PURPOSE OF THE STUDY: In this study, we aimed to evaluate acromiohumeral distance (AHD) and supraspinatus tendon (ST) thickness measurements and their relationship with pain and function in ST pathologies. MATERIAL AND METHODS: The study included 111 patients and 25 healthy controls (HC). Patients were divided into 3 groups according to their diagnosis: non-tear tendinopathy (NTT), partial thickness tear (PTT), and full thickness tear (FTT). The AHD and ST thickness of the participants were measured with ultrasound. The pain and functional status of the patients were evaluated with the Numeric Rating Scale (NRS), The QuickDASH shortened version of the DASH Outcome Measure - Disabilities of the Arm Shoulder and Hand (QDASH), and Simple Shoulder Test (SST). RESULTS: The AHD value was significantly higher in the NTT group (p=0.000). The AHD value was significantly lower in the FTT group (p=0.000). ST thickness value was significantly lower in the PTT group compared to the NTT group (p=0.000). There was a positive correlation between ST thickness and BMI (r=0.553,p<0.01). There was a negative correlation between ST thickness and SST and a positive correlation between ST thickness (r=-0.223,p<0.05) and QDASH (r=0.276,p<0.05). CONCLUSIONS: We found that AHD and SST thicknesses significantly differed in the NTT, PTT, FTT, and HC groups. This difference may be important for diagnosis. In addition, the effect of obesity on ST thickness and the relationship between ST thickness and functional scores may be considered. Weight control may be effective at this point. The coracoclavicular joint is a diarthrodial synovial joint that is eventually located between the upper surface of the horizontal part of the coracoid process and the conoid tubercle of the clavicle, and is considered an unusual anatomical alteration. The coracoclavicular joint has a low prevalence and can be diagnosed by imaging tests - radiography and computed tomography. Treatment can be performed both conservatively and surgically. We report a case of an 81-year-old female patient presenting of pain in her left shoulder due to coracoclavicular joint arthrosis. A radiograph of the left shoulder was performed, which detected a deformity in the lower portion of the middle third of the clavicle and the upper portion of the coracoid process, corresponding to the coracoclavicular joint, a finding confirmed by computed tomography. The patient was treated conservatively with analgesics (Dipyrone) and anti-inflammatories (Ibuprofen) with improvement in symptoms.
Horná končatina je dôležitým a komplikovaným orgánom, ktorý umožňuje širokú škálu pohybov potrebných pre prácu, tvorbu a realizáciu zámerov. Problémy v oblasti ramenného kĺbu majú výrazný dopad na kvalitu života človeka. Rameno, ako koreňový kĺb hornej končatiny, je najpohyblivejší a zároveň anatomicky a funkčne najzložitejší kĺb tela. Pre jeho špecifickú stavbu, veľký rozsah pohybu a neustálu aktivitu je ľahko zraniteľný a môže dôjsť k impingement syndrómu. Vyskytuje sa v čoraz mladšej populácii v produktívnom veku a býva príčinou dlhodobej práceneschopnosti. Cielenou fyzioterapiou je ale možné v danej oblasti funkčné zmeny a patologické svalové zreťazenie ovplyvniť, čím sa kvalitatívne zlepší hybný stereotyp. Vyšetrovali sme 20 pacientov s diagnózou impingement syndrómu bez predošlého operačného riešenia. Cielenou fyzioterapiou sme dokázali z väčšej miery úspešne odstrániť bolesť u 85 % pacientov, znormalizovať zvýšený tonus horných fixátorov a tíming u 60 % pacientov a správne zafixovať lopatku u 70 % pacientov.
The upper extremity is an important and complicated organ that enables a wide range of movements necessary for work, creation and realization of intentions. Problems in the area of the shoulder joint have a significant impact on a person’s quality of life. The shoulder, as the root joint of the upper limb, is the most mobile and, at the same time, the most anatomically and functionally complex joint of the body. Due to its specific structure, a large range of movement and constant activity, it is easily vulnerable and impingement syndrome can occur. It occurs in an increasingly younger population of productive age and is the cause of long-term incapacity for work. However, with targeted physiotherapy, it is possible to influence functional changes and pathological muscle concatenation in the given area, which qualitatively improves the movement stereotype. We investigated 20 patients with a diagnosis of impingement syndrome without a previous surgical solution. With targeted physiotherapy, we were able to largely successfully eliminate pain in 85% of patients, normalize the increased tone of the upper fixators and timing in 60% of patients, and correctly fix the scapula in 70% of patients.

PURPOSE OF THE STUDY Subacromial impingement is one of the most common reasons for shoulder pain. The surgical management of this condition has recently become the focus of criticism because of the rising number of surgical procedures and the lack of superiority of surgical over conservative treatment. In this prospective comparative study, we compared standardised conservative care with surgical treatment and placed special emphasis on the patients' ability to work. MATERIAL AND METHODS A total of 106 patients (25 women, 81 men; mean age: 45.4 12.3 years) were included in this prospective comparative clinical study. Patients in the non-operative arm (n = 42) received standardised physiotherapy. Patients in the surgical arm (n = 38) underwent arthroscopic subacromial decompression. All patients were followed up at 3, 6 and 12 months. Shoulder function (Constant score), pain (Numerical Rating Scale), and the duration of inability to work were assessed. RESULTS Shoulder function and pain improved significantly with both kinds of treatment. At no time of follow up we detected significant differences between the two treatment options. An analysis of the patients' ability to work showed that conservative treatment was superior to surgical treatment at 3-month follow-up (0.3 versus 5.0 weeks; p < 0.001) and between 4 and 6-month after intervention (0.2 versus 1.6 weeks; p = 0.032). DISCUSSION In the study presented here, significant improvements in function (Constant score) and pain (NRS) were achieved in both the non-operative and the surgical arm. There were no significant differences between the two groups at any time point. These results are similar to those reported by other authors in recent studies. Unlike other research work, however, our study demonstrated a major difference in the development and duration of inability to work. CONCLUSIONS Conservative and surgical treatment of subacromial impingement syndrome led to similar outcomes for shoulder pain and function at 3, 6 and 12 months after intervention. However, patients who were managed conservatively returned to work significantly earlier than patients who underwent surgery. PURPOSE OF THE STUDY The purpose of this study is to investigate the acute and long term effectiveness of kinesio taping applied following shoulder arthroscopy in relieving pain and reducing swelling. MATERIAL AND METHODS This study included 50 patients undergone shoulder arthroscopy between June 2016 and December 2017 in our clinic. Patients were randomly assigned into two groups. Group I consisted of patients who had kinesiotherapy; while Group II consisted of control patients whom we applied sham taping with no effect. Pre and postoperative pain and swelling status of patients were recorded and groups were compared. RESULTS Comparing two groups, we found that kinesio taping significantly reduced pain levels in the early post-operative period. However, we didn't detect any significant difference in reduction of shoulder swelling between two groups. CONCLUSIONS Kinesio taping could be an alternative treatment in relieving pain after shoulder arthroscopy. However, we didn't find a significant reduction in swelling in shoulder.
