BACKGROUND: As a step towards clinical use of AAV-mediated gene therapy, brains of large animals are used to settle delivery parameters as most brain connections, and relative sizes in large animals and primates, are reasonably common. Prior to application in the clinic, approaches that have shown to be successful in rodent models are tested in larger animal species, such as dogs, non-human primates, and in this case, minipigs. NEW METHOD: We evaluated alternate delivery routes to target the basal ganglia by injections into the more superficial corona radiata, and, deeper into the brain, the thalamus. Anatomically known connections can be used to predict the expression of the transgene following infusion of AAV5. For optimal control over delivery of the vector with regards to anatomical location in the brain and spread in the tissue, we have used magnetic resonance image-guided convection-enhanced diffusion delivery. RESULTS: While the transduction of the cortex was observed, only partial transduction of the basal ganglia was achieved via the corona radiata. Thalamic administration, on the other hand, resulted in widespread transduction from the midbrain to the frontal cortex COMPARISON WITH EXISTING METHODS: Compared to other methods, such as delivery directly to the striatum, thalamic injection may provide an alternative when for instance, injection into the basal ganglia directly is not feasible. CONCLUSIONS: The study results suggest that thalamic administration of AAV5 has significant potential for indications where the transduction of specific areas of the brain is required.

DYT1 gene mutations lead to early-onset dystonia that begins with focal limb onset and spreads to other body regions within 5 years, with typical sparing of the oromandibular muscles. In the present study, we describe two patients with an unusual presentation of the disease.

Během 12 let (1992–2004) podstoupilo na našem oddělení Nemocnice Na Homolce 19 pacientů s trigeminálními schwannomy radiochirurgickou léčbu Leksellovým gama nožem. Kontrolu po léčbě mělo dosud 15 pacientů (ženy :muži = 8 : 7, věk 16–71 let, medián 45 let), doba sledování byla 12–120 měsíců, medián 54 měsíců. U 7 nemocných předcházel pokus o mikrochirurgické odstranění či zmenšení nádoru. Cílový objem tumoru se pohyboval od 2 100 mm3 do 18 000 mm3 (medián 9 600 mm3). U 13 pacientů byl ozářen celý objem tumoru, u 2 se jednalo o parciální ozáření. Při záření jsme aplikovali na okraj nádoru minimální dávku (Dmin) 10–15 Gy (medián 12 Gy) na 50% izodóze. Kontroly nad růstem tumoru jsme dosáhli v 93 % (14 případů). U 3 pacientů došlo k úplné regresi, u 8 ke zmenšení tumoru, u 1 pacienta k zástavě růstu, u 3 se tumor zvětšil. U 11 (73 %) nemocných bylo zjištěno zlepšení klinického stavu, 2 nevykázali žádnou změnu a u 1 pacienta se vyskytly nové neurologické obtíže. Patnáctý pacient, kterému byla ozářena pouze část rozsáhlého tumoru komprimujícího kmen, zemřel 9 měsíců po léčbě na septický stav. U 5 pacientů byly schwannomy cystického charakteru, u 4 z nich se objevily po ozáření přechodné obtíže dané zvětšením cystické části tumoru. Ve 2 případech musela být stereotakticky odsáta jeho cystická část a u jednoho pacienta byla provedena marsupializace cysty. Radiochirurgie se ukázala být efektivní modalitou pro malé a středně velké trigeminální schwannomy jako léčba primární i sekundární, neboť kontroluje růst tumoru v 93 % a je zatížena malou morbiditou.
Within 12-year period (1992–2004) 19 patients with trigeminal schwannomas underwent Gamma Knife radiosurgery at our department of Na Homolce Hospital. Fifteen patients already had a follow-up after the treatment, and they were observed for the minimum of 12 and the maximum of 120 months, median 54 months. The group of patients consisted of 8 women and 7 men with the median age of 45 years. Seven patients underwent one to three microsurgical operations before radiosurgery. The tumour target volume ranged from 2 100 mm3 to 18 000 mm3 (median 9 600 mm3). Thirteen patients were subjected to irradiation of the whole tumour volume, in two cases the irradiation was only partial. The minimum tumour marginal dose ranged from 10 to 15 Gy (median 12 Gy) and it was delivered to 50 % isodose line. The tumour growth control was reached in 93 % (14 cases). In 3 patients, the tumour regressed completely, and in 8 patients there occurred a significant reduction of the tumour volume. No further tumour growth was seen in one case, progress appeared in three cases. Eleven (73 %) patients showed clinical improvement, 2 were without any change, and new neurological symptoms occurred in one patient. The fifteenth patient whose large tumour compressing the brain stem had been subjected to partial irradiation died of septicaemia 9 months after the treatment. Five patients had a cystic type of schwannomas, and four of them suffered from transient neurological deficits after irradiation as a consequence of an enlargement of the cystic part of the tumour. Two cases were tackled by stereopuncture, the third case was dealt with by the cyst marsupialisation. Radiosurgery has shown to be an effective primary as well as secondary treatment modality for small- and medium-size trigeminal schwannomas as the local tumour growth control is 93 % with a low morbidity.
