Fifty-five strains of enterococci isolated from the piglet intestine were characterized in vitro for probiotic activity. Identification of the isolates revealed Enterococcus faecium as the predominant species (84 %). Forty strains (73 %) were found to produce bacteriocin-like substances (only into solid media) with activity almost only toward Gram-positive genera. Thirty-eight % of strains were resistant to tetracycline, 27 % to chloramphenicol, 18 % to erythromycin and 16 % to vancomycin. In addition to control of strain safety, 6 % of isolates were beta-hemolytic and 16 % produced gelatinase. Seven strains selected for further probiotic assays exhibited sufficient survival rate at pH 3.0 after 3 h, in the presence of 1 % ox-bile and lysozyme after 1 d (over 107 CFU/mL in all tests). The adhesion of tested strains to porcine and human intestinal mucus was found in a similar range (1.4-14.0 % and 1.4-17.6 %, respectively). In accordance with current research effort to use and/or to combine various health promoting substances, the sensitivity of all isolates toward plant extracts and toward bacteriocins produced by animal and environmental strains was determined. All enterococci were sensitive toward oregano and sage extracts and toward one (E. faecium EF55--chicken isolate, activity of 25 600 AU/mL) of ten bacteriocin substances. It means that a similar anti-enterococcal potential of some bacteriocin substances may be observed as for certain plant extracts.

Poslední desetiletí přineslo zásadní změny v pochopení mechanismu tvorby žluče, transportu jednotlivých složek žluče ze sinusoidální do bazolaterální membrány a do žluče. Pokrok nastal objevením specifických transportních nosičů a vyvrcholil pochopením významu nukleárních receptorů při tvorbě žluče. Původně pouze empirická léčba je nyní opřena o přesně definované mechanismy zásahu do jednotlivých transportních mechanismů. Do popředí se dostalo studium adaptačních mechanismů provázejících cholestázu. Nové terapeutické možnosti vycházejí především z podpory a posílení adaptačních mechanismů. Centrální postavení v léčbě chronických cholestatických onemocnění zaujímá kyselina ursodeoxycholová (UDCA). Po prvých rozpacích při její aplikaci je dnes léčba opřena jak o experimentální výsledky, tak o dlouhodobé randomizované studie. Je možno konstatovat, že kyselina ursodeoxycholová změnila život nemocných s dlouhodobou cholestázou. Daří se, především při zahájení léčby v časném stadiu onemocnění, tyto nemocné dlouhodobě stabilizovat a zabránit rychlé progresi onemocnění. Další dosud nevyužité možnosti jistě přináší i dlouhodobá aplikace rifampicinu. Vzhledem k tomu, že obě tyto látky uplatňují svůj léčebný efekt přes různé skupiny transportérů, je v krátké době možno očekávat výsledky kombinované léčby UDCA s rifampicinem. Zcela nové možnosti přináší objevení centrální role nukleárních receptorů v mechanismu tvorby žluče a při adaptačních procesech při chronické cholestáze. Především využití vysoce účinných agonistů FXR by v brzké době mělo přinést další pokrok v léčbě těchto závažných stavů. Aplikace byly již provedeny experimentálně a výsledky prvých klinických studií je možno očekávat v krátké době. Chronická cholestáza je klasickým příkladem promítnutí výsledků základního výzkumu do oblasti terapie. Je to cesta dlouhá, ale velmi účinná.
The last decade brought about great advances in understanding the mechanism of bile formation and transport of bile constituents from the sinusoidal membrane to the basolateral membrane and to bile. The most important steps in this regard were discovering specific transport carriers and understanding the role of nuclear receptors in bile formation. The treatment, initially solely empirical, is currently based on the specifically defined mechanisms of intervention in the particular transport mechanisms. The study of adaptive mechanisms associated with cholestasis has become of concern. New therapeutic possibilities include primarily the promotion and enhancement of adaptive mechanisms. Ursodeoxycholic acid (UDCA) is of greatest relevance to the treatment of chronic cholestatic diseases. After the initial hesitations, the current therapy is supported by both experimental results and long-term randomized studies. It can be stated that ursodeoxycholic acid has changed the life of patients with prolonged cholestasis. We have succeeded, primarily by instituting the treatment at early stages of the disease, in stabilizing these patients on a long-term basis and in preventing rapid progression of the disease. Another so far unused potential is in long-standing administration of rifampicin. As the tharapeutic activity of both drugs is achieved through different groups of transporters, outcomes of the combined treatment with UDCA and rifampicin should be available in the foreseeable future. Completely new prospects have opened up with the discovery of the pivotal role of nuclear receptors in the mechanism of bile formation and adaptive processes in chronic cholestasis. Mainly the highly effective FXR agonists should contribute to further advances in the treatment of these serious conditions in the foreseeable future. They have already been used experimentally and soon, results of the first clinical trials should be reported. Therapy of chronic cholestasis is a classic example of the application of fundamental research outcomes to the treatment. Going this way is long but highly effective.
