Carotid body tumours (CBT), also called carotid body paraganglioma, are highly vascular and histologically portray paraganglion cells. They are typically found at the carotid bifurcation and result in the splaying of the internal and external carotid arteries (ICA and ECA). Recent literature supports the role of chronic hypoxia in the etiology of CBT. This pictorial essay discusses how CBT is an uncommon etiology for common clinical problems such as transient ischemic attacks. It also discusses imaging techniques to precisely map out the tumour for surgical resection using advanced imaging modalities and techniques.

OBJECTIVE: To evaluate the role of lateral skull base approaches in the management of benign parapharyngeal space tumors and to propose an algorithm for their surgical approach. METHODS: Retrospective study of patients with benign parapharyngeal space tumors. The clinical features, radiology and preoperative management of skull base neurovasculature, the surgical approaches and overall results were recorded. RESULTS: 46 patients presented with 48 tumors. 12 were prestyloid and 36 poststyloid. 19 (39.6%) tumors were paragangliomas, 15 (31.25%) were schwannomas and 11 (23%) were pleomorphic adenomas. Preoperative embolization was performed in 19, stenting of the internal carotid artery in 4 and permanent balloon occlusion in 2 patients. 19 tumors were approached by the transcervical, 13 by transcervical-transparotid, 5 by transcervical-transmastoid, 6, 1 and 2 tumors by the infratemporal fossa approach types A, B and D, respectively. Total radical tumor removal was achieved in 46 (96%) of the cases. CONCLUSION: Lateral skull base approaches have an advantage over other approaches in the management of benign tumors of the parapharyngeal space due to the fact that they provide excellent exposure with less morbidity. The use of microscope combined with bipolar cautery reduces morbidity. Stenting of internal carotid artery gives a chance for complete tumor removal with arterial preservation.

Tumory karotického glomu patří mezi vzácné, obvykle benigní tumory. Nebezpečí tumoru karotického glomu spočívá především v jeho hypervaskularizaci a intimním vztahu ke krčním cévám a hlavovým nervům. Na kazuistickém sdělení je dokumentováno, že mylná diagnóza a snaha o odstranění či získání biopsie z tumoru na pracovištích mimo cévní centra však může být pro nemocného mnohem nebezpečnější než povaha nádoru sama. Carotid body tumours are rare, usually benign tumours. The dangerous nature of carotid body tumours is due to their hypervascularization and the intimate relationship to cervical arteries and cranial nerves. In a case report, the authors document that misdiagnosis and efforts to remove or obtain a biopsy of the tumour outside vascular centres can be more dangerous for the patient than the nature of the tumour itself. Carotid body paragangliomas (CBP) are rare neoplasms arising from the small chemoreceptor organ in the adventitia of the common carotid bifurcation. The aim of this study is to present the diagnostic process, performed treatment and obtained results in patients with carotid body paragangliomas of the Department of Otolaryngology of Istanbul Education and Research Hospital between March 1997 and November 2008. Retrospective analysis was carried out, based on the medical documentation of 5 patients with carotid body paragangliomas (3 women and 2 men), age range 44 to 68 years with a mean of 59.6 years. Four of the patients were diagnosed and treated with Shamblin type II tumor, one of the patients with type I. Physical examination, radiological evaluation, method of the treatment and post-treatment complications were studied. The most common and single symptom was nonspecific neck mass. Preoperative diagnostic evaluation consisted of a color duplex ultrasonography, computerized tomography with contrast enhancement, magnetic resonance imaging and digital subtraction angiography. In all patients with Shamblin type I and II, blunt dissection of the tumor was conducted smoothly in the subadventitial plane. Postoperative vagus nerve and hypoglossal nerve deficit were reported in one case. Carotid body paraganglioma excision has higher risk of cranial nerve paresis and carotid artery injury, so it requires careful handling and good surgical skills to ensure complete removal.

Chemodektom, paragangliom glomus caroticum neboli tumor karotického glomu se vyskytuje v populaci s incidencí 1 : 30 000. Tumor je makroskopicky, mikroskopicky i biologicky velmi podobný adrenálnímu feochromocytomu. Významným znakem těchto tumorů je jejich bohatá vaskularizace. Patří mezi nejvíce prokrvené tumory vůbec. Histologicky se popisuje alveolární uspořádání nádorových buněk, na rozdíl od feochromocytomu, ostře od sebe ohraničených jemnými pruhy vaziva se širokými kapilárami a buňkami podobnými buňkám Schwannovým. Cytoplazma nádorových buněk hlavních je slabě eozinofilní a jemně granulovaná. Jádra buněk mohou být pleomorfní a hyperchromní, ale nutno konstatovat, že jaderná atypie není jednoznačnou známkou malignity. Biologicky se jedná o tumory převážně benigní. Klinicky se jedná o pomalu rostoucí, nebolestivý, tuhý a kulovitý útvar za úhlem dolní čelisti. Přestože chemodektom je všeobecně považován za radiosenzitivní, hlavní terapeutický postup je chirurgická exstirpace tumoru.
Chemodectoma, paraganglioma of glomus caroticum or carotid glomus tumor occurs in the population with an incidence of 1:30 000. The tumor is macroscopically, microscopically and biologically very similar adrenal pheochromocytoma. An important feature of these tumors is their rich vascularization. Among the most blood supply tumors at all. Histologically, alveolar described arrangement of tumor cells, in contrast to pheochromocytoma, sharply demarcated from each other soft tissue with broad stripes capillaries and cells like Schwann cells. The cytoplasm of tumor cells is a major weakly eosinophilic and finely granular. Nuclei of cells may be pleomorphic and hyperchromic, but must be stated that nuclear atypia is not unequivocal sign of malignancy. Biologically they are mostly benign tumors. Clinically, it is a slow-growing, painless, firm and globular in shape for the angle of the mandible. Although chemodectomas were found is generally considered to be radiosensitive, the main therapeutic approach is surgical extirpation of the tumor.

V kazuistice je diskutována problematika maligního paragangliomu metastazujícího do krčních uzlin, která byla komplikována chybným odečtením peroperační histologie. Metastatická uzlina byla peroperační biopsií označena jako metastáza medulárního karcinomu štítné žlázy a byla v jedné době provedená totální tyreoidektomie. Teprve definitivní histologie zjistila metastázu karotického paragangliomu. Exstirpace tumoru s resekcí karotických tepen a jejich náhradou byla provedena až v druhé době.
There is discussed case of malignant paraganglioma with cervical lymph node metastasis that was complicated by mistake during peroperative histology. Peroperative histological examination revealed metastasis of medular carcinoma of thyroid gland. Total thyroidectomy was performed. Postoperative histological assessment revealed metastasis of carotic paraganglioma. Tumour was resected together with carotic arteries. Their reconstruction was performed during second procedure.
