The Cow's Milk-related Symptom Score (CoMiSS) is an awareness tool for evaluating cow's milk-related symptoms. Previous studies have focused on providing CoMiSS values for healthy and symptomatic infants aged 0-6 months. However, there is a notable gap in the literature concerning CoMiSS values for infants older than 6 months. This cross-sectional study aimed to determine CoMiSS values in presumed healthy infants who have completed 6 months and are up to 12 months old, hereafter referred to as 6 to 12 months old. Physicians from six European countries prospectively determined CoMiSS values in infants attending well-child clinics. Exclusion criteria included preterm delivery, acute or chronic disease, and the consumption of a therapeutic formula, dietary supplements (except vitamins), or medication. The following information was collected: gestational age, gender, age, type of feed (breast milk or infant formula), and complementary feeding. Descriptive statistics were summarized with mean and standard deviation for normally distributed continuous variables, median and IQR for non-normally distributed variables, and differences in CoMiSS values were analyzed with appropriate tests. Data from 609 infants were obtained. The overall median (Q1-Q3) CoMiSS values were 3 (1-5). Significant differences were found across age groups (p < 0.001), but not across groups based on gender (p = 0.551) or feeding type (p = 0.880). Conclusions: This study provided CoMiSS values in presumed healthy infants aged 6-12 months. Additional studies should be conducted to establish the use of CoMiSS to assess cow's milk-related symptoms in infants 6 months and older. This review article presents new developments in the management of cow’s milk protein allergy (CMPA) in infants and children based on the updated 2023 recommendations of the European Society for Paediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN), which focus on the preventionof ABKM.

Alergie na červené maso tzv. neprimátních savců bývá poměrně vzácnou diagnózou. Častějším nálezem je primární alergie na hovězí maso, asociovaná s alergií na bílkovinu kravského mléka, která se vyskytuje především v raném dětském věku. V 90. letech minulého století byly identifikovány dvě nové formy alergických reakcí na červené maso: pork-cat syndrom a alfa-gal syndrom, vyskytující se především v dospělém věku. U obou jednotek je častá zkřížená reaktivita v rámci savčích alergenů a riziko spočívá v tendenci vystupňování alergických reakcí až do fáze systémové alergické reakce. Vzhledem k tomu, že červené maso představuje podstatnou složku stravy, je přínosné vědět o existenci této zkřížené reaktivity a znát rizika následků konzumace těchto potravin.
Allergy to the red meat also called non-primate mammals is a relatively rare diagnosis. More frequent is a primary allergy to beef as- sociated with an allergy to cow’s milk protein, occurring mainly in early childhood. In the 1990s, 2 new forms of allergic reactions to red meat were identified, pork-cat syndrome and alpha-gal syndrome, occurring mainly in adults. In both units, there is a strong cross- -reactivity within mammalian allergens and the risk is in the tendency for aggravated allergic reactions up to anaphylaxis. Since red meat is an essential part of the diet, it is beneficial to know the risks of these diseases.

The Cow's Milk-related Symptom Score (CoMiSSTM) is a scoring system that reflects the appearance and intensity of symptoms possibly related to consumption of cow's milk. The original tool was recently updated by changing the cut-off, and the stool scale and by adding angioedema. There is no data available regarding the natural evolution of CoMiSS in infants with no cow's milk allergy (no-CMA) or a comparison between original and updated CoMiSS values. We determined the original and the updated CoMiSS in infants not diagnosed with cow's milk allergy. The evolution of CoMiSS during the first year of life was assessed repetitively during predefined check-ups at 1.5, 3, 4, 6, 8, 10, and 12 months. The original and updated scores were compared with the Wilcoxon Signed-Rank Test. We also tested the impact of feeding type, age, gender, and order in the family on the CoMiSS. One hundred and twenty-two infants were included. CoMiSS values during the first year of life showed an inverse relation to age. The difference in CoMiSS between the original and updated versions was significant at 6,8,10, and 12 months (p < 0.001), related to the switch from the Bristol Stool Form Scale to the Brussels Infants and Toddlers Stool Scale (BITSS). The difference between both versions of CoMiSS was not significantly different in infants < 6 months (p = 0.999 at 1.5 and 4 months, and p = 0.586 at 3 months, respectively).Conclusion: CoMiSS decreases with age during the first year of life. While there is no difference between the two CoMiSS versions in healthy infants under 6 months of age, the CoMiSS value in the updated version is lower than the original 1 in infants aged 6 to 12 months. Exokrinní pankreatická insuficience je podmíněna nedostatečnou sekrecí pankreatických enzymů ev. jejich předčasnou inaktivací anebo jejich inadekvátní aktivací ve střevě. Hlavní příčinou u dětí je chronická pankreatitida a cystická fibróza. Klinicky se pankreatická malabsorpce projeví neprospíváním, steatoreou s průjmem, azotoreou s poklesem sérového albuminu a proteinů. Cystická fibróza je nejčastějším autozomálně recesivně dědičným onemocněním kavkazské rasy. Onemocnění má charakter multiorgánového postižení. Prognóza velmi závisí na včasné diagnostice a multioborovém léčebném přístupu. Autoři poukazují na stále platný význam potního testu v diagnostice cystické fibrózy prezentací kazuistiky batolete s cystickou fibrózou s negativním novorozeneckým screeningem.
The cause of exocrine pancreatic insufficiency is inadequate secretion of pancreatic enzymes or premature inactivation or inadequate activation in the bowel. The main cause is chronic pancreatitis and cystitic fibrosis. Clinical manifestations of pancreatic malabsorption are weight loss, steatorrhea with diarrehea, azotorhhea, loss of serum albumin and proteins. Cystic fibrosis is the most common autosomal recessive disease of the Caucasian population characterised by chronic multi-organ impairment. Early diagnosis and multi-professional centre care is necessery for optimal management and improved quality of life and survival of patients. The authors point out the importance of the sweat test in diagnosis of cystic fibrosis presentation of a case study of toddler with cystic fibrosis who had a negative newborn screening.

Alergie na bílkoviny kravského mléka (ABKM) je nejčastější alergií kojeneckého věku. Důležitým momentem v její diagnostice je včasné vytipování suspektního pacienta a provedení orálního eliminačně-expozičního testu. S cílem pomoci poskytovatelům zdravotní péče identifikovat rizika alergie vytvořili odborníci skóre symptomů potenciálně souvisejících s alergií na bílkovinu kravského mléka (Cow ́s Milk-related Symptom Score - CoMiSS). Dotazník přináší relevantní přehled o intenzitě symptomů ABKM a napomáhá v hodnocení rizika ABKM u symptomatických pacientů. V roce 2022 byl CoMiSS aktualizován: cut-off se snížil z ≥ 12 na ≥ 10, Bristolská škála stolic byla nahrazena Bruselskou škálou stolic pro kojence a batolata a angiodém byl zařazen jako nový příznak s identickým hodnocením jako kopřivka.
The cow ́s milk protein allergy (CMPA) is the most frequent allergy of infants and toddlers. It is challenging to early recognize the suspected patient and if indicated, start the diagnostic elimination followed by an oral challenge. To help the medical health care provider to identify infant at allergy risk experts published the Cow ́s Milk Related Symptom Score (CoMiSS). The questionnaire brings a relevant overview of CMPA symptoms ́ intensity and helps to work out the CMPA correctly. In 2022 the CoMiSS was updated: the cut-off decreased from ≥ 12 to ≥ 10, the Bristol Stool Scale was substituted by the Brussels Infant and Toddler Stool Scale intended for non-toilet trained subjects, an angioedema was included as a new symptom with equal rating as urticaria.

OBJECTIVES: The Cow's Milk-related Symptom Score (CoMISS) is an awareness tool for evaluating cow's milk-related symptoms in otherwise healthy infants <1 year of age. This study assessed whether replacing the Bristol Stool Form Scale (BSFS) with the Brussels Infants and Toddlers Stool Scale (BITSS) in non-toilet-trained infants would modify the overall CoMiSS and change the clinical approach regarding potential cow's milk allergy. METHODS: Non-toilet-trained infants aged <13 months were assessed by CoMiSS using the 7 images from the BSFS (CoMiSS-BSFS) compared to the 4 images of stools from BITSS (CoMiSS-BITSS). The Wilcoxon signed-rank test and Pearson correlation coefficient were calculated. A post hoc analysis using identical tests was performed in subsets of CoMiSS-BSFS scores ≥10, ≥12, ≤5, and ≥6. RESULTS: Eight hundred forty-four pairwise scores were collected. Applying the Wilcoxon test over the complete dataset, the difference between CoMiSS-BSFS and CoMiSS-BITSS was statistically significant ( P < 0.001). However, there was no significant difference in the subsets with CoMiSS-BSFS ≥10, ≥12, and ≥6 ( P = 0.84, P = 0.48, and P = 0.81, respectively). The significant difference remained restricted to the group with CoMiSS-BSFS ≤5, considered at low risk for CM-related symptoms ( P < 0.001). CONCLUSION: Replacing BSFS with BITSS does not change the cutoff for awareness of possible CM-related symptoms and will not impact the use of CoMiSS in clinical practice. Changes in CoMiSS remained limited to the subgroup with a low risk for CM-related symptoms.
