Úvod: Film Rašómon je mistrovské dílo režiséra Kurosawy, v němž svědkové popisují tentýž případ vraždy z různého pohledu. Existuje ale rozdíl v tom, co vidí a jak to vnímají. Tento jev se nazývá Rašómonův efekt. Cílem této studie bylo zjistit, zda nálada lékaře v různém pracovním prostředí má vliv na rozhodování o indikacích a léčbě. Materiál a metody: Náš vzorek tvořilo celkem 54 lékařů (29 kardiologů a 25 kardiochirurgů), kteří si prohlédli koronarogram pacienta se závažným postižením koronární tepny dvakrát, s odstupem 30-45 dnů v různém prostředí. Pro stanovení nálady po indikování invazivního postupu bylo lékařům položeno 6 otázek. Pro srovnání kategorických údajů byl použit chí kvadrát test. Opakovaná srovnání poměrů se analyzovala pomocí Bonferroniho korigovaného Z-testu. První a druhé klinické indikace se porovnávaly McNemarovým testem podle pacientova koronarogramu v rámci skupiny. Výsledky analýzy byly prezentovány jako frekvence (procento). Statistická významnost byla stanovena na úrovni p < 0,05. Výsledky: Podle kardiologů a kardiochirurgů existuje statisticky významný rozdíl v odpovědích na otázky v závislosti na náladě v prvním a druhém časovém bodu s výjimkou rozdílu v odpovědích v náladě "právě teď jsem v pohodě". V druhém časovém bodu, kdy 55,2 % kardiologů odpovědělo maximálně "naprosto pravda", odpovědělo 64 % operatérů "většinou pravda" (p = 0,024). Byl zjištěn statisticky významný rozdíl (p = 0,041) mezi kardiology a chirurgy v rozvrstvení klinických indikací podle koronarogramů pacientů při první konzultaci; tedy o samotě, v tichu a při větším soustředění. Zatímco 55,2 % kardiologů odpovědělo "perkutánní", zněla odpověď 76 % chirurgů "operace". Naopak neexistoval statisticky významný rozdíl (p = 0,136) v distribuci intervenčních výkonů ve druhé situaci, tedy v přeplněném a hlučném prostředí. Zatímco 51,7 % kardiologů odpovědělo "stent", 72 % chirurgů odpovědělo "operace". Nebyl nalezen žádný rozdíl mezi distribucemi indikací 1 a 2 mezi kardiology (p = 1) nebo mezi chirurgy (p = 1). Závěr: Náladu mohou faktory vnějšího prostředí ovlivňovat a někdy mohou působit zmatek. Kardiolog nebo chirurg mohou vědomě nebo nevědomky preferovat konkrétní metodu léčby. To, že nepřijmou opačné nebo jiné řešení, lze vysvětlit nepsanými, zaběhnutými zvyklostmi na daném pracovišti, osobními nebo klinickými postupy i způsobem výuky v době studia. To vše může vést k rozdílným rozhodnutím o léčbě v různou dobu u jednoho a téhož klinického případu.
Introduction: "Rashomon" is Kurosawa's masterpiece. Witnesses describe the same murder from different perspectives. But there is a difference between what they see and what they perceive. This is called the "Rashomon Effect". The aim of this study is to determine whether the mood of clinicians in different work environments has an effect on treatment indication decisions. Material and methods: The sample consisted of 54 clinicians, consisting of cardiologists (n = 29) and cardiovascular surgeons (n = 25). The angiography of a patient with severe coronary artery pathology was watched by the clinicians twice, at intervals of 30–45 days, in environments with different characteristics. 6 questions were asked to determine the moods after an indication for invasive treatment. Chi-square test was used to compare categorical data. Multiple comparisons of ratios were analyzed with the Bonferroni corrected Z test. The McNemar test was used to compare the first and second clinical indications according to the patient’s angiography within the group. Analysis results were presented as frequency (percentage). Significance level was taken as p <0.05. Results: According to cardiologists and surgeons, there is a statistically significant difference in the answers given to the questions about moods at the first and second time points, except for the difference in the responses given to the mood: “I’m calm right now.” At the second time point, where 55.2% of cardiologists answered “definitely true” at most, 64% of surgeons answered “mostly true” (p = 0.024). There was a significant difference (p = 0.041) between cardiologists and surgeons in the distribution of clinical indications according to the patient’s angiogra- phic images at the first consultation; namely during the isolated, silent, and more concentrated condition. While 55.2% of the cardiologists gave the answer “percutaneous”, 76% of the surgeons gave the answer “operation”. However, there was no significantly different (p = 0.136) distribution of interventional indications in the second condition, namely the crowded and noisy situation. While 51.7% of the cardiologists gave the answer “stent”, 72% of the surgeons gave the answer “surgery”. There is no difference between the distributions of indication 1 and indication 2 among cardiologists (p = 1) or among surgeons (p = 1). Conclusion: Mood can be affected by environmental factors. Environmental factors can sometimes be con- fusing. The cardiologist or surgeon may be knowingly or unknowingly next to a treatment method. The unwritten, standardized institutional habits, personal or clinical protocols, and the approach orientation of the training received may explain the practice of ignoring the opposite decision. All these may result in different treatment decisions being made at different times in the same clinical case.

OBJECTIVE: The initial data of the International Study on Acute Coronary Syndromes - ST Elevation Myocardial Infarction COVID-19 showed in Europe a remarkable reduction in primary percutaneous coronary intervention procedures and higher in-hospital mortality during the initial phase of the pandemic as compared with the prepandemic period. The aim of the current study was to provide the final results of the registry, subsequently extended outside Europe with a larger inclusion period (up to June 2020) and longer follow-up (up to 30 days). METHODS: This is a retrospective multicentre registry in 109 high-volume primary percutaneous coronary intervention (PPCI) centres from Europe, Latin America, South-East Asia and North Africa, enrolling 16 674 patients with ST segment elevation myocardial infarction (STEMI) undergoing PPPCI in March/June 2019 and 2020. The main study outcomes were the incidence of PPCI, delayed treatment (ischaemia time >12 hours and door-to-balloon >30 min), in-hospital and 30-day mortality. RESULTS: In 2020, during the pandemic, there was a significant reduction in PPCI as compared with 2019 (incidence rate ratio 0.843, 95% CI 0.825 to 0.861, p<0.0001). This reduction was significantly associated with age, being higher in older adults (>75 years) (p=0.015), and was not related to the peak of cases or deaths due to COVID-19. The heterogeneity among centres was high (p<0.001). Furthermore, the pandemic was associated with a significant increase in door-to-balloon time (40 (25-70) min vs 40 (25-64) min, p=0.01) and total ischaemia time (225 (135-410) min vs 196 (120-355) min, p<0.001), which may have contributed to the higher in-hospital (6.5% vs 5.3%, p<0.001) and 30-day (8% vs 6.5%, p=0.001) mortality observed during the pandemic. CONCLUSION: Percutaneous revascularisation for STEMI was significantly affected by the COVID-19 pandemic, with a 16% reduction in PPCI procedures, especially among older patients (about 20%), and longer delays to treatment, which may have contributed to the increased in-hospital and 30-day mortality during the pandemic. BACKGROUND: The majority of children with Noonan syndrome (NS) or other diseases from the RASopathy spectrum suffer from congenital heart disease. This study aims to survey cardiac care of this patient cohort within Europe. METHODS: A cross-sectional exploratory survey assessing the treatment and management of patients with NS by paediatric endocrinologists, cardiologists and clinical geneticists was developed. This report details responses of 110 participating paediatric cardiologists from multiple countries. RESULTS: Most paediatric cardiologists responding to the questionnaire were associated with university hospitals, and most treated <10 patients/year with congenital heart disease associated with the NS spectrum. Molecular genetic testing for diagnosis confirmation was initiated by 81%. Half of the respondents reported that patients with NS and congenital heart disease typically present <1y of age, and that a large percentage of affected patients require interventions and pharmacotherapy early in life. A higher proportion of infant presentation and need for pharmacotherapy was reported by respondents from Germany and Sweden than from France and Spain (p = 0.031; p = 0.014; Fisher's exact test). Older age at first presentation was reported more from general hospitals and independent practices than from university hospitals (p = 0.031). The majority of NS patients were followed at specialist centres, but only 37% reported that their institution offered dedicated transition clinic to adult services. Very few NS patients with hypertrophic cardiomyopathy (HCM) were reported to carry implantable cardioverter defibrillators for sudden cardiac death prevention. Uncertainty was evident in regard to growth hormone treatment in patients with NS and co-existing HCM, where 13% considered it not a contra-indication, 24% stated they did not know, but 63% considered HCM either a possible (20%) or definite (15%) contraindication, or a cause for frequent monitoring (28%). Regarding adverse reactions for patients with NS on growth hormone therapy, 5/19 paediatric cardiology respondents reported a total of 12 adverse cardiac events. CONCLUSIONS: Congenital heart disease in patients with NS or other RASopathies is associated with significant morbidity during early life, and specialty centre care is appropriate. More research is needed regarding the use of growth hormone in patients with NS with congenital heart disease, and unmet medical needs have been identified.

BACKGROUND: The Heart Team approach has become an integral part of modern cardiovascular medicine. To evaluate current opinions and real-world practice among lead extraction practitioners, an online survey was created and distributed among a pool of lead extraction specialists participating in the International Lead Extraction Expert Meeting (ILEEM) 2018. METHODS: The online survey consisted of 10 questions and was performed using an online survey tool (www.surveymonkey.com). The collector link was sent to 48 lead extraction experts via email. RESULTS: A total of 43 answers were collected (89% return rate) from lead extraction experts in 16 different countries. A great majority (83.7%) of the respondents performed more than 30 lead extraction procedures per year. The most common procedural environment in this survey was the hybrid operating room (67.4%). Most procedures were performed by electrophysiologists and cardiologists (80.9%). Important additional members of the current lead extraction teams were cardiac surgeons (79.1%), anesthesiologists (95.3%) and operating room scrub nurses (76.7%). An extended Heart Team is regarded beneficial for patient care by 86.0%, with potential further members being infectious diseases specialists, intensivists and radiologists. Team training activities are performed in 48.8% of participating centers. CONCLUSIONS: This survey supports the importance of establishing lead extraction Heart Teams in specialized lead extraction centers to potentially improve patient outcomes. The concept of a core and an extended Heart Team approach in lead extraction procedures is introduced.

Together, heart failure and arrhythmia represent the most important cardiovascular sources of morbidity and mortality among adults with congenital heart disease (ACHDs). Although traditionally conceptualized as operating within 2 distinct clinical silos, these scenarios frequently coexist within the same individual; consequently the mechanistic, therapeutic, and prognostic overlap between them demands increased recognition. In fact, given the near ubiquity of heart failure and arrhythmia among ACHDs, there is perhaps no other arena within cardiology where this critical intersection is more frequently observed. Optimal care for ACHDs therefore requires a heightened awareness of the relevant interactions as well as the pharmacologic and interventional resources that are increasingly available to the treating cardiologist. This review explores and highlights the overlap between these 2 fields to recommend a parallel, yet interactive, multidisciplinary approach to clinical management. Congenital heart disease categories are broken down into their archetypal subtypes to highlight subtleties of the pathophysiology, evaluation, and therapeutic approach.
