OBJECTIVE: The substantial material and legislative investments in establishing and maintaining cytological screening in the Czech Republic represent barriers to a direct transition to primary HPV screening. Therefore, the LIBUSE project was implemented to test the efficacy of phasing in HPV DNA testing as a co-test to cytology in routine screening of women >30 years of age. METHODS: Women aged 30 to 60 years who underwent regular annual Pap smears were co-tested for HPV DNA with selective 16/18 genotyping at 3-year intervals. All HPV 16/18-positive cases and/or cases with a severe abnormality in cytology were sent for colposcopy; HPV non-16/18-positive cases and LSILs were graded using p16/Ki67 dual-stain cytology, and positive cases were sent for colposcopy. RESULTS: Overall, 2409 patients were included. After the first combined screening (year 'zero') visit, 7.4% of women were HPV-positive and 2.0% were HPV16/18-positive; only 8 women had severe Pap smear abnormalities. Triage by dual staining was positive in 21.9% of cases (28/128). Biopsy confirmed 34 high-grade precancer lesions. At the second combined visit (year 'three'), the frequency of HPV infection (5.3% vs. 7.4%) frequency of HPV16/18 (1.1% vs. 2.0%), referrals for colposcopy (35 vs. 83), and biopsy verified high-grade lesions (5 vs. 34) were significantly lower (all P ≤ 0.001). CONCLUSION: The addition of HPV DNA testing with selective genotyping of HPV16/18 to existing cytology screening significantly increased the safety of the program. The gradual introduction of HPV testing was well received by healthcare professionals and patients, and can facilitate transformation of the cytology-based screening. PURPOSE: The effect of SAM vaginal gel, a medical device containing adsorptive silicon dioxide and antioxidative sodium selenite and citric acid, on histologically-proven cervical intraepithelial neoplasia type 2 (CIN2) as well as p16 positive CIN1, and on the presence of the onco-marker p16 was investigated. METHODS: 216 women aged 25-60 years were randomized to either receive an intravaginal daily dose of SAM gel for three 28-day periods, or be followed-up without intervention. The primary endpoint was efficacy, defined as a combined histological and cytological regression. At baseline and after 3 months participants had: a guided biopsy including p16 immunohistochemical (IHC) staining, only if a lesion was visible at colposcopy; a cervical smear for cytology, high-risk human papillomavirus (hr-HPV) and a p16/Ki-67 test. At 6 months a further cytology and p16/Ki-67 test was performed. RESULTS: Regression of CIN lesions was observed in 78 out of 108 patients (72.2%) in the SAM gel arm and in 27 out of 108 patients (25.0%) in the control arm. Similarly, the change in the p16/Ki-67 cytological test status was significantly in favor of the treatment arm. The prevalence of hr-HPV decreased significantly (p < 0.001) in the treatment arm, from 87.0% to 39.8%, while it slightly increased in the control arm, from 78.7% to 83.3%. At 6 months the cytological regression in the treatment group and the highly significant effect on p16/Ki-67 was still present. CONCLUSION: SAM vaginal gel enhances the regression of cervical lesions and clears hr-HPV and p16/Ki-67 in smears significantly, thus offering an active non-destructive management to prevent cervical cancer. Objective: To analyze the course of p16/Ki-67-positive abnormal cytological cervical findings and high risk (hr)-HPV- and p16/Ki-67-clearances in women treated with a vaginal gel. Methods: 172 women with a histological diagnosis of CIN2 or p16-positive CIN1 lesions were selected based on a positive cytological p16/Ki-67 test. For 3 months, 75 patients in the active arm (AA) daily administered 5 ml of a vaginal gel. Ninety seven patients in the control arm (CA) underwent no treatment ("watchful waiting"). Endpoints were cytological evolution, p16/Ki-67- and hr-HPV-clearances. Results: At 3 months, cytological regression was observed in 76% (57/75) of patients in the AA compared with 25% (24/97) in the CA. Progression occurred in 5% (4/75) of the AA compared with 15% (15/97) of the CA. The p16/Ki-67 status change was statistically significantly (p < 0.001) in favor of the AA: 77% (58/75) became negative compared to 21% (20/97) in the CA. hr-HPV prevalence decreased significantly (p < 0.001) in the AA from 87 to 44%, while increasing in the CA from 78 to 84%. Cytological regression and p16/Ki-67 changes persisted in the AA at 6 months. Conclusions: The vaginal gel significantly cleared hr-HPV and p16/Ki-67 and was associated with improved cytological findings, thereby potentially offering an effective option against oncogenic risk. Purpose: The contraceptive pill is an effective and safe method of preventing pregnancy. The progestins used for contraception either are components of a combined hormonal contraceptive (tablets, patches or vaginal rings) or are used alone in progestin-only formulations. Progestin-only contraceptives are available as daily oral preparations, subcutaneous or intramuscular injectables (every 1-3 months), subdermal implants (every 3-5 years) and intrauterine systems (every 3-5 years). Long-acting progestins are highly effective in typical use and have a very low risk profile and few contraindications.Material and Methods: A new progestin-only, oestrogen-free contraceptive, drospirenone, in a dosage of 4 mg/day in a 24/4 regimen, has received regulatory approval in the USA and the EU. The molecule has antigonadotropic, antimineralocorticoid, antiestrogenic and antiandrogenic properties.Results: The regimen was chosen to improve the bleeding profile; maintain plasma oestradiol levels at those of the early follicular phase, to avoid hypoestrogenism; and preserve efficacy even with a missed pill, as drospirenone has a half-life of 30-34 h.Conclusions: Clinical studies have shown good efficacy, very low cardiovascular side effects and a favourable bleeding pattern, as well as maintenance of ovulation inhibition after scheduled 24 h delays in pill intake.
Cíl studie: Práce hodnotí výsledky dvouletého sledování kohorty žen ve věku 35–36 a 45–46 let, které se účastní projektu LIBUŠE, jehož cílem je ověřit účinnost společného vyšetření HPV DNA testu, cytologického stěru a cytologického barvení p16/Ki67 v národním cervikálním screeningu. Typ studie: Prospektivní observační studie. Název a sídlo pracoviště: Gynekologicko-porodnická klinika VFN a 1. LF UK, Praha. Materiál a metodika: Ze všech žen zařazených do studie LIBUŠE byly vybrány pouze ty, kterým bylo při vstupu do studie 35–36 a 45–46 let. Pacientkám byl odebrán konvenční cytologický stěr a HPV DNA test (Cobas 4800, Roche Diagnostics). Ženy byly podle vstupních výsledků stratifikovány do tří úrovní rizika: 1. nerizikové, 2. vysoce rizikové a 3. se střední mírou rizika, které byly tříděny pomocí imunocytochemického barvení p16/Ki67. Vysoce rizikové pacientky a pacientky s pozitivitou duálního barvení byly referovány k expertnímu kolposkopickému vyšetření. Případy s biopticky verifikovanou prekancerózou nebo karcinomem byly považovány za „pozitivní nálezy“. Výsledky: Věkové kritérium splnilo 352 žen. U 26 (7,4 %) žen byla zjištěna HPV DNA pozitivita, z toho u devíti pozitivita HPV 16/18. Závažná cytologická abnormita byly zjištěna pouze u jedné pacientky (0,3 %), která byla současně HPV pozitivní. K expertní kolposkopii bylo primárně referováno deset (2,8 %) žen splňujících kritérium vysokého rizika. U dalších 18 pacientek bylo doplněno barvení p16/Ki67 a čtyři pozitivně testované byly rovněž referovány k expertní kolposkopii. Při kontrole po roce bylo identifikováno devět pacientek středního rizika, při druhé kontrole po dalším roce bylo identifikováno dalších šest pacientek středního rizika. Během dvouletého sledování bylo ke kolposkopii referováno dalších devět pacientek. Za dobu sledování byla zjištěna prekanceróza u deseti pacientek a u žádné pacientky nebyl zjištěn invazivní karcinom. Závěr: Doplnění stávajícího cytologického screeningu o HPV DNA test se selektivní genotypizací HPV 16/18 umožňuje významně zvýšit citlivost a bezpečnost našeho cervikálního screeningového programu.
Objective: The study evaluates results of 2-years follow-up of patients in ages 35–36 and 45–46, who are participating in the project LIBUSE, that deals with efficacy of HPV DNA and Pap smear co-testing and p16/Ki67 dual staining in the Czech national cervical screening. Design: Prospective observational study. Setting: Department of Obstetrics and Gynecology, General University Hospital and 1st Medical Faculty, Charles University, Prague. Materials and methods: Out of all women enrolled in the project LIBUSE only those who were at the beginning of the study 35–36 and 45–46 years old were sellected. Conventional Pap smear and HPV DNA test (Cobas 4800, Roche Diagnostics) had been collected at the baseline. Women were stratified according to their results in the three risk groups: 1. low-risk, 2. high-risk and 3. intermediate risk, who subsequently underwent p16/Ki67 dual staining. All high-risk patients and those with positive result of dual staing were refered to the expert colposcopy. The cases with biopsy proven precancers or cancers were considered as „positive findings“. Results: Altogether 352 women meet the age requirements. In 26 (7.6%) women had been proven HPV DNA positivity and out of the them 9 cases were HPV 16/18 positive. Severe cytological abnormality was found only in one patient (0.3%), who was simultaneously HPV positive. Ten women (2.8%) were classified as high-risk and directly refered to colposcopy. Another 18 patients underwent p16/Ki67 dual staining and 4 positive cases were refered to colposcopy too. After one year further 9 patients were classified as intermediate risk and 6 more were identified after two years of follow-up. Within two years 9 more patient were refered to colposcopy. After the entire period of follow-up in 10 patients biopsy confirmed precancer lesions, none of them had invasive cancer. Conclusions: Addition of HPV DNA testing with selective HPV 16/18 genotyping to the cytology based screening significantly increases sensitivity and safety of our cervical screening program.

Metodika, výsledky: Preeklampsie (Preeclampsia, PE) je závažné multiorgánové onemocnění komplikující těhotenství. Je celosvětově hlavní příčinou mateřské a perinatální mortality a morbidity. Z provedených studií vyplynulo, že u těhotných žen se zvýšeným rizikem je možná prevence rozvoje závažných forem onemocnění včasným zahájením léčby kyselinou acetylsalicylovou. Aby byla prevence účinná, měla by být riziková skupina těhotných žen identifikována v 11.–13. gestačním týdnu. Jediná, v mnoha studiích validovaná metodika pro provádění screeningu PE s dostatečnou diagnostickou účinností v I. trimestru těhotenství je dána The Fetal Medicine Foundation (FMF) a byla přijata a publikována v novém doporučení The International Federation of Gynecology and Obstetrics (FIGO). Závěr: Tento článek shrnuje nejnovější poznatky a postupy pro provádění screeningu preeklampsie v rámci kombinovaného screeningu v I. trimestru těhotenství a způsob prevence rozvoje závažných forem PE podáváním nízkých dávek kyseliny acetylsalicylové.

Methods, results: Preeclampsia (PE) is a multisystem disorder complicating pregnancy. It is the leading cause of maternal and perinatal mortality and morbidity worldwide. Recent studies have shown that high-risk pregnant women may benefit from low-dose acetylsalicylic acid early therapy in prevention of the development of severe forms of the disease. The risk group of pregnant women should be identified in 11–13 gestational week for effective prevention. The only procedure validated in many studies for performing PE screening with sufficient diagnostic accuracy in the first trimester of pregnancy is given by The Fetal Medicine Foundation (FMF) and has been adopted and published in a new recommendation by The International Federation of Gynecology and Obstetrics (FIGO). Conclusion: This article summarizes the recent findings and recommendation for performing screening of preeclampsia in 1st trimester of pregnancy and how to prevent the development of severe forms of PE by low-dose acetylsalicylic acid therapy.
