BACKGROUND/OBJECTIVES: Adverse effects of topical glaucoma medications (TGMs) may include development of ocular adnexal disorders. We undertook a study to determine the effect of TGMs on the risk of developing lacrimal drainage obstruction (LDO) and eyelid malposition. SUBJECTS/METHODS: All patients 66 years of age and older in Ontario, Canada initiating TGM and all patients diagnosed with glaucoma/suspected glaucoma but not receiving TGM from 2002 to 2018 were eligible for inclusion in this retrospective cohort study. Using validated healthcare administrative databases, cohorts were identified with TGM and no TGM patients matched 1:2 on sex and birth year. The effect of TGM treatment on risk of surgery for LDO and lid malpositions was estimated using Kaplan-Meier and Cox proportional hazards models. RESULTS: Cohorts included 122,582 patients in the TGM cohort and 232,336 patients in the no TGM cohort. Among the TGM cohort there was decreased event-free survival for entropion (log-rank P < 0.001), trichiasis (P < 0.001), and LDO (P = 0.006), and increased ectropion-free survival (P = 0.007). No difference in ptosis-free survival was detected (P = 0.78). For the TGM cohort there were increased hazards for entropion (hazard ratio [HR] 1.24, 95% confidence interval [CI] 1.12-1.37; P < 0.001), trichiasis (HR 1.74, 95% CI 1.57-1.94; P < 0.001), and LDO (at 15 years: HR 2.39, 95% CI 1.49-3.85; P = 0.004), and a decreased hazard for ectropion (HR 0.89, 95% CI 0.81-0.97; P = 0.008). No association between TGM treatment and ptosis hazard was detected (HR 0.99, 95% CI 0.89-1.09; P = 0.78). CONCLUSIONS: TGMs are associated with an increased risk of undergoing surgery for LDO, entropion, and trichiasis.

PURPOSE: To assess the efficiency and safety of cheek-midface lifting for the ophthalmoplastic reconstructive surgical repair of cicatricial lower eyelid malpositions after previously failed surgery. METHODS: In two ophthalmoplastic clinics, 14 eyelids of 13 patients underwent transconjunctival cheek-midface lifting. In a retrospective audit, changes in margin-reflex distance 2 (MRD2), snap-back test, the position of the eyelid after surgery including improving of the presurgical scleral show, the overall patient's satisfaction as well as complications after surgery were recorded. RESULTS: Following transconjunctival cheek-midface lifting, there was a significant improvement of MRD2, snap-back test, and the scleral show (p < 0.001, respectively). However, in three patients the scleral show persisted despite improved MRD2. All patients were satisfied with the results of the surgery. No post-surgical long-term complications were developed. CONCLUSION: Cheek-midface lifting can be a useful technique in severe cicatricial lower eyelid ectropion after previously failed surgery and is a safe and effective reconstructive method for ophthalmic surgeons with good cosmetical results and little postoperative long-term complications.

Cíl: Cílem práce bylo prokázat spolehlivost a výši kvality operační techniky laterálního kantálního závěsu u korekcí ektropia a entropia. V převážné většině případů byly příčinou nesprávného postavení víček změny involučního původu, ve zbylých případech se jednalo o změny způsobené mírným jizvením po předešlých operacích. Dalším cílem naší práce bylo prokázat, zda u provedených víčkových korekcí technikou laterálního kantálního závěsu došlo k vymizení či alespoň zmírnění symptomů, které jsou s výše zmiňovanými patologiemi neodvratně spjaty. Metodika: Retrospektivní studie proběhla od dubna 2018 do dubna 2020 v Ústřední vojenské nemocnici v Praze. Do souboru bylo zařazeno celkem 43 očí u 33 pacientů. U 23 očí se jednalo o ektropium, u zbývajících 20 očí jsme se zabývali problematikou entropia. Poměr mužů a žen ve sledovaném vzorku byl 17:16. Průměrný věk všech pacientů byl 79 let. Ze studie byli vyloučeni pacienti, u nichž byla použita jiná chirurgická technika. Práce je doplněna o fotodokumentaci zachycující průběh operace a pacienty v předoperačním a pooperačním období. Výsledky: U korekcí entropia nebyly zaznamenány žádné akutní pooperační komplikace či dehiscence v ráně. Sledování probíhalo jeden týden od operace. V 91,3 % případů došlo ke správné korekci postavení dolního víčka a úplnému vymizení symptomů. Pouze u dvou pacientů se nepodařilo požadovaného výsledku docílit. U korekcí entropia se správné korekce dolního víčka podařilo dosáhnout v 95 %, pouze u jednoho pacienta entropium přetrvávalo. Závěr: Korekce entropia a ektropia metodou laterálního kantálního závěsu se ve většině případů ukázala býti metodou bezpečnou, spolehlivou a kvalitní. Správné korekce postavení víček se v obou případech podařilo dosáhnout ve více než 90 % případů. Úleva od symptomů a spokojenost pacientů je vysoká.
Background: Ectropion and entropion are eyelid malpositions associated with a number of unpleasant symptoms. One of several surgical methods that can correct these problems is referred to as the lateral tarsal strip technique. In this paper, we evaluate the postoperative results and quality of this technique. We also examine the complications associated with this surgical method. Methods: The study took place from April 2018 to April 2020 at the Military University Hospital in Prague. The operation was performed by two surgeons. The study included 43 eyes of 33 patients (17 women and 16 men), average age was 79 years. Before the operation, 23 eyes were affected by ectropion and 20 eyes affected by entropion, 35 of the 43 were cases of involutional origin. The study does not include patients who underwent other surgical techniques. Results: There were no perioperative or postoperative complications in the entropion surgery group. Successful correction was achieved, and adverse symptoms disappeared in 91.3% of cases. Correct position of the eyelid was not achieved in two cases. For patients with ectropion, surgical correction was successful in 95% of cases. Ectropion persisted after surgery only in one patient. Conclusion: The lateral tarsal strip technique is a safe, reliable and highly effective surgical technique. Correction of eyelid malposition was achieved in more than 90% of cases. The vast majority of patients were satisfied and reported relief from preoperative symptoms.

We report a case of lamellar ichthyosis and sight-threatening complications of cicatricial ectropion in an adult male patient which was surgically managed with tectonic penetrating keratoplasty. We present a case of autosomal-recessive lamellar ichthyosis in a 47-year-old man who was referred to our outpatient eye clinic for treatment of primary keratouveitis of the right eye with keratolysis and exudation in the anterior chamber. A diagnosis of cicatricial ectropion with serious lagophthalmos was established on examination. The patient underwent tectonic penetrating keratoplasty, cataract extraction, and intra-ocular lens placement with no perioperative complications. The patient was subsequently treated with oral fluconazole 200 mg once daily for 12 days due to a positive fungal culture for Candida albicans and systemic oral acyclovir 250 mg 3 times per day for 12 days as prophylaxis for a labial herpetic infection. Post-operative complications included corneal rejection and nonhealing neurotropic epithelial defect of the graft. Long-term treatment with topical cyclosporine (Ikervis®) and dexamethasone led to resolution of the corneal rejection. Lubrication with artificial tears containing hyaluronic acid, perfluorohexyl octane (Evotears®), and vitamin A ointment led to symptomatic relief of dry eye disease. The patient was referred to a dermatologist and was started on systemic retinoid acitretin at a dose of 0.5 mg/kg per day. Ten months after surgery, the patient's visual acuity was 0.1 based on the Snellen chart and the corneal graft was stable. Infection in the cornea can rapidly progress to corneal melting in patients with severe cicatricial ectropion. A good patient outcome depends on the interdisciplinary approach to patient management by the ophthalmologist, dermatologist, and plastic surgeon.

Iatrogenic ectropion with sagging of the lower eyelid after failed excessive lower eyelid blepharoplasty is a severe complication in aesthetic surgery. Traditionally, free skin grafting is the method of choice for correction. This overview presents the cheek-midface lift as a useful and powerful method for the correction of ectropion following excessive lower eyelid blepharoplasty. This technique, which can be performed with the patient under local anesthesia, enables good functional as well as aesthetic outcomes.

Cíl práce: V současné medicíně je kladen velký důraz na časné uzavírání defektů povrchu těla, které nejen přispívá k dobrému a rychlému hojení, ale samozřejmě i uzavírá vstupní bránu možné infekce. Nejsnadnějším uzávěrem defektu je přímá sutura vzniklé rány. Některé defekty jsou však takového rozsahu, že je nutno jejich uzávěr řešit jiným způsobem, než prostou suturou, například otočnou lalokovou plastikou. V práci jsou hodnoceny výsledky lalokové operační techniky u 56 pacientů. Metodika: Jedná se retrospektivní přehled 56 pacientů odoperovaných autorem v roce 2011-2012, u kterých byla jako operační technika použita místní otočná laloková plastika. U 46 pacientů byl důvod lalokové plastiky tumor víček. U 35 pacientů pro bazaliom dolního víčka, u 8 pacientů pro bazaliom horního víčka a u 3 pacientů pro bazaliom vnitřního koutku. U 10 pacientů byl důvod k zákroku ektropium dolního víčka, u 2 pacientů oboustranně. Práce je doplněna fotodokumentací operačních postupů a v tabulce podrobným rozborem velikostí tumorů a odpovídajících laloků. Výsledky: Na základě zkušeností s 56 otočnými lalokovými plastikami, které byly uskutečněné autorem v průběhu 2 let na Oční klinice MNUL byly sledovány výsledky použití otočných kožních laloků u 46 bazaliomů víček, k úpravě 12 ektropií dolních víček. Sledována byla úšpěšnost a recidivy excize tumoru 2 mm do zdravé tkáně. Uvedená operační technika byla u všech pacientů schopna zabezpečit uzavření defektu víčka po odstranění tumoru nebo navrátit správné anatomické postavení víček do normy u ektropií. U 2 pacientů s bazaliomem dolního víčka byla použita doplňující technika laterální kantotomie k uvolnění tahu v ráně. Statistické výsledky počtu recidiv u excizí vedených 2 mm od okrajů tumoru do zdravé tkáně potvrzují relativní bezpečnost takto široké excize a naše výsledky (4,34 %) se pouze nepatrně liší od výsledků uvedených v literatuře (4,0 %).
Aim: In the contemporary medicine, strong emphasis is put on early closing of body surface defects, which contributes to good and early healing, but, of course, closes the entrance gate for possible infection as well. The easiest closing of the defect is the direct suture of the emerged wound. But some defects are of such an extent, so their closure is necessary to handle other way than simple suture, e.g. by rotating flap plastic surgery. In the paper are evaluated results of flap surgery technique in 56 patients. Material and Methods: It is a retrospective overview of 56 patients operated on by the author in the years 2011–2012, in whom the rotating flap plastic surgery was used as surgery technique. In 46 patients, the reason of flap plastic surgery technique was tumor of the eyelids. In 35 patients, it was due to the basalioma of the lower eyelid, in 8 patients due to the basalioma of the upper eyelid, and in three patients due to the basalioma of the medial cantus region. In 10 patients, the reason of the surgery was ectropion of the lower eyelid, and in two of them bilaterally. The paper is supplemented by surgical procedures photographs, and by a table, with detailed analysis of the sizes of tumors and corresponding flaps as well. Results: According to the experience with 56 rotating flap plastic surgeries, performed by author during the period of two years at the Department of Ophthalmology, Masaryk Hospital in Ústí nad Labem, Czech Republic. E.U., the results of the use of rotating skin flaps in 46 eyelids basalioma, of correcting 12 ectropions of the lower eyelid, and reliability of the tumor’s excision with 2 mm surrounding healthy tissue rim were followed-up. Using the mentioned surgery technique, it was possible in all patients to secure the closing of the eyelid defect, or to return to normal and correct anatomical position of the eyelids in ectropions. In two patients with basalioma of the lower eyelid, the additional technique of lateral cantotomy was used to relieve the tension in the wound. Statistical results of recurrences number in tumors’ excisions with 2 mm safety margin in to the healthy tissue confirm the relative safety of such wide excision, and our results (4.34 %) differ only slightly from the results published in the literature (4.0 %).
