BACKGROUND: Monoamine oxidase (MAO) inhibitors can interact with selective serotonin reuptake inhibitors (SSRIs)/serotonin-norepinephrine reuptake inhibitors (SNRIs). There is clinical interest surrounding use of ozanimod with SSRIs/SNRIs because the major metabolites of ozanimod are weak inhibitors of MAO-B in vitro. OBJECTIVE: To evaluate the incidence of treatment-emergent adverse events (TEAEs) potentially related to serotonin accumulation (SA) during concomitant ozanimod and SSRI/SNRI use by performing analyses of data from an open-label, oral ozanimod 0.92 mg trial (DAYBREAK; NCT02576717). METHODS: SA narrow (serotonin syndrome, neuroleptic malignant syndrome, and hyperthermia malignant) and broad (terms potentially associated with SA) MedDRA v24.0 searches were performed using TEAE data from participants with relapsing multiple sclerosis who entered DAYBREAK from phase 3 studies (cutoff February 1, 2022). Incidences of TEAEs matching terms from each search were stratified by SSRI/SNRI use. RESULTS: Of 2257 DAYBREAK participants, 274 (12.1%) used an SSRI/SNRI. No participants had TEAEs matching the SA narrow search terms. There was no significant difference in the percentage of participants with ⩾1 TEAE matching the SA broad search for those on versus off SSRIs/SNRIs (on: 12.4%, n = 34/274; off: 15.6%, n = 310/1982, nominal p = 0.1630). CONCLUSION: MedDRA searches showed no increase in TEAEs potentially associated with SA with concomitant SSRI/SNRI and ozanimod use.

BACKGROUND: Considering the recently growing number of potentially traumatic events in Europe, the European Psychiatric Association undertook a study to investigate clinicians' treatment choices for post-traumatic stress disorder (PTSD). METHODS: The case-based analysis included 611 participants, who correctly classified the vignette as a case of PTSD, from Central/ Eastern Europe (CEE) (n = 279), Southern Europe (SE) (n = 92), Northern Europe (NE) (n = 92), and Western Europe (WE) (N = 148). RESULTS: About 82% woulduse antidepressants (sertraline being the most preferred one). Benzodiazepines and antipsychotics were significantly more frequently recommended by participants from CEE (33 and 4%, respectively), compared to participants from NE (11 and 0%) and SE (9% and 3%). About 52% of clinicians recommended trauma-focused cognitive behavior therapy and 35% psychoeducation, irrespective of their origin. In the latent class analysis, we identified four distinct "profiles" of clinicians. In Class 1 (N = 367), psychiatrists would less often recommend any antidepressants. In Class 2 (N = 51), clinicians would recommend trazodone and prolonged exposure therapy. In Class 3 (N = 65), they propose mirtazapine and eye movement desensitization reprocessing therapy. In Class 4 (N = 128), clinicians propose different types of medications and cognitive processing therapy. About 50.1% of participants in each region stated they do not adhere to recognized treatment guidelines. CONCLUSIONS: Clinicians' decisions for PTSD are broadly similar among European psychiatrists, but regional differences suggest the need for more dialogue and education to harmonize practice across Europe and promote the use of guidelines.

Ketamine is clinically used fast-acting antidepressant. Its metabolite hydroxynorketamine (HNK) shows a robust antidepressant effect in animal studies. It is unclear, how these chemically distinct compounds converge on similar neuronal effects. While KET acts mostly as N-methyl-d-aspartate receptor (NMDAR) antagonist, the molecular target of HNK remains enigmatic. Here, we show that KET and HNK converge on rapid inhibition of glutamate release by reducing the release competence of synaptic vesicles and induce nuclear translocation of pCREB that controls expression of neuroplasticity genes connected to KET- and HNK-mediated antidepressant action. Ro25-6981, a selective antagonist of GluN2B, mimics effect of KET indicating that GluN2B-containing NMDAR might mediate the presynaptic effect of KET. Selective antagonist of α7 nicotinic acetylcholine receptors (α7nAChRs) or genetic deletion of Chrna7, its pore-forming subunit, fully abolishes HNK-induced synaptic and nuclear regulations, but leaves KET-dependent cellular effects unaffected. Thus, KET or HNK-induced modulation of synaptic transmission and nuclear translocation of pCREB can be mediated by selective signaling via NMDAR or α7nAChRs, respectively. Due to the rapid metabolism of KET to HNK, it is conceivable that subsequent modulation of glutamatergic and cholinergic neurotransmission affects circuits in a cell-type-specific manner and contributes to the therapeutic potency of KET. This finding promotes further exploration of new combined medications for mood disorders.
Dementia is a syndrome caused by a brain disease in which higher cortical functions, including memory, thinking, orientation, learning, language, judgement, and others are impaired. The impairment of these functions causes significant limitations in the patient’s self-sufficiency over time. The number of cases is increasing exponentially due to the ageing population. This diagnosis significantly negatively affects the quality of life not only of patients, but also of their caregivers. Therefore, emphasis is now being placed on comrehensive therapy. This article serves as a brief overview of the pharmacological treatment. However, non-pharmacological approaches are also a very important part of therapy
Demence je syndrom způsobený onemocněním mozku, při kterém dochází k narušení vyšších kortikálních funkcí, k nimž patří paměť, myšlení, orientace, schopnost učení, jazyk, úsudek a další. Porucha těchto funkcí způsobuje časem významné omezení soběstačnosti pacienta. Počet případů vzhledem ke stárnoucí populaci exponenciálně narůstá a tato nemoc výrazně negativně ovlivňuje kvalitu života nejen pacientů, ale i jejich pečovatelů. Proto je v současné době kladen velký důraz na komplexní terapii této choroby. Článek slouží jako stručný přehled farmakologické léčby, nicméně velmi důležitou součástí terapie jsou i nefarmakologické přístupy.

Touretteův syndrom (TS) je jednou z nejčastějších neuropsychiatrických poruch dětského věku. Obvyklý věk vzniku příznaků je mezi 5. a 6. rokem věku a naprostá většina případů vznikne do 12 let, až čtyřikrát častěji u chlapců než u dívek. Průběh onemocnění je charakteristicky měnlivý v čase, s exacerbacemi, remisemi a změnami rázu tiků. V dospívání se obvykle tiky zmírňují a výrazné pozdní exacerbace se vyskytují zřídka. Od roku 2020 však prudce vzrostl počet nově vzniklých případů tikové poruchy u adolescentních dívek. Jejich projevy jsou inkongruentní s klasickým obrazem TS a svědčí pro funkční (psychogenní) původ, přičemž vykazují nápadně shodné rysy s videozáznamy šířenými na sociálních sítích. Na případech několika našich pacientek ukazujeme typické projevy onemocnění. Uvádíme koncept masové sociogenní nemoci, popisujeme roli sociálních médií při jejím šíření a uvádíme paralely s historickými hromadnými psychickými jevy.
Tourette syndrome (TS) is one of the most common neuropsychiatric disorders of childhood. The usual age of symptom onset is between the ages of 5 and 6 years, and the vast majority of cases emerge by age 12, up to four times more often in boys than in girls. The course of the disease characteristically varies over time, with exacerbations, remissions, and changes in the tic pattern. In adolescence, the tics usually diminish and marked late exacerbations are rare. However, since 2020, the number of new cases of tic disorder has increased sharply in adolescent girls. Their manifestations are incongruent with the classical presentation of TS and are suggestive of a functional (psychogenic) origin. They show strikingly similar features to videos circulated on social media. We present the typical manifestations of the disease in several of our patients. Using the concept of mass sociogenic illness, we describe the role of social media in its spread and draw parallels with historical mass psychiatric phenomena.

Nejvýznamnější mikrovaskulární neurologickou komplikací diabetu mellitu je diabetická neuropatie. Tento termín zahrnuje kromě nejčastější distální senzitivní či senzitivně‐motorické polyneuropatie také neuropatii autonomní a řadu fokálních forem (mononeuropatie, plexopatie). Diagnostika je založena na anamnéze a klinickém vyšetření, volba dalších diagnostických metod (elektromyografické vyšetření, testy autonomních nervových funkcí, hodnocení intraepidermální inervace apod.) záleží na typu postižených nervových vláknech. Základním opatřením v terapii i prevenci rozvoje a progrese diabetické neuropatie je dlouhodobá optimální metabolická kompenzace diabetu a dalších kardiovaskulárních rizikových faktorů a pohybová aktivita. U algických forem se dále uplatňují preparáty ovlivňující neuropatickou bolest, tedy antiepileptika ze skupiny modulátorů funkce α2δ podjednotky kalciových kanálů (gabapentin, pregabalin) a antidepresiva ze skupiny inhibitorů zpětného vychytávání serotoninu a noradrenalinu (duloxetin, venlafaxin) či tricyklických antidepresiv (amitriptylin). Analgetický efekt má i kyselina thioktová (alfa‐lipoová) a řada opioidů. Diabetes mellitus je také nezávislým cerebrovaskulárním rizikovým faktorem a podílí se tedy na vzniku ischemických cévních mozkových příhod. Signifikantně zvyšuje také riziko rozvoje kognitivních poruch
The most important microvascular neurological complication of diabetes mellitus is diabetic neuropathy. In addition to the most common distal sensory or sensory-motor polyneuropathy, this term also includes autonomic neuropathy and a number of focal forms (mononeuropathy, plexopathy). Diagnosis is based on history and clinical examination; the choice of other diagnostic methods (nerve conduction studies and needle electromyography, autonomic nerve function tests, assessment of intraepidermal innervation from skin biopsy, etc.) depends on the type of nerve fibers affected. The basic intervention in the therapy and prevention of the development and progression of diabetic neuropathy is long-term optimal control of diabetes and other cardiovascular risk factors, and physical activity. In painful forms, drugs affecting neuropathic pain are also used, i.e. antiepileptics from the group of modulators of the α2δ subunit of calcium channels (gabapentin, pregabalin) and antidepressants from the group of serotonin and noradrenaline reuptake inhibitors (duloxetine, venlafaxine) or tricyclic antidepressants (amitriptyline). Thioctic acid (alpha-lipoic acid) and a number of opioids also have an analgesic effect. Diabetes mellitus is also an independent cerebrovascular risk factor and thus contributes to the development of ischaemic stroke. It also significantly increases the risk of developing cognitive impairment.

Kazuistika demonstruje úspěšnou ambulantní léčbu pacientky s farmakorezistentní depresí. Přestože moderní medicína již nabízí celou řadu antidepresiv, stále se mnohdy setkáváme s opožděným klinickým nástupem účinku v rozmezí tří až šesti týdnů, nedostatečnou efektivitou či intolerancí léčby, které jsou pro efektivní léčbu značným limitem. Výsledkem je určité procento pacientů, kteří nedosáhnou remise či dlouhodobého snížení symptomů ani po vyčerpání dostupných možností léčby – kombinace antidepresiv, elektrokonvulzivní terapie (ECT), repetitivní transkraniální magnetická stimulace (rTMS), chronoterapie apod. Oproti tomu je u esketaminu evidován promptní efekt a akutní okamžitá úleva od depresivních symptomů, a to včetně sebevražedných myšlenek či plánu k její realizaci (1). Pozitivní efekty léčby esketaminem prokazuje předložená kazuistika pacientky XY, u které se výskyt příznaků depresivního onemocnění od zahájení léčby LP Spravato rapidně snižoval a již po měsíci od zahájení léčby kleslo její skóre na škále MADRS (Montgomery Asberg depression rating scale) z původních 32 na 18 bodů. Pacientka však uváděla subjektivní zlepšení svého stavu již po druhé aplikaci léku. Předložená kazuistika prokazuje zároveň i dlouhodobý efekt léčby, jelikož skóre MADRS zůstalo po dvou měsících užívání stabilně na 18 bodech.
The case report demonstrates the successful outpatient treatment of a patient with treatment-resistant depression (TRD). Although modern medicine already offers a whole range of antidepressants, we still often encounter a delayed clinical onset of effect in the range of three to six weeks, insufficient effectiveness or intolerance of treatment, which are a significant limit for effective treatment. The result is a certain percentage of patients who do not achieve remission or a long-term reduction of symptoms even after exhausting all available treatment options - a combination of antidepressants, electroconvulsive therapy (ECT), repetitive transcranial magnetic stimulation (rTMS), chronotherapy, etc. In contrast, with esketamine, a prompt effect and acute immediate relief from depressive symptoms is recorded including suicidal thoughts or plans to commit suicide (1). The positive effects of esketamine treatment are demonstrated by the presented case report of the patient XY, in whom the occurrence of symptoms of a depressive illness decreased rapidly after starting treatment with Spravato. One month after starting treatment, her score on the MADRS (Montgomery Asberg depression rating scale) dropped from 32 to 18 points. However, the patient reported a subjective improvement in her condition already after the second application of the drug. The presented case report also demonstrates the long-term effect of the treatment, as after two months of using Spravato, as the MADRS score remained stable at 18 points after two months of use.

Sdělení shrnuje současné poznatky o závažných stavech, které mohou vzniknout v souvislosti s podáváním antidepresiv a antipsychotik. Tyto stavy obyčejně vyžadují okamžitou intervenci. Patří k nim akutní a tardivní dystonie, tardivní dyskinezy, neuroleptický maligní syndrom, serotoninový syndrom a syndromy vzniklé z jejich vysazení. Vzhledem k jejich závažnosti a někdy nečekané manifestaci představují závažné nežádoucí účinky podléhající povinnému hlášení farmakovigilančnímu oddělení Státního ústavu pro kontrolu léčiv.
The article summarizes current knowledge of severe states associated with taking antidepressants and antipsychotics. These states usually require immediate intervention and include acute and tardive dystonia, tardive dyskinesias, neuroleptic malignant syndrome, serotonin syndrome and withdrawal-induced syndrome. Regarding their severity, and sometimes unexpectedly manifestation, they represent severe adverse events subjected to compulsory reporting to State Institute for Drug Control, Department of Pharmacovigilance.
