, až v polovině případů predilekčně postihují páteř. Ohrožují vybranou skupinu pacientů tzv. kostními příhodami, kdy v popředí jsou obtížně zvládnutelné bolesti, patologické fraktury, míšní komprese, neurologické deficity, hyperkalcemie. Celkově jsou zdrojem výrazné morbidity, značně zhoršují kvalitu života pacientů a jsou indikátorem horší prognózy s mediánem přežití 6-7 měsíců od stanovení diagnózy. Doposud není stanovena účinná léčba cílená na skeletální metastázy NSCLC. V této kazuistice prezentujeme dva pacienty s NSCLC a osovými metastázami skeletu po operační stabilizaci. Systémově byl u obou zvolen pembrolizumab, a přestože si byly zpočátku oba případy podobné, každý se vyvíjel jinak.
Skeletal metastases are prevalent in 20-30 % of patients with non-smaii cell lung cancer (NSCLC) at the time of diagnosis, up to 60 % of patients will develop bone metastases - up to half of those are presenting with spine metastases. Selected demographic of patients is at great risk of developing skeletal related events, that include severe chronic pain, pathological fractures, spinal compression, neurologic deficit, hypercalcemia. They are a source of great morbidity and significantly worsen patients’ quality of life, with overall survival median of 6-7 months at the time of diagnosis. Here we are presenting two cases of patients with NSCLC with bone metastases, who underwent surgical spine stabilization, and were subsequently treated with pembrolizumab in the first line of treatment.
S příchodem imunoterapie v léčbě nemalobuněčného karcinomu plic došlo k prodloužení života velké části nemocných. Pokud organismus nemocného reaguje na léčebné impulsy a podaří se probudit protinádorovou imunitu, je to úspěch. V praxi jsme ale svědkem situací, kdy se to přes veškerou snahu nedaří podle našich plánů a nemocný neprofituje z této léčby. Stále existuje mnoho otázek, proč tomu tak je. Jednou z teorií, proč určitá skupina pacientů má z léčby prospěch, a jiná nikoli, je složení střevního mikrobiomu. Naše kazuistika dokládá krásnou léčebnou odpověď u nemocné s adenokarcinomem, která podstoupila fekální transplantaci. A po roce od diagnózy začínáme být optimisty.
The advent of immunotherapy in the treatment of non-small cell lung cancer has prolonged the lives of a large proportion of patients. If the patient’s body responds to treatment and manages to activate anti-tumor immunity, it is considered a success. However, in practice, we often encounter situations where, despite our best efforts, things do not go as planned and the patient does not benefit from the treatment. Many questions remain as to why this is the case. One theory suggests that the composition of the gut microbiome may determine why certain groups of patients benefit while others do not. Our case report shows a significant treatment response in a patient with adenocarcinoma who underwent fecal transplantation. A year after diagnosis, we are becoming optimistic.
Entrectinib a jeho toxicita v praxi u ROS1 pozitivního pacienta s nemalobuněčným karcinomem plic byla vlastně naší první zkušeností s touto cílenou léčbou a rovnou i zkušeností s možnou formou nežádoucích účinků. Přestože entrectinib má výborná data léčebné odpovědi a přežití, je nutno myslet i na nežádoucí účinky a nemocné bedlivě sledovat. Někdy může být léčba vykoupena tou nejdražší daní.
Entrectinib and its toxicity in a ROS1-positive non-small cell lung cancer patient was actually our first experience with this targeted therapy and also our experience with potential side effects. Although entrectinib has excellent data on treatment response and survival, it’s important to consider side effects and monitor patients closely. Sometimes the cost of treatment can be quite high.
Léčba bronchogenního karcinomu se rok od roku obohacuje o nová léčiva, rozšiřuje se spektrum cílených léků na jednotlivé řídicí mutace, a tak i standardy léčby se dynamicky vyvíjejí. Naše kazuistika není nijak výjimečná ohledně vzácnosti genů či léčiv, ale je ukázkou odolnosti nemocného. Jeho touha žít a optimismus jistě napomohly tomu, že jsme jej léčili jednou linií za druhou, až jsme pomalu, ale jistě narazili na dno. Nemocný absolvoval sedm linií léčby a možná to není ještě konec.
he treatment of bronchogenic carcinoma is continually enriched with new medications, the range of targeted drugs for individual driver mutations is expanding, and treatment standards are dynamically evolving. Our case report is not extraordinary in terms of rare genes or medications but serves as an example of the patient’s resilience. The patient’s will to live, and optimism undeniably influenced our approach, enabling us to administer successive lines of treatment until we gradually reached a limit. The patient has finished seven rounds of treatment, and there could still be more to come.
V současné době je téměř každý čtvrtý nemocný s nemalobuněčným plicním karcinomem v době stanovení diagnózy ve III. klinickém stadiu a bohužel častěji s nemožností radikální resekce.1 I přes kurativní standardní chemoterapii s radioterapií dochází u většiny nemocných k recidivě onemocnění. Imunoterapie durvalumabem se stala nadějí pro tyto nemocné, což dokládá naše kazuistika.
At the moment, almost every fourth patient with non-small cell lung cancer is in clinical stage III at the time of diagnosis and, unfortunately, more often with the impossibility of radical resection. Despite curative standard chemotherapy with radiotherapy, the disease relapses in most patients. Immunotherapy with durvalumab has become a hope for these patients, as evidenced by our case report.

Novou nadějí na delší přežití je pro nemocné s malobuněčným karcinomem plic imunoterapie. Doposud byla podávána jen v rámci studií; rok 2022 se stal zlomovým. V komplexních onkologických centrech mají tito nemocní šanci na hrazenou léčbu durvalumabem (s podáváním chemoterapie). Naše kazuistika je zaměřena na vhodnost výběru pacientky pro tuto léčbu, neboť dlouhodobé zkušenosti mimo klinické studie jsou zatím malé.
Immunotherapy is the new hope for longer survival for patients with small cell lung cancer. So far, it has only been given in trials, but 2022 has become a breakthrough year. In comprehensive cancer centers, these patients have a chance of receiving reimbursed treatment with durvalumab (with chemotherapy). Our case report focuses on the appropriateness of patient selection for this treatment, as long-term experience outside clinical trials is still limited.
