BACKGROUND: Hypospadias is a common male birth defect that has shown widespread variation in reported prevalence estimates. Many countries have reported increasing trends over recent decades. OBJECTIVE: To analyze the prevalence and trends of hypospadias for 27 international programs over a 31-yr period. DESIGN, SETTING, AND PARTICIPANTS: The study population included live births, stillbirths, and elective terminations of pregnancy diagnosed with hypospadias during 1980-2010 from 27 surveillance programs around the world. OUTCOME MEASUREMENTS AND STATISTICAL ANALYSIS: We used joinpoint regression to analyze changes over time in international total prevalence of hypospadias across programs, prevalence for each specific program, and prevalence across different degrees of severity of hypospadias. RESULTS AND LIMITATIONS: The international total prevalence of hypospadias for all years was 20.9 (95% confidence interval: 19.2-22.6) per 10000 births. The prevalence for each program ranged from 2.1 to 39.1 per 10000 births. The international total prevalence increased 1.6 times during the study period, by 0.25 cases per 10000 births per year (p<0.05). When analyzed separately, there were increasing trends for first-, second-, and third-degree hypospadias during the early 1990s to mid-2000s. The majority of programs (61.9%) had a significantly increasing trend during many of the years evaluated. Limitations include known differences in data collection methods across programs. CONCLUSIONS: Although there have been changes in clinical practice and registry ascertainment over time in some countries, the consistency in the observed increasing trends across many programs and by degrees of severity suggests that the total prevalence of hypospadias may be increasing in many countries. This observation is contrary to some previous reports that suggested that the total prevalence of hypospadias was no longer increasing in recent decades. PATIENT SUMMARY: We report on the prevalence and trends of hypospadias among 27 birth defect surveillance systems, which indicate that the prevalence of hypospadias continues to increase internationally.

INTRODUCTION: We found midline epithelial adhesions in the glandar urethral plate in patients with hypospadias. After dissolution, a blind epithelized channel becomes visualized inside of the plate pointing to immature embryonic luminization. In addition it reveals that the epithelized surface of the distal urethral plate is larger than previously considered. OBJECTIVE: To determine the incidence and extent of these new anatomical details of urethral plate in hypospadias patients. METHODS: We prospectively assessed the detailed anatomy of the urethral plate in 72 consecutive patients with hypospadias. We recorded the presence of adhesions in the middle of the glandar urethral groove that can be easily dissoluted (dissolution line - D-line). We recorded the plate width before and after D-line dissolution, the presence of the hidden blind channel at continuation of D-line (channel type-A) and of the visible blind channel between D-line and urethral hypospadiac meatus (type-B) (Figure). In 62 patients, where the urethral plate tubularization was considered (Duplay, TIP), septs between channels were opened in the midline and a final width of the plate was measured by rolling the plate around a tube. RESULTS: Midline adhesions (D-line) were found in all 72 patients. Mean length of D-line was 5.13 ± O.17 mm. Mean plate width before dissolution was 5.9 ± 0.15 mm, and after dissolution 7.8 ± 0.16 mm. A blind channel of type A was detected in 22 patients (31%), type B in 24 (33%), type A and B in 16 (22%), and none in 10 patients (14%). Mean final plate width after D-line dissolution and opening of septs between channels in 62 patients with urethral plate tubularization was 8.7 ± 0.15 mm. DISCUSSION: The main contribution of our study is a new perspective of distal urethral plate anatomy that enables enlargement of the epithelized surface of the distal urethral plate by dissolution of the preexisting epithelized groove and opening of epithelized channels within the plate. To the best of our knowledge, this anatomical anomaly has not been described previously. CONCLUSIONS: The distal urethral plate of all hypospadias patients is partially "folded" in the midline by epithelial adhesions of different depth and extent that may be easily dissoluted. In half of the patients (53%) the "folded" part of the plate continues proximally as a blind channel inside the urethral plate (type A channel). Opening of these structures together with the well-known urethral plate pits (type B channel) helps augment the width and the overall epithelized surface of the distal urethral plate.

Hypospadie je jednou z nejčastěji se vyskytujících vrozených vad močových cest. Jedná se o rozštěp močové trubice na spodní straně a postihuje jednoho ze 125–300 narozených chlapců. Na vzniku se podílejí genetické i hormonální vlivy. Dle uložení se dělí na přední (glandulární, koronární, subkoronární), střední (penilní) a zadní (penoskrotální, skrotální, perineální). Hypospadie mohou způsobovat postiženým psychické trauma, u jiných forem poruchy mikce a ejakulace, u vad spojených s chordou navíc nemožnost sexuálního styku. Vadu posuzuje dětský urolog a léčí se chirurgicky. Vysoká odpovědnost vložená do rukou lékaře i sestry při ošetřování malých pacientů s hypospadií vyžaduje maximální důraz na vlastnosti, jako je pečlivost, spolehlivost, emoční stabilita, trpělivost, schopnost týmové spolupráce, projevování vzájemné úcty a korektnosti v jednání v týmu včetně rodičů dítěte, schopnost improvizovat, disponovat kreativitou. Současně vyžaduje i fyzickou zdatnost, výdrž a zručnost. Cílem práce celého týmu je zdárně ukončený léčebný proces a spokojený dětský pacient včetně jeho rodičů.
Hypospadias is one of the most common birth defects of the urinary tract. This is a cleft of the urethra on the underside and affects oneof the 125–300 boys born. Genetic and hormonal influences are involved in the development. They are divided into front (glandular,coronary, subcoronary), middle (penile) and posterior (post-bodied, scrotal, perineal). Hypospadias can cause psychological trauma inother forms of mucosal and ejaculatory disorders, in addition to the possibility of sexual intercourse with the chord associated with thechord. The defect is assessed by a child urologist and treated surgically. High responsibility placed on nurses in the care of small hyperspacepatients requires the utmost emphasis on features such as care, reliability, emotional stability, patience, teamwork, mutual respectand correctness in teamwork, including the parents of the child, the ability to improvise. At the same time, physical fitness, enduranceand skills. The goal of the whole team is to successfully complete the healing process and a satisfied child patient including his parents.

Hypospadie je výsledkem vývojové poruchy penisu charakterizované hypoplazií ventrální části penisu. Diagnóza hypospadie je možná až po narození. Mírné formy hypospadie jsou pouze kosmetickým defektem, závažnější formy jsou spojeny s mikčními a sexuálními problémy. Pokročilé formy hypospadie, spojené s nehmatnou gonádou, mohou být projevem poruchy vývoje pohlaví a měly by být podrobně vyšetřeny včetně genetického screeningu.
Hypospadia is the result of abnormal development of penis characterised by hypoplasia of ventral part of penis. The diagnosis of hypospadia is possible after birth. Moderate forms of hypospadia bring only cosmetic defect, more sever forms are connected with voiding and sexual problems. Severe forms of hypospadia connected with unpalpable gonade could be a manifestation of sexual differentiation disorder and should by widely investigated included genetic screening.

Hypospadie představují častou vrozenou vadu mužského genitálu, která je charakterizována nedostatečným vývojem a hypoplazií tkání na ventrální ploše penisu. Z klinického pohledu představují hypospadie široké spektrum postižení od distálních forem, které jsou pouze kosmetickou vadou, po těžké proximální formy spojené s poruchou močení a sexuálních funkcí penisu. Léčba je výhradně chirurgická. Rekonstrukční výkon se skládá ze tří základních kroků: korekce ventrální anulace penisu (chordy), rekonstrukce uretry (uretroplastika) a rekonstrukce ventrální plochy penisu (meatoplastika, glandoplastika, rekonstrukce kožního krytu penisu a případná plastika předkožky). Detaily operačního postupu a perioperační péče se velmi liší mezi jednotlivými pracovišti. Je to důsledek neexistujícího obecně přijatého systému třídění hypospadií na základě přísného biometrického popisu a také malého počtu prospektivních studií hodnotících dlouhodobé kosmetické i funkční výsledky léčby a dopady na kvalitu života pacienta.
Hypospadias is a common malformation of male genitalia that can be defined as an insufficient development and hypoplasia of the tissues forming the ventral aspect of the penis. From clinical point hypospadias represent a large spectrum of malformations from mild distal to severe proximal forms. Distal hypospadias are mainly a cosmetic issue only; however proximal ones have an important impact on micturation and sexual functions of penis. Treatment is surgical only. In general, hypospadias surgery involves three main steps: straightening of the penis (i. e., correction of chordee); reconstruction of the missing urethra (i. e., urethroplasty); and reconstruction of the tissues forming the ventral radius of the penis (i. e., glans, corpus spongiosum, and skin). Surgical methods and perioperative care vary widely between different authors. It is the result of a nonexistent generally accepted grading system of hypospadias based on rigorous biometric description, and a small number of prospective studies evaluating the long-term cosmetic and functional outcomes of treatment and impact on patients' quality of life.

Smithův-Lemliho-Opitzův syndrom (SLOS) je ve většině zemí světa v pořadí třetí nejčastější dědičné metabolické onemocnění. Cílem příspěvku je poukázat na výjimečný případ dítěte s diagnózou SLOS. Prostřednictvím kazuistiky je prezentována ošetřovatelská péče během hospitalizace, jakož i v domácím prostředí. Zdrojem informací byla zdravotnická dokumentace dítěte a strukturovaný nestandardizovaný rozhovor s matkou dítěte.

Kryptorchizmus je porucha sestupu varlete, léčba je chirurgická. Rizikem kryptorchizmu je sterilita a vznik nádoru varlete u dospělého muže. Léčbu – stažení varlete do šourku (orchidopexe) doporučujeme provést před druhým rokem věku dítěte. Fimóza – zúžená předkožka – je do roku věku normální nález, poté doporučujeme lokální aplikaci kortikoidní masti po dobu 6 týdnů, při neúspěchu dle nálezu cirkumcizi. Konglutinace předkožky – dostatečně široká předkožka slepená s glandem – se neuvolňuje až do školního věku dítěte, kortikoidy nejsou indikované. Hypospadie je ventrálně rozštěpená močová trubice, léčba je operační po roce věku dítěte.
Cryptorchidism, or undescended testicle, is detected by the attending physician during physical examination of a boy. The treatment is surgical: the testis with accompanying cord structure is mobilized and brought into the subcutaneous pouch in the scrotum. Cryptorchidism is associated with a risk of sterility and the development of testicular cancer in adulthood. It is recommended to perform the treatment, orchidopexy, by age two. Phimosis, a tight foreskin, is a normal finding by age one; afterwards, local application of a corticoid ointment is recommended for a period of six weeks; if it fails, circumcision is warranted. Preputial adhesions, a condition in which a sufficiently wide foreskin is attached to the glans, are not separated until school age; corticoids are not indicated. Hypospadias is an abnormal ventral opening of the meatus of the urethra and is treated surgically after the child is one year old.
