Primární pyomyozitida je hnisavá infekce kosterního svalu často s tvorbou abscesů, která vzniká na základě hematogenní infekce. Primární infekce má obvykle subakutní začátek a nejčastěji postihuje jednu svalovou skupinu dolní končetiny nebo svaly kyčle a pánve. Prezentujeme 16letého diabetika I. typu s nově diagnostikovanou akutní lymfoblastickou leukemií. Agresivní indukční chemoterapie byla šestý den komplikována febrilní neutropenií. Pacient udával při chůzi bolest pravého adduktoru stehna, na kůži jsme pozorovali flegmonózní změny kůže, připomínající podkožní hematom. Shrnujeme diagnostické a léčebné postupy, které vedly k dosažení definitivní diagnózy a terapeutického úspěchu.
Primary pyomyositis is a purulent infection of the skeletal muscle often with abscess formation, which arises from a haematogenous infection. Primary infections usually have a subacute onset and most commonly affect one muscle group of the lower limb or the muscles of the hip and pelvis. We present a 16-year-old type I diabetic with with newly diagnosed acute lymphoblastic leukemia. Aggressive induction chemotherapy was complicated by febrile neutropenia on day 6. The patient reported right thigh adductor pain on walking, and we observed phlegmonous skin changes resembling a subcutaneous hematoma. We summarize the diagnostic and therapeutic procedures that led to a definitive diagnosis and therapeutic success.

Úvod: Anterolaterální lalok stehna (ALT) byl popsán jako fasciokutánní lalok. Lze jej elevovat jako stopkovaný a/nebo jako volný lalok. Většina volných ALT laloků je elevována jako fasciokutánní lalok. Jejich použití při rekonstrukci hlavy a krku a po traumatech končetin je dobře známo. Kromě výše uvedených má tento lalok ještě další způsoby využití, které jsou méně používané. Podobně lze ALT lalok použít jako myokutánní lalok spolu s m. vastus lateralis. V případě potřeby lze oba svalové i fasciokutánní laloky odebírat jako chimérické laloky, které mohou pokrýt dvě různé oblasti rány. Kromě toho je elevace stopkovaného laloku méně časově náročná než u volného laloku. Vzhledem k tomu, že má dlouhou cévní stopku, může při použití jako stopkovaný lalok dosáhnout až do gluteální oblasti. Tato studie byla provedena pro vyhodnocení zmíněných méně častých indikací stopkovaného ALT laloku. Cílem studie bylo zhodnotit vlastnosti ALT laloku z hlediska rozsahu kryté plochy, oblouku rotace a výhod zahrnutí m. vastus lateralis jako součásti laloku. Metody: V letech 2016–2018 byla provedena retrospektivní analýza všech stopkovaných ALT laloků užitých k rekonstrukci trochanterických oblastí, horní části stehna, gluteální oblasti a oblasti boku. Do studie bylo zahrnuto sedm pacientů s osmi defekty. Výsledky: Všechny laloky se úspěšně zhojily. Nedošlo k žádné rozsáhlé nekróze laloku a u tří pacientů byly nalezeny drobné komplikace, jako je dehiscence rány. Závěr: ALT-vastus lateralis laloky mohou mít velké rozměry a lze je snadno elevovat za kratší dobu. Odebraný sval vastus lateralis může být použit k vyplnění defektu nebo může být použit jako chiméra k překrytí defektu. Použití svalu na dlouhotrvající infikované proleženiny pomáhá udržet sterilní spodinu rány a slouží jako prevence recidivy. Vaskularita tohoto laloku je robustní a vysoce spolehlivá. I po maximálním oblouku rotace (až 170°) všechny laloky přežily bez větších komplikací.
Background: The anterolateral thigh (ALT) flap was described as the fasciocutaneous flap. It can be harvested as a pedicled and/or free flap. Majority of the free flaps are harvested as a fasciocutaneous flap. Their use in head and neck reconstruction and limb trauma is well established. Apart from these advantages, this flap has various applications which are less utilized. ALT flap can be used as a myocutaneous flap along with vastus lateralis muscle. When muscle and fasciocutaneous flaps are required, both can be harvested as a chimeric flap which can cover two different regions of the wound. Moreover, harvest of the pedicled flap procedure is less time-consuming than that of a free flap. Since it has a long vascular pedicle, when used as pedicled flap, it can reach up to the gluteal region. To evaluate these less applied advantages of pedicled ALT flap, our study was undertaken. This study aimed to evaluate the efficiency of ALT flap in terms of the surface area of coverage, arc of rotation and the advantages of including vastus lateralis muscle as part of the flap. Methods: A retrospective record analysis of all pedicled ALT flap reconstruction of trochanteric, upper thigh, gluteal and flank regions from 2016 to 2018 was undertaken; 7 patients with 8 defects were included. Results: All the flaps healed successfully. There was no major necrosis of the flap and minor complications like wound gapping were found in three patients. Conclusion: The ALT-vastus lateralis flap dimensions can be very large and can be easily harvested in a very short time. Vastus lateralis muscle harvested can be used to fill the defect or can be used as chimera to cover the defect. The use of muscle over long standing infective pressure sores can sterilize the wound bed and help in preventing recurrence. The vascularity of this flap is robust and highly reliable. Even after a maximum arc of rotation (up to 170°) all the flaps survived without any major complications.
Úvod: Laloky jsou podstatou rekonstruční chirurgie. Schopnost tyto laloky úspěšně navrhnout, provést a zahojit je specialitou plastické chirurgie. Volba laloku se řídí hlavně typem vady. Při rozhodování však hrají roli určité faktory, jako je proveditelnost, doba trvání operace a stav pacienta. Primární typ volného laloku (svalového nebo fasciokutánního) je určen defektem nebo charakterem rány. Volba laloku také závisí na různých faktorech, jako je požadovaná délka stopky laloku, snadnost odběru a morbidita donorského místa. Tensor fascia lata flap (TFL) je myokutánní lalok, který obsahuje kromě svalu další komponenty. Materiál a metody: Do studie byli zahrnuti pacienti přijatí do terciární nemocniční péče s diagnostikovaným kompozitním tkáňovým defektem v kterékoli oblasti lidského těla v intervalu od listopadu 2016 do listopadu 2018. Pacienti ve studii podstoupili rekonstrukci volným TFL lalokem. Byla analyzována délka trvání odběru laloku, anatomická lokalizace stopky, hojení laloku a potřeba sekundárních chirurgických výkonů. Výsledky: Celkem 14 pacientů podstoupilo rekonstrukci pomocí volného TFL laloku. Anatomická pozice defektů byla nejčastěji na dolních končetinách - 8 případů (58 %), následovaly horní končetiny a hlava a krk (3 případy po 21 %). Průměrná doba odběru laloku byla 62,07 min (rozmezí 45-80 min). Pozice vstupu stopky do laloku byla průměrně 8,7 cm od spina iliaca anterior superior. Ze 14 laloků bylo 10 (71 %) laloků zcela úspěšných a u 4 (29 %) laloků se vyskytla parciální ztráta. Závěr: Doba odběru volného TFL laloku je ve srovnání s jinými laloky velmi krátká, a proto je lalokem volby u pacientů, kteří jsou v kritickém stavu a netolerují dlouhodobou celkovou anestezii.
Background: Flaps are the essence of reconstructive surgery. The ability to successfully design, execute and manage the flaps makes plastic surgery an outstanding speciality. The choice of flap is mainly guided by the type of the defect. However, certain factors like technique feasibility, duration of the surgery and patient factors do have a role in decision making. The primary type of free flap (whether a muscle or a fasciocutaneous flap) is dictated by the defect or the wound characteristics. However, the choice of flap depends on various factors like the component of flap, pedicle length required, the ease of harvest and donor site morbidity. Tensor fascia lata (TFL) is one myocutaneous flap, which has well developed components other than a muscle. Materials and methods: The patients admitted to a tertiary care hospital with the diagnosis of composite tissue defect in any region of the body were enrolled for this study from November 2016 to November 2018. Patients undergoing free TFL flap reconstruction are studied. The duration of flap harvest, the anatomical site of pedicle, flap outcome and the need of secondary surgery were analysed. Results: Totally 14 patients were reconstructed with a free TFL flap. The anatomic location of the defect was more frequent on lower limbs – 8 cases (58%), followed by the upper limb and the head and neck area (3 cases, each 21%). The mean flap harvest time was 62.07 (45–80) min. The mean size of pedicle entry was 8.7 cm from the anterior superior iliac spine. Out of the 14 flaps, there were 10 (71%) flaps successful completely and 4 (29%) of them had partial loss. Conclusion: A free TFL flap harvest time is very short compared to any other flaps and hence makes it the flap of choice in patients who are critical and cannot withstand long operating time.
Large unresectable STS presents a therapeutic challenge. Several options are being explored to avoid amputation without compromising the oncological outcome. Neoadjuvant chemotherapy delivers inconsistent and rather unsatisfactory results, preoperative radiotherapy compromises healing, hence it can impede adjuvant systemic treatment. We present a case report of neoadjuvant use of isolated limb perfusion with TNF-alfa and Alkeran (Melphalan) in a patient with initially unresectable large myxoid liposarcoma of the thigh. We achieved 55% reduction in size of the tumor that allowed for wide resection with a safe margin. Pathology confirmed 99% tumor necrosis. The patient has a full function of his extremity and is disease-free at one year follow-up. ILP should be considered as a treatment option which, in selected cases, can contribute to limb sparing surgery. Článek je pokračováním cyklu o významu fascií pro regionální anestezii. Tato kapitola se zaměřuje na kožní inervaci steh na. Blokády kožního povrchu jsou nedílnou součástí centrálních blokád i zavedených blokád hlavních periferních nervů směřujících do oblasti stehna. Průběh kožních nervů je vázán na dispozici povrchní i hluboké fascie. Fasciální struktury jsou dobře identifikovatelné pomocí ultrazvukového obrazu, což usnadňuje identifikaci kožních nervů a umožňuje jejich selektivní blokádu. Blokáda periferních kožních nervů stehna neovlivní svalovou sílu a mobilitu. Selektivní blokády pod kožních nervů usnadňují diagnostiku chronických bolestí v jimi zásobené oblasti.
The article is a continuation of a series of articles on the importance of fascial layers for regional anaesthesia. This chapter focuses on skin innervation of the thigh. Skin surface is an integral part of neuroaxial blocks as well as of the established regional anesthesia of the main peripheral nerves directed to the thigh area. The anatomical course of the skin nerves is tied to the locations of superficial and deep fascias. Fascial structures are well identifiable using ultrasound guidance, which fa cilitates the identification of skin nerves and allows their selective blockage. Blocks of the peripheral skin nerves of the thigh do not affect muscle strength and mobility. Selective peripheral blocks of the subcutaneous nerves facilitate the diagnosis of chronic pain in their supply area.

Článek je pokračováním cyklu o významu fascií pro regionální anestezii. Tato kapitola se pokouší vnést do souladu anatomickou strukturu dolní končetiny s požadavky na regionální anestezii. Základní studovanou strukturou jsou hluboké fascie v oblasti stehna - fascia lata a hluboké myofasciální struktury. Fascia lata rozděluje svalstvo stehna do tří kompartmentů. Mohutnost fascia lata znemožňuje průnik lokálního anestetika do jiného kompartmentu, než do kterého bylo podáno. Lokální anesteti kum se šíří pouze v myofasciálních prostorech jednotlivých kompartmentů. Větve n. femoralis inervují přední kompartment. Mediální kompartment je inervován větvemi nervus obturatorius a zadní kompartment zásobuje nervus ischiadicus. Druhá část textu je věnována detailnějšímu rozboru oblastí vhodných pro aplikaci regionální anestezie v oblasti stehna.
The article is a continuation of the series on the importance of fascia for regional anesthesia. This chapter attempts to align the anatomical structure of the lower limb with the requirements for regional anesthesia. The basic structure studied are deep fascia in the thigh area - fascia lata and deep myofascial structures. Fascia lata divides the muscles of the thigh into three compartments. The thickness of the fascia late makes it impossible for the local anesthetic to penetrate into a different com partment than the one into which it was administered. The local anesthetic is spread only in the myofascial spaces of individual compartments. The anterior compartment of the thigh is supplied by the branches of n. femoralis. The medial compartment is supplied by the branches of the obturatorius nerve, and the posterior compartment is supplied by the sciatic nerve. The second part of the text is focused on a more detailed analysis of thigh areas suitable for an application of regional anesthesia.
Distant abscesses are uncommon during the episode of acute pancreatitis (AP). However, these are possible sequalae of necrotizing pancreatitis and should be treated appropriately to prevent serious septic complications. We demonstrate a case of a 56-year-old male patient who presented with severe necrotizing pancreatitis and distant retroperitoneal abscess that reached femoral region and was detected on diagnostic imaging scans. Combination of surgical and supportive therapy was employed, and the patient recovered well with no permanent consequences. Our article highlights the importance of quick and accurate diagnosis and timely intervention in this rare type of pancreatitis complication.

OBJECTIVE: To demonstrate that the use of platelet-rich plasma (PRP) enhances both the quality of healing and the time required for wound healing at a skin graft donor site. METHODS: Patients who had dermo-epidermal skin grafts taken from the thigh area were included in a prospective, randomised clinical study. PRP was applied to one donor site and then covered with Vaseline-impregnated, open-weave gauze and gauze, while the contralateral donor site on the other thigh served as a control and was covered with the open-weave gauze and gauze without PRP. RESULTS: A total of 24 patients took part in the study, of which three developed infections and were thus removed from the study. Use of PRP reduced the wound healing time of the dermo-epidermal graft donor sites by a mean 17.8% and median 18 days. On average, the treated donor sites healed in 14.9 days compared with 18.4 days for the control group. The median was 14 days compared with 18 days in the control group (p=0.026). In one patient, healing was slower on the side where PRP was applied. In 20 patients, healing of the donor site was accelerated where PRP was applied. CONCLUSION: The study demonstrated a beneficial effect of PRP, as healing time was shortened. Using PRP to heal wounds could be beneficial for patients for whom commonly available wound healing therapies have failed, as well as for high-risk patient groups for whom problematic wound healing may be expected.
