OBJECTIVES: Staphylococcus aureus bacteremia (SAB) is one of the most frequent bloodstream infections. High mortality of SAB can be significantly reduced by regular infectious disease (ID) consultations and appropriate clinical management. Because the pandemic of coronavirus disease 2019 (COVID-19) has had a negative impact on hospital ID service, it can be assumed that it has also led to decreased quality of care for SAB patients. METHODS: This study enrolled all (n = 68) patients with proven SAB who were hospitalized in Military University Hospital, Prague, in 2019 and 2020 and the quality of care indicators for SAB patients were compared. RESULTS: A total of 33 and 35 patients with SAB were hospitalized in our hospital in 2019 and 2020, respectively. The significant difference between the pandemic year 2020 and year 2019 was in ID consultations performed (74% vs. 100%; p = 0.002) and fulfilment of all quality of care indicators (66% vs. 93%; p = 0.012). Next, higher in-hospital mortality was observed in 2020 than in 2019 (6% vs. 23%; p = 0.085). There was no significant difference in the percentages of patients with performed echocardiographic examinations (66% vs. 83%; p = 0.156) and collected follow-up blood cultures (85% vs. 94%; p = 0.428). In addition, there was no difference between the two years in the adequate antibiotic therapy, sources, and bacterial origin of SAB. CONCLUSIONS: The quality of care of SAB patients significantly decreased during the COVID-19 pandemic in our institution.

Biofilm formation is an important physiological process in Staphylococcus aureus (S. aureus) that can cause infections in humans. In this study, the ability of 36 methicillin-resistant S. aureus (MRSA) clinical isolates to form biofilm was studied based on genotypic and phenotypic approaches. These isolates were genotyped based on the microbial surface components recognizing adhesive matrix molecules (MSCRAMMs) and biofilm-associated genes (icaAD) via polymerase chain reactions. Phenotyping was performed based on the determination of the strength of biofilm formation of MRSA isolates in vitro. The most prevalent MSCRAMMs and biofilm-associated genes were clfA, eno, and icaD, followed by clfB. The fnbB (38.9%) and ebpS (11.1%) occurred less frequently among the MRSA isolates, while bbp and fnbA genes were absent from all isolates. The MRSA isolates were mostly moderate to strong biofilm formers, despite the heterogeneity of the MSCRAMM profiles. MRSA isolates from different infection sources (primary, catheter-related bloodstream, or secondary infections) were capable of forming strong biofilms. However, persistent bacteraemia was observed only in 19.4% of the MRSA-infected individuals. This study suggested that persistent MRSA bacteraemia in patients might not be associated with the biofilm-forming ability of the isolates.
In the microbiological diagnosis of bloodstream infections (BSI), blood culture (BC) is considered the gold standard test despite its limitations such as low sensitivity and slow turnaround time. A new FDA-cleared and CE-marked platform utilizing magnetic resonance to detect amplified DNA of the six most common and/or problematic BSI pathogens (Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa, and Escherichia coli; referred to as ESKAPEc) is available and may shorten the time to diagnosis and potentially improve antimicrobial utilization. Whole blood samples from hospitalized patients with clinical signs of sepsis were analyzed using the T2Bacteria Panel (T2Biosystems) and compared to simultaneously collected BC. Discrepant results were evaluated based on clinical infection criteria, combining supporting culture results and the opinion of treating physicians. A total of 55 samples from 53 patients were evaluated. The sensitivity and specificity of the T2Bacteria panel was 94% (16 out of 17 detections of T2Bacteria-targeted organisms) and 100%, respectively, with 36.4% (8 of 22) causes of BSI detected only by this method. The T2Bacteria Panel detected pathogens on average 55 hours faster than standard BC. In our study, 9 of 15 patients with positive T2Bacteria Panel results received early-targeted antibiotic therapy and/or modification of antimicrobial treatment based on T2Bacteria Panel findings. Given the high reliability, faster time to detection, and easy workflow, the technique qualifies as a point-of-care testing approach.

Úvod: Včasná a kauzální aplikace antibiotik u pacientů s pozitivní hemokulturou patří mezi základní předpoklady úspěšné léčby infekce. Izolace a následná identifikace bakterií z hemokultury klasickými (kultivačními) metodami však může trvat až několik dní. MALDI-TOF MS patří mezi metody, které umožňují rychlou identifikaci bakterií, a to nejen kultur z kultivačních médií, ale i přímo v klinickém materiálu. Metodika: Do studie byly zařazeny vzorky pozitivních hemokultur, které byly v letech 2016 až 2018 odebrány od pacientů Fakultní nemocnice Olomouc a následně byly vyšetřeny na Ústavu mikrobiologie Lékařské fakulty Univerzity Palackého v Olomouci. Vzorky pozitivních hemokultur byly zpracovány pomocí vlastního modifikovaného postupu, zahrnujícího odstranění krevních buněk pomocí centrifugace za nižších otáček. Následně byla pomocí centrifugace za vyšších otáček a promývání vzorku získána peleta, která byla testována pomocí MALDI-TOF MS. Výsledky: Ve studii bylo metodou přímé identifikace vyšetřeno celkem 110 pozitivních hemokultur. Správná identifikace na úrovni druhu byla vyšší u gramnegativních bakterií (88 %) v porovnání s grampozitivními bakteriemi (79 %) a také u nich bylo dosaženo vyšších hodnot identifikačního skóre. Hodnota identifikačního skóre vyšší nebo rovna než 2,0 byla u 62 % hemokultur obsahujících gramnegativní bakterie a 17 % hemokultur obsahujících bakterie grampozitivní. Hodnota identifikačního skóre v rozmezí 1,7–2,0 byla zjištěna u 21 % hemokultur gramnegativních a 33 % hemokultur s grampozitivními bakteriemi. Závěr: Přímá identifikace mikroorganismů z pozitivních hemokultur metodou MALDI-TOF MS umožňuje rychlejší diagnostiku a v důsledku zkrácení doby potřebné k dosažení výsledku identifikace patogenu může pozitivním způsobem významně ovlivnit antibiotickou léčbu pacientů.
Background: Early and causal administration of antibiotics in patients with a positive blood culture is an essential prerequisite for successful treatment of infection. However, isolation and subsequent identification of bacteria in a blood culture by classical (culture) methods may last several days. MALDI-TOF MS is a method allowing rapid identification of bacteria, not only cultures from culture media, but also directly in clinical specimens. Methods: The study included samples of positive blood cultures taken from patients in the University Hospital Olomouc between 2016 and 2018 and examined at the Department of Microbiology of the Faculty of Medicine, Palacký University Olomouc. Positive blood culture samples were processed using an in-house method involving the removal of blood cells by low-speed centrifugation. Subsequently, a pellet obtained by high-speed centrifugation and sample washing was tested by MALDI-TOF MS. Results: A total of 110 positive blood cultures were examined using the method of direct identification. At a species level, more Gramnegative bacteria (88 %) than Gram-positive bacteria (79 %) were correctly identified, with higher identification score values being obtained for the former. Identification score values of 2.0 or higher were found in 62 % of blood cultures containing Gram-negative bacteria and 17 % of blood cultures containing Gram-positive bacteria. Identification score values ranging from 1.7 to 2.0 were found in 21 % of Gram-negative blood cultures and 33 % of blood cultures containing Gram-positive bacteria. Conclusion: Direct identification of microorganisms from positive blood cultures using MALDI-TOF MS enables more rapid diagnosis. By reducing the time required to obtain the result of pathogen identification, it may positively affect the antibiotic treatment of patients.

Cíl práce: Tento článek přináší přehled nejčastějších bakteriálních druhů izolovaných z hemokultur ve Fakultní nemocnici Olomouc v letech 2015–2019 a jejich profil rezistence k antibiotikům. Materiál a metody: Data byla získána z laboratorního informačního systému ENVIS LIMS. Za období 1. 1. 2015 až 31. 12. 2019 byly retrospektivně hodnoceny výsledky hemokultivací. Z pozitivních hemokultur byla vyhodnocena četnost bakteriálních izolátů a u nejčastějších původců bakteriémií zjištěna rezistence k vybraným antibiotikům. K identifikaci byly použity standardní mikrobiologické postupy za pomoci systému MALDI-TOF MS. Citlivost k antibiotikům byla stanovena standardní diluční mikrometodou podle kritérií EUCAST. Výsledky: V období 5 let bylo celkem hodnoceno 3 400 izolátů z pozitivních hemokultur. Nejčastěji byly izolovány koaguláza-negativní stafylokoky (37 %), Escherichia coli (16 %), Klebsiella pneumoniae (9 %), Staphylococcus aureus (7 %), streptokoky (5 %), Pseudomonas aeruginosa (4 %), Enterobacter cloacae (2 %), Enterococcus faecalis (2 %) a Enterococcus faecium (2 %). Escherichia coli byla v uvedeném období relativně málo rezistentní k cefalosporinům 3. generace, piperacilinu s tazobaktamem, gentamicinu a fluorochinolonům (7–33 %). Klebsiella pneumoniae naproti tomu vykazovala vyšší rezistenci k těmto antibiotikům (33–65 %). Rezistence druhu Staphylococcus aureus k oxacilinu se pohybuje v rozmezí 3–7 %. Pseudomonas aeruginosa se vyznačovala rezistencí alespoň k jednomu z následujících antibiotik – piperacilinu/tazobaktamu, ceftazidimu, cefepimu, meropenemu, ciprofloxacinu a gentamicinu v rozmezí 3–46 %. Závěr: Znalost prevalence původců bakteriémií a jejich profilů rezistence je důležitá pro iniciální antibiotickou terapii v případě probíhající infekce. Je vhodné epidemiologické přehledy tohoto typu pravidelně provádět a poskytovat klinickým lékařům jako součást „antibiotic stewardship“.
Objectives: To provide an overview of the most common bacterial species isolated from blood cultures in the University Hospital Olomouc in the years 2015–2019 and their antibiotic resistance patterns. Material and Methods: The data were obtained from the laboratory information management system ENVIS LIMS. The results were analyzed retrospectively for the period from January 1, 2015 to December 31, 2019. Among positive blood cultures, the prevalence of bacterial species was assessed and the most frequent species were evaluated for resistance to selected antibiotics. Each sample was processed using standard microbiology methods with the MALDI-TOF MS system. Susceptibility to antibiotics was tested with the microdilution method according to the EUCAST recommendations. Results: Over the study period, a total of 3 400 isolates from blood cultures were included. Coagulase-negative staphylococci were the most prevalent (37 %), followed by Escherichia coli (16 %), Klebsiella pneumoniae (9 %), Staphylococcus aureus (7 %), Streptococcus spp. (5 %), Pseudomonas aeruginosa (4 %), Enterobacter cloacae (2 %), Enterococcus faecalis (2 %) and Enterococcus faecium (2 %). Resistance of E. coli to 3rd generation cephalosporins, piperacillin/tazobactam, gentamicin and fluoroquinolones ranged from 7 % to 33 %. A high percentage of Klebsiella pneumoniae strains (33 %–65 %) was resistant to the above antibiotics. The prevalence of methicillin-resistant Staphylococcus aureus reached 3 %–7 %. Pseudomonas aeruginosa exhibited resistance to piperacillin/tazobactam, ceftazidime, cefepime, meropenem, ciprofloxacin and gentamicin ranging from 3 % to 46 %. Conclusion: Knowing the prevalence of bacterial species from blood cultures and their antimicrobial resistance patterns is important for empirical antibiotic therapy in case of an existing infection. It is advisable to conduct such epidemiological studies as part of antibiotic stewardship.

The aim of this study was to trace the dynamic changes of methicillin-resistant Staphylococcus aureus (MRSA) lineages in the local hospital in both the national and international context. We describe genotypic and phenotypic characterization of 62 non-duplicate MRSA isolates collected during 2010-2016 at University Hospital in Hradec Kralove, Czech Republic. The isolates were characterized by multilocus sequence typing (MLST), spa typing, and staphylococcal cassette chromosome mec typing (SCCmec typing). Eight different genotypes were described; ST225-t003-II (32/62, 52%), ST5-t002-II (13/62, 22%), and ST225-t014-II (12/62, 21%) were constantly detected over the 7-year follow-up period. The genotypes ST225-t151-II, ST225-t1282-II, ST225-t1623-II, ST78-t2832-II, and ST225-t8799-II occurred only once in the period reported. The majority of the strains, represented by ST225, belonged to clonal complex 5 (CC5).

Fast and accurate detection of causative agents of bloodstream infections remains a challenge of today's microbiology. We compared the performance of cutting-edge technology based on polymerase chain reaction coupled with electrospray ionization-mass spectrometry (PCR/ESI-MS) with that of conventional broad-range 16S rRNA PCR and blood culture to address the current diagnostic possibilities for bloodstream infections. Of 160 blood samples tested, PCR/ESI-MS revealed clinically meaningful microbiological agents in 47 samples that were missed by conventional diagnostic approaches (29.4% of all analyzed samples). Notably, PCR/ESI-MS shortened the time to positivity of the blood culture-positive samples by an average of 34 hr. PCR/ESI-MS technology substantially improved current diagnostic tools and represented an opportunity to make bloodstream infections diagnostics sensitive, accurate, and timely with a broad spectrum of microorganisms covered.

OBJECTIVES: Fully sequenced IncI1 plasmids obtained from CTX-M-1-producing Escherichia coli of human and animal origin were compared. METHODS: Twelve E. coli isolates sharing identical ESBL genes and plasmid multilocus STs sequenced on Illumina and MinION platforms were obtained from the Danish antimicrobial resistance surveillance programme, DANMAP. After de novo assembly, the sequences of plasmids harbouring blaCTX-M-1 were manually curated and ORFs annotated. Within-group comparisons were performed separately for the IncI1 ST3 plasmid type and the IncI1 ST7 plasmid type. The IncI1 ST3 plasmid group was obtained from 10 E. coli isolates (2 from patients with bloodstream infections, 6 from food and 2 from animals). The IncI1 ST7 plasmids originated from E. coli isolates obtained from a patient with bloodstream infection and from a pig. Sequences of IncI1 ST3 and IncI1 ST7 plasmids harbouring blaCTX-M-1 with determined origin were retrieved from GenBank and used for comparison within the respective group. RESULTS: The 10 IncI1 ST3 blaCTX-M-1 plasmids were highly similar in structure and organization with only minor plasmid rearrangements and differences in the variable region. The IncI1 ST7 blaCTX-M-1 plasmids also showed high similarity in structure and organization. The high level of similarity was also observed when including plasmids from E. coli of animal origin from Australia, Switzerland, the Netherlands and France. CONCLUSIONS: This study shows broad spread of a very successful CTX-M-1-producing IncI1 type plasmid among E. coli of both human and animal origin.

The pathogenic extended-spectrum-beta-lactamase (ESBL)-producing Escherichia coli lineage ST648 is increasingly reported from multiple origins. Our study of a large and global ST648 collection from various hosts (87 whole-genome sequences) combining core and accessory genomics with functional analyses and in vivo experiments suggests that ST648 is a nascent and generalist lineage, lacking clear phylogeographic and host association signals. By including large numbers of ST131 (n = 107) and ST10 (n = 96) strains for comparative genomics and phenotypic analysis, we demonstrate that the combination of multidrug resistance and high-level virulence are the hallmarks of ST648, similar to international high-risk clonal lineage ST131. Specifically, our in silico, in vitro, and in vivo results demonstrate that ST648 is well equipped with biofilm-associated features, while ST131 shows sophisticated signatures indicative of adaption to urinary tract infection, potentially conveying individual ecological niche adaptation. In addition, we used a recently developed NFDS (negative frequency-dependent selection) population model suggesting that ST648 will increase significantly in frequency as a cause of bacteremia within the next few years. Also, ESBL plasmids impacting biofilm formation aided in shaping and maintaining ST648 strains to successfully emerge worldwide across different ecologies. Our study contributes to understanding what factors drive the evolution and spread of emerging international high-risk clonal lineages.
