Background: Bone marrow stromal cells (BMSCs) are the source of multipotent stem cells, which are important for regenerative medicine and diagnostic purposes. The isolation of human BMSCs from the bone marrow (BM) cavity using BM aspiration applies the method with collection into tubes containing anticoagulants. Interactions with anticoagulants may affect the characteristics and composition of isolated BMSCs in the culture. Thus, we investigated how anticoagulants in isolation procedures and cultivation affect BMSC molecular characteristics. Methods: BM donors (age: 48-85 years) were recruited from the hematology clinic. BM aspirates were obtained from the iliac crest and divided into tubes coated with ethylenediaminetetraacetic acid (EDTA) or heparin anticoagulants. Isolated BMSCs were analyzed by flow cytometry and RNA-seq analysis. Further cellular and molecular characterizations of BMSCs including CFU, proliferation and differentiation assays, cytometry, bioenergetic assays, metabolomics, immunostaining, and RT-qPCR were performed. Results: The paired samples of isolated BMSCs obtained from the same patient showed increased cellular yield in heparin vs. EDTA samples, accompanied by the increased number of CFU colonies. However, no significant changes in molecular characteristics were found between heparin- and EDTA-isolated BMSCs. On the other hand, RNA-seq analysis revealed an increased expression of genes involved in nucleotide metabolism and cellular metabolism in cultivated vs. non-cultivated BMSCs regardless of the anticoagulant, while genes involved in inflammation and chromatin remodeling were decreased in cultivated vs. non-cultivated BMSCs. Conclusion: The type of anticoagulant in BMSC isolation did not have a significant impact on molecular characteristics and cellular composition, while in vitro cultivation caused the major change in the transcriptomics of BMSCs, which is important for future protocols using BMSCs in regenerative medicine and clinics.

Osteoporóza je systémové metabolické onemocnění skeletu charakterizované sníženou pevností kostí, která má za následek zvýšené riziko zlomenin. Zlomeniny jsou spojeny s vážnými klinickými následky, včetně bolesti, invalidity, ztráty soběstačnosti a předčasného úmrtí, stejně jako vysokých nákladů na zdravotní péči. Včasná identifikace a intervence u pacientů s vysokým rizikem zlomeniny je nutná ke snížení zátěže v důsledku zlomenin. Po identifikaci pacienta s vysokým rizikem zlomeniny by mělo následovat vyhodnocení faktorů přispívajících k nízké denzitě kostního minerálu (BMD) a špatné kvalitě kosti, pádům a zlomeninám. Osteologické vyšetření zahrnuje hodnocení BMD pomocí dvouenergiové rentgenové denzitometrie, anamnézu a fyzikální vyšetření zaměřené na osteoporózu, laboratorní vyšetření a případně další zobrazovací metody. Mohou se objevit i jiná metabolická nebo nádorová onemocnění skeletu než osteoporóza, která vyžadují jiné typy léčby. V tomto přehledu jsou shrnuty základní postupy diagnostiky a diferenciální diagnostiky osteoporózy, které jsou nezbytné před zahájením léčby.
Osteoporosis is a systemic metabolic disease of the skeleton characterized by low bone strength that results in an increased risk of fracture. Fractures are associated with serious clinical consequences, including pain, disability, loss of independence, and death, as well as high healthcare costs. Early identification and intervention with patients at high risk for fracture is needed to reduce the burden of osteoporotic fractures. The identification of a patient at high risk of fracture should be followed by evaluation for factors contributing to low bone mineral density (BMD) and/or low bone quality, falls, and fractures. Components of the osteological evaluation include an assessment of BMD by dual-energy X-ray absorptiometry, osteoporosis-directed medical history and physical exam, laboratory studies, and possibly skeletal imaging. Disorders other than osteoporosis, requiring other types of treatment, may be found. This overview summarizes the basic procedures for the diagnosis and differential diagnosis of osteoporosis, which are necessary before starting treatment.

Stárnutí populace povede k významnému nárůstu incidence osteoporotických zlomenin v následujících dekádách. Osteoporotické zlomeniny jsou ve vyšším věku zatíženy vysokou morbiditou a mortalitou a také vyššími ekonomickými náklady pro společnost ve srovnání se zlomeninami u mladších osob. Přesto je osteoporóza ve vysokém věku nedostatečně diagnostikována a zůstává neléčena. Přístupy k prevenci zlomenin u starší populace ovlivňuje mnoho přidružených faktorů: komorbidity, křehkost, pády, polypragmazie, lékové interakce a další faktory. Většina studií ukazuje pokračující účinnost farmakologických intervencí i ve věkové skupině ≥ 75–80 let, i když jsou data do značné míry limitována velikostí studované populace. V tomto přehledu uvádíme přehled dostupných důkazů o účinnosti a bezpečnosti farmakologické léčby pro snížení rizika zlomenin v této populaci.
With a growing elderly population, the incidence of osteoporotic fractures will increase over the coming decades. In old age, osteoporotic fractures are associated with a substantial burden in terms of morbidity and mortality and higher costs for society compared with fractures in younger individuals. Nevertheless, osteoporosis in old age continues to be underdiagnosed and undertreated. Multiple competing factors: frailty, comorbidities, falls, polypharmacy and drug interactions, and other factors influence our approach to fracture prevention in this population. Most studies show continued efficacy of pharmacologic interventions in age group ≥ 75–80 years, although they are largely limited by small sample sizes. We herein review the available evidence of pharmacologic interventions for fracture risk reduction in this population.

Nízkotraumatické zlomeniny u mladých dospělých osob jsou nejčastěji důsledkem sekundární osteoporózy. Méně často je příčinou zlomenin nebo nízké denzity kostního minerálu (BMD) monogenní onemocnění s opožděnou manifestací v dospělosti (např. osteogenesis imperfecta) nebo idiopatická osteoporóza. Samotná nízká BMD může souviset také s nedosažením vrcholové kostní hmoty u jinak zdravých osob a neznamená vždy zvýšené riziko zlomenin. Pro klinickou praxi je důležité vyloučit sekundární etiologii osteoporózy a jiná metabolická nebo nádorová onemocnění skeletu. Léčba má být kauzální. Antiosteoporotická léčba je indikována, pokud nejsou dostačující kauzální opatření nebo je nelze zajistit, např. při osteoporóze navozené chemoterapií nebo dlouhodobou léčbou glukokortikoidy.
Low-trauma fractures in young adults are most often the result of secondary osteoporosis. Less frequently, the cause of fractures or low bone mineral density (BMD) is a monogenic disease with delayed manifestation in adulthood (e.g. osteogenesis imperfecta) or idiopathic osteoporosis. Low BMD alone may reflect a physiologically low peak bone mass in otherwise healthy individuals and does not mean an increased risk of fractures. For clinical practice, it is important to rule out the secondary causes of osteoporosis and other metabolic or cancer induced bone diseases. Treatment should be causal. Anti-osteoporotic treatment is indicated if causal measures are insufficient or cannot be provided, e.g., in osteoporosis induced by chemotherapy or long-term treatment with glucocorticoids.
