Infections caused by Pseudomonas sp are difficult to resolve by antibiotics (ATBs) and local therapy. The aim of our pilot study was to assess the effect of different local agents-particularly acidifying solutions-on the healing of diabetic foot ulcers (DFUs), eradication of pathogens, and economic costs related to DFU therapy. In this case study, we monitored 32 DFU patients infected by Pseudomonas species. Patients were divided into 2 groups according to the local therapy provided: group 1 (n = 15)-modern local treatment; group 2 (n = 17)-acidifying antiseptic solutions. The study groups differed only with regard to ATB usage prior to enrolment in the study (P = .004), but did not differ with regard to age, diabetes control, peripheral arterial disease, or microcirculation status. During the follow-up period, DFUs healed in 20% of cases in group 1, but there were no cases of healing in group 2 (NS). The length of ATB therapy, the number of new osteomyelitis, lower limb amputations, and the changes of DFUs status/proportions did not differ significantly between study groups. Pseudomonas was eradicated in 67% of cases in group 1 and in 65% of cases in group 2. The local treatment given to group 2 patients was associated with lower costs (P < .0001). Conclusion. Acidifying agents had the same effect as modern healing agents on wound healing, the number of amputations, and the eradication of Pseudomonas. Moreover, therapy performed using acidifying solutions proved in our pilot study markedly cheaper.

The genes encoding aminoglycoside resistance in Enterococcus faecalis may promote collateral aminoglycoside resistance in polymicrobial wounds. We studied a total of 100 diabetic foot ulcer samples for infection and found 60 samples to be polymicrobial, 5 to be monomicrobial, and 35 samples to be culture negative. A total of 65 E. faecalis isolates were screened for six genes coding for aminoglycoside resistance, antibiotic resistance patterns, and biofilm production. Infectious Diseases Society of America/International Working Group on the Diabetic Foot system was used to classify the wound ulcers. Majority of the subjects with culture-positive wound were recommended conservative management, while 14 subjects underwent amputation. Enterococcal isolates showed higher resistance for erythromycin, tetracycline, and ciprofloxacin. Isolates from grade 3 ulcer showed higher frequency of aac(6')-Ie-aph(2″)-Ia, while all the isolates were negative for aph(2″)-Ib, aph(2″)-Ic, and aph(2″)-Id. The isolates from grade 3 ulcers showed higher resistance to aminoglycosides as well as teicoplanin and chloramphenicol. All the 39 biofilm producers were obtained from polymicrobial wound and showed higher resistance when compared to biofilm non-producers. Higher frequency of isolates carrying aac(6')-Ie-aph(2″)-Ia in polymicrobial community showing resistance to key antibiotics suggests widespread distribution of aminoglycoside-resistant E. faecalis and their role in worsening diabetic foot ulcers.

Diabetes mellitus is a major chronic disease that continues to increase significantly. One of the most important and costly complications of diabetes is foot ulceration that may be colonized by pathogenic and antimicrobial resistant bacteria, which may express several virulence factors that could impair treatment success. These bacterial communities can be organized in polymicrobial biofilms, which may be responsible for diabetic foot ulcer (DFU) chronicity. We evaluated the influence of polymicrobial communities in the ability of DFU isolates to produce biofilm, using a microtiter plate assay and a multiplex fluorescent in situ hybridization, at three time points (24, 48, 72 h), after evaluating biofilm formation by 95 DFU isolates belonging to several bacterial genera (Staphylococcus, Corynebacterium, Enterococcus, Pseudomonas and Acinetobacter). All isolates were biofilm-positive at 24 h, and the amount of biofilm produced increased with incubation time. Pseudomonas presented the higher biofilm production, followed by Corynebacterium, Acinetobacter, Staphylococcus and Enterococcus. Significant differences were found in biofilm formation between the three time points. Polymicrobial communities produced higher biofilm values than individual species. Pseudomonas + Enterococcus, Acinetobacter + Staphylococcus and Corynebacterium + Staphylococcus produced higher biofilm than the ones formed by E. faecalis + Staphylococcus and E. faecalis + Corynebacterium. Synergy between bacteria present in dual or multispecies biofilms has been described, and this work represents the first report on time course of biofilm formation by polymicrobial communities from DFUs including several species. The biological behavior of different bacterial species in polymicrobial biofilms has important clinical implications for the successful treatment of these infections.

Syndrom diabetické nohy je závažnou komplikací u pacientů s diabetes mellitus, ulcerace postihují 10–25 % diabetiků, infikováno je kolem 60 % ulcerací. Infekční komplikace jsou spojeny s nutností hospitalizace, s omezením hybnosti a se zhoršením kvality života. Infekční postižení se podle závažnosti rozlišuje na mírné, středně závažné a závažné. Ve většině případů se jedná o infekci polymikrobiální, s účastí grampozitivních koků (především stafylokoků), aerobních gramnegativních tyčinek a anaerobních mikroorganizmů. Terapie infekce musí být vždy komplexní, zahrnuje úpravu celkového stavu pacienta včetně lokální péče, často s chirurgickým ošetřením. Antibiotická terapie je významnou součástí celkové léčby. Iniciální antibiotická terapie je volena empiricky, volba se uskutečňuje především dle tíže onemocnění. Se znalostí kultivačních nálezů se antibiotická terapie upravuje na cílenou.
Diabetic foot infections (DFIs) are serious problems in persons with diabetes, about 10 to 25 % of patients with diabetes develop a foot ulcer and 60 % of them are infected. DFIs cause morbidity, limit mobility, worsen patients' quality of life. Infections are classified as mild, moderate, or severe. Most DFIs are polymicrobial, with Gram-positive cocci (especially staphylococci), Gram-negative bacilli and obligate anaerobes. Successful therapy of DFI requires proper topical care and often includes surgical interventions but appropriate antibiotic treatment plays a key role. Initial antimicrobial therapy of these infections is usually empirical, the antibiotic regimen should be based on the severity of the infection. Definitive therapy should then be tailored according to the results of culture and susceptibility tests from a reliably obtained specimen.

Nejčastější příčinou hospitalizace u pacientů s diabetes mellitus je infekce ulcerace při syndromu diabetické nohy. Asi polovina těchto ulcerací jeví již při primomanifestaci klinické známky infekce. Infekce souvisí se značnou morbiditou pacientů a je přímou příčinou asi pětiny všech provedených netraumatických amputací dolní končetiny. Infekce jsou nejčastější příčinou amputace při syndromu diabetické nohy. Při optimální terapii lze dosáhnout vyléčení u 80 % ulcerací neohrožujících končetinu a asi u 70 % ulcerací končetinu ohrožujících. Správná terapie vyžaduje komplexní přístup, včetně adekvátní léčby ischemie a léčby odlehčením.
Foot wounds are common problem in people with diabetes and now constitute the most frequent diabetes-related cause of hospitalization. Diabetic foot infections cause substantial morbidity and at least one in five results in a lower extremity amputation. They are are now the predominant proximate trigger for lower extremity amputations worldwide. One in five diabetic wounds present clinical signs of infection at primomanifestation. About 80 % of limb non-threating wounds can be succesfully healed using appropriate and comprehensive approach, including antimicrobial therapy, revascularisation and off-loading.

Levels of matrix metalloproteinase-2 (MMP-2), matrix metalloproteinase-9 (MMP-9) and tumor necrosis factor-α (TNF-α) may influence wound healing and wound closure in non-healing wounds. The aim of the study was to test the hypothesis that hydrogencalcium salts of oxidized cellulose change the production of matrix metalloproteinases (MMPs) and TNF-α, wound size and number of bacterial strains in non-healing wounds. We analyzed MMP-2, MMP-9 and TNF-α in the wound fluid from 20 patients by ELISA every fourteen days over six weeks. Wound size, pain, wound closure and bacterial strains in the wound were also investigated. The wound size was reduced in 14 patients and pain in 16 patients. Bacterial contamination of the wound decreased significantly after treatment. The level of MMP-2 correlated with TNF-α production. The level of MMP-9 was unchanged during the healing period. We conclude that hydrogencalcium salts of oxidized cellulose have a favorable effect on the reduction of bacterial contamination, wound size and pain.

Cíl Stanovit prevalenci jednotlivých anaerobních bakterií u pacientů s chronickou ránou. Materiál a metodika Do souboru byli zařazeni všichni pacienti hospitalizovaní s chronickou ránou na Klinice popálenin a rekonstrukční chirurgie FN Brno v období od 1. 1. 2008 do 31. 12. 2010. U těchto pacientů byl pravidelným a cíleným mikrobiologickým odběrem získán materiál z kožních defektů. V diagnostice anaerobních původců infekce chronického defektu má nezastupitelnou roli stěr na sterilní vatový tampón následně zanořený do transportního média, které umožňuje přežití anaerobních bakterií. Tato metoda je preferována před otiskem. Po transportu do mikrobiologické laboratoře se materiál vyočkuje na kultivační půdy včetně VL agaru pro růst anaerobních bakterií. Kultivace anaerobních mikrobů vyžaduje anaerobní prostředí a časový interval 48 hodin. Následná identifikace anaerobů se provádí pomocí sestavy diagnostických disků a diagnostických souprav Remel a API bioMérieux. Výsledky Celkem bylo do skupiny zařazeno 95 pacientů (32 žen) s pozitivní izolací anaerobní bakterie v chronickém defektu, kteří byli hospitalizovaní ve sledovaném období. 75 pacientů v souboru (78,95 %) bylo hospitalizováno se vstupní diagnózou dekubitu, 13 pacientů (13,68 %) s bércovým vředem a 7 pacientů (7,36 %) s diabetickým defektem dolní končetiny (diabetickou nohou). M:F ratio 2,97:1. U těchto pacientů bylo izolováno celkem 198 kmenů anaerobních bakterií. Nejčastějším anaerobem izolovaným z chronické rány je Bacteroides fragilis celkem 115 kmenů (58,08 %), dále Peptococcus sp. 20 kmenů (10,10 %) a Peptostreptococcus sp. 19 kmenů (9,60 %) a Prevotela melaninogenica 14 kmenů (7,07 %). Závěr Ve studii se nám podařilo stanovit prevalenci základních anaerobních bakterií u pacientů s chronickou ránou. Potvrdili jsme také skutečnost, že nejvyšší výskyt anaerobů je u pacientů s dekubity.
Aim To establish the prevalence of particular anaerobic bacteria in patients with chronic wound. Material and methodology All patients, hospitalized with a chronic wound in the Clinic of burns and reconstructive surgery of Faculty Hospital of Brno in the period from January 1st, 2008 till December 31st, 2010, were enrolled in our data set. In these patients, the material was collected by regular and targeted collection from skin defects. A swab made with a sterile cotton wool swab subsequently dipped into transport medium, which enables a survival of anaerobic bacteria, plays an insubstitutable role in a diagnostic procedure of anaerobic etiological agents of chronic defect infections. This method is preferred to an imprinting. Following the transport to microbiologic laboratory, the material is inoculated in culture media including VL agar for a growth of anaerobic bacteria. The cultivation of anaerobic germs demands an anaerobic environment and a time period of 48 hours. The subsequent identification of anaerobes is performed by a set of diagnostic discs and diagnostic kits Remel and API bioMérieux. Results In total, 95 patients (32 women) with positive isolation of anaerobic bacteria in a chronic defect, who were hospitalized in the monitored period, were enrolled in our group. 75 patients in the group (78.95 %) were hospitalized with an initial diagnosis of the pressure ulcer, 13 patients (13.68%) with the crural ulcer and 7 patients (7.36 %) with a diabetic defect of lower extremity(the diabetic foot). M:F ratio 2.97:1. There were a total of 198 anaerobic bacteria tribes isolated in these patients. The most common anaerobic bacteria isolated from a chronic wound is Bacteroides fragilis 115 tribes in total (58.08 %), followed by Peptococcus sp. 20 tribes (10.10 %), Peptostreptococcus sp. 19 tribes (9.60 %), and Prevotela melaninogenica 14 tribes (7.07 %). Conclusion In our study we established the prevalence of basic anaerobic bacteria in patients with chronic wound. We also proved the fact that the highest incidence of anaerobes is in patients with pressure ulcers.
