Východiska: Výzkum v oblasti lékařské psychologie dokládá, že to, jak pacient komunikuje, se zdá být nejsilnějším prediktorem chování lékaře během konzultace a že aktivita pacienta má vliv i na to, kolik informací lékař pacientovi o jeho nemoci a léčbě sdělí. Zdá se tak, že být pacientem je dovednost, kterou je třeba rozvíjet, a tím vyvážit odpovědnost lékařů a pacientů při každém jejich setkání. Jasná doporučení a edukace v této oblasti chybí, chování nemocných je řízeno spíše pravidly etikety a slušného chování. Cíl: Hlavním cílem článku je vytvořit seznam doporučení pro pacienty a lékaře, aby mohli dovednost kompetentního pacienta ve vzájemné komunikaci společně rozvíjet, a tato doporučení zpracovat do edukačního letáku, jehož výsledkem je podpora diagnostického a terapeutického procesu a personalizované péče. Materiál a metody: Literární zdroje byly vybrány nesystematicky pro potřeby narativního review vedeného otázkou: „Jaké dovednosti a kompetence pacienta podporují efektivní komunikaci s lékařem?“ Pro tvorbu seznamu jsme zvolili tzv. metodu sněhové koule, někdy též označovanou jako řetězový nebo referenční výběr. Výsledky: Výsledky stávajícího narativního review dokládají, že téma komunikačních kompetencí pacientů představuje zájem výzkumníků i klinických pracovníků napříč medicínskými obory. Souhrn kompetencí pacienta byl zpracován do edukačního letáku s názvem „Než půjdu k lékaři“. Leták lze volně šířit v tištěné podobě tak, jak je uveden v článku, nebo lze pacienty odkázat na jeho elektronickou verzi (pomocí QR kódu na letáku nebo na stránkách Ústavu lékařské psychologie a psychosomatiky LF MU v sekci edukační materiály: https: //upp.med.muni.cz/zdroje/edu). Edukační leták lze použít jako vzdělávací materiál v jakémkoli zdravotnickém zařízení. Závěr: Být kompetentním pacientem je ideální stav, o který z pozice nemocných i lékařů usilujeme, a rozvíjíme tak medicínu více symetricky, partnersky a s optimálním rozdělením odpovědnosti za zdraví, nemoc a léčbu. Informační leták je jednou z možných a základních intervencí, jak lze rozvíjet dovednost kompetentní komunikace na straně pacienta. Jde o nejčastěji používaný komunikační kanál v oblasti edukace pacientů. Stávající seznam doporučení není definitivní a zavazující. Otvírá však téma samotné. Pojmenovává rozměry komunikace mezi lékařem a pacientem a dává oběma stranám možnost uvědomit si, jak by měl jejich vztah vypadat, co jim v něm chybí a co je pro obě strany důležité.
Background: Medical psychology research shows that how a patient communicates appears to be the strongest predictor of physician behavior during a consultation and that patient’s activity also influences how much information a physician communicates to a patient about his or her illness and treatment. Thus, being a patient is a skill that needs to be developed to balance the responsibilities of doctors and patients in each of their encounters. There is a lack of clear recommendations and education in this area, and patients’ behaviour is instead governed by the etiquette of courtesy. Purpose: The main aim of this article is to create a list of recommendations for patients and physicians to develop the skill of a competent patient in mutual communication and to include these recommendations into an educational leaflet with the aim to support the diagnostic and therapeutic process and personalized care. Material and methods: Literature sources were selected unsystematically for the purpose of a narrative review, guided by the question: “What patient skills and competencies promote effective communication with the physician?” We adopted the snowball method, sometimes referred to as chain or reference selection, to generate the list. Results: The results of the current narrative review demonstrate that the topic of patient communication competencies interests researchers and clinicians across medical disciplines. A summary of patient competencies was developed into an educational handout entitled “Before I go to the doctor”. The leaflet can be freely distributed in a printed form, as presented in the article, or patients can be referred to its electronic version (via QR code on the leaflet or on the website of the Department of Medical Psychology and Psychosomatics of the Faculty of Medicine, Masaryk University, in the educational materials section at https: //upp.med.muni.cz/zdroje/edu). The educational leaflet can be used as educational material in any healthcare facility. Conclusion: Being a competent patient is an ideal state that we strive for from the position of patients and physicians in order to develop medicine more symmetrically, in partnership and with an optimal distribution of responsibility for health, illness and treatment. The information leaflet is one of the possible and fundamental interventions to develop the skill of competent communication on the part of the patient. It is the most commonly used communication channel in patient education. The current list of recommendations is not definitive and binding. However, it opens up the topic itself. It names the dimensions of doctor-patient communication and gives both parties the opportunity to consider what their relationship should be like, what is missing in this relationship, and what is important to both parties.

Aim: This study aims to provide an understanding of the concept of self-efficacy in its implications in promoting adherence to healthy behaviours among pregnant women. Design: Concept analysis guided by Wilson 1963 framework. Methods: An electronic search of the literature published from 2009-2019 was conducted using the following databases: CINAHL, PubMed, ERIC, PsycINFO, and Google Scholar. After an in-depth review of the literature, the data were analysed, and the findings were synthesized. Results: It was found that self-efficacy is a multidimensional, dynamic, and contextual concept that is shaped according to a pregnant woman's previous experiences, the gained knowledge, presence of family empowerment, professional support, and the woman's emotional status. Self-efficacy influences a pregnant woman's cognitive process by taking quality decision-making, setting practical goals, and having a plan to overcome barriers and challenges. Conclusion: Self-efficacy is an essential predictor of adherence to the recommended healthy behaviours; hence, it should be part of any prenatal health promotion interventions.

BACKGROUND: Historically, studies of adherence to airway clearance therapy in cystic fibrosis (CF) have relied on self-reporting. We compared self-reported airway clearance therapy adherence to actual usage data from home high-frequency chest wall compressions (HFCWC) vests and identified factors associated with overestimation of adherence in self-reports. METHODS: Pediatric patients who perform airway clearance therapy with a HFCWC vest were eligible to participate. Objective adherence data were obtained from the HFCWC device, which records cumulative utilization time. Two readings at least 5 weeks apart were collected. Objective adherence was recorded as a ratio of mean-to-prescribed daily use (%). Self-reported adherence data were collected with a caregiver survey at enrollment. Adherence rates were categorized as low (< 35% of prescribed), moderate (36-79% of prescribed), and high (≥ 80% of prescribed). An overestimation was present when self-reported adherence was at least one category higher than objective adherence. RESULTS: In the final sample (N = 110), mean adherence by usage data was 61%. Only 35% of subjects (n = 38) were highly adherent, and 28% (n = 31) were low adherent. In contrast, 65% of subjects (n = 72) reported high adherence and only 8% (n = 9) reported low adherence (P < .001). Nearly half of self-reports (46%) overestimated adherence. In a multiple regression analysis, overestimation was associated with multiple airway clearance therapy locations (odds ratio 7.13, 95% CI 1.16-43.72, P = .034) and prescribed daily use ≥ 60 min (odds ratio 3.85, 95% CI 1.08-13.76, P < .038). Among subjects with prescribed daily airway clearance therapy ≥ 60 min, the odds of overestimating adherence increased 3-fold (odds ratio 3.04, 95% CI 1.17-7.87, P = .02) in a lower-income (< $50,000/y) environment. CONCLUSIONS: Self-reports overestimated actual adherence to airway clearance therapy, and the overestimation increased with treatment occurring in multiple households and prescribed therapy duration. Among participants with prescribed airway clearance therapy ≥ 60 min, overestimation increased with lower income. Objective measures of adherence are needed, particularly for lower-income children and those receiving treatments in multiple locations.

BACKGROUND: Public awareness of colorectal cancer (CRC) and uptake of CRC screening remain challenges. The viewpoints of the target population (asymptomatic individuals older than 50) regarding CRC screening information sources and the reasons for and against participation in CRC screening are not well known in the Czech Republic. This study aimed to acquire independent opinions from the target population independently on the health system. AIM: To investigate the viewpoints of the target population regarding the source of information for and barriers and facilitators of CRC screening. METHODS: A survey among relatives (aged 50 and older) of university students was conducted. Participants answered a questionnaire about sources of awareness regarding CRC screening, reasons for and against participation, and suggestions for improvements in CRC screening. The effect of certain variables on participation in CRC screening was analyzed. RESULTS: Of 498 participants, 478 (96%) respondents had some information about CRC screening and 375 (75.3%) had participated in a CRC screening test. General practitioners (GPs) (n = 319, 64.1%) and traditional media (n = 166, 33.3%) were the most common information sources regarding CRC screening. A lack of interest or time and a fear of colonoscopy or positive results were reported as reasons for non-participation. Individuals aged > 60 years [adjusted odds ratio (aOR) = 2.30, 95% confidence interval (CI) (1.42-3.71), P = 0.001], females (aOR = 1.95, 95%CI (1.26-3.01) P = 0.003), and relatives of CRC patients (aOR = 4.17, 95%CI (1.82-9.58) P = 0.001) were more likely to participate in screening. Information regarding screening provided by physicians - GPs: (aOR = 8.11, 95%CI (4.90-13.41), P < 0.001) and other specialists (aOR = 4.19, 95%CI (1.87-9.38), P = 0.001) increased participation in screening. Respondents suggested that providing better explanations regarding screening procedures and equipment for stool capturing could improve CRC screening uptake. CONCLUSION: GPs and other specialists play crucial roles in the successful uptake of CRC screening. Reduction of the fear of colonoscopy and simple equipment for stool sampling might assist in improving the uptake of CRC screening.

INTRODUCTION: The evaluation of plasma levels of antidepressants may improve the treatment outcome. The aim was to verify adherence and adequacy of administered doses of antidepressants among patients hospitalized for inadequate outpatient therapeutic response. METHODS: Selective serotonin reuptake inhibitors or venlafaxine plasma levels were assessed on the first day of hospitalization and after 3 days of controlled administration. The patients were considered adherent if the plasma level on admission was within the interval of the minimum and maximum plasma level on the fourth day, expanded by 30%. The adequacy of antidepressant doses used during the outpatient treatment was assessed by comparing the plasma level on the fourth day with the therapeutic reference range. RESULTS: Out of 83 patients, 52 (62.7%) were adherent. The plasma levels of antidepressants on the fourth day were found to be within the therapeutic reference range in 35 (43.2%) patients. The same number manifested levels below the therapeutic reference range. In 11 (13.6%) patients, the levels were higher than recommended. No significant difference in rate of adherence was found among individual antidepressants. CONCLUSION: The results show that antidepressant nonresponders are frequently under-dosed or nonadherent.

Úvod: Agresia a násilie v zdravotníckych zariadeniach sa celosvetovo zvyšuje a často je vysvetľovaná stresujúcim prostredím. Zdravotníci prichádzajú do pomerne úzkeho kontaktu s pacientmi, ktorí bývajú často ovplyvnený svojou chorobou, bývajú v psychickej i fyzickej nepohode trpiaci pocitom bezmocnosti. Prostredníctvom výskumu analyzujeme problematiku výskytu agresívneho správania pacientov v zdravotníckych zariadeniach. Metódy: Na overenie hypotéz sme použili nástrojmi induktívnej štatistiky. Väčšinu premenných sme si zvolili nominálne, vzhľadom k tomu na overenie hypotéz sme použili test nezávislosti chí kvadrát χ2 test vhodný pre tieto premenné. Rozhodnutie o významnosti rozdielov vykonávame na základe vypočítanej p-hodnoty a hladine významnosti 0,05. Výsledky: Analýzou výsledkov sme zistili, že zdravotnícki pracovníci sa stretávajú s agresiou, agresívnym správaním pacientov či už odôvodnene alebo nie záleží na uhle pohľadu. Dôležité je, že vo veľkej miere dokážu rozpoznať príznaky agresívneho správania. Podstatná časť respondentov v zdravotníckych zariadeniach kde bol vykonaný výskum má záujem o získavanie ďalších informácii o danej problematike. Záver: Práca v zdravotníctve je z hľadiska odbornej pripravenosti náročným povolaním, obzvlášť práca sestier. Zdravotnícky pracovníci nesú veľkú profesionálnu zodpovednosť, je preto potrebné aby zvládali komunikáciu s pacientom obzvlášť agresívnym a musia sa neustále vzdelávať.
Introduction: Aggression and violence in healthcare facilities is increasing worldwide and is often explained by a stressful environment. Healthcare workers come into relatively close contact with patients who are often affected by their illness, are in a mental and physical disorder and suffering from a sense of helplessness. We analyze through research the issue of aggressive behavior of patients in healthcare facilities. Methods: We used inductive statistics to verify hypotheses. For most of the variables were chosen the nominals and we used the chi quadratic independence test x2 suitable for these variables. Decision on the significance of the differences is based on the calculated p value and the significance level of 0.05. Results: By analyzing the results we found out that health care workers are confronted with aggression, aggressive behavior of patients, whether reasonably or not, depending on the point of view. The important thing is that they can largely recognize the signs of aggressive behavior. Most of the respondents in the health care facilities where the research was performed is interested in obtaining further information on the issue. Conclusion: Healthcare work is a demanding profession, especially nursing work, in terms of professional readiness. Health care professionals have a great deal of professional responsibility, so they need to handle communication with particularly aggressive patient and they must be constantly educated.

Kazuistika ukazuje vynikající účinnost desetitýdenní léčby fixní kombinací depotního inzulínu degludek s analogem inkretinu liraglutid (preparát Xultophy). Xultophy byl nasazen u mladé ženy s diabetem 2. typu, kterou se předtím mnoho let nedařilo kompenzovat suplementární léčbou moderními analogy depotního a rychlého inzulínu a linagliptinem.
This case report demonstrates the outstanding effectiveness of a 10-week treatment with fixed combination of insulin degludec together with incretin analog liraglutide (Xultophy). Xultophy was administered in a young woman with type 2 diabetes mellitus previously uncontrolled under supplementary treatment using recent analogs of longacting and short-acting insulin preparation and linagliptin.

OBJECTIVE: Celiac disease (CD) is a chronic long-term disease with autoimmune disorder connected to an allergic reaction to the molecule of gluten. CD is manifested in persons who are genetically predisposed. The only therapy for CD is a strong gluten-free (GF) diet. The aim of our study was to follow the adherence to the gluten-free diet in the selected group of parents and caregivers of children with CD, and to compare factors influencing the adherence to GF diet and therefore influencing also clinical symptoms of CD. We compared and examined important food-related activities as well, by using a personal questionnaire in the group of CD children's parents or caregivers. METHODS: We designed a cross-sectional study; 325 parents or caregivers of children with CD were recruited in the sample. CD was confirmed by the European Society for Paediatric Gastroenterology Hepatology and Nutrition (ESPGHAN) criteria. The anonymous questionnaires were collected at summer camps within period data 2012-2016 and followed factors which might have an influence on compliance with GF diet, according to parents' opinions. RESULTS: Adherence to GF diet was statistically significantly higher among girls compared to boys. Parents' questionnaires confirmed that younger group of children and children with the family history of CD had statistically higher compliance with GF diet. Parents with higher education confirmed better adherence to GF diet, statistical analyses confirmed higher adherence in mother´s education compared to father's education. Duration of CD, socio-demographic factors of parents - age of parent, gender of parent, and residency were not statistically significant in terms of compliance with the diet. CONCLUSIONS: Our study confirmed important factors influencing adherence to the GF diet, which are connected to the symptoms and consequences of CD. The assessment of parents' opinions and burden on the families of children with celiac disease should be also reflected in the evaluation of patient's status. Important is also the role of parents in order to facilitate and support clinical interventions and prevention of CD.
