Transection of the radial nerve is frequently associated with humeral shaft fractures that are part of a very complex upper extremity injury. In the presented case, a 19-year-old man with a 10-cm radial nerve defect with a need for nerve grafting to recover complete sensory and motor deficit of the radial nerve. In our case, at the same time we provided the tendon transfer of musculus (m.) pronator teres to m. extensor carpi radialis brevis, m. flexor carpi ulnaris to m. extensor digitorum communis, m. palmaris longus to m. extensor pollicis longus, and long sural nerve graft because of an extensive zone of the injury. The assumption was that if these two procedures are performed in one surgery, it will accelerate overall recovery, restore the functionality of the upper limb more quickly, and thus enable a faster recovery.

OBJECTIVES: To compare brain responses to peroneal electrical transcutaneous neuromodulation (peroneal eTNM®) and transcutaneous tibial nerve stimulation (TTNS), two methods for treating overactive bladder (OAB), using functional magnetic resonance imaging (fMRI). The present study was not designed to compare their clinical efficacy. MATERIALS AND METHODS: This study included 32 healthy adult female volunteers (average age 38.3 years (range 22-73)). Brain MRI using 3 T scanner was performed during three 8-min blocks of alternating sequences. During each 8-min block, the protocol alternated between sham stimulation (30 s) and rest (30 s) for 8 repeats; then peroneal eTNM® stimulation (30 s) and rest (30 s) for 8 repeats; then, TTNS stimulation (30 s) and rest (30 s) for 8 repeats. Statistical analysis was performed at the individual level with a threshold of p = 0.05, family-wise error (FWE)-corrected. The resulting individual statistical maps were analyzed in group statistics using a one-sample t-test, p = 0.05 threshold, false discovery rate (FDR)-corrected. RESULTS: During peroneal eTNM®, TTNS, and sham stimulations, we recorded activation in the brainstem, bilateral posterior insula, bilateral precentral gyrus, bilateral postcentral gyrus, left transverse temporal gyrus, and right supramarginal gyrus. During both peroneal eTNM® and TTNS stimulations, but not sham stimulations, we recorded activation in the left cerebellum, right transverse temporal gyrus, right middle frontal gyrus, and right inferior frontal gyrus. Exclusively during peroneal eTNM® stimulation, we observed activation in the right cerebellum, right thalamus, bilateral basal ganglia, bilateral cingulate gyrus, right anterior insula, right central operculum, bilateral supplementary motor cortex, bilateral superior temporal gyrus, and left inferior frontal gyrus. CONCLUSIONS: Peroneal eTNM®, but not TTNS, induces the activation of brain structures that were previously implicated in neural control of the of bladder filling and play an important role in the ability to cope with urgency. The therapeutic effect of peroneal eTNM® could be exerted, at least in part, at the supraspinal level of neural control.

Přehled byl koncipován na podkladě prací z českých a zahraničních odborných urologických periodik.
We present a review of neuromodulation and neurostimulation methods used in children. For each method the results are described in detail. detailed description ofitsresults is mentioned. Neuropathophysiology of the lower urinary tract is also briefly described. The rReview is based on articles published in national (Czech) and international urology journals.

Poranění periferních nervů dolních končetin není časté. Dominují trakční poranění n. peroneus v oblasti kolene či iatrogenní úrazy pánevních nervů při nitrobřišních operacích. Ostrá poranění jsou vyjma válečné chirurgie vzácná. Indikace k operační revizi se řídí obecnými pravidly chirurgie nervů. Ostrá poranění mají být ošetřena co nejdříve, ideálně do 72 hodin. Zavřené léze se indikují k operaci při denervačním syndromu trvajícím alespoň 3 měsíce. Nejlepších výsledků je dosaženo při revizích do 6 měsíců od vzniku poranění. Iritační syndromy vzniklé útlakem úlomky či jizvením po zlomeninách nebo iatrogenně (svorky, cement, šrouby apod.) lze revidovat i později. Kruciální je však včasné vyšetření na specializovaném neurochirurgickém pracovišti.
Peripheral nerve injuries of the lower extremities are not frequent. The most common are traction injury of the peroneal nerve at the knee level or iatrogenic trauma of the pelvic nerves during abdominal surgery. Civil sharp injuries are rare. Indications for surgical revision follow the general rules of nerve surgery. Sharp injury should be treated as soon as possible, ideally within 72 hours. Closed lesions are indicated for surgery if a complete denervation remains unchanged three months after the injury. Best results can be achieved within six months from the injury. Irritations caused by bone fragments or scarring or by iatrogenic injury (clamps, cement, screws, etc.) may be revised later. However, the most important is early clinical examination in a specialized neurosurgical department.

PURPOSE OF THE STUDY: Purpose of this anatomic study was to develop a new and safe technique of minimal invasive dorsal plate osteosynthesis for tibia shaft fractures. MATERIAL AND METHODS: Sixteen paired adult lower limbs of eight different cadaveric specimens were examined. Anatomical prebending for each plate was done. Plates were inserted percutaneously, following plate fixation the neurovascular bundle was dissected out. The distance between the neurovascular bundle (posterior tibial nerve, posterior tibial artery) and the plate was measured at two different positions. The distance to the origin of the flexor digitorum longus muscle and the arch of the soleus muscle was measured. RESULTS: The mean distance between the neurovascular bundle and the plate amounted 1.4 cm (±0,2 cm; 1.0-1.7 cm) at hole number six and 1.1 cm (±0.4 cm; 0.6-2.0 cm) at hole number ten. The nerve was never directly in contact with the plate. The flexor digitorum longus muscle had its origin along the plate and was between the plate and the neurovascular bundle in all cases. CONCLUSIONS: Dorsal percutaneous plate insertion is a safe and easy method for osteosyntesis of tiba shaft fractures. Especially in case of poor skin and soft tissue conditions this technique offers a good alternative.

Diabetická neuropatie je významnou a častou pozdní mikrovaskulární komplikací diabetes mellitus 1. a 2. typu. Představuje heterogenní syndrom postižení periferních, kraniálních i autonomních nervů. Prevalence se odhaduje mezi 13–54 % v závislosti na použitých diagnostických kritériích a na délce trvání diabetu. V průměru se vyvine asi u 50 % diabetiků 1. i 2. typu. Její prevalence narůstá díky stoupajícímu počtu diabetiků 2. typu. Kauzální léčba diabetické neuropatie není známá, stěžejní význam má prevence. Zatímco u diabetiků obou typů je důležitá kompenzace hyperglykémie, u diabetiků 2. typu je důležité ovlivnit i další rizikové faktory (hypertenze, obezita, hyperlipidémie, kouření). Diabetická neuropatie je rizikovým faktorem pro vznik diabetické (neuropatické) nohy. Role neurologa spočívá jednak v dobré diferenciální diagnostice periferní neuropatie (vyloučit jiné příčiny neuropatie, než je hyperglykémie) a v léčbě její bolestivé formy, tj. neuropatické bolesti.
Diabetic neuropathy is a significant and common late-onset microvascular complication of type 1 and 2 diabetes mellitus. It is a heterogeneous syndrome affecting peripheral, cranial and autonomous nerves. The prevalence is estimated to be at 13–54%, based on the diagnostic criteria and the duration of the patient's case of diabetes. On average it develops in roughly 50% of diabetics of both types. Its prevalence is slowly growing due to the growing numbers of type 2 diabetics. There is no known causal treatment for diabetic neuropathy, prevention is crucial. For both types of diabetes it is important to compensate hyperglycaemia, in type 2 diabetics it is also important to influence other risk factors as well (hypertension, obesity, hyperlipidemia, smoking). Diabetic neuropathy is a risk factor for development of the diabetic foot syndrome. The role of a neurologist is firstly in proper differential diagnostics of peripheral neuropathy (to rule out other possible causes for neuropathy apart from hyperglycaemia) and second in treatment of neuropathic pain.

Tento dokument uvádí přehled terapeutických možností pro léčbu nonneurogenního hyperaktivního močového měchýře na základě aktualizovaných guidelines Americké urologické asociace a Society of Urodynamics, Female Pelvic Medicine & Urogenital Reconstruction z roku 2014. Guidelines vycházejí z celkem 223 článků v anglickém jazyce, které byly uveřejněny v letech 1966–2014 a zabývají se správným pořadím, ve kterém jednotlivé terapie indikovat. Léčba první volby spočívá v behaviorální terapii, v rámci léčby druhé volby se podávají antimuskarinika a agonisté β3-adrenergních receptorů, léčba třetí volby zahrnuje injekční aplikaci onabotulotoxinu A, stimulaci periferního tibiálního nervu a sakrální neuromodulaci. Zavedení katetru, augmentační cystoplastika a derivace moči jsou modality poslední instance vyhrazené pouze pro dobře vybrané pacienty.
This manuscript reviews the treatment options for non-neurogenic overactive bladder, based on the updated 2014 American Urological Association a Society of Urodynamics, Female Pelvic Medicine & Urogenital Reconstruction guideline. The guideline is based on 223 English language articles published between 1966 and 2014, and discusses the sequence in which therapies should be offered. Behavioral therapies are first-line treatments, anti-muscarinics and β3-adrenergic agonists are second-line treatments, onabotulinumtoxin A injections, peripheral tibial nerve stimulators and sacral neuromodulation are third-line treatments, and indwelling catheters, augmentation cystoplasty and urinary diversion are only offered as a last resort in very selective cases.

Cíl: Cílem studie bylo zhodnotit přínos elektrofyziologického vyšetření v diferenciální diagnostice lumbální spinální stenózy (LS) a diabetické polyneuropatie (DPN). Soubor a metodika: Elektrofyziologické vyšetření bylo provedeno u 68 pacientů s klinicky symptomatickou LS, u 28 pacientů s DPN a 32 zdravých dobrovolníků. Výsledky: Vyšetřené elektrofyziologické parametry z horních končetin (latence F vlny n. ulnaris a amplituda senzitivního neurogramu n. radialis), všechny hodnocené latence z dolních končetin (F vlny n. tibialis, H reflexu m. soleus, spinální latence MEP) a amplituda senzitivního neurogramu n. suralis signifikantně odlišovaly pacienty s LS a DPN. ROC analýza nicméně prokázala pouze dva elektrofyziologické parametry jako efektivní nezávislé faktory pro odlišení pacientů s LS a DPN: latenci ulnarisové F vlny (optimální diskriminační hodnota je 24,2 ms, senzitivita 82,7 % a specificita 63,9 %) a amplitudu senzitivního neurogramu n. radialis (optimální diskriminační hodnota je 10,5 ?V, senzitivita 75,5 % a specificita 58,2 %). Vícerozměrná diskriminační analýza poskytla kanonické skóre s nejsilnější prediktivní hodnotou. Závěry: Elektrofyziologické vyšetření z horních končetin je užitečné v diferenciální diagnostice LS a DPN, nicméně nejlepší diskriminační efekt vykazuje kanonické skóre, které zahrnuje několik elektrofyziologických parametrů. Aim: To evaluate validity of electrophysiological examination in the differential diagnostics of lumbar spinal stenosis (LS) and diabetic polyneuropathy (DPN). Methods: Electrophysiological parameters were examined in 68 patients with clinically symptomatic LS, in 28 patients with DPN, and in a group of 32 healthy volunteers. Results: Electrophysiological parameters evaluated from the upper extremities (F-wave latency of the ulnar nerve and radial SNAP amplitude), all the latencies measured in the lower extremities (tibial F-wave, soleus H-reflex and spinal latency of MEP), and the sural SNAP amplitude contributed significantly to distinguishing the LS from DPN patients. ROC analysis, however, disclosed only two electrophysiological parameters as effective in independent discrimination between the LS and DPN patient groups: ulnar F-wave latency (cut-off point at 24.2 ms, sensitivity 82.7% and specificity 63.9%); and radial SNAP amplitude (cut-off point at 10.5 ?V, sensitivity 75.5%, specificity 58.2%). Multivariate discrimination provided a canonical score with the most powerful predictive value of all. Conclusions: Electrophysiological examination evaluated from the upper extremities proved very useful in the differential diagnostics of LS and DPN, but the most powerful predictive value was obtained by using a canonical score involving several electrophysiological parameters.

A palsy of the brachial plexus elements caused by carrying a heavy backpack is a very rare injury usually occurring in soldiers or hikers, and recovery is usually spontaneous. We describe here the case of male civilian presenting with an isolated serious axillary nerve palsy associated with chronic backpack use. During the surgery, a dumbbell-shaped neuroma-in-continuity was found which was caused by direct pressure from the subscapular artery. After resection of the neuroma, a nerve graft from the sural nerve was used to reconstruct the nerve. Reinnervation was successful and the patient was able to abduct his arm to its full range, with full muscle strength, within 24 months.
