AIM OF STUDY: To determine whether a high dose of levodopa-carbidopa intestinal gel (LCIG), expressed as levodopa equivalent daily dose (LE daily dose), is a risk factor for acute polyneuropathy in patients treated with LCIG. CLINICAL RATIONALE FOR STUDY: Treatment with LCIG is an effective device-assisted therapy in the advanced stages of Parkinson's Disease (PD). Polyneuropathy is a well-known complication of PD treatment. Patients treated with oral levodopa usually suffer from sub-clinical or mild chronic sensory polyneuropathy. However, severe acute polyneuropathy occurs in patients treated with LCIG, which is causally related to the treatment and leads to its immediate discontinuation. The etiology is not yet clear, but some patients with acute polyneuropathy have been given high doses of LCIG. MATERIAL AND METHODS: A retrospective multicentre study of patients treated with LCIG was performed. Patients with acute polyneuropathy were subjected to a detailed analysis including statistical processing. RESULTS: Of 183 patients treated with LCIG in seven centres, six patients (five females, median age 63 years) developed acute polyneuropathy with LCIG discontinuation. The median (interquartile range) initial and final LE daily dose in patients with and without acute polyneuropathy was 3,015 (2,695-3,184) and 1,898 (1,484-2,167) mg, respectively. The final LE daily dose of 2,605 mg cut-off had 83% sensitivity and 93% specificity for the prediction of acute polyneuropathy. CONCLUSIONS AND CLINICAL IMPLICATIONS: The risk of acute polyneuropathy in LCIG-treated patients was associated with a daily LE dose of greater than 2,605 mg or with more than a 62% increase in the daily LE dose during LCIG treatment.

Industrial noise sources are among the environmental noise sources that are ranked second among the causes of ill health in Europe by the World Health Organization. The aim of this paper is to summarize and review of published information focusing on noise annoyance from industrial activities and mining. A search for articles was performed using the bibliographic databases platforms. The epidemiological evidence shows that environmental noise may be associated with cardiovascular and metabolic diseases, impaired cognitive development in children, mental health, post-irritability, and sleep disturbances. As a result of efforts to minimize the effects of industrial noise on human health, the New South Wales Environment Protection Authority published A Guide to the Noise Policy for Industry in 2017, which sets out recommended noise levels, methods, and procedures for noise management based on the latest scientific evidence. Social networks can be used to assess the population's noise annoyance and to verify the effectiveness of the measures. The industrial noise sources are typically defined by low-frequency noise. Low-frequency noise has very low attenuation and is only slightly affected by obstacles, therefore it can be a major cause of night noise annoyance. An association was confirmed between exposure to low-frequency noise and sleep disturbance, psychological problems, cognitive impairment, increased social conflicts, anxiety, emotional instability, nervousness, and reduced mental performance - concentration, and visual perception. In view of the long tradition of mining and industry, the assessment of noise from these activities from the perspective of its impacts on human health is an inherent part of legislative processes. Supraventricular tachycardia (SVT) is a frequent complication of pulmonary hypertension (PH). The most prevalent SVTs are atrial fibrillation (AF) and typical atrial flutter (AFL), followed by focal and macroreentrant atrial tachycardia (AT) and nodal arrhythmia (AV nodal reentry tachycardia or AV reentry tachycardia). SVT is frequently associated with functional deterioration and right ventricular failure in PH patients. According to some data, reestablishing sinus rhythm is associated with clinical improvement. Catheter ablation of typical AFL, nodal tachyarrhythmias, or other less complex focal ATs have been shown to be feasible, acutely effective, and safe in patients with PH. However, the long-term clinical outcome is modified by the recurrence of index arrhythmia and the onset of a new SVT. Due to right atrial dilatation, technical issues can arise when ablation is carried out. The role of catheter ablation in patients with AF or more complex AT is even less effective. The results mirror the success rate in the general AF population with non-paroxysmal AF. However, the data is limited, and electrophysiological procedures are also more often complicated by specific adverse events in a severely frail population. Despite these limitations, catheter ablation is the treatment of choice in less complex SVT, but the indications for AF ablation must be more individualized.
The strike force in combat sports strongly depends on the protective material's mechanical properties and energy absorption capacity. Therefore, this study aimed to estimate the effect of the protective layer thickness and repeated loading on the net force in a falling weight impact test. A falling 8-kg weight dropped from 15 cm, 25 cm, and 50 cm was used to simulate impact peak forces in an upper limb strike. Transfer linear regression functions were identified for three layers of different thickness (1.8 cm, 3.6 cm, and 5.4 cm) between the measured force and undamped force that would be measured if no protective layer was used. A decrease in damping performance under repetitive loading was assessed for the same amount of energy (30 J) absorbed by the specimens. There were 36 specimens examined in 126 tests. When the measuring apparatus was covered with one, two or three layers of Trocellen foam, the undamped force was approximately 2.8, 6.1, and 11.1 times higher, respectively (p < 0.05, R2 ≥ 0.95), than the force measured in the kinetic design. This allows researchers to select the number of layers according to the individual needs in terms of safety and injury prevention. A single layer of Trocellen foam used in this study may be insufficient to ensure the safety of athletes in upper limb strike experiments due to possible compaction of the foam structure for deformations exceeding 80% compression and forces exceeding 5 kN, although no injury was previously observed.

INTRODUCTION: Mumps data were analysed to assess the effect of vaccination on mumps complications and hospitalisation. METHODS: The mumps cases reported to the Czech nationwide surveillance system from 2013 to 2022 were analysed using logistic regression with an odds ratio (aOR) adjusted for age, sex, year of onset and administrative region to measure the association between vaccination and complications or hospitalisation. Adjusted vaccine effectiveness (aVE) was calculated: aVE=(1-aOR)x100. RESULTS: A total of 11,913 mumps cases were reported, of which 6,885 (58%) were male. The median age of the study participants was 16 (range: 0-89 years). No complications occurred in 91% of patients. Mumps orchitis occurred in 633 (9%) male cases. A total of 946 (8%) patients required hospitalisation. The highest proportion of complications and hospitalisations was in the age group 35-44 years. Two doses of vaccine reduced statistically significantly the risk of any complications and of hospitalisation compared with unvaccinated patients: aOR 0.48 (95% CI: 0.37, 0.62), aVE of 52% (95% CI: 38, 63); and aOR 0.43 (95% CI: 0.33, 0.56), aVE of 57% (95% CI: 44, 67), respectively. Two doses showed statistically significant aVE 50% (95% CI: 32, 64) against orchitis, and 59% (95% CI: 23, 79) against meningitis. Among the two-dose recipients, the proportion of complications increased gradually with the time from the second dose. CONCLUSIONS: Our findings demonstrated a protective effect of two-dose vaccination against mumps complications and hospitalisation for mumps. We recommend continuing routine childhood mumps vaccination and maintaining high MMR coverage in Czechia.

OBJECTIVES: Olfactory dysfunction (OD) is a common symptom associated with Covid-19. During the Covid-19 pandemic, the importance of psychophysical olfactory tests and electrophysiological olfactory assessment increased. The purpose of the study was to analyze the psychophysical olfactory tests and the post-covid curves of olfactory event-related potentials (OERPs) and trigeminal event-related potentials (TERPs). METHODS: The prospective study included 98 subjects (62 females / 36 males). The average age was 42 years (range 21-84 years). Group I (n = 77) contained participants who had been infected with Covid-19. They were enrolled in the study at least 1 year after Covid-19. Group II (n = 21) was the healthy normosmic control group. RESULTS: In Group I, the OERPs of 18% participants were absent. Patients in Group I were statistically more likely to have an absence of OERPs (p = 0.036) than subjects in Group II. We did not detect a statistical difference in amplitudes and latencies of the OERPs between Group I and Group II. In Group I, N1 latency of the TERPs was significantly longer (p = 0.002) than in Group II. The amplitude of the N1-P2 interval of the TERPs was significantly lower (p = 0.025) in Group I than in Group II. According to the psychophysical Sniffin stick identification test, hyposmia was detected in 39% in Group I versus 0% in the control Group II. CONCLUSION: OD is a common post-covid symptom. The presence of OERPs is a significant prognostic factor for olfactory function after Covid 19. We detected a lower percentage of absence of OERPs after Covid-19 compared to the previously published studies of post-viral OD and post-infectious OD. For TERPs, we detected a longer N1 latency and a lower amplitude for the N1-P2 interval after Covid-19. OERPs and TERPs can be considered valid biomarkers to evaluate the progress of post-covid OD.

Anti-N-methyl D-aspartate receptor (anti-NMDAR) encephalitis is an autoimmune disorder characterized by IgG antibodies targeting NMDAR. The prevalence is remarkably higher in women and some develop the condition during pregnancy. While immunotherapies have shown good outcomes for pregnant mothers and their infants, the impact on early neurodevelopment remains elusive. This study investigates the effects of anti-NMDAR antibody on the development of primary cortical cultures. Anti-NMDAR antibody was administered to the cultures at day in vitro 5 for the following 5 days to assess dendritic branching and arbor complexity, and at day in vitro 14 for measuring the expression of brain-derived neurotrophic factor (BDNF) and synaptic proteins. Immature cultured neurons treated with anti-NMDAR antibody exhibited impaired dendritic branching and arbor complexity. Interestingly, BDNF expression was unaffected in mature neurons. Additionally, GluN1 expression, a mandatory NMDAR subunit, was significantly reduced, while no significant alterations were observed in PSD-95, gephyrin and synaptophysin expression. These findings shed light on the structural and synaptic impacts of anti-NMDAR antibody on immature neurons, providing evidence for their consequences in early neuronal development.

Lenvatinib is an orally effective tyrosine kinase inhibitor used to treat several types of tumors, including progressive, radioiodine-refractory differentiated thyroid cancer and advanced renal cell carcinoma. Although this drug is increasingly used in therapy, its metabolism and effects on the organism are still not described in detail. Using the rat as an experimental animal model, this study aimed to investigate the metabolism of lenvatinib by rat microsomal enzymes and cytochrome P450 (CYPs) enzymes recombinantly expressed in SupersomesTMin vitro and to assess the effect of lenvatinib on rat CYP expression in vivo. Two metabolites, O-desmethyl lenvatinib, and lenvatinib N-oxide, were produced by rat CYPs in vitro. CYP2A1 and 2C12 were found to be the most effective in forming O-desmethyl lenvatinib, while CYP3A2 was found to primarily form lenvatinib N-oxide. The administration of lenvatinib to rats caused changes in the expression of mRNA and protein, as well as the activity of various CYPs, particularly in an increase in CYP1A1. Thus, the administration of lenvatinib to rats has an impact on the level of CYPs.

BACKGROUND: Previous studies have shown that personality traits (i.e., openness to experience, conscientiousness, and agreeableness) relate to prejudicial attitudes. However, one of the aspects of prejudice is social distance; its association with personality traits was overlooked by previous studies. Therefore, this study examines the connection between the Big Five personality traits and social distance toward certain social groups. PARTICIPANTS AND PROCEDURE: Participants from the general population were recruited through leaflets, the institutional webpage, Facebook, and through the project recruitment website and assessed via paper-and-pencil or online form. A total of 214 participants were included (of whom 68.2% were women and the mean age was 32.65, SD = 11.27, range 18-72) who completed the Bogardus Social Distance Scale and the 44-item Big Five Inventory questionnaire. RESULTS: The results showed a relationship between social distance, agreeableness, and openness to experience. Agreeableness seems to lower the social distance toward all studied groups. In comparison, openness to experience seems to lower the social distance towards groups that evoke more polarized attitudes in the majority (e.g., migrants). Furthermore, the influence of demographic characteristics (i.e., age, education level, and gender) is also significant. CONCLUSIONS: This study shows that personality is significantly related to social distancing and expression of prejudicial attitudes. In particular, agreeableness and openness to experience have different effects on social distance and attitudes towards different groups. Further implications are discussed.
