Sparganosis is a relatively neglected foodborne and waterborne disease caused by species of the tapeworm genus Spirometra, the global distribution of which has not been sufficiently recognized. Known mainly as a zoonosis of East Asia, its species are native to all inhabited continents including Europe. Spirometra has been reported from numerous wildlife species from 17 European countries, and a critical review confirmed 17 autochthonous and 8 imported human clinical cases. We present the first molecular evidence of the coincident presence of 2 species in Europe and review the current distribution to raise awareness of the parasite in this region. Spirometra erinaceieuropaei is restricted to Europe and Spirometra mansoni represents a lineage distributed mainly across Asia and Oceania that reaches Europe. The parasite is common in Eastern Europe and its distribution has potential to expand along with its invasive or migrating mammal hosts, spreading the risks of human infection.

Autoři prezentují kazuistiku 14letého chlapce, který byl odeslán na Oční kliniku FN Olomouc s diagnózou akutní přední uveitidy levého oka. Podrobným očním vyšetřením na štěrbinové lampě byla v přední komoře detekována pohybující se živá larva. Nález na zadním segmentu byl fyziologický. Jednalo se o parazitárně podmíněnou iridocyklitidu, což je v Evropě naprosto raritní příčina uveálního dráždění. Neporušená larva byla z přední komory chirurgicky extrahována. Následný pooperační průběh byl bez potíží, při lokální terapii Dexamethasonem a Atropinem došlo k rychlému zklidnění bulbu a úpravě stavu. Vzorek byl mikroskopicky determinován jako larvální stadium tasemnic řádu Pseudophyllidea (sparganum). Na základě podrobného rozboru anamnestických údajů, analýzy vývojových cyklů jednotlivých druhů tasemnic a Uterárních údajů o výskytu tasemnic na,a území České republiky se autoři přiklánějí k názoru, že infekce oka byla vyvolána larválním stadiem tasemnic rodu Spirometra. K nákaze pacienta došlo patrně při koupání ve vodní nádrži na Moravě, po napití vody obsahující infikované buchanky.
The authors report a case of 14-year-old boy presented to the Ophthalmologic Department of University Hospital in Olomouc with the diagnosis af acute anterior uveitis. A living parasite has been detected in the anterior chamber in a slit lamp examination. The vitreous and retina remained uninvolved. The acute iridocyclitis associated with parasitic infection is a very rare cause of anterior uveitis in Central Europe. The patient underwent surgical removal of the parasite via paracentesis. The inflammation resolved under the treatment with atropine and dexamethasone drops within a few days. Histopathological examination revealed the parasite as a young larval stage of tapeworm from family Pseudophyllidea (sparganum). Based on the analysis of development cycles of different types of tapeworms and according the literature data on tapeworms found in the Czech Repubhc genus Spirometra seemed to be the most plausible cause of the patient's disease. As the most probable source of viable parasites authors suspect swallowed water containing tiny infected crustaceans in the dam near the town Olomouc in Moravia.
