Cauda equina neuroendocrine tumors (CENETs) are neoplasms of uncertain histogenesis with overlapping features between those of paragangliomas (PGs) and visceral neuroendocrine tumors (NETs). We have explored their biological relationship to both subsets of neuroendocrine neoplasms. The clinical and radiological features of a cohort of 23 CENETs were analyzed. A total of 21 cases were included in tissue microarrays, along with a control group of 38 PGs and 83 NETs. An extensive panel of antibodies was used to assess epithelial phenotype (cytokeratins, E-cadherin, EpCAM, Claudin-4, EMA, CD138), neuronal and neuroendocrine features (synaptophysin, chromogranin A, INSM1, neurofilaments, NeuN, internexin-α, calretinin), chromaffin differentiation (GATA3, Phox2b, tyrosine hydroxylase), and possible histogenesis (Sox2, T-brachyury, Oct3/4, Sox10). The cohort included 5 women (22%) and 18 men (78%). The average age at the time of surgery was 48.3 years (range from 21 to 80 years). The average diameter of the tumors was 39.27 mm, and invasion of surrounding structures was observed in 6/21 (29%) tumors. Follow-up was available in 16 patients (median 46.5 months). One tumor recurred after 19 months. No metastatic behavior and no endocrine activity were observed. Compared to control groups, CENETs lacked expression of epithelial adhesion molecules (EpCAM, CD138, E-cadherin, Claudin-4), and at the same time, they lacked features of chromaffin differentiation (GATA3, Phox2b, tyrosine hydroxylase). We observed no loss of SDHB. Cytokeratin expression was present in all CENETs. All the CENETs showed variable cytoplasmic expression of T-brachyury and limited nuclear expression of Sox2. These findings support the unique nature of the neoplasm with respect to NETs and PGs.

Prezentujeme případ 42leté ženy s potvrzeným zděděným syndromem von Hippel-Lindau (VHL) a s recidivujícím tumorem endolymfatického vaku (ELST) vlevo, který se prezentoval jako nehomogenní, solidně – cystická expanze pyramidy vlevo. Histologicky byly zastiženy lamely kosti s přilehlým vazivem a s přítomností léze papilárního vzhledu tvořenou centrálně vazivovou a cévnatou tkání, na jejíž papily nasedají poměrně drobné kubické elementy s lehce nepravidelnými hyperchromními jádry. Ojediněle byly zastiženy i drobné cystické formace s obsahem PAS pozitivního eozinofilního materiálu. Imunohistochemicky vykazovaly kubické elementy difuzní expresi vimentinu, epiteliálního membránového antigenu (EMA), cytokeratinu AE1/AE3 a S100 proteinu (pouze slabě). Další vyšetřované markery zahrnující TTF1, PAX8 a CD10 byly negativní. Tumor endolymfatického vaku je vzácný low-grade maligní epiteliální tumor vycházející z endolymfatického vaku v temporální kosti, s incidencí 1:30 000, s popsanými méně než 300 případy. Asi 1/3 z nich je asociována s VHL, autozomálně dominantním familiárním nádorovým syndromem.
We report the case of a 42-year – old female with familiar form von Hippel-Lindau disease (VHL) and recurrent endolymphatic sac tumour (ELST), which was presented like non-homogenous, solid and cystic expansion of the left petrous temporal bone. Histologically, there was found lamellae of bone with adjacent ligament and with papillary projections with fibrovascular core. The papillae were lined by a single layer of cuboidal epithelium with hyperchromatic and lightly pleomorphic nuclei. Sporadically, small cystic formations with eosinophilic, PAS positive secretion were noted. Imunohistochemically, the cuboidal cells showed diffuse positivity for vimentin, epithelial membrane antigen (EMA), cytokeratin AE1/AE3 and S100 protein (weakly). Other markers examined, including TTF1, PAX8 and CD10, were negative. Endolymphatic sac tumour is rare low-grade malignant epithelial tumour arising from the endolymphatic sac in the temporal bone, which occurs in 1 out of 30 000 births, with just fewer than 300 cases reported in the literature. About one third of cases are associated with von Hippel- Lindau disease, an autosomal dominant familial cancer syndrome.

Východiska: Meningeomy jsou častými primární nádory mozku s roční incidencí 3–8 případů na 100 000 obyvatel. Přestože se většinou jedná o benigní onemocnění, zhoršuje často svým charakterem a lokalizací svému nositeli kvalitu života. Základními léčebnými metodami jsou neurochirurgická exstirpace nebo stereotaktické ozáření. Obě metody dosahují velmi dobrých léčebných výsledků, přesto část pacientů trpí recidivou onemocnění vyžadující další intervenci s rizikem vzniku nebo zhoršení neurologického deficitu. Nevelké meningeomy bez růstové tendence a s absencí symptomatologie bývají často sledovány. Na druhou stranu se nelze spokojit s dlouhodobými výsledky léčby meningeomů s agresivnějším biologickým chováním. Známým rizikem recidivy meningeomů je vyšší grade, často je však obtížné identifikovat podskupiny, které vykazují prospěch z kombinované neurochirurgické operace a radioterapie, resp. u kterých je vhodné hledat cesty k další eskalaci léčby, a to i za cenu jistých vedlejších účinků. Pro rozvoj personalizované medicíny se perspektivně jeví analýza hormonální, cytogenetické i epigenetické výbavy nádorů. Cíl: Cílem práce je shrnout poznatky o epidemiologii, léčebných algoritmech a prognóze meningeomů mozku a navrhnout perspektivní prognostické a prediktivní faktory pro maximálně individualizovanou terapii.
Background: Common primary brain tumors are meningiomas with year incidence 3–8 cases per 100 000 people. Even though we mainly speak about benign disease, with its character and localization, it worsens the life quality to its bearer. The main therapeutic methods are neurosurgery extirpation and stereotactic radiosurgery. Despite reaching very good therapeutic results with both methods, some of the patients suffer with recurrence of the disease requiring other interventions with the risk of creating or worsening a neurological deficit. Small meningiomas without growing tendency and with the absence of symptomatology are observed. On the other hand, we cannot be satisfied with long-term results of the therapy of meningiomas with more aggressive biological behavior. A higher grade is a known risk of meningioma recurrence; however, it is often difficult to identify subgroups that show advantage of a combination of neurosurgery and radiosurgery, respectfully at which it is appropriate to look for ways to treatment escalation, and that is with the risk of side effects. The analysis of hormonal, cytogenetics and epigenetics tumor equipment seems to be the most promising for the development of personalized treatment. Purpose: The aim of this work is to sum up the facts about epidemiology, treatment algorithms and the prognosis of brain meningiomas and to suggest perspective prognostic and predictive factors for maximally individualized therapy.

Calcifying pseudoneoplasm of neuroaxis (CAPNON) is a rare lesion of the central nervous system with uncertain histogenesis. We further explored phenotypic spectrum of the entity with respect to possible histogenesis. We collected 5 cases of CAPNONs, performed a detailed morphological assessment, and performed an extensive immunohistochemical analysis (EMA, progesterone receptors, MUC4, SSTR2A, cytokeratin AE1/3, cytokeratin 18, GFAP, neurofilaments, desmin, nestin, synaptophysin, S100 protein, SOX10, CD56, Podoplanin, SATB2, ERG, CD45, and CD163) to elucidate the histogenesis. Furthermore, we performed NGS analysis of one case. The clinical course was benign in all cases. All lesions showed extensively calcified matrix in multilobular arrangement, with a palisade of osteoblast-like cells. Characteristic fibrohyaline matrix was notable in 4/5 cases, while one case was myxoid with rod-like calcifications. Metaplastic lamellar bone was present in 4/5 cases and psammoma bodies were present in 2/5 cases. In 4/5 cases, areas of entrapped glial tissue were present. Expression of EMA was focally present in 3/5 cases, SSTR2A and nestin in 2/5 cases, and progesterone receptor in 2/5 cases in rare cells. We did not observe concomitant expression of EMA, SSTR2A, and progesterone receptor in the same cellular subsets. In one case, NGS showed multiple chromosomal alterations and missense mutation in PIK3CA, attributable to the admixed meningothelial population compatible with meningioma. In another case, biphasic proliferation with myoepithelial phenotype was present. The lesions showed no lineage-specific immunoprofile. Additional pathology was identified in two cases, furthermore suggestive of a possible reactive origin of the lesion.

Colorectal cancer (CRC) is one of the most common cancers worldwide. The patient's prognosis largely depends on the tumor stage at diagnosis. The pathological TNM Classification of Malignant Tumors (pTNM) staging of surgically resected cancers represents the main prognostic factor and guidance for decision-making in CRC patients. However, this approach alone is insufficient as a prognostic predictor because clinical outcomes in patients at the same histological tumor stage can still differ. Recently, significant progress in the treatment of CRC has been made due to improvements in both chemotherapy and surgical management. Immunotherapy-based approaches are one of the most rapidly developing areas of tumor therapy. This review summarizes the current knowledge about the tumor microenvironment (TME), immune response and its interactions with CRC development, immunotherapy and prognosis.

Gaucherova choroba je autozomálně recesivní onemocnění patřící do skupiny tzv. střádavých nemocí. Je známo více než 300 mutací genu GBA1 kódujícího enzym β-glukocerebrozidázu. V České republice jde o velmi vzácné onemocnění a v současné době je u nás léčených 35 pacientů. V naší kazuistice prezentujeme případ šestnáctileté pacientky sledované na Klinice dětského lékařství ve Fakultní nemocnici v Ostravě. Pacientka trpěla od roku 2007 dlouhotrvající trombocytopénií, toho času s progresí, a splenomegálií, která nebyla blíže došetřena. Na ústav patologie byla v květnu roku 2018 zaslána trepanobiopsie s podezřením na myelodysplastický syndrom. Při bioptickém vyšetření nevykazovaly jednotlivé krevní řady žádné dysplastické rysy a počet blastů nebyl zvýšen. Intersticium dřeně bylo ze 70% prostoupené buňkami gaucherova typu s intraplazmatickým vláknitým materiálem. Buňky nabývaly vzhledu „zmačkaného papíru“, exprimovaly protilátku CD68 v imunohistochemickém vyšetření a v histochemickém vyšetření byly pozitivní v barvení PAS a železo (Fe). Na základě morfologického nálezu bylo vyšetřujícím patologem vysloveno podezření na Gaucherovu chorobu. V opakovaně provedeném aspirátu kostní dřeně byly následně zachyceny gaucherovy buňky a dodatečně provedené biochemické vyšetření prokázalo sníženou aktivitu enzymu β-glukocerebrozidázy. Gaucherova choroba je postupně progredující onemocnění, které vyžaduje včasnou diagnózu se zahájením terapie.
Gaucher disease is an autosomal recessive disease belonging to the so-called storage diseases. More than 300 mutations of the GBA1 gene encoding the β-glucocerebrosidase enzyme are known. It is a very rare disease in the Czech Republic. Currently 35 patients are treated. In our case report, we present the case of a 16 year old female patient attending the Clinic of Pediatric Medicine at the University Hospital in Ostrava. Since 2007, the patient has suffered prolonged thrombocytopenia, at the time with progression, and splenomegaly, which has not been further investigated. Trepanobiopsy was sent to the Department of Pathology with suspicion of myelodysplastic syndrome in May of 2018. In the biopsy examination, the individual bloodline did not show dysplastic features and the number of blasts was not increased. The marrow interstitium was 70% permeated with gaucher cells with intraplasmatic fibrous material. Cells were in the appearance of „crumpled paper“ and expressed CD68 in immunohistochemical stain and in histochemical examination of PAS and iron (Fe) staining. Based on a morphological finding, Gaucher‘s disease was suspected. Repeated bone marrow aspirates were subsequently captured by gaucher cells, and a next biochemical examination showed a β-glucocerebrosidase enzyme decrease of activity. Gaucher disease is a progressive disease that requires early diagnosis with the onset of therapy.

Cílem kazuistiky je upozornit na velmi vzácnou patologii – meningeom spánkové kosti. Extrakraniální výskyt meningeomů je ojedinělý a v případě menigeomu spánkové kosti vzácný. Meningeomy spánkové kosti se projevují nespecificky známkami chronického středoušního zánětu s cholesteatomem. Při vyšetření počítačovou tomografií je typická změna architektoniky spánkové kosti bez destrukce kosti, kterou by měl otorinolaryngolog a radiolog znát a při podezření na meningeom indikovat magnetickou rezonanci. Volba léčebné strategie závisí na mnoha faktorech, nejčastější léčbou je kombinace neurochirurgického a kofochirurgického operačního výkonu. Alternativou může být stereotaktické ozáření.
The aim of this case report is to discuss a very rare pathology – temporal bone meningioma. The extracranial location of meningiomas and temporal bone meningioma is a very rare condition. The symptomatology of temporal bone meningiomas is nonspecific, imitating chronic otitis media with cholesteatoma. However, temporal bone meningioma has a distinctive image on computed tomography. There is a change in the architecture without bone destruction that should be known by otorhinolaryngologist and radiologist. Magnetic resonance paging should be performed when temporal bone meningioma is suspected. The management of temporal bone meningiomas depends on a variety of factors. The most common therapy includes a combination of neurosurgical and otological surgery. There are also alternatives like stereotactic irradiation.
