Cíl: Popis průběhu implementace a vyhodnocení úspěšnosti dodržování doporučených postupů ERAS protokolu v podmínkách českého zdravotnictví. Soubor a metodika: Do studie bylo zařazeno celkem 163 pacientek od března do září 2022, tedy celkem 7 měsíců. Pacientky byly podle typu operačního výkonu rozřazeny do tří skupin. Klinický protokol: Onkogynekologie, hysterektomie a laparoskopie. Implementace probíhala ve třech fázích (příprava, samotná implementace protokolu a vyhodnocení). Výsledky a závěry: Hlavním hodnotícím kritériem byla míra dodržování jednotlivých doporučení (tzv. adherence). Ve všech třech skupinách byla souhrnná adherence 90 % a více. Koncept ERAS jsme na základě pilotních výsledků na našem pracovišti vyhodnotili jako dobře implementovatelný nástroj pro gynekologická pracoviště v podmínkách českého zdravotního systému.
Objective: To describe the implementation process and evaluate the success of compliance with the recommended ERAS protocol in the Czech healthcare system. Methods: The study included 163 patients from March to September 2022, a total of 7 months. Patients were divided into three groups according to the type of surgery. Clinical protocol: Oncogynecology, hysterectomy and laparoscopy. The implementation was realized in three phases (preparation, implementation of the protocol itself and evaluation). Results and conclusions: The cumulative adherence rate was 90% or more in all three groups. Based on the pilot results at our department, we evaluated the ERAS concept as a well-implemented tool for gynaecological departments in the Czech healthcare system.
Úvod: V České republice dlouhodobou dostatečnou adherenci k terapii kontinuálním přetlakem (CPAP) vykazuje asi 70 % pacientů se syndromem obstrukční spánkové apnoe (OSA). Důležitým prostředkem pro udržení či zvýšení těchto hodnot může být dodatečná a opakovaná edukace, zahrnující i instruktáž ke cvičení. To v současné době není v tuzemských podmínkách běžně využíváno. Proto bylo cílem zjistit, jak opakovaná edukace s instruktáží ke cvičení ovlivní adherenci pacientů k používání CPAP terapie. Soubor a metodika: 6 pacientů se syndromem OSA podstoupilo vstupní edukaci, polovina z nich kromě úvodní edukace absolvovala 4týdenní edukačně motivační program, zahrnující instruktáž ke cvičení pod dohledem fyzioterapeuta. Druhá polovina byla pouze edukována, bez dodatečné intervence. Data o adherenci pacientů byla extrahována ze systému AirView, který zaznamenává aktivitu používaného přístroje. Výsledky a závěr: Výsledky naznačují pozitivní vliv edukačně-motivačního programu na adherenci pacientů se syndromem OSA. Dodatečná a opakovaná edukace a motivace, včetně instruktáže ke cvičení by se tak mohla stat novou úlohou fyzioterapeuta v rámci multidisciplinárního týmu věnujícího se léčbě pacientů se syndromem OSA.

Limited health literacy (HL), depression and anxiety are common in dialyzed patients and affect health outcomes and self-management. We explored whether depression and anxiety mediate the association of HL with diet non-adherence (DN-A) in dialyzed patients. We performed a cross-sectional study in 20 dialysis clinics in Slovakia (n = 452; mean age: 63.6 years; males: 60.7%). Hierarchical cluster analysis was performed to create three HL groups. Logistic regression adjusted for age, gender and education was used to explore whether depression and anxiety mediate the association of HL with DN-A. Patients in the moderate HL group were more likely to be non-adherent to diet (OR (Odds Ratio)/95% CI: 2.19/1.21-3.99) than patients in the high HL group. Patients in the low HL and moderate HL group more likely reported depression or anxiety. Patients reporting depression (OR/95% CI: 1.94/1.26-2.98) or anxiety (OR/95% CI: 1.81/1.22-2.69) were more likely to be non-adherent with diet. Adjustment for depression reduced the association between moderate HL and DN-A by 19.5%. Adjustment for anxiety reduced the association between moderate HL and DN-A by 11.8%. Anxiety and depression partly mediated the association of HL with DN-A. More attention should be paid to treating patients' psychological distress to ensure adequate adherence with recommended diet.

To date the only efficient treatment of celiac disease is a lifelong gluten-free diet (GFD), which involves relevant lifestyle changes. Numerous methods measure adherence to GFD, but none is completely reliable. The aim of the study was to compare three frequently used methods to measure adherence to GFD and study factors that influence adherence to GFD. Eighty-one celiac patients 15 years or older, on GFD were evaluated by dietitian interview, a Celiac Dietary Adherence Test (CDAT) and blood antitransglutaminase antibodies (tTG). Factors influencing adherence were assessed by an ad-hoc questionnaire following WHO criteria. Adherent and non-adherent patients were classified in the same category in 44.4% of cases (n=36), (non-adherent=35.8% and adherent= 8.6%). In general, methods identified better non-adherent than adherent individuals. Among the 5 realms defined by WHO, when tTG (positive/negative) defined adherence, logistic regression identified ten significant variables (information about disease, income, education, cost of gluten-free products, eating in restaurants, time on GFD, symptoms at diagnosis, number of symptoms at time of diagnosis, other chronic diseases present, allergy/food intolerance plus a chronic disease and CD). Using the interview as reference, two variables were significant (self-perception of knowledge of the GFD, and presence of gastrointestinal symptoms when gluten is consumed). Results illustrate the difficulties of measuring both adherence to GFD and the factors that influences it. Further studies should explore new markers able to measure the amount of gluten necessary to activate autoantibodies production and the time they take to stop their production once the patient stops gluten ingestion.

BACKGROUND: Historically, studies of adherence to airway clearance therapy in cystic fibrosis (CF) have relied on self-reporting. We compared self-reported airway clearance therapy adherence to actual usage data from home high-frequency chest wall compressions (HFCWC) vests and identified factors associated with overestimation of adherence in self-reports. METHODS: Pediatric patients who perform airway clearance therapy with a HFCWC vest were eligible to participate. Objective adherence data were obtained from the HFCWC device, which records cumulative utilization time. Two readings at least 5 weeks apart were collected. Objective adherence was recorded as a ratio of mean-to-prescribed daily use (%). Self-reported adherence data were collected with a caregiver survey at enrollment. Adherence rates were categorized as low (< 35% of prescribed), moderate (36-79% of prescribed), and high (≥ 80% of prescribed). An overestimation was present when self-reported adherence was at least one category higher than objective adherence. RESULTS: In the final sample (N = 110), mean adherence by usage data was 61%. Only 35% of subjects (n = 38) were highly adherent, and 28% (n = 31) were low adherent. In contrast, 65% of subjects (n = 72) reported high adherence and only 8% (n = 9) reported low adherence (P < .001). Nearly half of self-reports (46%) overestimated adherence. In a multiple regression analysis, overestimation was associated with multiple airway clearance therapy locations (odds ratio 7.13, 95% CI 1.16-43.72, P = .034) and prescribed daily use ≥ 60 min (odds ratio 3.85, 95% CI 1.08-13.76, P < .038). Among subjects with prescribed daily airway clearance therapy ≥ 60 min, the odds of overestimating adherence increased 3-fold (odds ratio 3.04, 95% CI 1.17-7.87, P = .02) in a lower-income (< $50,000/y) environment. CONCLUSIONS: Self-reports overestimated actual adherence to airway clearance therapy, and the overestimation increased with treatment occurring in multiple households and prescribed therapy duration. Among participants with prescribed airway clearance therapy ≥ 60 min, overestimation increased with lower income. Objective measures of adherence are needed, particularly for lower-income children and those receiving treatments in multiple locations.

Non-adherence to dietary and fluid intake recommendations (NADFIR) is an important factor for the effective treatment of dialyzed patients and may be hindered by low health literacy (HL). Therefore, we assessed whether low HL of dialyzed patients is associated with their NADFIR. We performed a multicentric cross-sectional study in 20 dialysis clinics in Slovakia (n = 452; response rate: 70.1%; mean age = 63.6 years; males: 60.7%). We assessed the association between nine domains of HL and non-adherence (high serum potassium, high serum phosphate, relative overhydration, and self-reported NADFIR) using general linear models adjusted for age and gender. Moreover, we assessed the moderation by socioeconomic status (SES). We found higher NADFIR among patients with less sufficient information for health management (high serum phosphate level; odds ratio (OR): 0.77; 95% confidence interval (CI): 0.63-0.94), with a lower ability to actively manage their health (self-reported diet non-adherence; OR: 0.74; 95% CI: 0.62-0.89), and those less able to actively engage with healthcare providers (overhydrated; OR: 0.78; 95% CI: 0.65-0.94). Moreover, SES modified this relation. Low HL affects the adherence of dialyzed patients. This shows a need to support patients with low HL and to train healthcare providers to work with these patients, taking into account their SES.

Attitudes towards psychiatric medication are an important factor influencing the success of treatment. Mental health care in the region of Central and Eastern Europe remains biologically oriented, yet the attitudes of people with severe mental illnesses are largely unknown. In the present study, we aimed to analyze factors of attitudes toward psychopharmacological drugs among people with schizophrenia spectrum disorder who use community social services in the Czech Republic. Drug Attitude Inventory (DAI) was used as a primary research instrument and data were analyzed using a multivariable linear regression. The majority of respondents (78%) had positive attitudes towards psychopharmacological treatment. Additionally, attitudes towards psychopharmaceutic drugs did not differ with regard to sociodemographic characteristics except for family status. There was a significant negative correlation with the level of self-stigmatization and a significant positive correlation with the duration of treatment. Respondents who had repeated experiences with discontinuation of medication without a prior consultation with a psychiatrist had significantly worse attitudes towards medication than respondents without this experience. This study revealed evidence of a strong relationship between the attitudes toward medication and a history of discontinuation of taking medication and duration of treatment. Therefore, we suggest that interventions focused preventing the internalization of stigmatizing attitudes towards mental illness should be available to people with schizophrenia spectrum disorder, especially in the early stages of the illness - irrespective of the patient's age.
