BACKGROUND: Ceftolozane/tazobactam is approved for treatment of hospital-acquired/ventilator-associated bacterial pneumonia (HABP/VABP) at double the dose approved for other infection sites. Among nosocomial pneumonia subtypes, ventilated HABP (vHABP) is associated with the lowest survival. In the ASPECT-NP randomized, controlled trial, participants with vHABP treated with ceftolozane/tazobactam had lower 28-day all-cause mortality (ACM) than those receiving meropenem. We conducted a series of post hoc analyses to explore the clinical significance of this finding. METHODS: ASPECT-NP was a multinational, phase 3, noninferiority trial comparing ceftolozane/tazobactam with meropenem for treating vHABP and VABP; study design, efficacy, and safety results have been reported previously. The primary endpoint was 28-day ACM. The key secondary endpoint was clinical response at test-of-cure. Participants with vHABP were a prospectively defined subgroup, but subgroup analyses were not powered for noninferiority testing. We compared baseline and treatment factors, efficacy, and safety between ceftolozane/tazobactam and meropenem in participants with vHABP. We also conducted a retrospective multivariable logistic regression analysis in this subgroup to determine the impact of treatment arm on mortality when adjusted for significant prognostic factors. RESULTS: Overall, 99 participants in the ceftolozane/tazobactam and 108 in the meropenem arm had vHABP. 28-day ACM was 24.2% and 37.0%, respectively, in the intention-to-treat population (95% confidence interval [CI] for difference: 0.2, 24.8) and 18.2% and 36.6%, respectively, in the microbiologic intention-to-treat population (95% CI 2.5, 32.5). Clinical cure rates in the intention-to-treat population were 50.5% and 44.4%, respectively (95% CI - 7.4, 19.3). Baseline clinical, baseline microbiologic, and treatment factors were comparable between treatment arms. Multivariable regression identified concomitant vasopressor use and baseline bacteremia as significantly impacting ACM in ASPECT-NP; adjusting for these two factors, the odds of dying by day 28 were 2.3-fold greater when participants received meropenem instead of ceftolozane/tazobactam. CONCLUSIONS: There were no underlying differences between treatment arms expected to have biased the observed survival advantage with ceftolozane/tazobactam in the vHABP subgroup. After adjusting for clinically relevant factors found to impact ACM significantly in this trial, the mortality risk in participants with vHABP was over twice as high when treated with meropenem compared with ceftolozane/tazobactam. Some strains of the genus Enterococcus are effective probiotic bacteria if they meet safety and probiotic criteria. In our study, 17 canine enterococci previously selected from a group of 160 isolates based on safety criteria were screened for some functional properties relevant to their use as probiotics. The results of antimicrobial resistance testing showed sensitivity of eleven strains to EFSA recommended antimicrobials. In contrast, the most frequent resistance was observed for cefotaxim (15/17) and oxacillin (13/17). PCR detection of resistance genes (vanA, vanB, vanC, tetM, tetL, ermB, and mefA) revealed the presence of mefA gene in five Enterococcus faecium strains and vanA gene in one strain. The production of enzymes commonly associated with intestinal diseases was in general rare (β-glucosidase 2/17, α-chymotrypsin 1/17, N-acetyl-β-glucosaminidase 0/17, and β-glucuronidase 0/17). The measurement of strain survival rate (%) under the conditions simulating gastric (pH 2.5) and bile juices (0.3% bile) showed considerable differences between strains (< 0.01 to 4.7% after 90 min for gastric juices, 48.0 to 254.0% after 180 min for bile). The concentration of produced L-lactic acid ranged between 83.1 to 119.3 mmol/L after 48 h cultivation depending on the strain. All strains fermented 16 out of 49 different carbohydrates (range from 17 to 23/49). Antimicrobial activity was recorded for two strains against some species of Listeria sp. and Enterococcus sp. Finally, two E. faecium candidates (IK25 and D7) were selected for testing in dogs, and hereafter they could possibly extend the currently limited range of beneficial bacteria of canine origin used as a dietary supplement for dogs.

Ranná infekce je jednou z nejčastějších místních komplikací hojení ran. Léčba infikované ulcerace vyžaduje multidisciplinární přístup, který splňuje požadavky na racionální antimikrobiální terapii, na střízlivou indikaci systémové antibiotické terapie a zejména na omezení lokálního používání dermatologických extern s obsahem antibiotik. Pro topickou léčbu infikovaných nehojících se ran je nutné využívat tzv. biocidy. Nedodržení těchto zásad vede k šíření rezistentních bakteriálních kmenů v populaci a znemožňuje nasazení dosud účinných antibiotik do budoucna.
Wound infection in one of the most common complication in chronic wound healing. The effective treatment of infected ulcer is based on the interdisciplinary teamwork which meets the principles of antimicrobial stewardship programme. The most important step is to reduce topical usage of remedies and products with antibiotics and to indicate systemic antibiotic treatment more prudently. It is necessary to use so called biocides for topical treatment of infected non-healing wounds. Breaking the rules leads to spreading resistant germs in patient population and makes impossible future application of antibiotics.

The effective treatment of infection with Pseudomonas aeruginosa in patients with cystic fibrosis requires early pathogen detection. Highly sensitive detection of the P.aeruginosa using nested PCR will be the indication to start an antibiotic therapy. The benefit from novel therapeutic protocol will be evaluated by comparison with clinical and microbiological data of historical controls (2001-2004). Their sputa will be retrospectively examined for the presence of P.aeruginosa using novel PCR. The potential occurrence of transmissible strain of P.aeruginosa in a cohort of our patients with cystic fibrosis will be checked using two genotyping techniques: RAPD and macrorestriction typing with subsequent PFGE.
Efektivní léčba infekce Pseudomonas aeruginosa u pacientů s cystickou fibrózou vyžaduje její časnou diagnostiku. Velmi citlivý přímý průkaz patogenu pomocí nested PCR bude indikací k zahájení antibiotické léčby. Prospěch z nového léčebného protokolu budestanoven porovnáním s klinickými daty historických kontrol (2001 až 2004) a s jejich mikrobiologickými výsledky, jež budou doplněné o retrospektivní PCR vyšetření sputa na přítomnost P.aeruginosa. Existence přenosného kmene P.aeruginosa v komunitě pacientů s cystickou fibrózou bude zjišťována pomocí dvou genotypizačních technik: RAPD a makrorestrikční typizace s využitím PFGE.
