Specifická alergenová imunoterapie (SAIT) je jediný kauzální způsob léčby alergie, který zasahuje přímo do imunopatologických mechanismů, jejichž důsledkem jsou klinické symptomy alergie pacienta. V současnosti je SAIT indikována k léčbě alergické rinokonjunktivitidy, alergického astmatu a alergie na jed blanokřídlého hmyzu projevující se anafylaktickou reakcí. Během léčby se do organismu alergika v pravidelných časových intervalech vpravují definované dávky terapeutického alergenu, na který je tento pacient přecitlivělý. Cílem je snížit reaktivitu jedince na konkrétní alergen. Formy aplikace v současnosti jsou injekční subkutánní forma a forma sublingvální buď kapková, nebo tabletová. Doba léčby je 3-5 let. Budoucnost v oblasti alergenové imunoterapie bude patřit přípravkům s vyšší imunogenicitou a nižší alergenicitou, preferenčně v neinjekční tabletové formě.
Specific allergen immunotherapy (SAIT) is the unique treatment affecting directly the immunopathological mechanisms of IgE mediated allergies. Allergic rhinitis, allergic asthma bronchiale and hymenoptera venom allergy with the history of anafylaxis are the indications for SAIT nowadays. The treatment consists of the consecutive application of causal allergen standardized extract in the ascending order. The aim is to lower the hypersensitivity to the causal allergen. The application routes of SAIT are sublingual (SLIT) or subcutaneous (SCIT). The duration of treatment varies from 3 to 5 years. The future perspectives in SAIT intend to alleviate the allergenicity and to enhance the immunogenicity of SAIT, preferentially via the SLIT application route.

Alergická onemocnění jsou chápána jako systémová onemocnění s orgánovými příznaky. Alergická rýma je definována jako soubor nosních příznaků, vznikajících na podkladě IgE – zprostředkovaného zánětu po alergenové expozici nosní sliznice. Základní charakteristikou alergické rýmy je perzistující eozinofilní zánět provázený typickými klinickými projevy. Alergická rinitida se rozvíjí na podkladě četných vlivů zevního prostředí u geneticky disponovaného jedince. Projevy rýmy mohou být spontánně reverzibilní nebo ustupují po léčbě. Plán vedení léčby alergické rýmy obsahuje edukaci pacienta a eliminaci alergenů, farmakoterapii, specifickou alergenovou imunoterapii. Sublingvální tabletová forma alergenové imunoterapie (pyly trav, roztoči domácího prachu) je moderní alternativou léčby subkutánní. Tato léčba snižuje závažnost alergických příznaků, snižuje riziko vzniku nových alergických senzibilizací a inhibuje rozvoj klinického astmatu u pacientů léčených pro alergickou rýmu.
Allergic diseases are seen as systemic diseases with organ symptoms. Allergic rhinitis is defined as a group of nasal symptoms,arising from an underlying IgE-mediated inflammation following exposure of the nasal mucosa to allergens. The basic feature ofallergic rhinitis is a persistent eosinophilic inflammation accompanied by typical clinical manifestations. Allergic rhinitis developson the basis of numerous environmental factors in a genetically predisposed individual. The symptoms of rhinitis may be spontaneouslyreversible or resolve following treatment. The management of allergic rhinitis involves patient education, eliminationof allergens, pharmacotherapy, and specific allergen immunotherapy. Sublingual tablet dosage form of allergen immunotherapy(grass pollens, house dust mites) is a modern alternative to subcutaneous treatment. This treatment reduces the severity ofallergy symptoms, lowers the risk of developing new allergic sensitizations, and prevents the development of clinical asthma inpatients treated for allergic rhinitis.

Pimecrolimus (Elidel, SDZ ASM 981) is an anti-inflammatory and immunomodulatory 33-epichloro-derivative of macrolactam ascomycin, with low potential for affecting systemic immune responses compared with other calcineurin inhibitors, cyclosporin A and tacrolimus. Despite numerous studies focused on the mechanism of pimecrolimus action on mast cells, only the single report has addressed pimecrolimus effects on other typical FcεRI-expressing cells, the basophils. Patients allergic to birch pollen (n = 20), hymenopteran venoms (n = 23) and 10 non-allergic volunteers were examined. Primary human basophils pre-treated or not with 0.5-50 μMol pimecrolimus were exposed to various concentrations of recombinant Bet v 1a allergen, bee or wasp venom extracts and anti-IgE for 20 min, and then examined for the expression of CD45, CD193, CD203c, CD63 and CD164 using flow cytometry. The externalization of basophil activation markers (CD63 and CD164) was equally inhibited through pimecrolimus in cells activated by recombinant pollen allergen, hymenopteran venom extracts and anti-IgE. Although the individual response rate was subject to strong variation, importantly, pre-treatment with pimecrolimus lowered the number of activated basophils in response to any of the stimuli in the basophils from all patients. The inhibition was concentration-dependent; approximately half of the basophils were inhibited in the presence of 2.5 mMol pimecrolimus. Pimecrolimus is a valuable new tool for the inhibition of hyper-reactive basophils in patients with pollen allergy and a history of anaphylactic reactions to bee or wasp venoms. Further research should address short-term use of pimecrolimus in vivo in a wide spectrum of allergic diseases.

Autor podává přehled o léčebné skupině antihistaminik se zaměřením na přípravky II. generace se zvýrazněným imunomodulačním účinkem, které se stále významněji uplatňují u všech onemocnění, jejichž základem je alergický zánět. Účinnost a bezpečnost těchto léčiv by měly být široce využity nejen specialisty, ale hlavně praktickými lékaři, ke včasnému zvládnutí alergického zánětu.
The author gives an overview of the antihistaminic category of medications, focusing on 2nd generation preparations with an enhanced immunomodulatory effects, which are finding ever more significant application in all diseases which are underlined by an allergic inflammation. The efficacy and safety of these drugs should be taken advantage of by not just specialists but especially by general practitioners, in order to quickly and efficiently manage allergic inflammations.

Mast cells and basophils (MCs/Bs) play a crucial role in type I allergy, as well as in innate and adaptive immune responses. These cells mediate their actions through soluble mediators, some of which are targeted therapeutically by, for example, H1- and H2-antihistamines or cysteinyl leukotriene receptor antagonists. Recently, considerable progress has been made in developing new drugs that target additional MC/B mediators or receptors, such as serine proteinases, histamine 4-receptor, 5-lipoxygenase-activating protein, 15-lipoxygenase-1, prostaglandin D2, and proinflammatory cytokines. Mediator production can be abrogated by the use of inhibitors directed against key intracellular enzymes, some of which have been used in clinical trials (eg, inhibitors of spleen tyrosine kinase, phosphatidylinositol 3-kinase, Bruton tyrosine kinase, and the protein tyrosine kinase KIT). Reduced MC/B function can also be achieved by enhancing Src homology 2 domain-containing inositol 5' phosphatase 1 activity or by blocking sphingosine-1-phosphate. Therapeutic interventions in mast cell-associated diseases potentially include drugs that either block ion channels and adhesion molecules or antagonize antiapoptotic effects on B-cell lymphoma 2 family members. MCs/Bs express high-affinity IgE receptors, and blocking their interactions with IgE has been a prime goal in antiallergic therapy. Surface-activating receptors, such as CD48 and thymic stromal lymphopoietin receptors, as well as inhibitory receptors, such as CD300a, FcγRIIb, and endocannabinoid receptors, hold promising therapeutic possibilities based on preclinical studies. The inhibition of activating receptors might help prevent allergic reactions from developing, although most of the candidate drugs are not sufficiently cell specific. In this review recent advances in the development of novel therapeutics toward different molecules of MCs/Bs are presented.

This study was undertaken to evaluate possible antiallergic effects of an extract of pigments from green sea urchin (Strongylocentrotus droebachiensis) shells. Effects were studied on animal models - guinea pig ileum contraction, rabbit eyes allergic conjunctivitis, and rabbit local skin irritation. The extract significantly reduced, in a dose-dependent manner, the histamine-induced contractions of the isolated guinea pig ileum with ID50 =1.2 µg/mL (in equivalents of spinochrome B), had an inhibitory effect on the model of ocular allergic inflammation surpassing the reference drug olopatadine, and did not show any irritating effect in rabbits. The extract predominantly contained polyhydroxy-1,4-naphthoquinone which would be responsible for the pharmacological activity. The active compounds of the extract were evaluated in silico with molecular docking. Molecular docking into H1R receptor structures obtained from molecular dynamic simulations showed that all spinochrome derivatives bind to the receptor active site, but spinochrome monomers fit better to it. The results of the present study suggest possibilities for the development of new agents for treating allergic diseases on the base of pigments from sea urchins shells.
