Autoři popisují raritní případ solitární metachronní metastázy sterna renálním karcinomem u 59leté pacientky 12 let po primární levostranné nefrektomii. Vzhledem k velikosti metastázy a rozsahu resekce byla jako náhrada sterna využita polyesterová zdvojená síťka, mezi jejíž vrstvy byl aplikován kostní cement. Operace byla provedena bez komplikací bez indikace k adjuvantní léčbě. Pacientka ve sledovaném pooperačním období 20 měsíců je v celkově dobrém stavu a bez známek recidivy základního onemocnění.
In this report, the authors describe a rare case of complete sternal resection for a metachronous metastasis from renal carcinoma in a 59-year-old female patient 12 years after primary left nephrectomy. Due to the large extent of resection, a polyester double layer mesh with bone cement was used for chest wall reconstruction. The postoperative course was uneventful without any indication for adjuvant treatment. The patient has been followed up for 20 months without any signs of complications and recurrence of her malignancy.

Pancreatic paragangliomas are extremely rare with less than 20 cases ever described in the world literature. There is no detailed report of the vascular anatomy in this entity and its possible impact on patient management. We present a case of large pancreatic head paraganlioma in a 53-year-old woman. The tumour had a predominant arterial blood supply via both the hepatic artery and the superior mesenteric artery. Complex inflow was complemented by supplementary branches from the right renal artery. The arteriovenous communications within the lesion represented the most dangerous aspect of excision and the tumour removal was accompanied with a considerable blood loss. After pancreaticoduodenectomy, patient experienced transient elevation of liver function tests with no other identifiable cause than a change in portal haemodynamics. It is advisable that the precise knowledge of vascular anatomy in pancreatic head paraganglioma should be obtained prior to any intervention.
