Cystická hydatidóza neboli echinokokóza (CE) patří k vzácně se vyskytujícím onemocněním na území České republiky. V letech 2005-2009 bylo zaznamenáno 11 případů CE, přičemž se většinou jednalo o případy diagnostikované u imigrantů z Balkánu. V předložené práci uvádíme kazuistiku 38letého pacienta s hydatidózou jater a následným rozvojem bronchobiliární píštěle jako vzácné komplikace CE. Předoperačně byla u pacienta zahájená léčba mebendazolem. Poté za 10 dnů byla z hrudního přístupu resekována postižená část plic a přes bránici byly založeny drény do jaterních cyst. Pooperační průběh byl komplikován únikem žluči do hrudní dutiny. Únik žluče souvisel s trvající komunikací mezi jaterní cystou a hrudní dutinou, která se po odstranění drénu spontánně neuzavřela. Endoskopicky provedená nasobiliární drenáž snížila tlak ve žlučových cestách, což po 14 dnech vedlo ke spontánnímu uzavření komunikace mezi echinokokovou cystou jater a hrudní dutinou. Pacient je za 7 měsíců po operaci v dobrém klinickém stavu.
Cystic hydatid disease or cystic echinococcosis (CE) rarely occurs in the Czech Republic. In 2005-2009, eleven cases were recorded, mostly among immigrants from the Balkans. Presented here is a case report of a 38-year-old patient with hepatic CE complicated by bronchobiliary fistulae. Ten days before surgical removal of the hydatid cysts, treatment with mebendazole was started. During surgery the affected part of the lungs was resected and the liver cysts were drained using transthoratic access. The follow-up was complicated by leakage of bile into the pleural cavity. The leakage was associated with continued communication between the liver cyst and the pleural cavity which did not close spontaneously after removal of the drain. Endoscopic nasobiliary drainage decreased pressure in the bile duct and within 14 days, it led to the spontaneous closure of the communication between the liver cyst and the pleural cavity. Seven months after the operation, the patient was in a very good clinical condition.

The problem of prophylaxis of incompetence of bronchial stump and bronchial fistula is many sided. Notwithstanding the development of new technologies, perfection of surgical technique, its frequency still remains on high level that persistently requests the search of new ways for resolving this problem, which preserved its topicality till nowadays. We have found that: The most efficiency of BFS is achieved during treatment of bronchial fistulas with diameter up to 3 mm and with effectiveness at 83.2% patients after rightside PE and at 92.3% patients after leftside PE. In case of BPF with diameter more than 3 mm the number of satisfactory results of BFS reaches only 33.3%. but, it is necessary to mention that among patients with BPF sized over 3 mm after leftside PE the effectiveness of BFS significantly exceeds as general index (50.0% against 33.3%) so after rightside PE (50.0% against 25.0%).
Autori prezentujú prípad pacienta s empyémom hrudníka a veľkou bronchopleurálnou fistulou, ktorý riešili po neúspešných predchádzajúcich operáciách otvorenou drenážou podľa Eloessera a po vyčistení pleurálnej dutiny, uzatvorili kýpeť hlavného bronchu sutúrou s využitím omentoplastiky. Použili lalok omenta podľa Kiricutu na cievnej stopke, ktorý transponovali cez bránicu do pleurálnej dutiny. Lalokom prekryli sutúru bronchu a vyplnili pleurálnu dutinu, výkon ukončili torakoplastikou a uzatvorením hrudníka. Pooperačný priebeh bol priaznivý, bez komplikácií, pacient je v súčasnosti bez ťažkostí. Liečba postpneumonektomického empyému s bronchopleurálnou fistulou si vyžaduje komplexný viacetapový prístup. V poslednej fáze po drenáži, vyčistení pleurálnej dutiny a po uzatvorení dehiscentnej priedušky je možné využiť na prekrytie sutúry a na zabezpečenie obliterácie postpneumonektomickej pleurálnej dutiny lalok omenta transponovaný do hrudníka z brušnej dutiny. Význam omentoplastiky spočíva v jeho vlastnostiach ako sú: novotvorba ciev, rezorpčná a absorpčná schopnosť, imunologická a mechanická funkcia, ktoré v konečnej fáze pomôžu pri vyhojení vážnej postpneumonektomickej komplikácie .
The authors present a case reivew of a patient with thoracic empyema and large bronchopleural fistula. Following preceeding failing procedures, the condition was managed using Eloesser open drainage, the pleural cavity was cleared and the main bronchus fistula was sutured using omentoplasty. A pediculated omental flap (Kiricutu) was transposed into the pleural cavity through the diaphragm. The flap was used to cover the bronchial suture and fill the pleural cavity. Finally, thoracoplasty and suturing of the thorax was performed. Postoperative course was positive, with no complications recorded and, currently, the patient is asymptomatic. Treatment of postpneumonectomic empyema with bronchopleural fistula requires complex approach in several steps. During the final phase, following the pleural cavity drainage and clearing and when the dehiscent bronchus is sutured, omental flaps, transposed from the abdominal into the thoracic cavity, may be used to cover the suture and to obliterate the postpneumonectomic pleural cavity. Significance of omentoplasty consists in its features, such as: neovascularization, resorption and absorption qualities, immunological and mechanical functions, which, finally, facilitate healing of serious postpneumonectomic complications.

Rozpad bronchiálního pahýlu po plicní resekci se vznikem bronchopleurální píštěle a pooperačním empyémem hrudníku nadále představuje obávanou komplikaci plicních resekci především po pneumonektomiích. Autoři v kazuistice informují o úspěšné konzervativní léčbě, která následovala po neúspěšné chirurgické revizi a po pokusu o zavedení endobronchiálního stentu. Nemocný je 20 měsíců po uzavření píštěle bez známek recidivy.
Disintegration of the bronchial stub following the lung resection procedures together with development of the bronchopleural fistule and the postoperative empyema of the thorax, remain a feared complication following all lung resections, but especially pneumonectomies. In this case review, the authors report on a successful conservative management case, which followed an unsuccessful surgical revision and an attempt for the endobronchial stent introduction. 20 months following the closure of the fistule, the patient shows no signs of a relapse of the disorder.

Kazuistika popisuje průběh úspěšné léčby abscedující bronchopneumonie s bronchopleurální píštělí vedoucí k pyopneumotoraxu. Je popsána strategie umělé plicní ventilace se selektivní intubací a ventilací postižené plíce pomocí vysokofrekvenční tryskové ventilace a objemové ventilace zdravé plíce v režimu BIPAP. Rozbor se týká problematiky antibiotické léčby s důrazem na důležitost toalety bronchiálního stromu cíleným pravidelným bronchoskopickým odsáváním.
The case report describes a patient with pneumonia with abscess formation who developed a bronchopleural phistula leading to pyopneumothorax. The patient was intubated with biluminaltube. The affected lung was ventilated with high frequency jet ventilation, the healthy lung was ventilated in volume control BIPAP mode. Antibiotic treatment is discussed with proper bronchial tree toilette using targeted bronchial lavage and suction.
