Point-of-care ultrasonografia (POCUS) predstavuje formu ultrasonografie, keď ošetrujúci lekár vykonáva ultrasonogra- fické vyšetrenie pri lôžku pacienta v reálnom čase, čo mu umožňuje hodnotiť aj dynamické zmeny USG obrazu a nálezy priamo korelovať so symptómami pacienta. Lekár vykonávajúci POCUS má zvyčajne na mysli konkrétnu klinickú otázku, ktorú si chce sonografickým vyšetrením zodpovedať. POCUS má potenciál zefektívniť diagnostiku, liečbu, monitoring a skríning mnohých ochorení u interne chorých pacientov, jeho postavenie v rámci vnútorného lekárstva však zatiaľ nie je jasne zadefinované. Článok sa venuje potenciálnym výhodám POCUS-u v kontexte vnútorného lekárstva, dátam o využití POCUS-u v reálnej klinickej praxi relevantnej pre internistu a dátam o využití tzv. vreckových USG prístrojov.
Point-of-care ultrasonography (POCUS) is a form of ultrasonography where the attending physician performs an ultrasonographic examination at the patient's bedside in real-time, which allows him to evaluate dynamic changes in the sonographic image and directly correlate the findings with the patient's symptoms. The doctor performing POCUS has a specific clinical question in mind that he wants to answer with the sonographic examination. POCUS has the potential to streamline the diagnosis, treatment, monitoring, and screening of many diseases in internally ill patients, but its position within internal medicine is not yet clearly defined. The present work is devoted to the potential advantages of POCUS in internal medicine, data on the use of POCUS in an actual clinical practice relevant to the internist, and data on the use of the so-called pocket ultrasound devices.

Hluboká žilní trombóza představuje přetrvávající výzvu z hlediska diagnostiky, léčby i follow-upu pacientů. Všechny tyto kroky se často odehrávají v interních ambulancích, které tak musí obsáhnout celý tento proces. Jeho klíčovou částí je cévní sonografie, bez které nelze validně stanovit diagnózu žilní trombózy, její formy, a vhodné léčby. Rozeznáváme dva režimy cévní sonografie, Point-of-Care a expertní vyšetření, ve dvou různých fázích (diagnostická a follow-upová sonografie). Nároky na každé z nich se liší, a provádí je lékaři s různým stupněm expertízy. Oba mody mají přesně stanovené požadavky na způsob provedení a výstupy, jejich dodržení vede ke korektnímu stanovení diagnózy, včasnému zahájení léčby a stanovení optimálního postupu po základní době léčby.
Deep vein thrombosis still represents a challenge regarding the diagnostics, treatment and follow-up. All this steps are often performed in the internal medicine ambulatory centers and such clinics therefore need to be aable to manage the whole process. Its key part is vascular sonography, which is needed to establish the diagnosis, the form of thrombosis and proper treatment course. There are two types of vascular sonography - Point of Care a expert sonography, being performed in two different regimes (diagnostic and follow up sonography). There are different demands for each of these two types, and each type is performed by physicians with different level of expertise. There are well defined criteria for performance a conclusions made for each type, and their precise fulfillment leads to establishment of diagnosis of DVT, early treatment initiation and setting of optimal strategy following the baseline treatment period.

Nádorové onemocnění je významným rizikem vzniku žilní trombózy a plicní embolie (Venous Thrombembolic Event, VTE). Incidence trombózy spojené s onkologickým onemocněním (Cancer Associated Thrombosis, CAT) stoupá vzhledem ke zlepšení možností protinádorové léčby a prodloužení života pacientů. CAT činí až čtvrtinu všech případů žilní tromboembolie, způsobuje vyšší morbiditu i mortalitu a také zhoršení kvality života. Standardem léčby byly po léta nízkomolekulární hepariny (Low Molecular Weight Heparins, LMWH). V posledních letech se objevila data o účinnosti a bezpečnosti léčby v této populaci také přímými orálními antikoagulancii (Direct Oral Anticoagulants, DOAC), a ta byla proto zavzata do mezinárodních doporučení. V akutní fázi prokázaly orální inhibitory faktoru Xa (xabany) lepší účinnost v prevenci recidivy VTE oproti nízkomolekulárním heparinům, nicméně u edoxabanu a rivaroxabanu s vyšším rizikem krvácivé příhody. Proto pro léčbu DOAC jsou vhodní selektovaní onkologicky nemocní s nízkým rizikem krvácení, s nádory mimo gastrintestinální nebo genitourinální trakt a také ti, u nichž není předpoklad interakce DOAC s jinými léčivy.
Cancer is a major risk factor for venous thromboembolism (VTE). The incidence of cancer-associated thrombosis (CAT) in cancer patients is increasing due to improved antitumor treatment options and the prolongation of patients' lives. It constitutes up to 25% of all VTE cases. CAT causes higher morbidity and mortality, as well as a deterioration in the quality of life. Daily subcutaneous low-molecular-weight heparin (LMWH) has been the standard treatment for CAT for years. Data on the safety and efficacy of direct oral anticoagulants (DOACs) in this population have emerged in recent years and have been included in recent international guidelines. For the acute-phase treatment, the anti-Xa inhibitors (apixaban, edoxaban, rivaroxaban) showed better efficacy than LMWH in preventing VTE recurrence; however, rivaroxaban and edoxaban were also associated with an increased risk of bleeding events. Recent guidelines recommend DOACs for the treatment of CAT in selected cancer patients (eg, low bleeding risk, no luminal gastrointestinal or genitourinary malignancies, no interfering medications).

Intraoperative physiologic changes related to the steep Trendelenburg position have been investigated with the widespread adoption of robot-assisted pelvic surgery (RAPS). However, the impact of the steep Trendelenburg position on postoperative complications remains unclear. We conducted a meta-analysis to compare RAPS to laparoscopic/open pelvic surgery with regards to the rates of venous thromboembolism (VTE), cardiac, and cerebrovascular complications. Meta-regression was performed to evaluate the influence of confounding risk factors. Ten randomized controlled trials (RCTs) and 47 non-randomized controlled studies (NRSs), with a total of 380,125 patients, were included. Although RAPS was associated with a decreased risk of VTE and cardiac complications compared to laparoscopic/open pelvic surgery in NRSs [risk ratio (RR), 0.59; 95% CI 0.51-0.72, p < 0.001 and RR 0.93; 95% CI 0.58-1.50, p = 0.78, respectively], these differences were not confirmed in RCTs (RR 0.92; 95% CI 0.52-1.62, p = 0.77 and RR 0.93; 95% CI 0.58-1.50, p = 0.78, respectively). In subgroup analyses of laparoscopic surgery, there was no significant difference in the risk of VTE and cardiac complications in both RCTs and NRSs. In the meta-regression, none of the risk factors were found to be associated with heterogeneity. Furthermore, no significant difference was observed in cerebrovascular complications between RAPS and laparoscopic/open pelvic surgery. Our meta-analysis suggests that the steep Trendelenburg position does not seem to affect postoperative complications and, therefore, can be considered safe with regard to the risk of VTE, cardiac, and cerebrovascular complications. However, proper individualized preventive measures should still be implemented during all surgeries including RAPS to warrant patient safety.

Žilní tromboembolismus (ŽTE) představuje stále závažný medicínský problém s neklesajícím výskytem nových případů i přes prováděnou profylaxi v rizikových situacích. Jedná se o multifaktoriální onemocnění, na kterém se výrazně podílí i dědičná složka. Cílem stávajícího výzkumu je vyhledávání nových polymorfismů, které se mimo klasických trombofilií (deficitu přirozených inhibitorů koagulace a mutací FVL a FII protrombinu) podílejí na trombogenezi. V článku je podán přehled o výsledcích již provedených celogenomových asociačních studií ŽTE a jejich použití pro výpočet tzv. polygenního skóre rizika, které by po standardizaci metody mohlo být používáno pro individualizovanou prevenci ŽTE.
Venous thromboembolism (VTE) is still a serious medical problem with the non-decreasing incidence of new cases despite prophylaxis in risky situations. It is a multifactorial disease, in which the hereditary component is also significantly involved. The aim of the current research is to search for new polymorphisms that are involved in thrombogenesis in addition to classical thrombophilia (deficiency of natural coagulation inhibitors and FVL and FII prothrombin mutations). The article provides an overview of the results of already performed genome-wide association studies of VTE and their use for the calculation of the so-called polygenic risk score, which could be used for individualized prevention of VTE after standardization of the method.

The decision on treatment after a first venous thromboembolism (VTE) to prevent recurrences may be influenced by many factors. The prospective, observational, WHITE study aimed to analyze how this issue was tackled in every-day clinical practice in various countries, which have sensibly different socio-economic conditions and healthcare systems. Doctors active in 79 Internal or Vascular clinical centers in 7 countries (China, Czechia, Poland, Portugal, Russia, Slovakia, and Tunisia) enrolled VTE patients after the maintenance treatment phase. The present report analyzed information, collected in the central database, regarding the baseline characteristics, index events, type and duration of anticoagulant therapy and decision on post-maintenance treatment. From April 2018 to December 2020, 1240 patients were enrolled, 58% with an unprovoked index event. Direct oral anticoagulants (DOACs) were used in > 85% of all cases in China, Poland, Portugal, Russia and Czechia, in 52% in Slovakia and in no patient in Tunisia. The maintenance anticoagulation lasted in average approximately 6 months. Altogether, anticoagulation was stopped in 20%, extended in about 50%, regardless of whether the event was unprovoked or provoked and shifted to antithrombotics (mainly sulodexide or aspirin) in the remaining patients. In conclusion, some differences in VTE patient management were found between countries. The provoked/unprovoked nature of the index event, instead, was not the prevalent criterion to drive the decision on extension of anticoagulation, without large variations between countries. DOACs were the most widely used anticoagulant drugs, whereas > 25% of patients received antithrombotic drugs instead of anticoagulants as extended treatment.
Narastajúci objem dát a skúseností s použitím priamych orálnych antikoagulancií (DOAK) v primárnej aj sekundárnej prevencii venózneho tromboembolizmu u onkologických pacientov (CAVTE) viedol ku zmenám v početných medzinárodných odporúčaniach. Reflektujeme tieto zmeny s ohľadom na podmienky v SR. V primárnej prevencii CAVTE rozlišujeme onkologických pacientov na tých, ktorí podstupujú v súvislosti s ochorením chirurgický zákrok a na nechirurgických pacientov: hospitalizovaných a ambulantne liečených. Za základné liečivá v primárnej prevencii naďalej považujeme heparíny s nízkou molekulovou hmotnosťou (LMWH). V liečbe a sekundárnej prevencii CAVTE odporúčame uvažovať vždy aj o možnosti použiť DOAKy ako z hľadiska účinnosti rovnocennú alternatívu LMWH. LMWH je potrebné preferovať pred DOAKmi aj warfarinom u všetkých pacientov s klinicky nestabilnou situáciou s vysokým rizikom krvácania a vysokým rizikom interakcie so systémovou liečbou. Predovšetkým ide o pacientov s intraluminálnymi nádormi hornej časti gastrointestinálneho traktu a urogenitálnymi malignitami s vysokým rizikom krvácania. Pre nedostatočný objem dát sú LMWH stále preferované aj u pacientov s primárnymi nádormi a metastatickým postihnutím centrálnej nervovej sústavy a u hematoonkologických pacientov.
The increasing volume of the data and experience with direct oral anticoagulants (DOACS) in the primary and secondary prevention of venous thromboembolism in oncologic patients (CAVTE) has recently lead to changes in several international guidelines. We reflect these changes within the conditions in Slovak republic. In the primary prevention of CAVTE we recognise oncosurgical patients and nonsurgical patients: hospitalised and out patients. Low molecular weight heparins are still dominant in the primary prevention of CAVTE. Regarding the treatment and the secondary prevention of CAVTE, we recommend always to consider the possibility to use DOACs as they proved to be non inferior to LMWH. However, LMWH should be prefered over DOACs as well as over warfarin (VKA) in all patients who are in a clinically unstable condition with the high risk of bleeding and/or interaction with the systemic treatment. Primarily in the patients with intraluminal tumours of the upper part of the gastrointestinal tract and genitourinary tumours with the high risk of bleeding. As for the lack of data, LMWH are still preferd also in patients with primary tumours and metastatic disease of the central nervous system and in hemato oncology.
Importance: Prior studies have observed an association between the burden of atherosclerotic vascular disease and the risk of venous thromboembolism (VTE). The association is not well described in peripheral artery disease (PAD) after lower extremity revascularization (LER). Objective: To describe the risk of, factors associated with, and outcomes after VTE, as well as the association of low-dose rivaroxaban plus antiplatelet therapy with VTE after LER. Design, Setting, and Participants: This global, multicenter cohort study used data from the Vascular Outcomes Study of ASA (acetylsalicylic acid) Along With Rivaroxaban in Endovascular or Surgical Limb Revascularization for PAD (VOYAGER PAD) randomized clinical trial, which enrolled patients from 2015 to 2018 with median follow-up of 28 months. Participants included patients with PAD undergoing LER. Patients with an indication for therapeutic anticoagulation were excluded. Data were analyzed from September 2020 to September 2021. Exposure: Randomization to rivaroxaban 2.5 mg twice daily or placebo on a background of aspirin 100 mg daily; short-term clopidogrel was used at the discretion of the treating physician. Main Outcomes and Measures: Symptomatic VTE was a prespecified secondary outcome and prospectively collected. Results: Among 6564 patients (median [IQR] age, 67 [61-73] years; 4860 [74.0%] men), 66 patients had at least 1 VTE. The 3-year rate of VTE in patients receiving placebo was 1.7%, and the pattern of risk was linear (year 1: 0.5%; year 2: 1.1%). After multivariable modeling, weight (hazard ratio [HR], 3.04; 95% CI, 1.09-8.43), hypertension (HR, 2.11; 95% CI, 0.91-4.89), prior amputation (HR, 2.07; 95% CI, 0.95-4.53), and older age (HR, 1.81; 95% CI, 1.06-3.11) were associated with increased risk of VTE. VTE was associated with risk of subsequent mortality (HR, 7.22; 95% CI, 4.66-11.19). Compared with aspirin alone, rivaroxaban plus aspirin was associated with lower VTE risk (HR, 0.61; 95% CI, 0.37-0.998; P = .047), with benefit apparent early and sustained over time. This association was not modified by use of clopidogrel at randomization (without clopidogrel: HR, 0.55; 95% CI, 0.29-1.07; with clopidogrel: HR, 0.69; 95% CI, 0.32-1.48; P for interaction = .67). Conclusions and Relevance: In this cohort study, there was continuous risk for VTE after LER in patients with PAD, with greater risk in patients who were older and had obesity and those with more severe PAD, as reflected by prior amputation. Low-dose rivaroxaban plus aspirin was associated with lower VTE risk compared with aspirin alone, with benefits apparent early and continued over time. The spectrum of venous and arterial thrombotic events and overall benefits of more potent antithrombotic strategies for prevention should be considered after LER for PAD.
