Úvod: Migrace síťky je jednou z nejméně častých komplikací plastiky tříselné kýly se síťkou. Bylo publikováno jen několik malých sestav pacientů s anamnézou migrace síťky. Plné porozumění léčbě této komplikace je limitováno nedostatkem dat. Většina pacientů z publikovaných sestav byla léčena chirurgicky. V tomto článku bychom rádi prezentovali naši zkušenost s pokusem o endoskopickou léčbu této komplikace. Kazuistika: Pacient podstoupil transabdominální preperitoneální plastiku tříselné kýly v roce 1999. O čtyři roky později podstoupil reoperaci stejnou metodou pro recidivu tříselné kýly. Po dvaceti letech po primární operaci měl pacient pozitivní test na okultní krvácení ve stolici. Subjektivně byl zcela asymptomatický. Kolonoskopicky byla zjištěna přítomnost migrované síťky v colon sigmoideum. Navzdory opakovaným pokusům o odstranění síťky endoskopicky byla tato forma terapie neúspěšná. Migrovaná síťka musela být extrahována chirurgicky a byla provedena resekce sigmatu. Kromě infekce rány (komplikace IIIb dle Clavien-Dindo) byl pooperační průběh nekomplikovaný. Závěr: V tomto případě se nepodařilo endoskopicky odstranit síťku migrovanou do colon sigmoideum. Navzdory naší negativní zkušenosti se domníváme, že má smysl se pokusit síťku migrovanou do dutého intraabdominálního orgánu odstranit endoskopicky.
Introduction: Mesh migration is one of the least common complications that arise after inguinal hernia repair with a mesh. Only small case series have been reported, and an understanding of this issue is limited due to a lack of data. Most of the cases were treated surgically. In this paper, we wish to present the potential of treating this condition using endoscopic techniques. Case report: A male patient underwent transabdominal preperitoneal repair of a primary inguinal hernia in 1999. In 2003, the patient required the same procedure for a recurrent inguinal hernia. Twenty years after the primary hernia repair, the patient had a positive faecal occult blood test but was completely asymptomatic. A colonoscopy revealed mesh migration into the sigmoid colon. Despite multiple attempts to remove the mesh endoscopically, endoscopic treatment was unsuccessful. The migrated mesh was surgically removed and obligatory resection of the sigmoid colon was carried out. Apart from wound infection (Clavien-Dindo IIIb), the postoperative course was uneventful. Conclusion: In our case, the mesh that had penetrated the colon could not be removed endoscopically. Despite our experience, it is advisable to attempt endoscopic removal of mesh that has migrated into a hollow intra-abdominal viscus.

OBJECTIVE: To demonstrate the use of a single-stapler technique during rectosigmoid resection in women with deep infiltrating endometriosis (DIE). DESIGN: A step-by-step video demonstration of rectosigmoid resection and end-to-end anastomosis using two circularly placed sutures and one circular stapler. SETTING: Institute for the Care of Mother and Child, Prague, Czech Republic. PATIENT(S): A 39-year-old woman presented with primary sterility and deep infiltrating endometriosis, and an EZIAN score of A2,B2,C3. A nodule was located 9 cm from the anus and was 38 × 9 mm in size. This included an intramural fibroma of 6 cm and a left-sided ovarian endometriotic cyst of 6 cm. Her pain on the visual analogue scale were dysmenorea 6, dyspareunia 5-6, dyschezie 7, dysuria 0, and acyclic pain 5. INTERVENTIONS: The primary objective was to replace the linear-stapler resection with two simple, strictly circularly placed sutures, to cut the intestinal wall between them, and to form the end-to-end anastomosis with a circular stapler. The one-stapler technique consisted of the following steps: intestinal wall cleansing as in the limited segmental resection; placement of one strictly circular suture just below the DIE nodule, without fixation; placement of the first circular suture just below the DIE nodule, ideally with at least three full-thickness "bites" of the intestinal wall; placement of the second circular stitch approximately 2 cm below the first one in a similar manner (three full-thickness "bites"); interruption of the intestinal wall with a harmonic scalpel; end-to-end intestinal anastomosis with a circular stapler; and airtightness test of the anastomosis. This results in only one incision line and therefore a lower risk of leakage. Intestinal resection time was on average 10 minutes longer compared to that for the linear stapler technique. So far, we have successfully performed the procedure in 25 women. Perioperative leakage was observed in two of these 25 patients in the classical procedure group and in none of the 25 patients in the group with the one-stapler technique. There were no differences in C-reactive protein (CRP) on third and fifth postoperative days or in other complications such as bleeding and pyrexia). The cost of procedure is lowered by the decrease in the number of staplers from 3 to 1. The patients' postoperative follow-up was uneventful, and they were discharged from the hospital at the same time as the women in whom the classical stapler technique was performed. MAIN OUTCOME MEASURES(S): The primary outcome was the development of a new surgical approach to resection rectosigmoid endometriotic nodules that would decrease the number of incision lines on the intestine. The secondary outcome measures were peri- and postoperative complications (i.e., bleeding, intestinal leakage, postoperative infection, CRP), length of the surgery and hospitalization, and cost of the procedure. CONCLUSION: Multiple incision lines following resection of the rectosigmoid colon and end-to-end anastomosis are risk factors for postoperative intestinal leakage. Therefore, a single incision line formed with two circular sutures, and one circular stapler may reduce the risk of postoperative complications and also financial expenses of the procedure. We believe that this method is suitable and easiest for nodules located less than 6 cm from the anal verge because of possible complications with angulation of linear stapler.

INTRODUCTION AND HYPOTHESIS: Endometriosis is a gynecological condition characterized by endometrial tissue outside of the uterus. It affects up to 15% of women of reproductive age. In the case of bowel infiltration, about 90% of lesions are localized on the sigmoid colon or the rectum and may interfere with bowel function. Three surgical approaches are possible: (1) shaving technique, (2) discoid resection of the nodule, and (3) segmental resection with end-to-end anastomosis. A rectovaginal fistula is feared as a postoperative complication mainly in simultaneous resection of the vaginal and the rectosigmoid nodules. Its prevention is a two-step surgery (the first operation on the vagina and the second on the colon) or a preventive colostomy, both of which are often thought to be too invasive for a benign condition. Herein, we suggest a one-step surgery to prevent its development. METHODS: In three women, a concomitant laparoscopic resection of the vaginal and rectosigmoid endometrial nodule was completed with interposition of a mesorectal flap. RESULTS: All surgeries were uncomplicated with no rectovaginal fistula in the postoperative period. CONCLUSION: In the hands of skilled surgeons, this one-step technique can be used to prevent rectovaginal fistula development.

Anterior penetrating sacral injuries in children are extremely rare. These injuries are coupled with both a high energy mechanism (combat injury, motor vehicle accidents) and with foreign body impalement. The treatment is individual, laparotomy with penetrating wound exploration is indicated, osteosynthesis is performed in case of grossly displaced fractures, in an unstable injury to the posterior pelvic ring, and urgently in case of a neurological injury. The case report describes a 14-year-old girl with a left-sided anterior penetrating sacral injury at the level of S2/S3, who was injured during a bicycle accident (impalement on handlebars). The emergent laparotomy was performed first to treat the lesion of the sigmoid mesocolon. After 16 days the patient underwent the second operation, when open fragment reposition and sacral bone suture were performed. Both the sacral fracture and soft tissues were healed in 6 weeks. The patient was fully weight bearing and without pain. Key words:pediatric sacral fracture, penetration, treatment.

Autoři prezentují dva případy invaginace u dospělého pacienta. V prvním případě se jedná o 84letou polymorbidní pacientku, akutně indikovanou k operační revizi pro ileus, s nálezem invaginace v oblasti sigmatu, bez průkazu organické příčiny. V druhém případě o 80letou polymorbidní pacientku, akutně indikovanou k operační revizi pro ileus, s nálezem invaginace na tenkém střevě, s průkazem neuroendokrinního nádoru. Střevní invaginace v dospělosti je poměrně vzácná a ve většině případů vzniká na podkladě organické příčiny, nejčastěji benigního tumoru tenkého střeva [2]. Na rozdíl od invaginace v dětském věku, kdy je příčina většinou nejasná [1,7]. Práce poukazuje na vzácnou příčinu náhlé příhody břišní u dospělého člověka, kterou může být i invaginace.
The authors present two cases of intestinal invagination in adult patients. The first case is an 84-year-old polymorbid female patient indicated for acute surgical revision due to ileus with a finding of invagination in the sigmoid area, with no evidence of an organic cause. The second case, an 80-year-old polymorbid female patient, was indicated for acute surgical revision also due to ileus with a finding of invagination on the small intestine and with a proven neuroendocrine tumor. Unlike invagination in childhood where its cause is usually unknown[1,7], intestinal invagination is relatively rare in adults and it is mostly due to an organic cause, most frequently a benign tumor of the small intestine[2]. This study focuses on invagination as a rare cause of acute abdomen in adult patients.

Úvod: Cílem studie je zhodnotit výsledky zavedení metody detekce perfuze anastomózy po resekci sigmoidea a rekta do praxe na pracovišti autorů a vyhodnotit incidenci nutnosti změny linie resekce v návaznosti na kvalitu perfuze stěny střevní a zároveň zaznamenat komplikace v hojení anastomózy. Metoda: Retrospektivní unicentrická analýza prospektivně sbíraných dat u pacientů s resekcí sigmoidea a rekta a konstrukcí primární anastomózy. Do souboru bylo zařazeno 50 pacientů, 27 mužů a 23 žen, s mediánem věku 64,5 roku (33–80). 44 pacientů bylo k resekci indikováno pro karcinom sigmoidea anebo rekta, 6 pacientů pro benigní onemocnění. Ve 29 případech se jednalo o totální excizi mezorekta s koloanální mechanickou nebo ručně konstruovanou anastomózou a ve 21 případech byla provedena resekce sigmoidea anebo horního rekta s mechanickou anastomózou. U všech pacientů před konstrukcí anastomózy byla vykonána kontrola perfuze anastomozovaných segmentů metodou NIR (near infrared) fluorescenční angiografie detekcí indocyaninové zeleně (ICG). Byla hodnocena kvalita perfuze mezokolon a stěny střevní s impaktem na nutnost posunutí resekční linie a komplikace v hojení anastomózy v 30denním pooperačním intervalu. Výsledky: U všech 50 pacientů byla metoda kontroly perfuze fluorescenční angiografií technicky úspěšně realizována. V 5 případech (10 %) bylo nutné posunout linii resekce pro známky špatné perfuze stěny střevní. U 4 pacientů (8 %) byla v pooperačním období zaznamenána komplikace v hojení anastomózy. U pacientů s totální excizí mezorekta byla dehiscence verifikována ve 3 případech (10,3 %) a v 1 případě (4,8 %) se o dehiscenci jednalo po resekci sigmoidea a horního rekta. Závěr: Prezentované výsledky naznačují, že metoda fluorescenční angiografie může představovat cestu, jež povede ke snížení incidence dehiscence anastomózy kolorektálních resekcí detailním mapováním perfuze anastomózovaných segmentů.
Introduction: The aim of the study was to evaluate the results of fluorescence angiography for assessing anastomotic perfusion after resection of the sigmoid colon and rectum since its introduction into clinical practice at the authors´ workplace and to evaluate the incidence of necessary resection line repositioning based on the quality of perfusion, and also to record any complications in anastomotic healing. Method: Retrospective unicentric analysis of prospectively collected data from patients with resection of the sigmoid colon and rectum with primary anastomosis. The patient set included 50 patients, 27 males and 23 females; the median age was 64.5 years (33–80). Forty-four patients were indicated for resection for cancer of the sigmoid colon or rectum, while 6 patients had a benign disease. Twenty-nine patients underwent total mesorectal excision with coloanal mechanical or hand-sewn anastomosis and 21 underwent resection of the sigmoid colon or upper rectum with mechanical anastomosis. Prior to the construction of the anastomosis, assessment of perfusion of the anastomotic segments by near infrared (NIR) indocyanine green (ICG) fluorescence angiography was performed in all patients. The quality of perfusion of the mesocolon and bowel wall and its impact on moving the resection line and complications of anastomotic healing 30 days postoperatively were all evaluated. Results: Assessment of perfusion using fluorescence angiography was technically successfully performed in all 50 patients. In 5 cases (10%) the resection line had to be moved for signs of poor perfusion of the bowel wall. Postoperatively, healing of the anastomosis was complicated in four patients (8%). Dehiscence was recorded in 3 patients (10.3%) with total mesorectal excision and in 1 patient (4.8%) after resection of the sigmoid colon and upper rectum. Conclusion: The presented results indicate that fluorescence angiography may lead to a decrease in the incidence of anastomotic dehiscence after colorectal resections by mapping in detail the perfusion of the anastomosed segments.

BACKGROUND: Both over-the-scope clip (OTSC) and KING (endoloop + clips) closures provide reliable and safe full-thickness endoscopic closure. Nevertheless, OTSC clip demonstrated significantly inferior histological healing in the short-term follow-up. AIM: To compare OTSC versus KING closure of a perforation with regard to long-term effectiveness and macroscopic and histological quality of healing. METHODS: We performed a randomized experimental study with 16 mini-pigs (mean weight 43.2 ± 11.2 kg). A standardized perforation was performed on the anterior sigmoid wall. KING closure (n = 8) was attained by approximation of an endoloop fixed to the margins of a perforation with endoclips. OTSC closure (n = 8) was performed by deploying OTSC (OVESCO) over the defect. Pigs underwent a control sigmoidoscopy 8 months after the closure to assess the macroscopic quality of healing. Then, autopsy was performed and the rectosigmoid was sent for histopathological assessment. RESULTS: All closures were completed successfully without air leaks. The duration of closure was similar in both techniques (OTSC 17.8 ± 7.6 min vs. KING 19.6 ± 8.8 min). At autopsy, all KING closures (100 %) were healed with a flat scar without signs of leakage. Microscopically, no inflammatory changes were observed after KING closure. In the OTSC group, microscopic ulcers were present in two pigs (25 %), cryptal abscesses in three pigs (38 %) and significant neutrophil accumulation in all eight pigs (P < 0.01). Giant cell granulomas, dysplasia or abundant scarification was not observed in either group. CONCLUSIONS: Both OTSC and KING closures offer a long-term reliable seal of a gastrointestinal perforation without stenosis or fistulas. KING closure provides long-term histologically superior healing.

Hlboká infiltrujúca endometrióza (DIE) je chronické progresívne estrogén dependentné ochorenie, ktoré môže postihovať aj štruktúry gastrointestinálneho traktu. Ochorenie je zvyčajne dlhodobo asymptomatické, respektíve sa prejavuje veľmi nešpecifickými príznakmi. Jeho neskorý záchyt môže viesť k ireverzibilným zmenám na jednotlivých štruktúrach. Táto kazuistika popisuje raritný prípad rozsiahlej hlbokej infiltrujúcej endometriózy zadného kompartmentu malej panvy, ktorá vyústila do stavu obštrukčného ilea. Sústreďuje sa na zásadnú úlohu „virtuálnej kolonoskopie“ pomocou trojdimenzionálnej transvaginálnej sonografie (3D-TVUS) pri hodnotení rozsahu ochorenia.
Deep infiltrating endometriosis (DIE) is a chronic progressive estrogen-dependent disease that can also affect structures of the gastrointestinal tract. The disease is usually asymptomatic in long-term, respectively, is manifested with very nonspecific symptoms. Its late detection can lead to irreversible changes in the individual structures. This case report describes a rare case of extensive deep infiltrating endometriosis of posterior pelvic compartment, which resulted into the state of obstructive ileus. It focuses on the essential role of „virtual colonoscopy“ using three-dimensional transvaginal sonography (3D-TVUS) in evaluating the extent of the disease.
