Užívanie tabaku je na epidemickej úrovni a je hlavnou príčinou invalidity a predčasných úmrtí, pričom neúmerne postihuje zraniteľné skupiny obyvateľstva. Zatiaľ čo spotreba cigariet je na historickom minime, používanie elektronických cigariet prudko vzrástlo, najmä u tínedžerov, ktorí sú najviac ohrození celoživotnou závislosťou od tabaku. Odvykanie od fajčenia je ťažké dosiahnuť. Sestry zohrávajú dôležitú úlohu pri poskytovaní poradenstva a vzdelávania. Pri odvykaní od fajčenia u svojich pacientov by mali využívať účinné metódy. Produkty náhradnej nikotínovej terapie obsahujú rôzne množstvá nikotínu a sú účinné pri znižovaní túžby po nikotíne, ako aj pri abstinenčných príznakoch. Behaviorálna terapia a podpora sú nevyhnutné na to, aby pomohli fajčiarom rozvíjať zručnosti a aby sa dokázali vzdať cigariet a fajčenia.
Tobacco use is at epidemic levels and a leading cause of disability and premature death, disproportionately affecting vulnerable populations. While cigarette consumption is at an all-time low, e-cigarette use has soared, especially among teenagers, who are most at risk of lifelong tobacco addiction. Quitting smoking is difficult to achieve. Nurses play an important role in providing counseling and education and should use effective smoking cessation methods for their patients. Nicotine replacement therapy products contain varying amounts of nicotine and are effective in reducing nicotine cravings as well as withdrawal symptoms. Behavioral therapy and support are essential to help smokers develop the skills to give up cigarettes and smoking.
Prevalence kouření tabáku je u nemocných schizofrenií vysoká, podle metaanalýz 2–3× vyšší než v běžné populaci a dosahuje 45–80 %. Tito nemocní mají navíc silnější závislost ve srovnání s ostatními kuřáky (kouří větší množství cigaret, častěji a s větší intenzitou) a také případný odvykací stav od nikotinu bývá těžší. Mají vyšší mortalitu i morbiditu v důsledku kouření. Kouření cigaret bylo prokázáno jako rizikový faktor rozvoje schizofrenie a zhoršení její prognózy. Práce shrnuje možné důvody kuřáctví u této skupiny nemocných: genetické faktory, používání kouření k sebemedikaci příznaků schizofrenie a nežádoucích účinků psychofarmak, ale také sociální faktory. Přes jasně prokázaná negativa kouření jak obecná, tak specifická pro pacienty se schizofrenií, je kouření u těchto nemocných v ČR přehlíženo a léčba závislosti na tabáku není běžně doporučována a nabízena.
The prevalence of tobacco smoking in patients with schizophrenia is high (45–80 %), according to meta-analyses 2–3 times higher than in the given population. In addition, these patients have a stronger addiction compared to regular smokers (they smoke a larger number of cigarettes, more often and with greater intensity) and the eventual withdrawal from nicotine tends to be more difficult. They have higher mortality and morbidity due to smoking. Cigarette smoking has been shown to be a risk factor for developing schizophrenia and worsening its prognosis. The thesis summarizes the possible reasons for smoking in this group of patients: genetic factors, the use of smoking to self-medicate the symptoms of schizophrenia and the side effects of psychotropic drugs, but also social factors. Despite the clearly proven negative effects of smoking, both general and specific to patients with schizophrenia, smoking is overlooked in the Czech Republic and tobacco addiction treatment is not routinely recommended and offered.

AIMS: To compare success rates and characteristics of smokers treated a second time by a smokers' clinic with success rates of their first treatment. DESIGN: Retrospective cohort study. SETTING: Tobacco Dependence Treatment clinic in Prague, Czech Republic, between 2005 and 2017. PARTICIPANTS: A total of 5225 smokers treated either once (n = 5006, single treatment sample, SS) or also second time (n = 219, re-treated sample, RS), on average 4.47 years after the first visit. INTERVENTION: Smokers received intensive treatment of tobacco dependence with pharmacotherapy options. Outcomes were evaluated after 1 year. In case of failure or relapse, participants could undergo re-treatment in the same setting at least 1 year after the start of the first treatment. MEASUREMENTS: Twelve-month self-reported continuous abstinence; CO-validated (≤ 6 parts per million); number of visits; type of pharmacotherapy; mental health history; Fagerström Test for Cigarette Dependence; time between first and second treatment. RESULTS: The abstinence rate in the SS was 34.8% [95% confidence interval (CI) = 33.4%, 36.1%] and in the RS was 37% (95% CI = 30.6%, 43.8%) and 39.7% (95% CI = 33.2%, 45.5%) for their first and second treatments, respectively. The samples were comparable on smoking and socio-demographic characteristics and pharmacotherapy used, but the RS in the second treatment had a higher prevalence of diagnosed mental health disorder at 39.3% (95% CI = 32.8%; 46.1%) compared with 23.7% (95% CI = 22.5%; 24.9%) in the SS. Participants who initiated their second quit attempt 1 to 2 years after the first one were less successful than those who initiated their second quit attempt later (25 versus 43%; P < 0.05). The results of the first treatment cycle were not found to be a reliable predictor for outcomes of the second cycle of treatment in univariate or multivariate logistic regression (odds ratio = 1.35, 95% CI = 0.70-2.63, P = 0.373). CONCLUSION: In Prague, Czech Republic, smokers re-attending stop-smoking treatment more than 2 years after their previous quit attempt appear to achieve similar success rates to those being treated for the first time.
