Jaterní cirhóza i Cushingův syndrom jsou izolovaná onemocnění s charakteristickým klinickým obrazem, který obvykle vede ke stanovení diagnózy. Prezentujeme případ 46letého pacienta, u kterého souběh jaterní cirhózy a Cushingovy nemoci významně ovlivnil jak diagnostický, tak zejména terapeutický postup obou onemocnění.
Liver cirrhosis as well as Cushing's syndrome have quite characteristic clinical picture that usually leads to an early diagnosis. Here we present a rather complex case of a 46-year-old patient with an unusual coincidence of Cushing's disease and cirrhosis causing complications during diagnostic evaluation and particularly during the treatment of both diseases.

OBJECTIVE: The aim of this study was to evaluate the prevalence of venous thromboembolism (VTE) in patients included in the European Registry on Cushing's syndrome (ERCUSYN), compare their clinical characteristics with those who did not develop VTE and identify risk factors for VTE. DESIGN: A retrospective observational cohort study. METHODS: Data extraction from the registry was taken on February, 7, 2022. At the time there were 2174 patients diagnosed with Cushing's syndrome (CS) and 95 VTEs were reported in the database. RESULTS: Of 95 VTE events 70 (74%) were in pituitary-dependent CS patients, 12 (12.5%) in adrenal-dependant CS, 10 (10.5%) in ectopic CS, and 3 (3%) in CS due to other causes. Sex, 24-hour urinary free cortisol (UFC) value at diagnosis, as well as the number of operations remained statistically significant predictors of VTE. Of patients who were treated with at least one surgery, 12 (13%) VTE occurred before and 80 (87%) after the surgery. Nearly half of these VTEs occurred within six months since the operation (36; 45%). Over half of the centers that reported VTE did not routinely anticoagulate CS patients. Anticoagulation schemes varied widely. CONCLUSION: Patients with CS have an elevated risk of developing VTE for an extended period of time. From ERCUSYN cohort patients have higher risk for VTE if they need multiple surgeries to treat CS, are males and have high UFC values at the diagnosis of CS. Since there is no agreement on thromboprohpylaxis, a protocol for VTE prevention that is widely adopted appears to be necessary for patients with CS.

Článek informuje o vybraných novinkách v klinické endokrinologii. Část věnovaná endokrinologii shrnuje především pokroky v medikamentózní léčbě hypofyzárních onemocnění, zejména Cushingova syndromu a akromegalie. Krátce jsou zmíněny inovované doporučené postupy pro management Cushingova syndromu. V sekci věnované štítné žláze je uvedena informace o ETA konsenzu o indikacích pooperační léčby diferencovaného karcinomu štítné žlázy radiojódem a informace o nové léčbě endokrinní orbitopatie monoklonální protilátkou proti 1. typu IGF receptoru teprotumumabem. Rozsáhlejší část článku je věnována novým doporučením pro management incidentalomů nadledvin publikovaným v letošním roce. Nakonec je krátce zmíněna recentně publikovaná studie TRAVRSE studující kardiovaskulární bezpečnost substituční léčby testosteronem u mužů s hypogonadismem.
Present article informs about selected recent developments in clinical endocrinology. Neuroendocrinology section is devoted mainly to developments in medical therapy of pituitary disorders, namely Cushing's syndrome and acromegaly. Update of guidelines on management of Cushing' syndrome is also mentioned. Thyroid section informs about ETA consensus on indication for post-surgical radioiodine therapy in differentiated thyroid cancer and also about new therapy for thyroid-associated ophthalmopathy-monoclonal antibody against IGF-1R teprotumumab. Large section reviews major changes covered in recent clinical practice guidelines on the management of adrenal incidentalomas in comparison with previous ones from 2016. Finally, new study on cardiovascular safety of testosterone replacement therapy in hypogonadal men TRAVERSE is discussed.

Nejčastějším typem dětské obezity, která se vyvíjí v důsledku kombinace genetických a environmentálních faktorů, je obezita prostá. Příčinou sekundární obezity mohou být, vedle vzácných genetických forem obezity a vlivu některých léků, endokrinní choroby, mezi které patří hypotyreóza, Cushingův syndrom, deficit růstového hormonu, hypogonadismus, pseudohypoparatyreóza typu Ia, inzulinom, hypotalamická obezita a syndrom polycystických ovarií. Časné odhalení endokrinní příčiny obezity vede ke snížení morbidity a v některých případech i mortality těchto dětí. Úkolem lékaře je tyto stavy mezi ostatními obézními dětmi rozpoznat a řešit. Při vyšetření příčiny obezity je, vedle rodinné a osobní anamnézy, důležité klinické a laboratorní vyšetření, zhodnocení růstové křivky a křivky BMI (body mass index). Popisujeme patofyziologické mechanismy odpovědné za rozvoj obezity u jednotlivých endokrinopatií, typické klinické a laboratorní nálezy, patologické růstové křivky a křivky vývoje BMI vedoucí k jejich diagnóze.
The most common type of childhood obesity is simple obesity which develops as a result of a combination of certain genetic and environmental factors. Secondary obesity is caused by rare genetic forms of obesity, by certain drugs and by the following endocrine diseases: hypothyroidism, Cushing’s syndrome, growth hormone deficiency, hypogonadism, pseudohypoparathyroidism type Ia, insulinoma, hypothalamic obesity and polycystic ovary syndrome. Early detection of the endocrine cause of obesity leads to a reduction in morbidity and, in some cases, mortality in these children. Doctors must diagnose secondary obesity, distinguish it from simple obesity, and treat it accordingly. When examining causes of obesity, not only personal medical history, but also the family medical history is necessary; clinical examination and the evaluation of the child’s growth curve and BMI (body mass index) are crucial. This text describes pathophysiological mechanisms resulting in endocrinopathy-related obesity, as well as typical clinical and laboratory findings, pathological growth and BMI curves confirming a specific endocrinopathy.

OBJECTIVE: Currently available methods for endogenous cortisol monitoring in patients with hormonal insufficiency rely on measurements of plasma levels only at a single time point; thus, any kind of chronic exposure to cortisol is challenging to evaluate because it requires collecting samples at different time points. Hair cortisol levels acquired longitudinally better reflected chronic exposure (both cortisol synthesis and deposition) and may significantly contribute to better outcomes in glucocorticoid replacement therapies. DESIGN: Twenty-two patients on cortisol substitution therapy were monitored for plasma, urinary, and hair cortisol levels for 18 months to determine whether hair cortisol may serve as a monitoring option for therapy setting and adjustment. METHODS: Plasma and urinary cortisol levels were measured using standardized immunoassay methods, and segmented (∼1 cm) hair cortisol levels were monitored by liquid chromatography coupled to mass spectrometry. A log-normal model of the changes over time was proposed, and Bayesian statistics were used to compare plasma, urinary, and hair cortisol levels over 18 months. RESULTS AND CONCLUSIONS: Hair cortisol levels decreased over time in patients undergoing substitutional therapy. The residual variance of hair cortisol in comparison to plasma or urinary cortisol levels was much lower. Thus, longitudinal monitoring of hair cortisol levels could prove beneficial as a noninvasive tool to reduce the risk of overdosing and improve the overall patient health.

Cíl studie: Popis vzácného případu Cushingova syndromu vyvolaného adrenálním adenomem v těhotenství s úspěšnou terapeutickou strategií. Vlastní pozorování: Prezentujeme výjimečný případ Cushingova syndromu u 30leté tercipary s rozvíjejícími se příznaky od 18. týdne gravidity. Pomocí laboratorních testů byl zjištěn primární Cushingův syndrom s nadprodukcí kortizolu a s nízkým adrenokortikotropním hormonem. Nativní magnetickou rezonancí potvrzen tumor pravé nadledviny. Ve 28. gestačním týdnu byla provedena laparotomická pravostranná adrenalektomie a histologicky potvrzen adrenální adenom. S nasazenou glukokortikoidní substitucí gravidita pokračovala do 38. týdne, kdy byl vaginálně porozen zdravý chlapec. Závěr: Cushingův syndrom se v těhotenství vyskytuje zřídka, často není rozpoznán nebo je diagnostikován postpartálně. Diagnostiku v průběhu gravidity komplikuje fyziologická nadprodukce kortizolu s aktivací hypotalamo-hypofyzární osy a snadná záměna symptomů za potíže vyvolané graviditou. Hlavním laboratorním parametrem je několikanásobně zvýšený volný kortizol v moči a vymizení cirkadiánního rytmu sérového kortizolu. Při včasné diagnostice a chirurgické terapii jsme schopni významně snížit míru výskytu mateřských a fetálních komplikací.
Objective: The description of a rare case of Cushing's syndrome caused by an adrenal adenoma in pregnancy with successful treatment. Case presentation: 30-ear-old Gravida 3 female was admitted to our hospital with hypertension at the 18th week of gestation. Hormonal analyses revealed primary Cushing's syndrome with high plasma cortisol levels and low levels of adrenocorticotropic hormone. Magnetic resonance imaging demonstrated a mass on the right-side of the adrenal gland. Adrenalectomy was performed in the 28th week of gestation and the following histopathology revealed an adrenocortical adenoma. Pregnancy continued until the 38th week of gestation with glucocorticoid replacement therapy and the patient gave birth vaginally to a healthy boy in the 38th week of gestation. Conclusion: Cushing's syndrome in pregnancy rarely occurs; diagnosis may be dismissed or determined after birth in most cases. Misdiagnosis of Cushing's syndrome is common because of physiological increase of corticotropin hormones and cortisol levels and overlapping symptoms that can occur even during physiological pregnancy. Cushing's syndrome should have a place in the differential diagnosis of hypertension in pregnancy (especially before the 20th week of gestation). Analysis of the urinary free cortisol level and circadian rhythm blood cortisol can provide a reasonable strategy to diagnose Cushing's syndrome in pregnant women. Early diagnosis and surgical treatment can significantly reduce maternal and fetal complications.

Cushingův syndrom (CS) je nepříliš časté onemocnění s přibližnou populační prevalencí 40 pacientů/1 milion obyvatel. Prevalence CS je však pravděpodobně vyšší mezi pacienty s diabetes mellitus (DM). U pacientů s DM randomizovanými do studií z ambulantních provozů, u nichž lze předpokládat lepší kontrolu diabetu a nižší výskyt komplikací, byl subklinický CS potvrzen u 0–2,9 %. U pacientů, kteří byli v době randomizace hospitalizováni, se CS vyskytoval v 2–9,4 %. Na základě dostupných dat není v tuto chvíli plošný skrínink CS u pacientů s DM 2. typu doporučován. Přesto je třeba na CS i v jeho subklinické podobě myslet jako na možnou příčinu diabetu, a to u pacientů s hypertenzí, obezitou, mikrovaskulárními komplikacemi, dlouhodobě špatnou kompenzací onemocnění či při nutnosti neobvykle vysokých dávek inzulinu, zejména pak při kombinaci těchto faktorů. Samozřejmostí je hormonální vyšetření pacientů se zachyceným incidentalomem nadledviny.
Cushing´s syndrome is a rare disease with the population prevalence about 40 patients per 1 million inhabitants, the number among patients with diabetes mellitus (DM) is probably higher. The screening of CS among the DM patients showed the prevalence between 0–2.9% in the groups of outpatients and 2–2.9% among the patients admitted to the hospital at the time of randomization. The routine screening of CS among patients with type 2 DM is not recommended at the moment. Nevertheless, the possibility of CS should be taken into the account mainly in patients with hypertension, obesity, microvascular complications, bad level of metabolic control or high insulin doses, especially, if more of these features are present. A hormonal profile of patients with adrenal incidentaloma should be evaluated.
