In acute myeloid leukaemia (AML) t(8;16)(p11;p13)/MYST3-CREBBP is a very rare abnormality. Previous small series suggested poor outcome. We report on 59 patients with t(8;16) within an international, collaborative study. Median age was 52 (range: 16-75) years. AML was de novo in 58%, therapy-related (t-AML) in 37% and secondary after myelodysplastic syndrome (s-AML) in 5%. Cytogenetics revealed a complex karyotype in 43%. Besides MYST3-CREBBP, whole-genome sequencing on a subset of 10 patients revealed recurrent mutations in ASXL1, BRD3, FLT3, MLH1, POLG, TP53, SAMD4B (n = 3, each), EYS, KRTAP9-1 SPTBN5 (n = 4, each), RUNX1 and TET2 (n = 2, each). Complete remission after intensive chemotherapy was achieved in 84%. Median follow-up was 5·48 years; five-year survival rate was 17%. Patients with s-/t-AML (P = 0·01) and those with complex karyotype (P = 0·04) had an inferior prognosis. Allogeneic haematopoietic cell transplantation (allo-HCT) was performed in 21 (36%) patients, including 15 in first complete remission (CR1). Allo-HCT in CR1 significantly improved survival (P = 0·04); multivariable analysis revealed that allo-HCT in CR1 was effective in de novo AML but not in patients with s-AML/t-AML and less in patients exhibiting a complex karyotype. In summary, outcomes of patients with t(8;16) are dismal with chemotherapy, and may be substantially improved with allo-HCT performed in CR1.

To study the prognostic relevance of rare genetic aberrations in acute myeloid leukemia (AML), such as t(16;21), international collaboration is required. Two different types of t(16;21) translocations can be distinguished: t(16;21)(p11;q22), resulting in the FUS-ERG fusion gene; and t(16;21)(q24;q22), resulting in RUNX1-core binding factor (CBFA2T3). We collected data on clinical and biological characteristics of 54 pediatric AML cases with t(16;21) rearrangements from 14 international collaborative study groups participating in the international Berlin-Frankfurt-Münster (I-BFM) AML study group. The AML-BFM cohort diagnosed between 1997 and 2013 was used as a reference cohort. RUNX1-CBFA2T3 (n = 23) had significantly lower median white blood cell count (12.5 × 109/L, P = .03) compared with the reference cohort. FUS-ERG rearranged AML (n = 31) had no predominant French-American-British (FAB) type, whereas 76% of RUNX1-CBFA2T3 had an M1/M2 FAB type (M1, M2), significantly different from the reference cohort (P = .004). Four-year event-free survival (EFS) of patients with FUS-ERG was 7% (standard error [SE] = 5%), significantly lower compared with the reference cohort (51%, SE = 1%, P < .001). Four-year EFS of RUNX1-CBFA2T3 was 77% (SE = 8%, P = .06), significantly higher compared with the reference cohort. Cumulative incidence of relapse was 74% (SE = 8%) in FUS-ERG, 0% (SE = 0%) in RUNX1-CBFA2T3, compared with 32% (SE = 1%) in the reference cohort (P < .001). Multivariate analysis identified both FUS-ERG and RUNX1-CBFA2T3 as independent risk factors with hazard ratios of 1.9 (P < .0001) and 0.3 (P = .025), respectively. These results describe 2 clinically relevant distinct subtypes of pediatric AML. Similarly to other core-binding factor AMLs, patients with RUNX1-CBFA2T3 rearranged AML may benefit from stratification in the standard risk treatment, whereas patients with FUS-ERG rearranged AML should be considered high-risk.

V oblasti liečebného prístupu k dyslipidémiám sa asi pred 5–6 rokmi objavili inhibítory CETP (cholesteryl ester transfer protein). Ich benefit spočíval v zvyšovaní sérových hladín HDL-cholesterolu (HDL-C). Tento prístup podporovali epidemiologické a klinické sledovania, nakoľko osoby s vyššími sérovými hladinami HDL-C mávali nižší výskyt kardiovaskulárnych (KV) príhod. Tri štúdie s inhibítormi CETP (ILLUMINATE s torcetrapibom, dal-OUTCOMES s dalcetrapibom a ACCELERATE s evacetrapibom) však neboli úspešné, a v prvom prípade s torcetrapibom boli pacienti dokonca poškodení zvýšenou hladinou sérového aldosterónu u aktívne liečených. Dalcetrapib bol bezpečným liekom, ale benefit nepreukázal. Podobné je to s evacetrapibom. Ešte prebieha štúdia s anacetrapibom (REVEAL), ale efekt sa tu neočakáva. Evacetrapib a anacetrapib oproti dalcetrapibu výraznejšie redukujú sérový LDL-cholesterol (LDL-C), a podobné sú výsledky s redukciou LDL-C pomocou ďalšieho CETP-inhibítoru TA-8995 (štúdia TULIP). Autori rozoberajú možné vysvetlenia neúčinnosti CETP-inhibítorov (dysfunkčný HDL-C, iný mechanizmus účinku ako reverzný cholesterolový transport). Genetická analýza v štúdiách s dalcetrapibom (dal-OUTCOMES a dal-PLAQUE-2) preukázala, že z liečby profitujú len chorí s genotypom (homozygoti) AA v géne ADCY9 na 16. chromozóme, a takých je asi len 20 % chorých. Klinická štúdia DAL-301 túto skutočnosť bude potvrdzovať. Framinghamská štúdia v Offspring kohorte sledovaných preukazuje, že asociáciu „hladina HDL-C – budúce KV riziko“ významne ovplyvňujú sérové hladiny LDL-C a triacylglycerolov (ak sú zvýšené, predikcia pomocou HDL-C zaniká). HDL častice sú zložité a nevieme, ktorý jej podtyp je pre KV prognózu dôležitý. V tejto oblasti výskum pokračuje.
In the treatment of dyslipidemias about 5–6 years back a new class of drugs emerged, CETP (cholesteryl ester transfer protein)-inhibitors. Their benefit was due to an increase of HDL-cholesterol (HDL-C) serum levels. This treatment mode was supported by epidemiological and clinical studies, as people with high serum HDL-C levels suffered less from cardiovascular (CV) events. Three studies with CETP inhibitors (ILLUMINATE with torcetrapib, dal-OUTCOMES with dalcetrapib and ACCELERATE with evacetrapib) were unfortunately negative, and torcetrapib was even harmful to patients due to an increase of aldosterone serum levels. Treatment with dalcetrapib was safe, but without benefit. Similar it was with evacetrapib. There is still running also a study with anacetrapib (REVEAL), but a benefit is here not expected. Evacetrapib and anacetrapib in comparison to dalcetrapib can reduce serum LDL-cholesterol (LDL-C) much more, and similar results were found also in another CETP-inhibitor TA-8995 (TULIP study). Authors try to explain why there is no benefit with CETP-inhibitors (dysfunctional HDL particle, another effective mechanism than reverse cholesterol transport). A genetic analysis in dalcetrapib studies (dal-OUTCOMES and dal-PLAQUE-2) showed, that a cardiovascular benefit from this treatment is concentrated only to a subgroup of patients with a genotype AA in the gene ADCY9 on 16th chromosome. In the population there are about 20% of people with this AA genotype. A clinical study DAL-301 will test this data in a near future. Framingham Offspring study showed that the association of“ HDL-C serum level – CV risk in the future” is greatly influenced by serum levels of LDL-C and triglycerides (if these are increased, than the CV future prediction with HDL-C levels is lost). HDL particles are complex and we do not know which subtype of HDL particles is for cardiovascular prognosis important. The research here continues.

Metabolic syndrome is a highly prevalent human disease with substantial genomic and environmental components. Previous studies indicate the presence of significant genetic determinants of several features of metabolic syndrome on rat chromosome 16 (RNO16) and the syntenic regions of human genome. We derived the SHR.BN16 congenic strain by introgression of a limited RNO16 region from the Brown Norway congenic strain (BN-Lx) into the genomic background of the spontaneously hypertensive rat (SHR) strain. We compared the morphometric, metabolic, and hemodynamic profiles of adult male SHR and SHR.BN16 rats. We also compared in silico the DNA sequences for the differential segment in the BN-Lx and SHR parental strains. SHR.BN16 congenic rats had significantly lower weight, decreased concentrations of total triglycerides and cholesterol, and improved glucose tolerance compared with SHR rats. The concentrations of insulin, free fatty acids, and adiponectin were comparable between the two strains. SHR.BN16 rats had significantly lower systolic (18-28 mmHg difference) and diastolic (10-15 mmHg difference) blood pressure throughout the experiment (repeated-measures ANOVA, P < 0.001). The differential segment spans approximately 22 Mb of the telomeric part of the short arm of RNO16. The in silico analyses revealed over 1200 DNA variants between the BN-Lx and SHR genomes in the SHR.BN16 differential segment, 44 of which lead to missense mutations, and only eight of which (in Asb14, Il17rd, Itih1, Syt15, Ercc6, RGD1564958, Tmem161a, and Gatad2a genes) are predicted to be damaging to the protein product. Furthermore, a number of genes within the RNO16 differential segment associated with metabolic syndrome components in human studies showed polymorphisms between SHR and BN-Lx (including Lpl, Nrg3, Pbx4, Cilp2, and Stab1). Our novel congenic rat model demonstrates that a limited genomic region on RNO16 in the SHR significantly affects many of the features of metabolic syndrome.

Alveolární kapilární dysplazie s vychýlením plicních žil (ACD/MPV) je velmi vzácná a fatální porucha plicního vývoje. Toto onemocnění se pojí s perzistující plicní hypertenzí (PPHN) refrakterní ke konvenční terapii, která vede k hypoxemickému respiračnímu selhání a k úmrtí pacienta. ACD/MPV se často sdružuje s mnohočetnými vrozenými vadami napříč orgánovými systémy, postižen bývá zejména gastrointestinální, kardiovaskulární a urogenitální systém. Autoři prezentují případ donošeného novorozence, který zemřel ve stáří 21 hodin na respirační selhání při farmakorezistentní PPHN. U pacienta byly prokázány mnohočetné vrozené vývojové vady: hypoplazie levostranných srdečních oddílů, inkompletní AV kanál s dysplazií mitrální chlopně, arteria lusoria, zdvojená levá renální tepna, anomální rozdělení pravé plíce (2 laloky), mesenterium commune, pancreas annulare, hypoplazie žlučníku, hydronefróza s hydroureterem a hemivertebra Th10. Zlatým standardem v diagnostice ACD/MPV je histologické vyšetření plicní tkáně dětským patologem obeznámeným s touto jednotkou. Genetický podklad onemocnění představují změny na 16. chromozomu, které byly v prezentovaném případě způsobeny delecí 11 genů, včetně klíčových FOXF1, FOXC2 a FOXL1. Alveolar capillary dysplasia with misalignment of the pulmonary veins (ACD/MPV) is a rare, fatal developmental lung disorder of neonates and infants. This condition is associated with persistent pulmonary hypertension (PPHN) leading to severe respiratory failure and death. ACD/MPV is usually accompanied by a complex of congenital anomalies, especially involving gastrointestinal, cardiovascular and urogenital systems. A term newborn is presented in this case report. The patient suffered from multiple inborn defects: hypoplastic left heart, atrioventricular septal defect with malformed mitral valve, retroesophageal subclavian artery, double right renal artery, two lobes of right lung, intestinal malrotation, annular pancreas, hypoplastic gallbladder, bilateral hydronephrosis, hydroureters, and hemivertebra Th10. Despite complete resuscitation care the girl died because of pharmacoresistant pulmonary hypertension and hypoxemic respiratory failure at the age of 21 hours. The gold standard in diagnostics of ACD/MPV represents histologic evaluation of the lung tissue by a pediatric pathologist familiar with the disorder. The characteristic histological features of ACD/MPV were found in this case. Targeted genetic testing revealed deletion in the chromosomal region 16q24.1 containing the FOX transcription factor gene cluster (including FOXF1, FOXC2, and FOXL1 genes). No deletions in this genomic region are found in different databases of genomic variants, indicating that these changes are pathogenic for ACD/MPV. Autosomal dominant tubulointerstitial kidney disease (ADTKD) refers to a group of conditions characterized by autosomal dominant inheritance, a bland urinary sediment with minimal blood and protein, pathologic changes of tubular and interstitial fibrosis, and slowly progressive chronic kidney disease. This commentary discusses recent advances in our medical knowledge of these conditions, including the recent identification of mutations in the MUC1 gene as a cause of ADTKD and changes in terminology proposed by Ekici et al.

We report 11 individuals, each presenting with few (2-4) adnexal neoplasms histologically confirmed as belonging to the spectrum of lesions typical for Brooke-Spiegler syndrome (BSS) and/or multiple familial trichoepitheliomas. These include spiradenoma, cylindroma, spiradenocylindroma, and trichoblastoma variants. Our objective was to clarify whether this is merely a sporadic, albeit unusual, occurrence of multiple neoplasms in these patients or whether they are related to BSS and its phenotypic variant, multiple familial trichoepithelioma. Six patients presented with 2 neoplasms, 4 had 3 lesions and the last had 4 lesions. In none was there any family history of similar lesions. The 28 neoplasms consisted of 7 spiradenomas, 6 cylindromas, 5 spiradenocylindromas, and 11 trichoblastomas (6 trichoepitheliomas and 5 with mixed patterns). In 1 patient only with 2 spiradenomas, both tumors harbored identical CYLD sequence alterations (c.1112C>A/S371X) in the CYLD gene and both showed loss of heterozygosity on chromosome 16q. The remaining cases yielded neither germ line nor somatic alterations in CYLD. It is concluded that the presentation with few (2-4) cylindromas, spiradenomas, spiradenocylindromas, and trichoepitheliomas is a sporadic occurrence, and that these patients do not have any relationship to BSS.

A 70-year-old woman sought treatment for confluent flesh-colored papules on all 4 eyelids. Sixteen family members were reported to have similar lesions involving the face and scalp. Initial histopathologic examination of the lesions was interpreted as basal cell carcinoma, but on further review, the lesions were deemed to be consistent with trichoepitheliomas as seen in Brooke-Spiegler syndrome. Cylindromatosis gene mutation analysis confirmed this unique presentation of Brooke-Spiegler syndrome, and revealed a previously unidentified mutation in the cylindromatosis gene.
