BACKGROUND: Despite high initial sensitivity to chemotherapy, TNBC is associated with a poor prognosis, highlighting the need for novel therapeutic strategies. The aim of this multicenter, randomized, open-label phase II trial was to assess the efficacy of ixabepilone as monotherapy, and the combination of ixabepilone with cetuximab, as first-line treatment in patients with triple-negative locally advanced nonresectable and/or metastatic breast cancer. PATIENTS AND METHODS: Women were randomly assigned to receive either ixabepilone (40 mg/m(2)) every 21 days (n = 40), or ixabepilone (40 mg/m(2)) every 21 days with cetuximab (400 mg/m(2) loading dose, followed by 250 mg/m(2)) once weekly (n = 39). The primary end point of the trial was to estimate the response rates of ixabepilone monotherapy and ixabepilone with cetuximab combination therapy. RESULTS: Of 79 randomized patients, 77 were treated. Based on an intent-to-treat analysis, an objective response rate of 30% (95% confidence interval [CI], 16.6-46.5) was observed in the monotherapy arm, and 35.9% (95% CI, 21.2-52.8) in the combination arm. Median progression-free survival was 4.1 months in both treatment groups. Safety findings were consistent with the known individual toxicity profiles of ixabepilone and cetuximab. Skin and subcutaneous tissue disorders were more common with combination therapy, as were discontinuations because of adverse events. CONCLUSION: Ixabepilone monotherapy and the ixabepilone and cetuximab combination demonstrated similar levels of clinical activity in first-line treatment of advanced TNBC, with a predictable safety profile. Further investigation of novel therapies for TNBC is required to improve patient outcomes.

An important aspect of synaptic plasticity in the brain is axonal transport of essential components such as mitochondria from the soma to the synapse. For uninterrupted transport of cellular cargo down the axon, functional microtubules are required. Altered microtubule dynamics induced by changes in expression of microtubule-associated tau protein affects normal microtubule function and interferes with axonal transport. Here we investigate the effects of the nontaxoid-binding-site microtubule-stabilizing agents peloruside A (PelA) and laulimalide, compared with the taxoid-site-binding agents paclitaxel (Ptx) and ixabepilone, on axonal transport of mitochondria in 1-day-old rat pup cerebral cortical neuron cultures. The differences in effects of these two types of compound on mitochondrial trafficking were specifically compared under conditions of excess tau expression. PelA and laulimalide had no adverse effects on their own on mitochondrial transport compared with Ptx and ixabepilone, which inhibited mitochondrial run length at higher concentrations. PelA, like Ptx, was able to partially reverse the blocked mitochondrial transport seen in ECFP-htau40-overexpressing neurons, although at higher concentrations of microtubule-stabilizing agent, the PelA response was improved over the Ptx response. These results support a neuroprotective effect of microtubule stabilization in maintaining axonal transport in neurons overexpressing tau protein and may be beneficial in reducing the severity of neurodegenerative diseases such as Alzheimer's disease.

Karcinom prostaty je nejčastějším solidním nádorovým onemocněním a druhou nejčastější příčinou úmrtí na onkologická onemocnění v České republice, západních zemích a v USA. Úmrtí nemocných způsobuje kastračně rezistentní varianta onemocnění, progredující lokálně a metastazující, nejčastěji do kostí. V širším slova smyslu rozumíme progresí nárůst hladiny prostatického specifického antigenu, vznik nových ložisek či zvětšování již diagnosticky ověřených a/nebo klinické zhoršení obtíží. Primární citlivost nádorové buňky na klasickou androgenní deprivaci mizí a nádor i přesto progreduje. U případů, kdy není dostatečně účinná nebo kdy není možná dosud nejvíce používaná chemoterapie docetaxelem, měl lékař jen velmi omezené další možnosti léčby. Nové objevy mechanismů signálních cest stimulujících buněčný androgenní receptor (AR) v prostředí androgenní deprivace vedou logicky k hledání molekul, které stimulaci inhibují. Cílem je zpomalit průběh onemocnění a prodloužit přežití. Stávající přípravky lze rozdělit do skupin podle místa a způsobu účinku. Vedle cíleného zásahu zaměřeného na AR zahrnuje nové léčebné armamentarium non-androgenní signální cíle, dále možnost imunoterapie, imunomodulace, některé enzymatické blokátory, nová chemoterapeutika a cílené ovlivnění buněk kostních metastáz. Tento nadějný směr je označován jako cílená léčba. Článek předkládá aktuální přehled přípravků, jejichž ověřování ve studiích velmi pokročilo a přináší slibné výsledky.
Prostate cancer is the most common type of solid tumors and the second leading cause of cancer deaths in the Czech Republic, Western countries and the USA. Death of the patients is caused by castrate-resistant variant of the disease, which progresses locally and metastasizes mostly to the bones. In a broader sense, the progression is understood as an increase in the level of prostate-specific antigen, the emergence of new foci or enlargement of already diagnostically proven ones, and/or clinical worsening of symptoms. Primary sensitivity of tumor cells to conventional androgen deprivation disappears and the tumor still progresses. In cases where the docetaxel chemotherapy was impossible or not efficient enough, physicians had only very limited additional treatment options. New discoveries in mechanisms of cellular signaling pathways stimulating the androgen receptor (AR) in an androgen-deficient environment lead logically to the search of drug molecules that inhibit the Stimulation. The aim is to slow the progression of the disease and to prolong survival. Existing products can be divided into groups according to site and mechanism of action. Apart from targeted intervention aimed atAR, new therapeutic armamentarium includes non-androgenic signaling targets, the possibility of immunotherapy immunomodulation, certain enzymatic blockers, new chemotherapeutics, and targeted interference of cells of bone metastases. This encouraging way is referred to as targeted therapy. The paper presents an overview of current products, whose verification in studies progressed remarkably and is yielding promising results.

BACKGROUND: New agents that are active in patients with metastatic colorectal cancer are needed. Patupilone (EPO906; epothilone B) is a novel microtubule-stabilising agent. METHODS: Patients with advanced colon cancer who progressed after prior treatment regimens received intravenous patupilone (6.5-10.0 mg m(-2)) once every 3 weeks by a 20-min infusion (20MI), 24-h continuous infusion (CI-1D) or 5-day intermittent 16-h infusion (16HI-5D). Adverse events (AEs), dose-limiting toxicities (DLTs), pharmacokinetics and anti-tumour activity were assessed. RESULTS: Sixty patients were enrolled. The maximum tolerated dose (MTD) was not reached in the 20MI arm (n=31), as no DLTs were observed. Three patients in the CI-1D arm (n=26) experienced 1 DLT each at 7.5, 8.0 and 9.0 mg m(-2), but MTD was not reached. However, the prolonged 16HI-5D arm was terminated at 6.5 mg m(-2) after two of the three patients developed a DLT. Diarrhoea was the most common AE and DLT, with increased severity at the higher doses (9.0 and 10.0 mg m(-2)). Grade 3 or 4 diarrhoea was observed in 11 (35%) of the patients in the 20MI arm, 4 (15%) of the patients in the CI-1D arm and 2 (67%) of the patients in the 16HI-5D arm. Patupilone activity was observed in the 20MI arm with a disease control rate of 58%, including four confirmed partial responses. The disease control rate in CI-1D arm was 39%. CONCLUSION: Patupilone given once every 3 weeks as a 20-min infusion had promising anti-tumour activity and manageable safety profile at doses that demonstrated therapeutic efficacy.

Chemoterapie je jednou ze základních léčebných metod používaných v léčbě zhoubných nádorových onemocnění. V kombinaci s ostatními léčebnými modalitami dosahuje vynikajících výsledků. Cytostatika se liší různými mechanismy účinku. Jejich neselektivita a toxicita vede ke snaze identifikovat nové molekuly, které by posunuly terapeutické možnosti onkologie. Snahou je vytvořit takové látky, které by využívaly rozdílné pochody mezi zdravými a nádorovými buňkami a snažily se o diferencovanost zásahu. Hormonální terapie je další z onkologických systémových léčebných modalit. Pracuje na principu přítomnosti specifického znaku na nádorové buňce a jeho interakci s hormonálním preparátem. Klinická data prokázala efektivitu hormonální terapie pouze u karcinomu prostaty, karcinomu mléčné žlázy a karcinomu endometria.
Chemotherapy is one of the basic treating modalities applied in the treatment of malignant tumours. It reaches excellent effects in combination with other treating modalities. Cytostatics differ in their mechanisms of effect. Their non-selectivity and toxicity results into effort to identify new molecules that would shift forward therapeutical possibilities of oncology. The effort is to create such substances utilizing different procedures between healthy and oncogenous cells and to try to produce differentiated affect. Hormonal therapy is another oncological system treating modalities. It works on the principle of the presence of a specific feature found on the tumorous cell and its interaction with hormons. Clinical data have proved effectiveness of hormonal therapy only in the prostate cancer, breast cancer and endometrial cancer.

The taxanes that target microtubules are among the most active drugs in breast cancer treatment; however, resistance to these agents remains a significant issue for many patients. The epothilones are a novel class of nontaxane, microtubule-targeting agents, currently being evaluated in varying stages of clinical trials. Ixabepilone is the first epothilone analogue to receive US Food and Drug Administration approval in the United States for the treatment of metastatic breast cancer and as such will be the primary focus of this review. RECENT FINDINGS: Multiple phase II trials evaluating ixabepilone in different populations of patients with metastatic breast cancer as well as a phase III trial in combination with capecitabine have recently been published. SUMMARY: Phase II trials clearly demonstrate the activity of single-agent ixabepilone in both taxane-untreated and taxane-treated metastatic breast cancer. Although the highest activity was seen in early lines of therapy, there was also clear evidence of activity in heavily pretreated patients. Ixabepilone has also been evaluated in combination with capecitabine in a randomized, phase III trial demonstrating a benefit for the combination compared with single-agent capecitabine for patients resistant to anthracyclines and taxanes. In general, ixabepilone administered as a single-agent and in combination with capecitabine has been reasonably well tolerated.

Ixabepilon je semisyntetický analog epothilonu B, jehož mechanismus účinku spočívá v inhibici depolymerizace mikrotubulů. Jeho vazebné místo se však liší od vazebného místa taxanů. Ixabepilon je odolný vůči několika mechanismům lékové rezistence.

Epothilony jsou novou skupinou cytotoxických molekul, původně identifikovaných jako metabolity produkované myxobakterií Sorangium cellulosum. Klinický úspěch paclitaxelu a docetaxelu stimuloval výzkum dalších látek se stejným mechanismem účinku a vedl k objevu tří nových typů přírodních látek netaxanového typu: epothilony A a B z půdní bakterie, discodermolid z mořské houby a eleutherobiny a sarcodictyiny z korálů. Všechny tři skupiny látek stabilizují mikrotubuly a kompetitivně inhibují vazbu paclitaxelu na polymery tubulinu, což svědčí o tom, že jejich vazebné místo je stejné. Epothilony jsou přírodní látky stabilizující mikrotubuly se silnou in vivo a in vitro protinádorovou aktivitou. Proto byly epothilony uvažovány jako potenciální chemoterapeutické léky v důsledku jejich zásahu do cytoskeletu. Nedávné studie na rakovinných buněčných liniích a u lidí s nádorovým onemocněním signalizují jejich vyšší účinnost ve srovnání s taxany. Mechanismus jejich účinku je podobný s taxany, ale jejich chemická struktura je jednodušší a jsou lépe rozpustné ve vodě. Navíc mohou být epothilony připravovány ve velkém množství fermentací bakterií a vykazují rovněž aktivitu proti buněčným liniím a tumorům, které jsou rezistentní vůči mnoha jiným lékům. Kvůli vysoké aktivitě a klinické potřebě léčení rakoviny jsou epothilony cílem mnoha studií. Cílem tohoto článku je přiblížit problematiku epothilonů českému čtenáři.
Epothilones are a new group of cytotoxic molecules, originally identified as metabolites produced by the myxobacterium Sorangium cellulosum. Clinical success of paclitaxel and docetaxel stimulated the research of further substances with the same mechanism of effects and resulted in discovering three new types of natural substances of non-taxane type: Epothilones A and B from a soil bacterium, discodermolide from a sponge and eleutherobines and sarcodictyines from corals. All the three groups of substances stabilize microtubules and competitively inhibit the bond of paclitaxel to polymers of tubulin, which suggests that their binding sites are the same. Epothilones are natural substances stabilizing microtubules with strong in vivo and in vitro anti-cancer activity. Thus, epothilones were considered as potential chemotherapeutic drugs due to their intervention into the cytoskeleton. Recent studies in cancer cell lines and in humans with cancer diseases indicate their higher efficacy compared with taxanes. The mechanism of their effects is similar to that of taxanes, but their chemical structure is simpler and their solubility in water is higher. In addition, epothilones may be prepared in large amounts by the fermentation of bacteria and they also exert activities against cell lines and tumours, which are resistant to many other drugs. Due to their high activity and to clinical requirements for the treatment of cancer, epothilones are targets of many studies. The purpose of the present article is to familiarize Czech readers with problems of epothilones.
