Incidence adenokarcinomu žaludku v České republice postupně klesá, nicméně i přes rozvoj onkologické terapie zůstává mortalita stále vysoká. U velmi časných stadií lze dosáhnout dlouhodobého přežívání jen endoskopickou intervencí. Toto je však možné pouze u minority pacientů. Ve stadiu II a III je základem kurativního postupu radikální chirurgický výkon s D2 lymfadenektomií. I přes radikální výkon však prognóza zůstává závažná vzhledem k vysoké míře recidivy. Je tedy zřejmé, že u lokálně pokročilých onemocnění je pouhá resekce nedostatečná a v současné době je standardem léčby perioperativní chemoterapie, která vede k prodloužení parametrů přežívání a zlepšení prognózy. Systémová terapie u lokálně pokročilého onemocnění se vyvíjela zejména v posledních 20 letech. Efekt neoadjuvantní a adjuvantní chemoterapie a chemoradioterapie byl potvrzen již v několika randomizovaných klinických studiích fáze III. V současné době se výzkum zaměřuje i na přínos cílené terapie v perioperačním období.
The incidence of gastric adenocarcinoma in the Czech Republic is decreasing. However, despite the development of oncology treatment, mortality remains high. In the very early stages, long-term survival can be achieved by endoscopic intervention. Nevertheless, it is possible only in a minority of patients. In stage II and III, radical resection with D2 lymphadenectomy is the main approach of a curative treatment. Even so, the prognosis remains serious due to a high rate of recurrence. Thus, it is now clear that in locally advanced cancers, radical resection alone is not sufficient and perioperative systemic chemotherapy is currently the standard of treatment. This approach leads to the improvement of survival and prognosis. Systemic therapy for locally advanced disease has developed in particular in the last 20 years. The effect of neoadjuvant and adjuvant chemotherapy and chemoradiotherapy has been confirmed in several randomized phase III clinical trials. Currently, research is also focusing on the benefits of target therapy in perioperative setting.

Aim: The main aim was to describe the current state of perioperative safety processes with reference to perioperative nurses' work. Design: To achieve the stated goal, a custom-built "ad hocˮ research design was created, using a combination of qualitative and quantitative methods. The findings of the exploratory qualitative survey were verified by the quantitative survey focused on in this article. Methods: The exploratory survey and Surgical Safety Checklist were the basis for a questionnaire which was designed to be used as a tool for investigation among a set of perioperative nurses in Czech hospitals. The target group of respondents was operating room managers, one representative per healthcare institution. The obtained data were analysed by exploratory statistical methods. Results: More than 96% of the healthcare institutions included in the research sample have formally established procedures: patient identification, verification of the type and side of the operation, checks for patient's history of allergies, and checks of the number of medical devices (n = 68). The survey showed that the respondents perceive safety checks to be of utmost importance in the perioperative procedure (modal value of seven on a scale from one to seven). Conclusion: Using the Surgical Safety Checklist should lead to safety improvements in nursing perioperative care. In most cases, the nursing safety practices formally introduced at respondents' workplaces correspond to WHO recommendations in the form of this Checklist.
