Cíl: Zjistit, zda existuje spojitost mezi integritou rodiny, kvalitou rodinného prostředí a úrazovostí dětí do pěti let věku. Materiál a metody: Výzkumný soubor vychází z Evropské longitudinální studie rodičovství a dětství (ELSPAC). Pro účely této práce jsme vyhodnotily data z dotazníků určených rodičům, a to jednak z doby gravidity matky a dále z věku osmnácti měsíců, tří a pěti let sledovaných dětí. Porovnávány jsou dvě kontrastní skupiny dětí vzhledem k podmínkám (materiálním, psychologickým, sociálním atd.), které existují v rodinách, kde tyto děti vyrůstají. První kontrastní skupina (N = 315) zahrnuje děti, u nichž se nevyskytl do jejich věku pěti let žádný úraz, druhá skupina o stejném počtu (N = 315) pak děti, kde byl výskyt úrazů ve stejném věkovém rozpětí vysoký, konkrétně jde o 3–8 úrazů. Výsledky: V rodině, která vykazuje prvky harmonie, symetrie a komplementárnosti, a to v případě vztahů mezi dospělými i mezi dospělými a dětmi, je riziko úrazovosti dětí do pěti let výrazně nižší, než tomu je u rodin, které tyto znaky nevykazují.
Target: To find, whether there is an association of the family integrity and family environment quality with the injury frequency in children aged five years and younger. Material and methods: The research group was established based on the European Longitudinal Study of Parenthood and Childhood (ELSPAC). For the purpose of the work presented here, we evaluated data from questionnaires assigned to parents concerning periods of mother´s pregnancy on the one hand and times of age of the children monitored of 18 months, 3 years and 5 years on the other. Two contrast groups were compared in terms of conditions (material, psychological, social, etc.) existing in the families, where the children lived. The first contrast group (N=315) included children who did not suffer any injury till their age of 5 years and the second group comprised the same number of children (N=315) who experienced a rather high injury frequency within the same age interval: 3–8 injuries. Results: In families exerting features of the harmony, symmetry and complementary nature in relationships between individual adults as well as between adults and children, the risk of injuries in children aged five years and younger is considerably lower compared to families not exerting these characteristics.

Z populace 3 580 otců, kteří podali údaj o úrazech do narození svých dětí (průměrný věk 27,7 let), podalo ještě 2 255 i druhý údaj za dalších pět let. Jejich úrazová morbidita je sledována podle příčin (v prvním období 17, ve druhém 25) individuálních počtů úrazů, incidencí na 100 mužů a místa léčení (nemocnice, ambulance, jen doma). Dva tisíce šestnáct otců vytvořilo longitudinální kohortu s údaji z obou období, ostatní tvoří skupinu alternativní. Celý soubor otců byl rozdělen podle počtu úrazů do kategorií bez úrazu, s úrazovostí nízkou, střední a vysokou. Incidence jednotlivých příčin úrazů se od jedné kategorie ke druhé zvyšovaly různým tempem, což vyjadřují relativní rizika (RR) se základem v kategorii otců s nízkou úrazovostí. V důsledku nestejného rozložení příčin mezi kategoriemi se struktura příčin úrazů mezi otci s různě vysokou úrazovostí lišila. I v longitudinální kohortě a v alternativní skupině bylo složení příčin úrazů jiné u otců se střední a s vysokou úrazovostí než u otců s úrazovostí nízkou. Úrazovost otců ve druhém období byla studována podobně jako v prvním. Počty úrazů z druhého období, přiřazené do kategorií jejich minulé úrazovosti, byly přímo odstupňované podle velikosti jejich předchozího úrazového postižení, RR = 1 – 1,19 – 1,75 – 2,48 (p<0,000): tendence k podobné úrazovosti je v mužské populaci silná a dlouhodobá.
From the population of 3580 fathers, who provided data on injuries from the birth of their children (aged on average 27.7 years), 2255 of them also submitted the second set of data from a period of next five years. The morbidity associated with these injuries was considered based on causes (17 and 25 causes in the first and second periods, respectively) of individual injuries, incidence per 100 men and treatment environment (hospital, outpatient department, solely at home). Total of 2016 fathers formed a longitudinal cohort with data from both periods; the remaining ones formed an alternative group. The whole group of the fathers was divided by the occurrence of injuries into categories as follows: without injuries, with low injury frequency, intermediate injury frequency and high injury frequency. The incidences of particular causes of injuries from one category to the other increased at different rates, which was expressed by the relative risk (RR) based on the category of fathers with the low injury frequency. Due to unequal distribution of causes between the categories, the structure of injury causes was different between fathers with different levels of the injury frequency. In the longitudinal cohort and alternative group, the structure of causes of injuries was different in fathers with intermediate and high injury frequencies compared to fathers with the low injury frequency. The frequency of injuries of fathers in the second period was studied in a similar manner as in the first period. Cíl práce: Zmapovat chronickou nemocnost rodičovských párů očekávajících dítě a souvislost mezi jejím výskytem u matek a otců. Zjistit vztah mezi dosavadní chronickou nemocností rodičů a jejich dosavadní i budoucí pětiletou úrazovostí. Materiál a metoda: Studie má průřezovou a prospektivní část. Celkem 3 550 párů budoucích otců a matek z Brna uvedlo v dotaznících své chronické nemoci a dosavadní úrazy. Po pěti letech byly u zbývajících 1 995 párů (56,2 %) zjištěny úrazy nastalé po narození dětí. Nezávislou proměnnou jsou chronické nemoci, sledované jednak podle druhu neduhů, jednak podle počtů připadajících na jednotlivé rodiče a jejich páry. Šetřena je souvislost nemocnosti jednoho rodiče s nemocností druhého a rozložení podle toho, zda postihují oba rodiče – jen matku – jen otce – žádného z nich. K takto tříděné nemocnosti rodičů jsou přiřazeny údaje o počtu jejich úrazů do narození dětí a v následujících pěti letech. Hledána je jejich závislost na jednotlivých druzích nemocí a na jejich celkovém počtu (chronicitě). Data jsou zpracována v systému SPSS pomocí průměrů, měr variability a F-testů, procent a jejich podílů (relativní rizika), dle chí2 testů. Sledovány jsou korelace (Pearson, Spearman) a míry shody. Výsledky: Matky měly více chronických nemocí a méně úrazů než otcové. Počet chronických nemocí jednoho z rodičů byl přímo úměrný chronicitě druhého. Prevalence chronických nemocí v později zmenšeném souboru rodičů se proti původní situaci nezměnila. Oba členy rodičovského páru postihovaly nejčastěji bolesti v zádech (16,0 %), zažívací potíže (13,0 %), ekzémy (7,0 %) a bolesti hlavy (4,0 %). U ostatních neduhů byla zjišťovaná konkordance mnohem slabší. Úrazy vykázaly signifikantní asociace se 14 chronickými nemocemi ze 23 šetřených v průřezové části studie a s devíti nemocemi v prospektivní části. Na celkové chronické nemocnosti byla následná pětiletá úrazovost otců i matek závislá signifikantně – chronické nemoci představují riziko zvýšené budoucí úrazovosti. Závěr: V letech 1991 a 1992 se děti v Brně rodily do rodin, kde žádný z obou rodičů neměl v 7,5 % chronickou nemoc a v 7,0 % žádný dosavadní úraz, ale bez nemoci i úrazu zároveň jich bylo jen 1,4 %. Naproti tomu silnou chronicitu spolu s vysokou úrazovostí mělo 10,7 % rodičovských párů.
Target of the work: Mapping of chronic diseases in parent pairs expecting children and association between rates of diseases in mothers and fathers. Finding of relationship between still existing chronic diseases of parents and their still existing and future five-year rate of injuries. Material and methods: The study includes its cross-sectional and prospective parts. Total of 3550 pairs of future fathers and mothers from Brno specified their chronic diseases and still existing injuries in questionnaires. After five years, injuries occurring after the child birth were determined in the remaining 1995 pairs (56.2%). The independent variable corresponds to chronic diseases monitored on the one hand according to their type and, on the other hand, according to their occurrence per particular parents or pairs. Association between the rate of diseases of one parent and that of another parent and distribution depending on the fact whether the diseases involve both parents – only the mother – only the father – or none of them, was examined. To the rate of diseases of parents classed in this manner, data are assigned concerning the number of their injuries from the child birth and in a subsequent five-year period. Their dependence on particular types on diseases and on their total number (chronic nature) is searched for. The data are processed in the SPSS system with the help of average values, variability measures and F-tests, percent proportions and their ratios (relative risks) based on squared chi tests. Correlations (Pearson, Spearman) and degrees of their agreement are followed. Results: Mothers experienced higher numbers of chronic diseases and lower numbers of injuries compared to fathers. The number of chronic diseases in one of the parents was directly proportional to the chronic nature of diseases of the other parent. The prevalence of chronic diseases in a later reduced group of parents remained unaltered compared to the original situation. Both members of the parent pairs most frequently experienced pain in the back (16.0%), digestive problems (13.0%), eczemas (7.0%) and cephalgia (4.0%). In other diseases, the concordance was found to be much weaker. Injuries demonstrated significant associations with 14 chronic diseases of 23 diseases examined in the cross-sectional part of the study and nine diseases examined in the prospective part. There was a significant dependence of the five-year rate of injuries in fathers as well as mothers on the rate of chronic diseases – chronic diseases present enhanced risk of future injuries. Conclusion: In 1991 and 1992, in Brno, children were born in families where none of both parents suffered from chronic diseases in 7.5% and none of them has experienced any injury in 7.0%, but only 1.4% was free of any disease or injury. In contrast to this, a high occurrence of chronic diseases together with high rate of injuries was found in 10.7% of parent pairs

Matky z brněnské části mezinárodní studie ELSPAC (Evropská longitudinální studie rodin a dětí) podaly zprávu o svých úrazech za dvě životní období: do poloviny těhotenství s očekávaným dítětem (v průměrném věku 24,5 let) a za dalších pět let po jeho porodu. Tyto informace ukázaly, kolik úrazů připadalo na 100 žen a jaké bylo složení jejich úrazové morbidity podle příčin v celých souborech a v kategoriích matek s různě vysokou úrazovostí. V prvním termínu vyplnilo dotazníky 3 848, ve druhém 2 997 respondentek, z nich 2 429 dalo informace v obou termínech. Úrazovost této longitudinální kohorty se ve srovnání s ostatními matkami jevila v prvním období jako příznivější (p < 0,025), ve druhém jako stejná. Pro matky z longitudinální kohorty rozdělené do čtyř kategorií podle počtů úrazů v prvním období (žádný úraz – jeden – dva a tři – čtyři a více případů se 757 – 686 – 771 – 215 respondentkami) byl zjištěn počet jejich úrazů po dalších pěti letech. Mezi úrazy z obou srovnávaných období byla přímá a těsná asociace (p < 0,000), kterou charakterizovaly kategoriální incidence 36,3 – 64,0 – 85,1 – 112,1 úrazů na 100 osob. Příslušná relativní rizika byla RR 1 – 1,76 – 2,34 – 3,09: v běžné ženské populaci se projevil dlouhodobý sklon držet se i nadále na podobné úrovni úrazové morbidity, jaká ji postihovala v minulosti.
Participants in this epidemiological study were women living in Brno, mothers who were taking part in the international project ELSPAC (European Longitudinal Study of Pregnancy and Childhood). This study obtained data about injuries which happened in two periods of their lives, up to the middle of pregnancy (mean age of participants at that time 24.5 years) and during five years following the childbirth. The data showed the number of injuries per 100 women and the distribution of injury morbidity classified according the cause of injury in the whole groups or in categories of mothers divided according their accident rate. The data were obtained from information reported by respondents in a questionnaire. The initial interviews included 3848 women and the follow up interviews 2997 women; the real cohort represented 2429 women. While in the first period the accident rate of this longitudinal cohort was lower (p<0.025) than the injury incidence of the rest of women, in the second period both incidences did not differ significantly. The mothers in the longitudinal cohort were divided into four categories according the number of injuries in the first period: no injury (757) – one injury (686) – two or three injuries (771) – four or more injuries (215 respondents) and the data about the number of their injuries during the five years following the childbirth were collected. There exists a close association between the injury incidences from both compared periods (p<0.000), which was characterized by injury incidence for the categories: 36.6 – 64.0 – 85.1 – 112.1 injuries per 100 persons. The relative risks for these categories were 1 – 1.76 – 2.34 – 3.09, respectively; our findings suggest that the general female population tends to maintain a stable level of injury morbidity.

Účel: Přispět k prevenci úrazovosti dětí na podkladě srovnání jejich úrazovosti s úrazovostí rodičů. Zjistit, zda takový mezigenerační vztah vůbec existuje a pokud ano, je-li stejný u synů i dcer a zda se mění s věkem dětí. Metody: Podle úrazů otců a matek, zjištěných za období před narozením dlouhodobě sledovaných dětí a v následujících pěti letech, byli rodiče rozděleni do kategorie bez úrazů, s nízkým, se středním a s vysokým počtem úrazů. Úrazy jejich dětí za věk 0–3, 3–5, 5–7, 7–11 let a za souhrnná období 0–7 a 0–11 let byly srovnávány mezi rodičovskými kategoriemi. Výsledky: Úrazovost dětí byla přímo úměrná úrazovosti rodičů. Mezi dětmi s rodiči bez úrazů a s rodiči se střední či vysokou úrazovostí byly signifikantní rozdíly v incidenci úrazů a v podílech dětí s vysokým počtem úrazů. Naopak podíly dětí bez úrazů se při rostoucí rodičovské úrazovosti výrazně zmenšovaly. Tyto vztahy se v předškolním období s věkem dětí zesilovaly, ve školním věku se ustálily. Do 5 let byly výraznější u děvčat, později – zejména za období 0–7 a 0–11 let se projevovaly silněji u chlapců. Závěr: Úrazovost rodičů je vysoce rizikovým faktorem pro úrazovost jejich dětí. Zábrana úrazů v generaci dnešních dětí je důležitým vkladem do snížení úrazovosti jejich potomstva. Příčinu zjištěných vztahů neznáme, je však možné, že kromě sdíleného životního stylu a sdíleného sociálního prostředí mohou být mediátorem i procesy v této souvislosti dosud nezkoumané, např. metabolické či neurobiologické, podobné v obou generacích.
Aim of the study: To help in prevention of children´s injuries by comparing them with parental injury morbidity. To investigate whether such a relation exists at all – if yes, whether the relations are the same in boys as well as in girls, and whether they change during their progressing age or not. Methods: According to numbers of their injuries before and five years after the birth of their children, the parents were divided into four categories: without any injury and with low, medium, and high numbers of them. Injuries of their children in age periods 0–3, 3–5, 5–7 and 7–11 years as well as during cummulated periods of 0–7 and 0–11 years were compared between parental categories. Results: Injuries of children were directly and closely associated with the parental ones. Between the offspring of parents with no injury and of the parents with medium or high numbers of injuries, significant differences have been found in injury incidence per 100 children and in the rate of injury repeaters, gradually increasing on the parental scale. In the same direction, the rate of injury – free children decreased. During preschool age, these relations grew stronger, in school age they seemed to get stabilized. Up to 5 years of age, they were more evident in girls and later – namely in cummulative age periods, they were stronger in boys. Conclusion: Parental injuries are significant risk factor(s) for injury morbidity of their children: prevention of parental injuries is the best prevention of injuries for their offspring. The cause of this phenomenon is not clear, beside the life style and social environment shared by both generations, it might be some processes not yet investigated in families, eg. of metabolic or neurobiolocal nature that are common in both generations.
